Clinical Observation of Changes in Subjective Evaluation Index and Electrogastrography Parameters of Patients with Functional Dyspepsia before and after Oriental Medical Interventions (기능성 소화불량 환자의 한방치료 전후의 주관적 평가지표와 Electrogastrography Parameters를 이용한 객관적 지표의 변화 관찰)

  • Objectives : Functional dyspepsia is a type of disease characterized by bothersome postprandial fullness, early satiation, epigastric pain and burning without structural gastrointestinal disease. The aim of this study was to analyze the characteristics of functional dyspepsia patients and evaluate changes in symptoms, quality of life, and electrogastrography parameters before and after oriental medical interventions. Methods : We recruited forty-six functional dyspepsia patients who visited gastroenterology clinic in the oriental medicine hospital of Kyung Hee University between November 2009 and February 2011. Patients were assessed for their frequency of dyspepsia (based on short form-Leeds Dyspepsia Questionnaire: SF-LDQ), quality of life (based on functional dyspepsia-related quality of life questionnaire: FD-QoL), gastric motility (based on electrogastrography: EGG) on the first visit. Then, the effect of oriental medical interventions was evaluated using EGG on the second visit. Results : The majority of patients had symptoms of nausea and indigestion. The largest decrease in EGG parameters was found in the indigestion group. The frequency of regurgitation and postprandial EGG power % bradygastria showed a significant correlation. Also, significant correlations were found between some items of FD-QoL and some EGG parameters. Compared to the EGG parameters before oriental medical interventions, some parameters after treatment had positive results, implying the improvement of gastric motility disorder. We also found improvement of EGG parameters in both digestant medicinal group and digestant combined with qi-tonifying medicinal group. Conclusions : The results of this study suggest that clinical application of EGG can be an objective diagnostic tool in functional dyspepsia patients visiting oriental medical hospital.

Influences of Inter-electrode Distance on Electrogastrography Measurements (위전도 측정을 위한 전극간 부착거리에 관한 연구)

  • Cutaneous electrogastrography is the measurement of electrical activity of the stomach on the abdominal surface. The validity of cutaneous electrogastrography is dependent upon the quality of the recording technique. The locations of electrodes are an important issue. We examined the influences of the inter-electrode distance of bipolar leads on electrogastrography measurements. The sensitivity distributions of EGG leads were calculated based on a 2D body fat model and evaluated according to the region of interest sensitivity ratio (ROISR). We simulated the ROISR of the inter-electrode distance in relation to various body fat thicknesses. The distance between the electrodes was proportional to the distance between the ROI and the surface of the abdomen. The results imply that inter-electrode distance can be applied in electrogastrography according to human body fat thickness.

  • Electrogastrography(EGG), the cutaneous recording of the myoelectrical activity of the stomach using surface electrodes, is attractive due to its non-invasiveness. We propose feasible methods to detect tile gastric contraction by vector analysis of the surface EGG signals obtained from three paired electrodes. The amplitude and direction of the calculated EGG vector were analyzed and compared with the gastric contraction signals such as the strain gauge signal from the gastric serosa in dog. From the vector analysis of the EGG signal, we found a typical change of the amplitude and direction of the EGG vector, which can indicate occurrences of the gastric contraction.

Association between Minimal Change Esophagitis and Gastric Dysmotility: A Single-Center Electrogastrography and Endoscopy Study in Children

  • Purpose: Minimal change esophagitis (MCE) is a reflux disease without mucosal breaks, known to be partially associated with abnormal gastric motor function. Electrogastrography (EGG) is commonly applied to assess gastric motor function in a noninvasive fashion. We aimed to determine the relationship between MCE and gastric myoelectrical activity (GME) recorded on EGG in children. Methods: We retrospectively assessed the records of 157 children without underlying disease who underwent both EGG and upper gastrointestinal endoscopy at Gachon University Gil Medical Center between January 2010 and June 2015. The children were stratified according to the appearance of the esophagus (normal vs. MCE). Between-group differences in EGG parameters and their correlation with each MCE finding were statistically analyzed. Results: Only the power ratio, one of the EGG parameters analyzed, differed significantly between the two groups (MCE, $1.68{\pm}3.37$ vs. normal, $0.76{\pm}1.06$; p<0.05), whereas the other parameters, such as dominant frequency, dominant power, and the ratio of abnormal rhythm, showed no differences. Among children with MCE, significant correlations were noted between erythema and power ratio (p<0.05), friability and postprandial dominant frequency (p<0.05), and edema and/or accentuation of mucosal folds and pre-prandial frequency (p<0.05). Helicobacter pylori infection correlated with postprandial arrhythmia (MCE, $33.59{\pm}15.52$ vs. normal, $28.10{\pm}17.23$; p<0.05). EGG parameters did not differ between children with normal esophagus and those with biopsy-proven chronic esophagitis. Conclusion: In children with MCE, gastric dysmotility may affect the development of MCE, manifesting as EGG abnormalities. H. pylori infection may also affect GME. However, larger prospective investigations are needed to confirm these findings.

  • Objectives : To review to determine difference in the effect of age, gender, location of electrode, test meal and period of recording that influence Electrogastrography (EGG) parameters and to study its oriental medical application. Methods : We investigate the research results through internet search engines, Pub Med, Karger, Mary Ann Libert and Pro Quest. Results : Age and gender influence postprandial Dominant Frequency Instability Coefficient (DFIC). Body mass index (BMI) influence fasting and postprandial EGG parameters. Menstrual cycle phase influence fasting Dominant Frequency (DF). Test meal volume and composition influence postprandial EGG parameters. But reduced-calorie test meal have no effect on EGG parameters. Conclusions : Because the amplitude of surface-recorded SW(slow wave) is very weak, EGG is influenced by many factors. Age, gender, BMI, menstrual cycle phase, test meal volume and test meal composition influence EGG parameters. but reduced-calorie test meal have no effect on EGG parameters.

  • Objectives: To review the history, method, parameters, system of electrogastrography(EGG) and to study its oriental medical application in Traditional Chinese Medicine. Methods: We investigate the research results through internet search engine, CNKI (China National Knowledge Infrastructure). Results: In China, EGG was studied on various regions, including multifarious regions of Traditional Chinese Medical. And also regions of Traditional Chinese Medical could divide classification of diseases, herbal medicine, acupuncture and the like. Conclusions: EGG is a noninvasive, easy and painless method that is kept recordings of the electrical activity of the stomach, therefore it has used for studies and clinics widely. EGG in Traditional Chinese Medicine has used an objective parameter that verified efficacies of oriental medicine. I would therefore like to conclude that there should be continued study and active application of EGG in the wide variety of oriental medicine.

  • Electrogastrography(EGG) is the technique by which gastric myoelectrical activity is recorded noninvasively, from electrodes on the abdominal skin. This gastric myoelectrical activity consists of two type signals : 1) slow wave, which is gastric basal rhyemic activity and is not closely related to gastric contraction 2) spike wave, which is generated only during contraction of the stomach. Despite many attempts made over the decades, analysis of surface EGG has not led to identification of the spike wave (gastric contraction) that would help the clinician to diagnose functional or organic diseases of the stomach. In this paper, we propose a feasible methods to detect gastric contraction by frequency-domain signal analysis of surface ECG signal. Synchronous measurement of gastric pressure by the antropyloric manometry with surface EGG established feasibility of this method.

Postprandial Epigastric Fullness and Pyloric Valve Disturbance in Patients with Functional Dyspepsia;Analysis of Bowel Sounds and Electrogastrography (기능성(機能性) 소화불량증(消化不良症) 환자(患者)의 식후(食後) 심하비만과 pyloric valve의 기능장애(機能障碍);장음과 위전도를 중심으로)

  • Background & Object : Postprandial epigastric fullness is one of common symptoms in functional dyspepsia, but its pathophysiological mechanism has still been unknown. In this study, its association between postprandial epigastric fullness and pyloric valve disturbance was studied through analysis of bowel sounds and electrogastrography. Methods : Bowel sounds and electrogastrography were recorded together for fasting 15 min and postprandial 40 min. Parameters of bowel soundincluding motility index, sound to sound interval, standard deviation of sound to sound interval, sounds per minute, % of bowel sound, sound length, intensity, dominant frequency of sounds, and postprandial / fasting ratio of dominant frequency of sounds were analyzed with a specialized program. By electrogastrography, regularity of slow waves and power ratio were obtained. Results : Significances of bowel sounds appeared in motility index (p=0.046), dominant frequency of fasting (p=0.048), postprandial (p=0.003), and the ratio of postprandial/fasting (p=0.000); those of EGG parameters were shown in postprandial regularity of slow waves (p=0.006) and power ratio (p=0.011). Conclusion : Pyloric valve disturbance was a cause of postprandial epigastric fullness in patients with functional dyspepsia. Analysis of bowel sound might be useful in diagnosing its existence.

Development of Detection and Analysis System for Electrogastrographic Signal (위전도신호의 측정 및 분석시스템 개발)

  • Electrogastrography(EGG), the cutaneous recording of the myoelectrical activity of the stomach using surface electrodes, is a non-invasive technique to detect gastric motility disorder, We developed a detection and analysis system for the EGG signal, which consists of hardware(bio-amplifier, filter) and softwere(user interface, analysis algorithm, patient database). The EGG signal was amplified and filtered by 3 channel bio-amplifiers, and simultaneously digitized and stored on IBM PC with a sampling frequency of 16 Hz. The stored EGG signal was analyzed using developed algorithm to extract clinically useful information from the signal. The developed system has tested through animal experiments, and is under clinical evaluation.

Correlation Study between Electrogastrography and Heart Rate Variability in Dyspeptic Patients (소화불량 환자에서 위전도 검사와 HRV 검사와의 연관성 연구)

  • The aim of this study was to find correlation between EGG and HRV parameters in dyspeptic patients. 39 patients (male 8, female 31) were enrolled. EGG and HRV was measured in all patients. EGG parameters like % of gastric slow wave and power ratio were used to classify patients as normal or abnormal EGG groups. HRV parameters which were included TP (total power), LF(low frequency density), HF (high frequency density), and LF/HF ratio were compared between them. EGG parameters were compared among normal, sympathetic hyperactivity, and parasympathetic hyperactivity groups classified by LF/HF ratio. There was no difference in HRV parameters between normal and abnormal EGG groups. Comparing EGG parameters among normal, sympathetic hyperactivity, and parasympathetic hyperactivity groups, both sympathetic and parasympathetic hyperactivity groups had lower value of postprandial regularity of gastric slow wave than normal group. Abnormal value of LF/HF ratio(both sympathetic and parasympathetic hyperactivity) may be related with disturbance of electrical gastric activity in patients with dyspepsia.
