Predictors of Successful Aging in Community-dwelling Elders and Mediation Effect of Transcendence (지역사회 노인의 성공적 노화 예측요인과 초월감의 매개효과)

  • The purpose of this study was to identify predictive factors of successful aging and to determine mediating effect of transcendence between successful aging and influencing factors of community-dwelling elders in Korea. 162 elders were participated in this study who visited seniors welfare center and senior citizen center. Data were analyzed by PASW 18.0 for t-test, Pearson's correlation coefficients and hierarchical regression analysis. Transcendence, parent-child relationship, depression, spirituality and perceived economic status were statistically significant influencing factors, explained 77% of successful aging in community-dwelling elderly. The mediating role of transcendence was identified about parent-child relationship, depression, spirituality and perceived economic condition. The study findings demonstrated that transcendence was significant variable affecting successful aging. The variables affecting successful aging such as transcendence, parent-child relationship, depression, spirituality, perceived economic status can be utilized for nursing strategies to successful aging in community-dwelling elders.

A Policy Implementation Analysis on the Care Voucher for the Aged -Focusing on Choice and Competition- (노인돌보미바우처 정책집행분석 -선택과 경쟁은 실현되는가?-)

  • As a case study in the field of policy implementation research, this study focuses on how the logic of consumer choice and provider competition operates on the front line of policy processing. To find the implementation process of the program, 39 interview data were analyzed, including voucher users, care workers, social workers in 4 agencies and local public officers in one of the district in Seoul, and relevant officials from the Ministry for Health, Welfare and Family affairs and the Center for Social Service Management. The main results are as follows: In the level of policy implementation, user choice and competition, which was the main logic behind the implementation of the voucher program, did not occur as expected by policy makers. Instead of user choosing his/her provider, it was found that the providers were choosing its users. Secondly, the case study found that providers have formed a caucus which allocated the local users equally amongst the providers. In this process, local public officers have supported the meeting by providing them with a list of users. Such results may be interpreted as a habitual execution from the tradition of supply-side subsidy, rather than the way of implementation in the market system. Thirdly, although voucher users could not choose their preferred agency in the first stage of service, some other choices exists so that users may choose their preferred care-giver and time for service. Finally, the change of agency and care-giver in the way of delivering services were observed.

Elderly Suicide and It's Related Factors : Focused on the Role of Social Support and Mastery in the Effects of Hopelessness and Depression on Suicidal Ideation (노인 자살관련 요인: 무망감과 우울증이 자살생각에 미치는 영향에 대한 사회적 지지와 자아통제감의 효과를 중심으로)

  • This study examined the role of social support and mastery in the effects of hopelessness and depression on suicidal ideation in order to identify elders' suicide and it's related factors. Especially, it focused to find out whether the hopelessness excluding depression has an independent effect on elders' suicidal ideation, and psychosocial factors such as social support and mastery has an interaction effects with elders' hopelessness variables and depression on suicidal ideation. This study recruited 297 elders over 65(older persons in the elder education programs of senior welfare center in Masan, Changwon and Jinhae) and employed a self-administered survey method during January, 2007. The following are the major results of the study. First, the level of hopelessness, depression and suicidal ideation related elders' suicide indicated differences according to their educational level, marital state, income, and their chronical disease numbers. Especially, the lower income and higher chronical disease numbers showed higher level of hopelessness, depression and suicidal ideation among the elder subjects. Second, interaction effects of social support and depression were significant in suicidal ideation, the more level of social support in elder' subjects, the lower the suicidal ideation. While, interaction effects of social support and hopelessness, and mastery and depression/hopelessness in suicidal ideation were not statistically significant. Third, psychosocial factors such as social support and mastery on elders' suicidal ideation have a significant effect when other relevant variables are controlled. From these results, hopelessness is other significant factor predicting suicidal ideation(suicide) among elders. Also, social support and mastery is very significant factor influencing to prevent or decrease suicidal ideation among elders.

A Study on the factors of longevity, ADL and IADL of the eldery people over 100 years in Gyeongnam province (경남지역 백세인의 장수요인과 일상생활수행능력(ADL) 및 도구적 생활수행능력(IADL)에 대한 연구)

  • This study conducted surveys of 88 eldery people over 100 years in Gyeongnam on April to June, 2011 during 90 days. It was intended to find the factors of longevity, Activity Daily of Living (ADL), and Instrumental Activity Daily of Living (IADL). Major results of this study were as follows: The factors of longevity are getting enough sleep over than seven hours(95.5%), taking in less salty food (69.8%), optimism without even thinking about suicide (86.9%), nonsmoking habit (92%), and moderation in drink (86.4%). The most difficult part of Activity Daily of Living (ADL) is taking a bath, and that of Instrumental Activity Daily of Living (IADL) is taking a bus or subway alone. All items of ADL and IADL, female was higher than the male, but not statistically significant(p<0.05). Therefore, further study is expected to develop the supplementary policy to support the elderly people with declining to carry out physical activities. It is also expected to develop the public welfare programs and policies for the aged, allowing them to take responsibility and to participate as a member of the society.

The Changes of Types of Nutritional Risk and Nutritional Intake according to Depression and Self-esteem among the Elderly in Chunnam Province (전남지역 노인에서 우울 및 자아존중감의 인식정도에 따른 영양위험도 및 영양소 섭취량의 변화 양상)

  • This study was conducted in order to identify the relationship between psychological factors, such as depression and self-esteem, and nutritional status, such as nutritional risk index and nutrient intake, among the elderly in Chunnam Province. The participants were 119 elderly individuals over the age of 65 years who visited the Senior Welfare Center in Chunnam province between January 29 and February 2, 2007. This study was conducted using a structured questionnaire that included, the Center for Epidemiologic Studies Depression Scale (CES-D), General Self Efficacy Scale (GSES), Nutritional Screening Initiative Checklist (NSI checklist), questions pertaining to the general characteristics of the participants and an estimation of nutrient intake using the 24-hour recall method. Data were analyzed by the SPSS program. Analysis of the participant's CES-D scores revealed that 43.7% of the subjects were normal and 56.3% had more than mild depression. The mean GSES score was 45.9 for the entire group of subjects (51.9 for men, 43.6 for women). The mean nutritional risk index value was 4.30 (5.03 for men, 4.01 for women). Analysis of the participant's scores on the NSI checklist revealed that 69.7% of the subjects were normal and 30.3% exhibited a moderate nutritional risk. The CES-D was positively correlated with the NSI checklist (p < 0.05) but negatively correlated with nutrient intake. However, the GSES was negatively correlated with the NSI checklist (p < 0.05), but positively correlated with nutrient intake (P < 0.01 for protein, calcium, phosphorus, zinc etc.). The results of this study indicate that it is necessary to manage psychological factors, including depression and self-esteem, in the elderly in order to decrease their nutritional risk and increase their nutrient intake.

A Qualitative Study on Coping strategies of Older adults with Depression: Focused on the Experience of Coping with Depression in Older Adults Living in Seoul (노인의 우울 대처 전략에 관한 질적 연구: 서울시 거주 도시 노인의 우울 대처 경험을 중심으로)

  • The purpose of this study is to explore qualitatively what kind of coping strategy the Korean older adults use when they are depressed and why. Participants were users, older than 60, of an elderly welfare center in Seoul, and answers of 34 respondents who experienced depression were included in the analysis. The collected data were analyzed through content analysis. The results of the analysis showed that strategies to cope with depression used by participants were composed of 6 domains and 11 sub-domains: health behavior (medical approach / exercise and diet), family and social contact (social interaction / going out and going on a trip / communication with family), Religious activities (Religious activities), lifelong education (hobbies / educational activities), productive activities (labor), health risk behaviors (drinking and gambling / resignation). The depression coping strategies of the older adults and their characteristics are as follows. First, older adults used diverse problem-focused coping strategies to cope with depression. Second, older adults considered various coping strategies together and used them simultaneously. Third, the facility space for older adults functions as a shelter. Fourth, although there were cases where medical approach was used, the intention to reuse was very low. Through this study, the following suggestions were made to help older adults cope with depression more successfully. First, access to various coping strategies should be promoted. Second, more places where older adults can spend their time comfortably need to be provided. Third, resistance to mental health care should be resolved.

The Structural Relationship among Job Stress, Human Rights Behavior, Social Support and Job Satisfaction (노인장기요양시설 종사자들의 직무스트레스와 인권옹호행동, 직무만족의 구조적 관계: 사회적 지지의 매개효과를 중심으로)

  • Recently, as the number of patients with dementia using nursing homes increases, the interest about the quality of care services is increasing. Job satisfaction of caregivers is very important factor in the quality of care service. In addition, as perception of human rights is heightened, human rights behavior in nursing homes is also recognized as an important factor. The purpose of this study is to investigate the structural relationship between job satisfaction and human rights behavior, job stress, and social support of dementia workers who have significant influence on quality of care service. For this purpose, 210 questionnaires were collected from 300 dementia workers working in the elderly nursing home, day night care center, and domiciliary visit care center in Gyeonggi province. After excluding 13 questionnaires, the questionnaires were analysed by AMOS and SPSS 21.0. The results of the analysis are following. It was found that job stress significantly affects the social support directly (-.276). Also, social support significantly affects job satisfaction (.315). On the other hand, human rights advocacy has a direct effect on job satisfaction (.175) and an indirect effect (.102), showed a total effect of (.277). This shows that social support partially mediates human rights advocacy and job satisfaction. Job stress had a direct effect on job satisfaction(-.217) and an indirect effect(-.095), indicating a total effect of (-.312), and social support partially mediated job stress and job satisfaction Could know. The results of this study suggest that in order to improve the job satisfaction of the dementia workers, it is necessary to increase the human rights advocacy behavior and reduce the job stress in an environment that recognizes the social support.

Development of Dementia Care Model in a Community (지역사회 치매관리 모형 개발 : 광명시의 경우)

  • There has been a dramatic increase in public awareness regarding dementia during recent years. However, dementia remains a family affair and patients do not receive adequate care in Korea. This study aims to assist patients and their caregivers by establishing Home and Community based Long-Term Care in a city. The data collected for analysis include five main categories: dementia prevalence, limitations of daily activities of patients, burden of caregivers, the services that patient's family want to utilize, the resources that handle dementia in the community. Major findings can be summarized as follows: 1)The prevalence rate of dementia for elderly people is 13.1 per 100 persons. Alzheimer's disease amount to 38.9% of dementia patients and vascular dementia account for 36.7% of them 2)Eight out of ten patients have mild dementia. Almost all patients have normal ADL. IADL, however, shows different picture. In every items of IADL, about 60% of patients reveals some limitations. 3)The proportion of patients who had medical diagnosis is as low as 20%. Families of patients think dementia as normal aging process and medical doctors in the community do not give special concern to dementia patients. 4)Caregivers does not have proper social support. They suffer from long care time, experience large obstacles in respect of health, daily living, and social activity. 5)Health center and Community welfare center have launched some programs-consultation, home-visiting nursing, day care center, voluntary force mobilization and so on-for dementia patients. But they do not perform expected roles and functions because of lack of skilled personnels and inadequate coordination of relevant organizations for dementia care. 6)Families of dementia patients prefer home helper and home-visiting nurse to hospitalization. For the future, however, demand for institution-based long-term services will increase. We develope community dementia care model based on above findings as follows: 1)Health center execute community cardiovascular control program for the prevention of vascular dementia. 2)Refer to epidemiologic characteristics of patients and preference of family, the most urgent task for dementia care in this city is to expand and organize Home and Community based Long-Term Care. 3)For the continuous and comprehensive care, care plan for a patient must be prepared. Case management team should be builded to prepare this plan and coordinate relevant resources. 4)Special long-term care unit for dementia will be needed in a near future. This unit should have multiple functions, such as day-care center, short stay facility, training center for relevant personnels, besides long-term nursing home considering effective care of dementia and efficient operation of the facility. 5)Voluntary workers deserve their due efforts. Incentive mechanisms must be developed to activate voluntary activities.

Comparative analysis of food intake according to the family type of elderly women in Seoul area (서울 일부지역 여자 노인들의 가구유형에 따른 영양소 섭취실태 및 식사의 질 평가)

  • Purpose: As the rate of senior citizens living alone increases in the current aging society, there is much concern regarding the health and nutritional intake of solitary senior citizens. Therefore, this study compared the nutritional intake of senior citizens according to their family type. Methods: In July and August of 2011, two senior citizen welfare centers in Seoul were visited to survey 267 elderly women. Excluding 54 subjects for which the data were incomplete, information from 213 subjects was analyzed. The subjects were divided into three family types, living alone (LA, n = 74), living with spouse (LS, n = 78), and living with children (LC, n = 61). Results: The mean age of the LA group was the highest, while the mean age of the LS group was the lowest (p < 0.001), and WHR of the LC group was the highest (p = 0.049). Income was the highest in the LS group (p < 0.001). Frequency of eating out was the lowest in the LA group (p = 0.031). By Duncan's multiple analysis, the amounts of energy intake, vegetable protein, fat, calcium, phosphorus, potassium, selenium, Vit D, Vit E, $Vit\;B_2$, niacin, $Vit\;B_6$, $Vit\;B_{12}$, and cholesterol were significantly higher in the LS group compared with the LA or LC group (p < 0.05). The intakes of calcium, Vit D, $Vit\;B_{12}$, and cholesterol were still significantly different among the three groups, even after adjustment for age and monthly income. The LA group ate less fruit and fish than the LS or LC group (p < 0.05). The LA group showed the lowest dietary diversity and the LS group showed the highest diversity (p = 0.014), however, the significance of dietary diversity score among the three groups disappeared after adjustment for age and monthly income. Conclusion: Elderly women living with spouse were receiving better nutrition than elderly women living alone or living with children. Therefore, solitary elderly women who do not live with their spouse or children should be offered greater opportunities to receive a balanced meal at a congregational kitchen or welfare center. To ensure their healthy diet, it is essential to provide continuous nutrition education with these groups in mind.
