Clinical Observation on Hyperthyroidism (갑상선기능항진증(甲狀腺機能亢進症)의 임상적(臨床的) 관찰(觀察))

  • A clinical analysis was made on 161 cases of hyperthyroidism seen at the Radioisotope Laboratory of Kyungpook National University Hospital. This series consisted of 144 cases of diffuse goiter and 17 cases of nodular goiter. 1) Hyperthyroidism was most prevalent in the 4th decade and male to female ratio was 1:4.6. 2) Cardinal symptoms in the order of frequency were weakness, easy fatigability, palpitation, weight loss, nervousness, perspiration, heat intolerance, increased appetite, insomnia and dysmenorrhea. 3) Major physical findings in the order of frequency were goiter, fine tremor, tachycardia, wide pulse pressure, emaciation, warm moist skin, exophthalmos, systolic hypertension and atrial fibrillation. 4) The complications were ophthalmopathy (34.2%), thyrotoxic heart disease (5.6%), thyroid crisis (1 case), pretibial myxedema (1 case) and thyrotoxic myopathy (1 case). 5) Mean values of the six hour and twenty-four hour $^{131}I$ uptakes by the thyroid glands were 67.5% and 71.6%, respectively, in diffuse goiter and 64.5% and 65.0%, respectively, in nodular goiter. 6) Mean values of twenty-four hour $PB^{131}I$ conversion ratio were 76.3% in diffuse goiter and 70.2% in nodular goiter and those of the basal metabolic rate was +51% in the former and +41% in the latter. Mean serum cholesterol level was 152mg% in diffuse goiter and that in nodular goiter was 175mg%. 7) Among the 134 cases treated with $^{131}I$, 66 cases (49.3%) were successful1y controlled with single dose and in the majority of the cases the initial therapeutic dose required was $4.1{\sim}5.0mC$ in diffuse goiter and $5.1{\sim}6.0mC$ in nodular goiter. 8) With $^{131}I$ treatment the symptoms improved in the following order: heat intolerance, emaciation, nervousness, insomnia, easy fatigability, weakness, fine tremor, goiter, perspiration, exertional dyspnea and palpitation. And in a few cases improvement of even exophthalmos was seen. 9) Following $^{131}I$ treatment myxedema occurred in 4 cases (3%) and reccurrence in 9 cases (6.8%).

An Outbreak of Lnugworm Disease in a Flock of Korean Native Cattle (한우사육단지(韓牛飼育團地)에 발생(發生)한 우폐충증(牛肺蟲症)에 대하여)

  • In a pasture located at Kyong Puk area, a group of 200 Korean native casttle showed severe coughing, dyspnea and emaciation, of which 30 cattle died from the respiratory disease. To the alived cattle, clinical, pathologicoparasitological observations and treatment with terramycin, Mintic and tetramisole (Bonasol) were carried out and the following results were obtained. 1) Paroxymal coughighings, increased respiratory rate and heart rate were observed in almost all cases. But, larval output in feces was recognized in 32 percent of the cases. 2) Morderate eosinophilia was observed in a half of the cases. 3) At autopsy, lungs were enlarged, and areas of dark consolidations were scattered in all lobes. In the bronchi, numerous adult lungworms were found. 4) The adult worms detected from the bronchi were morphologically identified as Dictyocaulus viviparus. 5) The clinical signs and larval output were disappeared after the administration of Mintic or Bonasol. 6) From these results, it was concluded that the respiratory disease of the cattle was caused by the infestation of lungworm, Dictyocaulus viviparus.

A Necropsy Case of the Huge Pseudo-diverticulous Cyst at the Rumen of a Cattle (소의 제1위(第一胃)에 형성(形成)된 거대(巨大)한 가성게실(假性憩室)에 관(關)한 일부검례(一部檢例))

  • In a cattle which had died of extreme emaciation caused by cachexic condition, a huge cyst of $45{\times}35{\times}20$cm, in size was found at the rumen. The cyst having with the narrow stalk which ended in blind sac attached firmly to the rumen by fibrinous or fibrous adhesion and, contained the lightly yellowish, clear exudative fluid of about 20 l, in volume. Grossly, the thickness of the cyst wall was 10~14mm, and its inner portion of the half was very rigid and colored with milky white in contrast with soft and edematous outer portion of the half. Microscopically the inner portion of the cyst wall was consisted of scarred fibrous tissue and possessed at its inner margin a little amount of the muscle fibers which had degenerated passably. These were confirmed as the muscle fibers in specific staining property by Van-Gieson's and Mallory-Azan stain. On regarding to above findings of gross and microscopic pictures, it was considered to be a pseudo-diverticulous cyst composed of the serosa and the muscle layer deriving from the wall of the rumen. And it was suggested that the cyst had been growing up to big size by storage of the plenty exudate arisen from its wall and was separated from the rumen at the end of its stalk.

Hemangiosarcoma in a Female Pointer (개에서 발생한 혈관육종 일례)

  • A 12-year old female Pointer was referred to the Veterinary Teaching Hospital, College of Veterinary Medicine, Kyungpook National University for evaluation of emaciation. The owner had noted a dysuria and dyschesia 2 months before admitting to the hospital. The patient showed anorexia, depression and mild anemia. Radiographic findings indicated the mass below the urinary bladder, which pushed the intestine up to the spine. The patient was necropsied and grossly, two large masses were found on the heart and cranial thorax. Congestive nodules of 0.5-1.5 cm in diameter were scattered on both sides of the lungs. The left ventricular wall was hypertrophied and adult heartworms were found in right atrium. A large mass, filled with blood, 9-10 cm in diameter, originated from the rectum, compressed urethra. The gall bladder was filled with bile juice and the spleen was mild atrophied. The cortex of the kidney and the intestine were congested. On microscopic observation, there were neoplastic cells forming irregular vascular channels in the lung, kidney and heart. The vascular channels contain variable amount of blood. In the lung, the cells were almost elongated and the nuclei of neoplastic cells were round or ovoid and hyperchromatized. Based on the observation, the case was diagnosed as a hemangiosarcoma.

Pulmonary Aspergillosis and Renal Oxalosis in an Elk (엘크의 폐장 아스퍼질러스증 및 신장 옥살산증)

  • A four-year-old male elk (Cervus elaphus nelsoni) was diagnosed as pulmonary aspergillosis and renal oxalosis. Clinical signs were coughing, sneezing, respiratory distress, salivation, moderate anorexia, and progressive emaciation. Main gross lesions were fibrinopurulent tonsillitis, diffusely fibrinous pleuritis, and distinct lobar pneumonia with purple red in color. Most of the pulmonary lobes had numerous well demarcated 0.5 to 2 cm yellowish white discrete or confluent nodules that were surrounded by pale red zones. Histopathologically, the affected lungs were disseminated necrotizing pyogranulomas including fungal hypae, vasculitis, and diffusely fibronecrotic pleuritis. The renal lesions were composed of extensive tubular necrosis with large numbers of rosette-formation by birefringent oxalate crystals. Aspergillus fumigatus was isolated from lesions of the lungs. It seems to be a first report for pulmonary aspergillalis and renal oxalosis of a farmed elk in Korea.

Toxoplasmosis in piglets (자돈의 톡소플라즈마증 발생)

  • Suckling piglets and weaned pigs showed anorexia, dehydration, severe abdominal breathing, emaciation and paresis from Oct. 1993. to Nov. 1993. Five 2-week-old piglets were submitted for diagnosis in Kangwon National University. At necropsy, the pin-point well demarcated yellowish white foci were scattered on the surface of the lung, heart, liver, spleen and kidney. Histologically, multifocal areas of necrosis with mononuclear cells infiltration were found in the lung, heart, liver, lymph node, spleen, kidney and small intestine. These lesions tended to be associated with blood vessels. Variable round to ovoid tachyzoites were located at the periphery of the lesions. The organisms were demonstrated as Toxoplasma gondii by immunohistochemical staining method. Ultrastructurally, this parasite was surrounded with parasitophorous vacuole in alveolar macrophage. The parasite was crescent-shaped and $6{\sim}8{\times}1{\sim}2{\mu}m$ in size. It was enclosed by an thick outer membrane and an underlying thin inner membrane. Several club-shaped paired organelles and conoids lay in the cytoplasm at the anterior. Numerous round body and one to several mitochondria were presented in the cytoplasm. Based on the gross findings, histopathology, immunohistochemical and electron microscopic findings, this case was diagnosed as toxoplasmosis in piglets.

Mandibular Nerve Block Improves Nutritional Status and Liver Function in the Patient of Trigeminal Neuralgia -A case report- (하악신경차단으로 도움받은 삼차신경통환자의 영양섭취 및 간기능회복 -증례 보고-)

  • It is well known that many patients with trigeminal neuralgia suffer from electric shock-like stabbing pains. The pain can be triggered by nonnoxious stimuli such as touching of the face, chewing, talking or swallowing. This 62 year old woman was urgently admitted to the internal medicine department due to abdominal distention and severe general weakness. She has suffered characteristic violent pain triggered by chewing and swallowing for little over 4 years. This resulted in poor oral feeding for prolonged period which left her severely debilitated. The large amount of ascites that developed 20 days before admission and extreme emaciation forced her to bed rest. She also suffered from Herpes Zoster. After medical treatment to improve liver function and severe pain was persisted, the patient was referred to our department for control of pain. We performed right mandibular block with 1% dibucaine 0.4 ml and the effect was excellent. After the pain had subsided, patient was able to take meals more comfortably and improved liver function returned.

Pathological and serological detection of bovine viral leukosis in a dairy farm in Jeonbuk province (유우농장에서 발생한 소바이러스성 백혈병의 병리학적 및 혈청학적 조사)

  • Bovine viral leukosis is a viral disease of cattle characterized by the development of tumors in the lymphatic tissue. A female Holstein, 3-year-old, was submitted for diagnosis at the Diagnostic laboratory, Chonbuk National University. Clinical sign of the affected animal showed emaciation, enlargement of superficial lymph node and mild diarrhea. Remarkable lesions were enlargement of many internal lymph nodes. Histopathology revealed excessive neoplastic lymphoid cells characteristic of BVL infection. Subsequently, serums from all cattle were collected and serological examination was done where a 85% seropositive rate was detected using ELISA test. ELISA method showed a comparatively 75% higher detection rate than the agar gel immunodiffusion (AGID) test (85% vs 40%). Serologically positive cattle were variably detected in all ages from under 1 year to over 6 year of age. Hematological examination consistently showed leukocytosis and a differential lymphocytosis of seropositive cattle. Detailed comparative pathological and serological data diagnosed the presence of bovine viral leukosis.

A Case of Diencephalic Syndrome Presented as Failure to Thrive from Three Month of Age (생후 3개월부터 성장장애를 보이기 시작한 간뇌 증후군(Diencephalic Syndrome) 1례)

  • A 14-month-old child visited emergency room with stuporous mental state. He had been suffering from failure to thrive (FTT) and emaciation since three months of age, but he had good appetite and had been euphoric. A large mass was found in the third ventricle by brain CT and MRI. His parents refused operative removal of the mass and he expired 14 days after admission. It is necessary to include diencephalic syndrome in the differential diagnosis of failure to thrive when infants show FTT despite of good appetite and euphoric status.

Squamous Cell Carcinoma in Pere David Deer (사불상에서 자연발생한 편평상피암)

  • Metastatic squamous cell carcinoma was described in two female Pere David's deer reared at Everland zoo. Both deer with chronic emaciation had the mass on right maxilla(9$\times$11$\times$15cm or 20$\times$17cm) which was composed of miliary tan creamy contents and encapsulated by connective tissue. The undulating contents in the mass was a1so extended in the underlying or adjacent soft palate, maxillary and frontal sinus causing severe bone destruction. In one deer, two fistula were also noted in the right periocular area. Histologically, the neoplastic masses of both deer consist of anastomosing cords or nests of squamous epithelial cells with intercellular bridge or keratin pearl formation. The neoplastic cells invade deep into the subcutis and bony tissue. Mitotic figure was rare. Multifocal areas of necrosis and hemorrhages were also noted in the dermis. Metastasis to maxilla and ethmoid bones and/or to submandibular lymph node was noted in both cases. The diagnosis was based on the results of histopathology.
