Case Report of Takayasu's Arteritis (다카야수 동맥염 증례 1례 보고)

  • There were a few case reports on the treatment of Takayasu's arteritis. We had a good effect on one patient with Takayasu's arteritis by oriental medicine therapy. We use acupuncture, herbal acupuncture, electro-acupuncture, moxa, negative thrapy, kinesio taping therapy, physical therapy and exercise for 82 days. In this case, Left hemiparesis, dysarthria, pulseless, disturbances of urination, headache, dizziness improved after treatment. We experienced improvement in this symptoms of Takayasu's arteritis by oriental medicine treatment.

  • In the Clinical study on 1 Case of Patient with Obstruction of the retinal artery, the results were as follows. 1. Obstruction of the retinal artery is acute disease that vision isdiminished fastly and become a blindness. 2. It belong to the cathegory of Pock Maeng(暴盲) in Oriental Medicine. 3. The causes of Obstruction of the retinal artery were the yin-deficiency of liver and kidney, the stagnation of vital energy and blood stasis, the abundance of phlegm-heat, the yang-deficiency of spleen and kidney, the yin-deficiency of spleen and kidney, the nonfastness of primordial energy etc. 4. We could get the effective result by providing the patient with Jinkansikpung-tang-gami(鎭肝熄風湯加味) in the acute period 5. In Acupuncture treament and other treament, Electro-acupuncture theraphy ana Lodestone theraphy were useo for neurologic stimulation, Body acupuncture and Auricula-acupuncture were used for promoting the circulation of vital energy and blood.

Clinical diagnostic study of Physiological Signal data measured on 58 cases of numbness with EAV(Electro-puncture According to Voll) (비증환자(痺證患者) 58례(例)에 대(對)한 EAV측정치(測定値)의 진단적(診斷的) 고찰(考察))

  • Background and Objective : Most diagnostic method for numbness were invasive and complex. So we need to simplify and objectify diagnostic method for numbness. Some study with EAV which is one of Physiological Signal Measuring Instruments, report significantly result as objective diagnostic method for other clinical symptom. By using EAV, we have obtained some physiological signal data from meridian-acupoints of 58 numbness cases. Objective and Methods : This study researched into the clinical statistics for 58 case who ware in numbness, and they ware treated with oriental medical care at the Dong-shin university oriental hospital during 1 year from April 3 2000 to March 30 2001. The data were analyzed and interpreted to compare with traditional differentiation of symptom-complexes, then further evaluated as the Five Evolutive Phases to make them differentiated. The EAV valus of Five Evolutive Phases were identified with the sequence of wood(木), fire(火), earth(土), steel(金), water(水). Results and Conclusion : These values of physiological signal were identical with standard differentiation of symptom-complexes of numbness which is the main cause of dishannonious flow of Qi and blood of the in the liver and deficiency of Qi and blood of the bladder with stagnancy of dampness. Among Five Evolutive Phases, Earth and wood values were increased, steel, fire and water were decreased significantly. This data imply the possibility of somewhat generalization from measuring instruments.

  • Twenty five cases of the patient admission-treatment from chiefly complained of neck pain observed clinically from first January 1997 to thirty first December 1997 in the Dept. of Acupuncture and Moxibustion, Oriental Hospital, Taejon University, Taejon, Korea. And the results were obtained as follows; 1. The distribution of sex was male 13(52%) cases, female 12(48%) cases and the male to female ratio was 1.1 : 1. The distribution of age was the most predominant as the fifties 6(24%) cases. 2. The contributing factors were the most predominant as the reason unknown 9(36%) cases, unstable position 5(20%) cases. 3. The distribution of duration was the most predominant of 9(36%) cases in a week. 4. The distribution of before admission-treatments were the most predominant as west-medication 13(32.5%) cases. 5. The distribution of patient's condition of first treatment was the most predominant as Gr.III 13(52%) cases. 6. The distribution of duration for admission was the most predominant as 13(52%) cases in a week. 7. The distribution of radiological studies were the nust predominant as the HIVD 26(63.4%) cases. 8. The distribution of clinical symptoms were the most predominant as neck pain 25(21.0%) cases, radiating pain to the upper extremities 23(19.3%) cases. 9. The distribution of located on clinical syrnptoms were the most predominant as neck-shoulder and back-upper extremities 11(44%) cases. 10. The distribution of clinical diagnosis was the most predominant as HIVD 16(45.7%) cases. 11. The distribution of methods of treatment were the most predominant as Acupuncture -­ Herb Medication - Electro Acupuncture treatment 8(32%) cases. 12. The effect of treatment by discharge was the most predominant of 11(44%) cases as Good result.

Effects of Acupuncture and Moxibustion at Lower Sea Points on the Intestinal Motility with Loperamide-Induced Constipation in Rats (위, 소장, 대장의 하합혈 침구자극이 Loperamide로 유발된 변비의 장관 운동성에 미치는 영향)

  • Objectives : The aim of this study was to observe effects of manual acupuncture(MA), electro-acupuncture(EA) and moxibustion (MO) on the rat with loperamide-induced constipation. Methods : This study measured small intestinal motility and number of fecal pellets in rats with loperamide induced constipation. MA and EA(intensity, 5 times muscle twitch threshold) was applied for 30 minutes and MO was treated on 5 times moxa cautery to the groups divided with age and sex. Results : The small intestinal motility was decreased by ST36 EA and ST37 EA in 5 weeks male group and ST37 EA in 7 weeks male and female groups, and ST36 MO in 7 weeks female group, but it was increased by ST39 EA in 7 weeks male group and ST37 MO and ST39 MO in 7 weeks male group. The number of fecal pellets was decreased by MA, EA, MO of ST36 in all group except 5, 7 weeks male groups only, and MA, EA of ST37 in 5, 7 weeks male group and ST37 MO in 7 weeks female group, and ST39 EA in 5 weeks male group and 7 weeks female group and ST39 MO in 7 weeks female group, but increased by ST37 EA in 5 weeks female group and ST39 EA in 7 weeks male group. Conclusions : Although these differences do not have a established tendency, it suggests that acupuncture and moxibustion are experimentally dependent upon the sex and age on intestinal motility in rats.

Experimental Study Trends on the Acupuncture Moxibustion Treatment for Visceral Hypersensitivity: Based on the Data of PubMed (내장감각과민의 침구 치료에 대한 실험연구 현황: PubMed를 중심으로)

  • Objectives : The aim of this study is to review the current trends in experimental studies on the acupuncture moxibustion treatment for visceral hypersensitivity. Methods : PubMed was searched for experimental studies about visceral hypersensitivity and acupuncture/moxibustion. Data were extracted and tabulated from the selected articles about experimental method, intervention, result and mechanism. Results : Total 23 articles were reviewed. Chronic visceral hypersensitivity animal model was applied in 17 studies (74%). Visceral hypersensitivity was measured by abdominal withdrawal reflex scoring or/and abdominal electromyogram. Acupoints like ST25, ST36, ST37, BL25, LI11, BL32 and PC6 were treated by electroacupuncture or moxibustion. All articles reported that electroacupuncture or moxibustion treatment is significantly effective in reducing visceral hypersensitivity. Treatment mechanisms were studied, related to mast cell, serotonin (5-HT) and receptor (5-HT3R and 5-HT4R), substance P (SP), vasoactive intestinal polypeptide (VIP), c-fos positive cell, corticotropin-releasing hormone (CRH), purinergic 2X (P2X)2, P2X3, P2X4, P2X7, N-methyl-D-aspartate (NMDA) receptor (NR1 and NR2B), prokinectin (PK) 1 and PK2. Conclusions : Evidences on acupuncture/moxibustion treatment for visceral hypersensitivity in animal studies warrant more research on effective acupoins, electro-acupuncture methods and treatment durations.

Investigation of Clinical Research of Acupuncture Intervention in Assisted Reproductive Technology in Pubmed (보조생식술에 있어서 침의 중재에 대한 임상연구 동향 - Pubmed를 중심으로 -)

  • Purpose: The purpose of this study is to examine acupuncture intervention suggested in the papers of assisted reproductive technology(ART, hereafter) in medicine journals and to establish the further direction. Methods: The papers published between 1996 and 2009 in Pubmed were reviewed. Results: 1. A variety of randomized controlled trials and case series were found in Pubmed concerning influence of acupuncture in ART. It was investigated into improvement of pregnancy rate, analgesic effect during oocyte retrieval, change of serum hormone, and reduction of anxiety. 2. It was on clinical pregnancy rate that main focus of the investigation was placed and the improvement of pregnancy rate differed from studies. 3. For the analgesic effect during oocyte retrieval, the electro acupuncture was mainly used and the studies reported the reduced pain rate, tiredness, and nausea. 4. Acupuncture was reported as significant in improving the state of anxiety during ART. Conclusion: It would be necessary to further study the period of acupuncture intervention, the selection of acupoint through well-designed study, and establish standard of evidence of them.

Literature Review on Parkinson's Disease in Oriental Medicine (파킨슨병의 한의학적 고찰 -병인병리(病因病理)와 침구요법(鍼灸療法)을 중심으로-)

  • Objective: In order to find oriental medical therapies on Parkinson's disease and to make a fundamental basis for clinical application, this study was performed. Methods: We reviewed 35 kinds of the ancient and modern text, and related articles. Results: Parkinson's disease is an extrapyramidal disease characterized by akinesia, tremor at rest, rigidity, and slowness of movement. In old oriental medical text, Parkinson's disease is described as Jinjeon(振顫), Chi, Gyeong (痙), Gyejong, Ryeon(攣) and Pyeongo(偏枯), Tantan, Jungbu(中腑症) of Jungpung(中風). According to the text, major pathological causes were Wind(風), Fire(火), Phlegm(痰), and Blood stasis(瘀血). And Parkinson's disease can be classified into four clinical types as liver & kidney yin-deficiency, qi & blood deficiency, retention of phlegm-heat, and qi-stagnation & blood stasis. Standardized acupuncture points are GV20, GB20, GV14 on head, CV12, ST25 on abdomen, GV26, ST7, GB1, S14, LI20 on face, LI4, LI11, TE5, SI3, HT3, LI15, SI6 on upper extremity, and ST36, GB34, SP6, LR3, KI1, GB30, BL40 on lower extremity. Other methods, such as scalp acupuncture, electro-acupuncture, and herb-acupuncture, can be applied to treat Parkinson's disease. Conclusions: We find out that there are oriental medical concepts related with Parkinson's disease such as Jinjeon(振顫), Chi, Gyeong (痙), Gyejong, Ryeon(攣) and Pyeongo(偏枯), Tantan, Jungbu(中腑症) of Jungpung(中風), of which major causes are Wind(風), Fire(火), Phlegm(痰), and Blood stasis(瘀血). We can also apply many effective acupuncture points and acupuncture therapies according to differential diagnosis, for example, liver & kidney yin-deficiency. qi & blood deficiency, retention of phlegm-heat, and qi-stagnation & blood stasis.

Electric Stimulation for Pain Relief Using Acupuncture Needles (침을 이용한 전기자극 통증치료)

  • For pain relief my collegue and I used thin acupuncture needles as electrodes in electric stimulation. The needles were inserted into a trigger point and into another point located in the same muscle instead of meridian points of electro-acupuncture. Low frequency electric stimulation was given through the needles to 130 patients for 15 min. The results were as follows In 25 acute sprain patients electric stimulation was given $3.14{\pm}1.12$ times and the pain was reduced on the average by $83.00{\pm}6.77%$ (VAS). In 45 chronic sprain patients electric stimulation was given $5.51{\pm}1.38$ times and the pain was reduced on the average by $70.22{\pm}8.98%$ (VAS). In 28 myofascial pain syndrome patients electric stimulation was given $6.22{\pm}1.25$ times and the pain was reduced on the average by $66.48{\pm}8.75$(%). In 7 muscle contraction headache patients electric stimulation was given $4.14{\pm}1.57$ times and the pain was reduced on the average by $75.00{\pm}9.57%$ (VAS). In 25 radiculopathy patients electric stimulation was given $4.73{\pm}1.131$ times and the pain was reduced on the average by $21.37{\pm}9.31%$ (VAS). We he conclude that electric stimulation therapy using acupuncture needles is very effective in acute sprain, chronic sprain, myofascial pain syndrome and muscle contraction headache. Any doctor with knows anatomy and trigger points can practice this method without studying oriental medicine or difficult acupuncture techniques.

Scoping Review of Prospective Clinical Studies, Projects, and Clinical Study Protocol for Korean Medicine Treatment on Dementia (국내 치매에 대한 한의치료 전향적 임상연구, 과제, 임상시험 프로토콜의 현황조사)

  • Objectives: The aim of this study is to review and analyze the trends in prospective clinical research (PCR), project, clinical study protocol of Korean medicine (KM) for dementia. Methods: We searched PCRs of KM on dementia in six electronical databases, up to January 28, 2021. Moreover, the clinical research protocol and projects of KM for dementia were searched. Results: Total eight PCRs, nine projects, and three clinical study protocols were included. In the PCRs, there were one randomized controlled trial, five before and after study, and two comparative group before-after studies. Four of them used herbal medicine, two used acupuncture, one study used both herbal medicine and acupuncture, and the other one used moxibustion. Jowiseungchung-tang was the most frequently used herbal medicine. BL62, KI6 were used in electro-acupuncture, auricular-Shenmen in auricular acupuncture, and GV20 in moxibustion. The most frequent outcome was Korean-Dementia Rating Scale (K-DRS). One reported significant increase in K-DRS score, one reported 60% improvement, and the other studies mostly reported no significant difference. In the projects including 20 clinical studies of dementia, herbal medicine, integrative medicine and acupuncture were mainly used. In the protocols, herbal medicine, complex KM intervention, and integrative medicine were used. Conclusions: Currently, the number of PCRs of KM for dementia is very scarce. Therefore, the researcher's interest in this field and national research support should be made more, and the quality of clinical research in the future can be further improved by supplementing the limitations of previously published clinical research.
