This study purports to investigate the determinant of job satisfaction among workers working at Long-term care hospitals. The independent variables contain three groups of determinants: organizational characteristics variables(job autonomy, job variety, distributive justice, role conflict, supervisor support, job suitability, job significance, job security, organizational support, job growth, promotional opportunity), environmental variables(job opportunity), and psychological variables (met expectation, job efficacy, positive affectivity, and negative affectivity). The sample used in this study consisted of 250 workers from 4 Long-term care hospitals nationwide. Data were collected with self-administered questionnaires and analyzed using multiple regression analysis. The results of the study are as follows: 1) the following variables, listed in order of size, have significant effects on job satisfaction: negative affectivity(-), job significance(+), job growth(+), age(+), positive affectivity(+), organizational support(+), job opportunity(-). 2) the variance of job satisfaction explained by the variables used in the study are 53.8%. When demographic variables added to Model I, job satisfaction explained by variables are 55.4%. 3) the results of this study indicate that three variables of negative affectivity, job significance, job growth are especially important for improving the level of job satisfaction among workers at Long-term care hospitals.
Purpose: In Korea, the prevalence of dementia patients has increased, which makes the care burden of nurses important. The purpose of this study is to identify factors affecting the care burden of nurses caring elderly patients with dementia. Methods: A cross-sectional design was conducted using a convenience sample of 127 nurses from two hospitals and a nursing home in Korea. Participants completed questionnaires on knowledge of and attitudes toward dementia, social support, self-esteem, dementia problematic behavior (DPB), and professional caregiver burden index. The data were analyzed by using the t-test, ANOVA, Pearson correlation, and multiple regression with the SPSS/windows version 21.0 program. Results: The influencing factors for nurse burden include day shift, DPB, self-esteem, social support, which explain 28.0% of care burden of nurses. Conclusion: To reduce the burden of the nurses, there needs to be an administrative system that focuses on enhancing their self-esteem and social support. Active institutional support may be necessary for the nurses taking care of elderly patients with dementia.
Objectives: The purpose of this study is to investigate human health risk assessment of indoor air pollutants at small-sized public-use facilities (e.g., daycare centers, hospital and elderly care facilities) that the susceptible population is mainly used. Methods: To assess indoor air quality (IAQ), the concentrations of indoor air contaminants such as HCHO, benzene, toluene, ethylbenzene, xylene, styrene, PM-10, CO, $NO_2$ and $O_3$ in air samples were measured according to the Indoor Air Quality Standard Method. By conducting the questionnaire survey, the major factors influencing IAQ were identified. Human health risk assessment was carried out in the consideration of type of use (user and worker) at 75 daycare centers, 34 hospitals and 40 elderly care facilities. Results: As a result of measurement of indoor air contaminants, the average concentration of HCHO and TVOCs in hospitals was higher than daycare centers and elderly care facilities, about 8.8 and 23.5% of hospitals were exceeded by IAQ standard. In human health risk assessment, for the user of daycare centers and elderly care facilities, the mean carcinogenic risk of HCHO inhalation was higher than acceptable value. Except for HCHO, other values were determined under acceptable risk. Similarly, for the worker of hospitals, the mean carcinogenic risk of HCHO inhalation was higher than acceptable value and other values were evaluated under acceptable risk. In contrast, the risk levels of other contaminants measured in elderly care facilities were acceptable. In the determination of factors influencing IAQ, the construction year, building type, ventilation time, and the use of air cleaner were identified. Conclusions: This study provides the information for establishing the plans of public health management of IAQ at small-sized public-use facilities that have not yet been placed under the regulation. The findings suggest the consideration of human health risk assessment results for the IAQ standards.
Advanced countries such as the USA and Japan are eagerly seeking ways to improve health and welfare of the elderly. One of the services is home health care service using the telephone. Various types of services using the telephone have been developed, improved and are being utilized ranging from the basic consulting to emergency response systems in the area of health care for the elderly. A demonstration project was launched to study the feasibility of a consulting system and telemedicine for the elderly using the public phone system in Korea. For this project, a gathering site for the elderly was selected and those who visited this place were interviewed to find out what kinds of services they wanted and what kind of system they needed to provide the required services. Based on the users' requests and the surrounding environment, a telephone consulting facility was established at the Research Institute of Nursing Science at Seoul National University and consulting personnel was recruited, trained and posted at the center. An Application program for home health care nurses to use when they visited the patients at their homes was developed. This system operates on a notebook Computer and allows nurses to communicate with a doctor at a local hospital through a modem and telecommunication line. These systems were implemented for three months and problems which developed during operation of the systems were identified and progressively modified. Through system evaluation, it was found that a consulting system using phone service will be an invaluable system for the welfare of the elderly in the future. But in order to meet the elderly's need, more services than mere consultation are needed. That is, communication with physicians and hospitals are needed. Thus, when there is any need for physicians' attention, physicians or hospitals should be contacted directly. Similarly for telemedicine, when the home health care nurse visits elderly patients she can assess the patient's problem and provide nursing care, access a physician or hospital to refer her patient to or consult directly using the telecommunication the system. The above mentioned system is a basic form of futuristic telemedicine for the elderly and those who have chronic disease problems. This kind of system will be of great value when it is used on the national information super-highways in the future. In order to get to that stage, of course, this project needs great improvement in the technical, academic, and legal aspects.
Purpose: This study was conducted to examine the degrees of depression and activities of daily living (ADLs), and to identify factors that predict depression in elderly patients. Methods: The sample was149 patients (80% female; mean age 79.1) from three geriatric hospitals located in small cities and who agreed to participate in the study. Data were collected by staff nurses involved in direct patient care from October to September 2009. The level of depression was measured on a daily basis for seven days using a 13-item scale. Patients' ADLs was measured using a modified 10-item Bathel ADLs scale. Data were analyzed using SPSS/WIN 17.0. Results: Mean depression score was $10.05{\pm}4.77$ out of 26 points and about 27% was assessed as depressed with more than 13 points. Multiple regression analysis showed that low educational level, absence of spouse and fewer family visits predict depression. In addition, low ADLs and sleep disturbance predict depression. Conclusion: Nursing care providers need to be concerned about ADLs of older patients and should develop nursing activity programs to increase their in-hospital physical activities. It is also important care for older patients' sleep and encourages family visits to decrease their depression.
Objectives: The purpose of this study was to contribute to the development of an oral hygiene care program for patients with dementia by understanding the oral care status and oral health knowledge of care workers and caregivers with regard to patients with dementia. Methods: For about two months from May to June 2018, a survey of care workers and caregivers working in long-term nursing homes and elderly-specialized nursing hospitals was performed, and 442 people were selected for the final analysis. The frequency and percentage were calculated to understand the oral care status and oral health knowledge regarding patients with dementia, and an independent t-test was conducted to determine the difference between the elderly oral health knowledge of care workers and caregivers. Multiple regression analysis was also carried out to examine factors affecting elderly oral health knowledge. Results: With respect to the factor of oral health education in elderly oral health knowledge, oral health knowledge was shown to be highest when participants responded that education was unnecessary (p<0.001), and regarding the will to participate in oral health education, oral health knowledge was highest when participants responded as having no desire to participate (p<0.05). Conclusions: These results suggest that a standard manual should be developed for the accurate and qualitative management of oral hygiene care tasks performed by care workers and caregivers who are in charge of oral care for patients with dementia in order to provide continuous and systematic oral care.
The main purposes of the study were to identify factors affecting health-related quality of life of elderly stroke survivors, and to suggest social work interventions for improving their quality of life. The participants were 328 elderly stroke survivors over 55 years old recruited from 18 long-term care hospitals and 13 nursing care facilities in Chung-Nam Province, Korea. The data was collected through a direct survey method from Dec. 16, 2008 to Jan. 17, 2009. Statistical analysis revealed that the overall health-related quality of life among participants was relatively low, especially in the social relationships domain. As a result of hierarchical multiple regression analysis, depression emerged as the most significant predictor of elderly stroke survivors' health-related quality of life, followed by financial burden in future, activities of daily living, acceptance of disability, comorbidity, and major financial supporter(spouse). Health-related quality of life of elderly stroke survivors in long-term care facilities was affected by a variety of variables from disease-related factors to environmental ones. Particularly psychosocial variables were the most powerful factors to predict HRQOL of elderly stroke survivors. Based on the results, social work implications were discussed in the context of developing clinical strategies to alleviate elderly stroke survivors' health-related quality of life in long-term care facilities. Living with chronic illness and disability is a great challenge to the elderly. Most of all long-term residence in care facilities can cause deterioration of quality of life in old age. From now on, studies on health-related quality of life of elderly with chronic illness and disability should be activated in social work field.
In order to prevent aspiration pneumonia, oral hygiene care is important; thus proper tips for oral hygiene care should be distributed among hospitals and care facilities for elderly. Malpractice of oral hygiene itself may cause aspiration pneumonia. For efficient and effective oral hygiene care, engagement of professionals such as dentists or dental hygienists is essential. At the point where necessity of systemized dental $professionals^{\circ}{\phi}$ involvement at care facilities for elderly is being emerged, it is important to determine less risky approaches for oral health care tailored to elderly. Among many approaches, oral health care practice without usage of water is considered safe, reducing risk of possibility of aspiration. Since this practice is quite easy to implement, many dental professionals may utilize it when practiced at care facilities for elderly.
Purpose: This study of this study was to identify factors influencing the burden of main family caregivers who take care of elderly patients with brain and spinal diseases. Methods: This was conducted as descriptive research and data were collected from 255 main family caregivers who were taking care of elderly patients with brain and spinal diseases from 4 hospitals in Daegu and Gyeongbuk Province. Stepwise-multiple regression was used to identify the influencing factors of burden felt. Results: As the score of burden felt by the main family, economic, social, physical, interdependent and emotional burdens were high in order. Factors influencing burden felt by main family care givers taking care of elderly patients with brain and spinal diseases were changed relation with patient after hospitalization, daily life ability, marital status, education and family caregiver's personality (explanatory power of 24.6%). Family caregivers felt a heavier burden when their relation with the patient was changed negatively or when the patient's activity of daily living was low. Conclusion: Based on these results, we need to develop coping measures and interventional programs for reducing the burden felt by the main family caregivers of elderly patients with brain and spinal diseases.
Purpose: We investigated the association between empathy for the elderly, the nursing work environment, person-centered care, and geriatric nursing practice among university hospital nurses and factors that affect geriatric nursing practice. Method: We administered questionnaires to 178 nurses between May 23 and June 21, 2022 at a university hospital in city D. We used descriptive statistics, the t-test, one-way analysis of variance, the scheffé test, Pearson correlation coefficients, and stepwise multiple regression analysis for data analysis. Results: We observed a statistically significant positive correlation between nurses' geriatric nursing practice and empathy for the elderly, the nursing work environment, and person-centered care. Empathy for the elderly was positively correlated with the nursing work environment and person-centered care, and the nursing work environment showed a statistically significant positive correlation with person-centered care. Empathy for the elderly and the nursing work environment significantly affected geriatric nursing practice. The overall explanatory power of the regression model was 33.4%. Conclusion: Greater awareness and implementation of various educational programs that promote empathy for the elderly are necessary to improve nursing care for the elderly population. Furthermore, adequate manpower and material support are important to improve the nursing work environment among nurses employed at university hospitals.
