Purpose: This study were to examine the relationship among chronic pain, pain beliefs, pain coping, and fatigue and to define the main factors influencing chronic pain in the elderly. Method: Data were collected by self-reported questionnaires from 276 Korean elderly. Data analysis was done with SPSS 10.1 for descriptive statistics, t-test, ANOVA, Pearson Correlation Coefficient, and Stepwise multiple regression. Results: Chronic pain score was 5.45, which was slightly higher than the average. There were significant differences in chronic pain according to age, marital state, economic state, and duration of pain. There was a significant positive correlation between chronic pain and pain beliefs, passive pain coping, physical fatigue and mental fatigue. In addition, there was a significant negative correlation between chronic pain and active pain coping. Stepwise multiple regression analysis showed that 44% of the variance in chronic pain was significantly accounted for by passive pain coping(31%), pain beliefs(8%), physical fatigue(4%), and active pain coping(1%). Conclusion: These results suggested that pain beliefs, pain coping, and fatigue can be influencing factors on chronic pain for the elderly. Also, the findings can provide a basis for nursing intervention development to effectively manage chronic pain for the elderly.
Purpose: The aim of this study was to investigate the effects of chronic musculoskeletal pain and depression on health-related quality of life (HRQoL) according to gender in community-dwelling older adults. Methods: The subjects of this study were 209 elderly individuals who were receiving visiting nursing services from a public health center located in Gangwon-do. Data were collected using a structured questionnaire from March to April, 2008. SPSS/WIN 13.0 was used for data analysis. Results: Depression and HRQoL showed a significant difference between male and female subjects. There was a negative correlation between chronic musculoskeletal pain, depression, and HRQoL. In the male elderly, depression was the most significant predictor of HRQoL, while in the female elderly, chronic musculoskeletal pain was the most significant predictor of HRQoL. Conclusion: The study showed that chronic musculoskeletal pain was the variable with the highest explanatory power for HRQoL in the female elderly. Therefore, chronic musculoskeletal pain needs to be assessed and managed first in nursing interventions to improve HRQoL of the female elderly.
Purpose: The purpose of this study was to investigate the effects of Interferential current stimulation therapy(ICT) on pain, balance, and walking ability of elderly patients with chronic low back pain. Methods: Twenty participants were randomly assigned to ICT group (2 males and 8 females) and placebo ICT group (3 males and 7 females). Participants were ICT and placebo ICT for 20 minutes, and the pre- and post-VAS, TUG and postural balance were measured. Results: The ICT group showed significantly decreased pain scores in the elderly patients with chronic low back pain (p<.05) than the placebo ICT group. The postural fluctuation with eyes opened and closed was significantly lower than the placebo ICT group(p<.05), and the ICT group showed remarkable improvement (p<.05). Conclusion: ICT was expected to improve decrease pain, improve postureal sway and walking ability remarkably. Therefore, It was expected that the application or ICT would be an effective method for elderly patients with Chronic Low Back Pain.
Objective: This study covered the effect of stabilization exercise on muscle activity, shoulder painanddisability index(SPADI) oftheelderly people with chronic shoulder pain by tele-rehabilitation village hall in rural area. Design: A randomized controlled trial. Methods: The study subjects are 29 elders with chronic shoulder pain aged over 65-year-old who reside in the rural area (experimental group: 14 elders and control group: 15). The elderly people in the experimental group were asked to perform shoulder stabilization exercise 60 minutes per one time for 4 weeks and three times in a week and the others in the control group were asked to receive education related to pain management for the initial one time. Before and after the experiment, we measured the subjects' muscle activity of upper trapezius, serratus anterior and the lower trapezius. In addition, SPADI is measured through the questionnaire. Results: Compared to the control group, the experimental group showed significant differences in the muscle activity of the three muscles (upper trapezius, serratus anterior, lower trapezius), and SPADI. Conclusions: It is found that shoulder stabilization exercise is effective in muscleactivity, pain and ability of the elderly people through tele-rehabilitation service. This study should be used for improving the quality of the elderly people's lives through active tele-rehabilitation service for areas where have poor medical benefits.
Purpose: This study was to examine the effects of electrical muscle stimulation therapy on chronic knee pain in the elderly. Method: The research design was a one-group pretest-posttest design. The subjects were 45 (TE: 17, SE: 15, SY: 13) elderly, 65 years old and above with chronic knee pain. Pain was measured by the S-F McGill Pain Questuionnaire (S-F MPQ) and Arthritis Impact Measurement Scale (AIMS). Electrical muscle stimulation therapy experimental treatment was applied for 12 weeks, 3 times/week, 15 min/time. Data was collected from March 2005 to February 2006. Data was analyzed using the SPSS PC+ 12 version. Results: After receiving electrical muscle stimulation therapy, chronic knee pain in TE (S-F MPQ: t=-62.143, p=.000, AIMS: t=-29.155, p=.000), SE (S-F MPQ: t=-76.345, p=.000, AIMS: t=-39.323, p=.000), and SY (S-F MPQ: t=-43.691, p=.000, AIMS: t=-30.306, p=.000) groups were significantly decreased. Conclusion: Electrical muscle stimulation therapy can be a better effective primary nursing intervention for chronic knee pain for community dwelling elderly people with TE, SE, and SY.
Purpose: This study was to examine the effects of electric stimulation therapy on chronic knee pain, exercise self-efficacy, and quality of life in Korean elderly women. Methods: The design was an unequivalent control pretest-posttest study. Samples were total of 60 (experimental: 30, control: 30) elderly women with healthy cognitive and communication abilities aged 65 years old and above. The experimental treatment involved was the electric stimulation of both thigh quadriceps muscles for 15 minutes per treatment, 3times per a week, for a total of 12 weeks. Measurements taken were S-F MPQ and AIMS for chronic knee pain, exercise self-efficacy measurement for exercise self-efficacy, and S-F 36 scale for quality of life. Data were analyzed using SPSS PC+ 12. Results: Chronic knee pain according to the Short-Form McGill Pain Questionnaire (t=43.563, p=.000) and Arthritis Impact Measurement Scale (t=31.364, p=.000) were significantly decreased in the experimental group by the application of electrical stimulation therapy. Exercise self-efficacy (t=107.116, p=.000) and quality of life (t=76.429, p=.000) were significantly increased in the experimental group by the application of electrical stimulation therapy. Conclusion: Electrical stimulation therapy could be a more effective primary nursing intervention in decreasing chronic knee pain, and on increasing exercise self-efficacy and quality of life for Korean elderly women.
Purpose: This study was conducted to evaluate differences in pain, pain interference, and fatigue, according to the level of physical activity in the elderly with chronic pain. Methods: Data were collected between January and March 2011 from 116 elders with chronic pain living in community settings. The data were analyzed using the SPSS/WIN 17.0 program for descriptive statistics, $x^2$ test, t-test, ANOVA and ANCOVA. The reliability of the instruments was tested with Cronbach's ${\alpha}$ coefficient, which ranged from .91 to .93. Results: The results were as follows. The mean total physical activity was 2287.4 MET-min/week, and 41.4% of the investigated subjects were classified into the low physical activity group. After adjusting for age and sex, the moderate and high physical activity groups were significantly lower in pain (F=6.33, p=.002), pain interference (F=11.57, p<.001), and fatigue (F=3.16, p=.046) than the low physical activity group. Conclusion: Results from this study suggest that the level of physical activity can influence pain, pain interference and fatigue. Therefore, incorporating more physical activities into daily routines, inactive elderly individuals may improve their pain, pain interference and fatigue.
PURPOSE: Chronic low back pain is one of the main factors that affect the quality of life and cause problems of gait and balance in elderly people. This study investigated the correlation between the Oswestry disability index (ODI), Berg balance scale (BBS), and kinematic data measured while analyzing the gait of elderly people with chronic back pain. METHODS: A total of 29 subjects participated in this study. The ODI, BBS, and kinematic data of lower extremities were measured while walking. All data were analyzed using Pearson's correlation coefficients and the significance was measured at .05. RESULTS: ODI had a significant correlation with 1, 13, and 14 items of BBS (p < .05), and left hip external rotation and right ankle abduction respectively in the stance and swing phase of gait (p < .01). 13 items of BBS had a significant correlation with the right ankle abduction in the stance phase of gait (p < .01). In addition, 14 items of BBS had a significant correlation with right and left ankle abduction in the stance and swing phase of gait (p < .01). CONCLUSION: While attempting to predict chronic low back pain and balance issues, it may be useful to check the right ankle abduction in the stance and swing phase of gait. In the future, it would be helpful if some simple tests could be designed to assess balance in elderly people with chronic low back pain.
Purpose: This study was to examine effects of muscle electric stimulation on chronic knee pain, activities of daily living, and living satisfaction for Korean elderly women. Methods: The design was a nonequivalent control group pretest-posttest study. Subjects were 60 (experimental: 30, control: 30) elderly women 65 years old or above with good orientation and communication. The experimental treatment was electric stimulation on both thigh quadriceps muscles for 15 minutes per time, 3 times per week, for a total of 12 weeks. Measures were the S-F McGill Pain Questionnaire and Arthritis Impact Measurement Scale for chronic knee pain, activities measurement of daily living for activities of daily living, and living satisfaction measurement for living satisfaction. Data was analyzed through the SPSS Win 12.0. Results: Chronic knee pain by S-F MPQ (t=43.563, p=.000) and chronic knee pain by AIMS (t=31.364, p=.000) were significantly decreased in the experimental group, and the activities of daily living (t=124.353, p=.000) and living satisfaction (t=71.268, p=.000) were significantly increased in the experimental group for Korean elderly women. Conclusion: Muscle electrical stimulation decreased chronic knee pain, and increased the activities of daily living and living satisfaction for Korean elderly women. Further studies for muscle electric stimulation need to be done.
Purpose: This study investigates the status of medication use of the elderly with chronic disease taking non-opioid analgesics and attempts to identify factors influencing medication adherence. Methods: Data were collected from September 1 to October 19, 2016. A structured questionnaire was used for face-to-face interview with a convenience sample of 161, elderly people with chronic disease taking non-opioid analgesics. The survey included questions about status of medication use, medication adherence, symptom experience, depression and family function. Data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson's correlation coefficients, and stepwise multiple regression with IBM SPSS 23.0 program. Results: The mean score of medication adherence of the elderly with chronic disease was $4.48{\pm}2.35$. Experiences of side effects (${\beta}=.31$, p< .001), use of over-the-counter pain medication (${\beta}=.19$, p= .009), and family function (${\beta}=.16$, p= .031) were identified as significant predictors. The final model explained 18.0% of the variation of medication adherence of the elderly with chronic disease taking non-opioid analgesics (F= 12.30, p< .001). Conclusion: Therefore, as a strategy to improve medication adherence of the elderly with chronic disease, therapeutic intervention should be developed to improve family function and to manage with personalized plans considering experiences of side effects and use of over-the-counter pain medication.
