Welfare Science and Technology Responsive to Ageing (고령화에 대응하는 복지 과학기술)

  • This paper explores into the potential contribution of welfare science and technology(WST) to meeting the challenges of ageing. Drawing on the literature about gerontechnology, this paper considers the application of scientific knowledge for welfare of an aging population. WST for the improvement of the daily functioning of the elderly people can not only prevent, delay or compensate for the declines or impairments of coginitive and physical functions (by assistive technology) but also enhance personal ability to overcome problems either by supporting the person's abilities (by supportive technology) or by changing the environment (by public welfare technology). This paper draws attention to a paradigm change in the conceptualization of disability and its ramifications for the role of technology and aging. A range of approaches such as Barrier-Free design, Universal Design points to new directions of WST in which products and services are devised to improve the quality of life for all people, and hence enhancing the socio-economic vitality. It is suggested that the future of welfare science and technology will be based in large measure on its goal to reduce or prevent individual and structural lag in the relationships between people and their environments. Addressing issues of the broader physical and social environment will emphasize the goal of prevention and the view that environmental needs for aging population are changing over time.

Risk Factors of Primary Lung Cancer and Spirometry (원발성 폐암의 위험인자와 폐활량 측정)

  • Background: Lung cancer and chronic obstructive lung disease often coexist in the same person who are elderly and cigarette smoking. There are several reports that the presence of chronic obstructive pulmonary disease constitutes an independent risk factor for the development of lung cancer. Moreover, the association between mucus hypersecrtion and lung cacer has been reported. Method: In 72 cases with primary lung cancer which were confirmed histopathologically at Chonbuk University Hospital from August 1986 to July 1991, We evaluated the relationship between spirometry and lung cancer characteristics. Results: Six cases(8.3%) showed normal lung function, 16(22.2%) cases showed pure restrictive lung disease, 46(63.9%) cases showed moderated obstructive lung disease and 4(5.6%) cases showed severe obstructive lung disease. $FEV_1$(%) was lower in central type than in peripheral type, lower in advanced non-small cell cancer and lower in subjects with phlegm. $FEV_1$/FVC(%) was higher in small cell cancer than in squamous cell cancer and higher in patients without previous pulmonary disease than with previous pulmonary disease. But there was no statistically significant difference in lung function according to histologic types and smoking history. Lung cancers with $FEV_1$/FVC less than 75% consisted of 35 cases of squamous cell cancer, 7 of small cell cancer(14%), 5 of adenocarcinoma(10%), 2 of large-cell carcinoma and 1 of unclassified carcinoma. Squamous cell carcinoma occured more in patients with $FEV_1$/FVC<75% than with $FEV_1$/FVC$\geq$75%(p<0.05). Conclusion: It was suggest that low $FEV_1$/FVC, as reflection of obstructive lung disease, may be at greater risk for squamous cell carcinoma in cigarette smoker.

A Home-Based Remote Rehabilitation System with Motion Recognition for Joint Range of Motion Improvement (관절 가동범위 향상을 위한 원격 모션 인식 재활 시스템)

  • Patients with disabilities from various reasons such as disasters, injuries or chronic illness or elderly with limited body motion range due to aging are recommended to participate in rehabilitation programs at hospitals. But typically, it's not as simple for them to commute without help as they have limited access outside of the home. Also, regarding the perspectives of hospitals, having to maintain the workforce and have them take care of the rehabilitation sessions leads them to more expenses in cost aspects. For those reasons, in this paper, a home-based remote rehabilitation system using motion recognition is developed without needing help from others. This system can be executed by a personal computer and a stereo camera at home, the real-time user motion status is monitored using motion recognition feature. The system tracks the joint range of motion(Joint ROM) of particular body parts of users to check the body function improvement. For demonstration, total of 4 subjects with various ages and health conditions participated in this project. Their motion data were collected during all 3 exercise sessions, and each session was repeated 9 times per person and was compared in the results.

A Study on Installation of Maritime Passenger Service Environment for the Disabled Persons (장애인을 위한 해상교통환경 구축 방안에 관한 연구)

  • Modern countries have almost ratified the UN Convention on the rights of persons with disabilities and are creating environments to prevent discrimination against these individuals. In 1996, the International Maritime Organization presented a recommendation for the design and operation of passenger ships to respond to elderly and disabled persons' needs. In Korea, an act on the promotion of transportation convenience for mobility disadvantaged persons has been implemented, and facilities for the disabled have been rapidly installed in ships, vehicles, aircraft, railways and passenger terminals. However, the maritime transport environment is less concerned about disabled persons than safety. In this study, basic research was conducted to improve the maritime transport environment for disabled people in Korea through a field survey and literature survey. As a result, it was found that relevant laws and regulations have not been properly applied to excursion ships and ferries as well as small-scale ports, which are responsible for a large part of coastal passenger transportation. In addition to low installation rates of facilities for the transportation of persons with disabilities in ships and terminals, loose equipment was also considered. Therefore, it is necessary to protect the human rights of persons with disabilities, which are universal values of humanity, by complementing laws and introducing improved facilities.

A Convergence Study on the Effect of Chronic Diseases and Quality of Life on Subjective Health Cognition in Single-Household Women Aged 60 or over (60세 이상 1인 가구 여성의 만성질환과 삶의 질이 주관적 건강인지에 미치는 영향에 대한 융합적 연구)

  • The purpose of this study was to provide a convergence suggestion of health care and social welfare services by empirically analyzing the effects of chronic diseases and quality of life on subjective health cognition in single-household women aged 60 or over. This study was based on the 2016-2019 Korea National Health and Nutrition Examination Survey, and 1,111 women who had been divorced or bereaved in single-household aged 60 or over were selected. As a result, it was arthritis that showed the largest average difference in subjective health cognition according to the prevalence of chronic diseases. And in the multiple regression analysis that was controlled for demographic characteristics, chronic disease had a negative (-) effect on subjective health cognition, and quality of life had a positive (+) effect on subjective health cognition. The negative (-) factor that had the greatest effect on subjective health cognition was diabetes(-0.084) among chronic diseases, and the positive (+) factor was mobility(+0.173) among the quality of life. Therefore, it suggests that health care services for chronic diseases should be strengthened in order to improve the subjective health cognition of single-person households over 60 years old, and social welfare policies to improve the quality of life should be considered together in a convergence.

A Study on the Relation between Degree and Physical & Mental Health of Old People in Interpersonal Relationship Network (대인관계 네트워크에서 연결정도와 노인의 신체적 건강 및 정신적 건강과의 관련성 연구)

  • The purpose of this study is to see if we can predict the health of seniors of community by analyzing the connection between social network degree and mental and physical health of old people who live in the areas of Gangwha Island. The subjects of the study were men and women aged 65 or over, a total of 643 that resided in Ganghwa A-county. The survey was conducted on Korean Social Life, Health and Aging Project from the year 2011 to 2012. Regression analysis was carried out using the data. The analysis results were as follows. First, it showed the relationships between income, gender, age out of demographic variables used as control variable and old persons'physical health. The research results showed that physical health was better in case of the higher incomes, men, and lower age. Second, out of demographic variables, educational background, income, age was shown to correlate with mental health. The research results showed that mental health was better in case of the higher incomes, higher educational background, and lower age. Third, in social network including direction, both out-degree and in-degree were shown to predict old people's physical and mental health. The results of this study suggest that not only out-degree but also in-degree should be considered in predicting the health of elderly persons by a person's human relationship. Also, two indicators of degree are meaningful in the dimension of health promotion and welfare of the old in that they can be used for finding isolated individuals that can be physically and mentally vulnerable.

Change of Life of the Older due to Social Admission in Long-Term Care Hospital (노인의 사회적 입원으로 인한 요양병원에서의 삶의 변화)

  • The purpose of this study is to explore the phenomenon in-depth with its context that what are changes of life in the older patients with social admission by applying the phenomenological research method, and to understand the essential meanings and structure of change experiences of life in elderly patients with social admission. The participants were 15 patients with chronic disease over 65 ages who were selected by convenience sampling. Data were collected through individual in-depth interviews from April to July 2016, and were analyzed using Colaizzi (1978)'s phenomenological analysis method. The essential meanings and structure of change experiences of life in the older patients with social admission were derived into six clusters of themes as follows: 'A lot of concerns are solved', 'New relationship', 'Life as a ordinary person that it looks nothing like a patient', 'The body and mind are comfortable', 'Fear of discharge','Social isolation'. The results of this study contributed to promoting a deeper understanding of change of life in older patients with social admission, and provided for the basic data of strategic approach in solving the social admission problem in long-term hospital.

  • The development of bio-sensing technology made it easy to collect various biometric information that was only available in large medical devices. The miniaturization of sensors makes it simple to carry out various health checkups that It did in person to the hospital by improving the portability of diagnostic devices. It is able to combine sensors into portable devices such as Smartphones, apply advanced Internet of Things (IOT) technology, and create new form factors for medical devices such as ultra-small modules that can be inserted or attached to their bodies. The results can be checked immediately through portable information devices such as smart phones. Although commercialization is still slow in Korea, new technologies are being applied in various ways in countries such as the United States that have granted remote medical services. Medical demand, supply and cost in South Korea are growing ahead of a super-aged society. Under these circumstances, attention is focusing on whether smart healthcare, a new concept, can complement the existing medical system. This study identifies the technology trends associated with smart health care and categorizes various healthcare products in the UX design aspects. In addition, the UX design approach and guidelines for applying smart healthcare technologies to the elderly, the intended users, are presented. This research will provide a reference to a new social issue, the UX-design approach to solving the problems of the aged society.

A Study of Influences Transportation Welfare Facilities on the Mobility Handicapped: Focusing on the Factors of Characteristics Public Transit (교통복지시설이 교통약자에 미치는 영향: 대중교통특성 요소를 중심으로)

  • As of the end of 2019, the total population nationwide was 51,849,861, and over the past five years, the population per household has decreased by 1.52% annually from 2.45 in 2015 to 2.31 in 2019. Looking at the current status of population distribution by age group, Busan metropolitan city had the highest proportion of senior citizens aged 65 or older at 18.2%, and Sejong Special Self-Governing city had the lowest at 9.4%. In particular, as of 2019, the population of the mobility handicapped was 15,219,000 nationwide, showing a ratio of about 29.4% of the total population. Therefore, it is important to secure the right to move according to the mobile facilities so that the mobility handicapped can move safely and conveniently. Against this background, this study places value on transportation welfare facilities centered on the mobility handicapped for safe and convenient movement, and in particular, in proposing measures to cope with the entry of an aging society and the continuous increase of the mobility handicapped, the transportation facilities, The purpose of this study is to analyze the impact of welfare facilities on the mobility handicapped and suggest implications. As a research method, the results of statistical analysis methods were presented through major preceding studies and expert surveys.

Need Assessment for Enlargement of Oriental Medical Care Service Room in Rural Community Health Center (농촌지역 보건소 한방진료실 확대설치 요구도 및 관련요인 - 일개 군 보건소 한방진료실 내소자를 대상으로 -)

  • Objectives: The oriental medical care has been getting popular in community health centers because of uniqueness, changing of disease patterns, and increasing of elderly population. From 1998, oriental medical doctors has been working in several rural community health centers for their military obligations. At this point of time, it is necessary to evaluate the oriental public health doctors system. This study was performed to investigate the utilization patterns, the degree of satisfactions, needs of oriental medical care service provided by community health center in a designated Gun area. This study focused on the need for extending over Myun area of community health center's oriental medical care services. Methods: Person-to-person interview survey method through a structured questionnaire was done by a personnel at a oriental medical care service room in a Gun community health center. The major statistical method used for the analysis were the t-test, ANOVA, and logistic analysis. Results: The total number of responded subjects in this study was 163 residents. Among these 65.0% were aged 61 or over, and only 13.5% recognized themselves were healthy. 73.7% of the respondents demanded establishment of more oriental medical care services provided by community health center to other Myun area. Factors affecting the need for enlargement of oriental medical care service room were education level, subjective awareness of access to community health center, and cost satisfaction of oriental medical service provided by community health center. Thus, a resident who had graduation of middle school achievement or above(OR=3.35), had a long way to center(OR=2.47), satisfied with oriental medical service cost(OR=2.78) had demonstrated increased chance of need by logistic regression analysis.
