Correlates of life satisfaction in the Elderly : Systematic Review and Meta-analysis (노인의 생활 만족도 영향 요인: 체계적 문헌고찰 및 메타분석)

  • The purpose of this study is to provide convenience to the senior center staff and policy makers through a research of "Life satisfaction" in the senior centers with a systematic review and meta-analysis of the considerations of the elderly, published in foreign journals for the past 10 years. We searched published studies in the Medline Complete, Pub-med central and Scopus databases from January 2007 to November 2017. The main key words used "Elderly AND(And OR aged) AND Personal Satisfaction/ or life satisfaction" and out of 595 searched articles a total of 6 studies were selected. After qualitative analysis of the contents of the study, meta - analyzes were conducted for factors with more than three quantitative values. The factors affecting life satisfaction of the elderly were physical health (pooled effect size 0.582, p = .000), mental health (pooled effect size 0.423, p = .000), economic problems (r = .51, p =. 001), social activities, and daily living. It is expected that this study which provides an understanding of life satisfaction of the elderly would contribute to provide general information for developing social consensus on the employment in the super aging society should help improve the quality of welfare facilities for senior citizens in the future.

  • The subjects of this study were 514 people aged 65 and older who voluntarily participated in the senior welfare center in Seoul and Gyeonggi. The purpose of this study is to analyze the relationship between muscular strength, cognitive function, activities of daily living, depression variables of elderly people in a comprehensive evaluation. As for the data collection methods, were measured the upper and lower body strength, grip strength(left, right), cognitive function(mini-mental state examination, trail marking test, digit span test, stroop test), activities of daily living and depression was measured using a questionnaire. The results are as follows First, there was a statistically significant correlation between the elderly's muscular strength, cognitive function, daily living ability, and depression. Second, the statistical significance was found among all the variables as a result of the analysis of the elderly's integrated muscular strength, cognitive function, activities of daily living and depression. This study can provide basic data for successful aging and independent retirement life of the elderly by confirming the relationship between the elderly's muscular strength, cognitive function, Activities of daily living, and depression.

Physical Activity- and Alcohol-dependent Association Between Air Pollution Exposure and Elevated Liver Enzyme Levels: An Elderly Panel Study

  • Objectives: The deleterious effects of air pollution on various health outcomes have been demonstrated. However, few studies have examined the effects of air pollution on liver enzyme levels. Methods: Blood samples were drawn up to three times between 2008 and 2010 from 545 elderly individuals who regularly visited a community welfare center in Seoul, Korea. Data regarding ambient air pollutants (particulate matter ${\leq}2.5{\mu}m$ [$PM_{2.5}$], nitrogen dioxide [$NO_2$], ozone [$O_3$], carbon monoxide, and sulfur dioxide) from monitoring stations were used to estimate air pollution exposure. The effects of the air pollutants on the concentrations of three liver enzymes (aspartate aminotransferase [AST], alanine aminotransferase [ALT], and ${\gamma}$-glutamyltranspeptidase [${\gamma}$-GTP)]) were evaluated using generalized additive and linear mixed models. Results: Interquartile range increases in the concentrations of the pollutants showed significant associations of $PM_{2.5}$ with AST (3.0% increase, p=0.0052), ALT (3.2% increase, p=0.0313), and ${\gamma}$-GTP (5.0% increase, p=0.0051) levels; $NO_2$ with AST (3.5% increase, p=0.0060) and ALT (3.8% increase, p=0.0179) levels; and $O_3$ with ${\gamma}$-GTP (5.3% increase, p=0.0324) levels. Significant modification of these effects by exercise and alcohol consumption was found (p for interaction <0.05). The effects of air pollutants were greater in non-exercisers and heavy drinkers. Conclusions: Short-term exposure to air pollutants such as $PM_{2.5}$, $NO_2$, and $O_3$ is associated with increased liver enzyme levels in the elderly. These adverse effects can be reduced by exercising regularly and abstinence from alcohol.

The effects on rehabilitative program on isokinetic muscle power of patients with osteoarthritis of Knee joint (재활프로그램이 슬관절 골관절염 환자의 등속성 근력에 미치는 영향)

  • The purpose of this study was to investigerate the changes of isokinetic muscular function in elderly people who have been to take from osteoarthritis in both knee joints after 36 weeks' rehabilitative therapy programs. In this study the subjects were 20 women residing in S-Tower(n=10) and H-welfare Town(n=10) respectively. The rehabilitative group(equal to experimental group) had taken part in exercise program 5 days per week. And then was performed by warm up, workout(involving aerobic exercise and weight training), cool down, physical therapy(cryotherapy, TENS, ultrasound). Also its programs were classified in conditioning phase($0{\sim}12$ weeks), improvement phase($13{\sim}24$ weeks), and maintenance phase($25{\sim}36$ weeks) respectively. The results of inspections were as followed: In the isokinetic muscular function, there were significant differences in right leg's flexor and extensor in $60^{\circ}$/sec. And there were significant differences in right leg's flexor and left leg's extensor in $180^{\circ}$/sec. At last, there were significant differences in right leg's flexor and left leg's extensor in $240^{\circ}$/sec. In other words, the rehabilitative programs for 36 weeks could increase the muscular function in elderly with OA. In conclusion, the rehabilitative programs of this paper has shown the positive results, which involved in the muscular function variables in elderly people with OA in both knee.

  • A research project, the "Construction of Frailty Cohort for Elderly and Intervention Study," funded by the Ministry of Health and Welfare, has been ongoing since December 2015. The Korean Frailty and Aging Cohort Study (KFACS) aims to identify risk factors for adverse outcomes associated with frailty in community-dwelling older adults, as well as means of prevention. KFACS is a multicenter, longitudinal study, with the baseline survey being conducted in 2016-2017. The sample (n=3,000) consists of those aged 70-84 years, stratified by age and gender, recruited from urban and rural regions nationwide. An in-person interview and health examination are performed every 2 years. This project is also conducting many intervention studies. Project interventions focus on nutrition and exercise. The nutritional intervention study compares the effects of 1.2 g/kg versus 1.5 g/kg daily protein intake on sarcopenia and frailty in older Koreans. The exercise trial examines the effectiveness of an information and communication technology-based program in preventing or reducing frailty.

Current Status and Policy Issues of Senior Clubs: With Focus on Senior Employment Programs (시니어클럽의 현황 및 정책과제: 노인일자리사업을 중심으로)

  • The study aimed to investigate the current status and policy issues of senior clubs, focusing on the senior employment programs in South Korea. The current status of senior clubs was reviewed based on their legal basis, organization, project type, finance, project performance, and operational difficulties(low revenue yields, high labor intensity and low wages, lack of funding, weakness in provision of a legal basis, etc.). The policy directions of such clubs were also defined as follows: (1) establishment of the role of senior clubs(a local senior center of job creation projects, an execution entity of business senior employment programs matching the regional characteristics, and a center for community change through the promotion of awareness of the problems of and related to the elderly and through empowerment of the elderly); (2) improvement of the quality of jobs(need-based and sustainable job creation, quality improvement in education, improvement of the elderly practitioners' working conditions, and collaboration and partnerships among the public-private sectors); and (3) legislation and amendment of senior employment programs.

A Study of Cognitive Function and Driving Status of Elderly Owner Driver (노인운전자들의 인지기능과 운전실태에 대한 연구)

  • The purpose of this study is to provide some basic information for elderly driving safety, by analyzing the current state. This study analyzed 128 survey questionnaires given to senior welfare center user in P city and evaluated Montreal Cognitive Assessment Korean Version(MoCA-K) and Neurobehavioral Cognitive Status Examination(NCSE) for checked 41 owner drivers by questionnaires. 32% of total elderly persons was owner driver, and 56.1% of elderly drivers was driving everyday. 56.1% of elderly persons was driving for 1-2hours per day. 46.3% of elderly persons was using a car for leisure activity and 31.7% of elderly drivers was experienced a car accident. As cognitive function evaluation, some elderly drivers showed scores lower than average in subcategories of cognitive functions for driving a car. So, methodical evaluation system for accurate evaluation of deficit ability and driving re-education system will be required for safety driving of elderly person. In future, a legal approach of government as driver licence management and renewal system will be required for safety driving of elderly person.

Food Safety and Nutrition Education Program for Elderly and Assessment of Program Effectiveness Based on Health Belief Model (건강신념모델에 근거한 노인 대상 식품안전·영양교육 프로그램 효과 평가)

  • Most elderly have difficulties in managing food safety and nutrition by themselves. Various nutrition educations for the elderly have been developed, but food safety and nutrition education program and educational tools for the elderly are very limited. The aim of the study was to evaluate a food safety and nutrition education program based on the Health Belief Model (HBM) for the elderly. Education program was implemented for 220 seniors (137 educated group and 83 control group) aged over 65 years at senior welfare community centers. The intervention study was carried out on a weekly basis during each of 5 weeks, and each session lasted about 35 or 40 minutes. The effectiveness of the program was assessed with a questionnaire before and after education. Education program consisted of a 5 week program, and topics were 'Dietary changes for the elderly's healthy life', 'Prevention of food poisoning in everyday life', 'Safe food handling for my health', 'Healthy dietary life to prevent chronic disease', and 'Safety! nutrition! healthy dietary life'. Education program materials such as slides, handouts, videos, leaflet, and booklets were developed. As a result, there were score improvements in knowledge, dietary behaviors, and health belief after intervention in the intervention group, which were higher than those of the control group. In particular, there was a meaningful interrelation between dietary behavior and health belief (r=0.520, P<0.001). This finding suggests that changing beliefs is very important to make desirable dietary behavioral changes. For this reason, we can conclude HBM theory is an effective model to educate nutrition and food safety for the elderly. Furthermore, food safety and nutrition education programs are implemented and delivered continuously at various settings such as a health center or community welfare center, and those will contribute significantly to enhance perception and change their desirable dietary behaviors for the elderly.

A Delphi Survey for Seeking Progress in Family Policy : Focusing on Integrated Family Policy Perspective (가정정책 방향 모색을 위한 전문가 조사 연구 : 통합적 가정정책 관점을 중심으로)

  • This study attempted to develop integrated family policy. For this purpose, a delphi survey was conducted twice among 15 professionals regarding the general contents of family policy, the goals and outcomes of president Roh's administration, the direction and plan of action for future family policy. The results of this study are as follows: Firstly, the main results of president Roh's administration were the creation of the Ministry of Women and Families, and the Healthy Families Law. Nevertheless, that government had no long-term vision or action programs. Secondly, there are two aspects of family policy which one is family members based and the other is family life cycle based. Accordingly the subjects of this study insist that family policy has to cover people's whole life, all families regardless of family structure, and has to integrate the plans and programs which have been served to individuals, for example, women, child, youth, and elderly. Thirdly, the orientations of future family policy are to be toward happiness, health, equality, coexistence, harmony, growth, generosity, diversity, self-control, and a sustainable society. So the concept of the integrated family policy has to cover all aspects of the family and the diversity of family life. Therefore, the service delivery system for family policy has to converge into the Healthy Family Center.

Development of a Community-based Preventive Health Care Model for the Elderly in Korea through the Evaluation of a Japanese Counterpart (일본의 노인건강관리체계 검토를 통한 한국 지역사회노인 예방 건강관리 방안모색 연구)

  • Purpose: Through a thorough examination of the CCSC (Community Comprehensive Support Center) system in Japan, this study suggests a scheme to provide community-based preventive health care services for the elderly in Korea. Methods: The study inquired into the applicability of the Japanese model by reviewing the data related to the CCSC project, aided by both in-depth interviews with staff in the field and consultations with specialists. Results: Rearrangement of the Visiting Health Management Project system is needed to manage the collective or individual visiting care management for frailty prevention of the elderly in communities. The delegated service system for preventive care in the community, including direct management by one of the public health centers, also needs to be reviewed and the application of stricter standards for the selection of the agency or corporation to run the delegated service is necessary. Long-Term Care Insurance, along with national and local grants, is to be considered as a financial resource for the community-based preventive health care model for the elderly. By making active use of education rooms at district offices, senior citizen centers in neighborhoods for the elderly with easy access can be created. The project needs to raise active supports from communities, develop programs which can be absorbed into particular local cultures, and promote the understanding of the preventive project in local communities. The preventive program should focus on first solving the problems of depression, seclusion, and lack of mobility of the elderly. Second, the program should instruct physical self-management for exercise-nutrition-dental maintenance, and third, the program should strengthen the cognitive abilities of the elderly. In addition, it is necessary to systematize and implement counter-plans of the family and community to protect the elderly who has mental and cognitive problems. Finally, by establishing a network of public health welfare resources based upon research on a community level, assessment and planning for the health of the elderly should be one with their family, and comprehensive consultation and recommendations should be provided to the family. Conclusion: Taking into consideration the experience Japan has had with respect to a similar project, it is appropriate to develop and implement a service system which would combine the Visiting Health Management Project system which has already been established and a preventive health care model for the elderly on a community level.
