The purpose of this study is to measure the resource use of the elderly in long-term care services and to examine the effects of patient and facility characteristics on their use of resources. The data were collected from 510 old people over sixty years of age, residing in five long-term care hospitals and two skilled nursing homes during the period between December 1, 2000 and February 28, 2001. For a full sample, when the first level of RUG(Resource Use Group)-III categories were employed as the proxy of patient severity, facility characteristics, such as location, size and ownership, have large effects on the resource use measured by service intensity, whereas patient characteristics such as severity have little or no effect. The resource use is significantly high if the facility: (1) is located in rural areas (gun): (2) has mare than 200 beds; (3) is a long-term care hospital; (4) is private; and (5) has a low percentage of medical aid patients. The analysis of the resource use in each RUG-III categories, for which ADL(Ability of Daily Living) were employed as the prosy of patient severity, shows a similar result. The loose relationship between the needs of residents and the resource use seems to be closely associated with the ineffective reimbursement system for providers. The current reimbursement system has no provision for quality improvement and reimburses facilities simply according to their types: fee-for-service for long-term care hospitals, and monthly-flat-rate or full-coverage-national-aid for skilled nursing facilities. It will be necessary to develop a more reasonable reimbursement system that takes patient's severity into account and gives incentives for long-term care providers to offer cost-effective services.
Objective: Pain is very common in the elderly, so there is a high prevalence of analgesic use among this population. The purpose of this study was to assess patterns of analgesic use and evaluate factors associated with analgesic use in elderly patients. Method: The subjects of this study were patients over 65 years old hospitalized in a teaching hospital located in Chuncheon-si, Korea between January 1, 2014 and March 31, 2014. Data collection regarding analgesic prescriptions and baseline characteristics was conducted using computerized hospital database by medical information team. Logistic regression analysis was used to identify factors related to analgesic use. Results: A total of 2,394 patients were finally included. Among these patients, 700 (29.2%) took analgesics; 521 (74.4%) out of these 700 patients were received opioid analgesics and 179 (25.6%) were received only non-opioid analgesics. The most frequently prescribed opioid analgesic was pethidine (45.7%), and the most frequently prescribed non-opioid analgesic was acetaminophen (44.1%). Fracture was associated with increased odds of opioid analgesic prescriptions (OR = 2.766, 95% CI = 2.019-3.790, p < 0.001) and any analgesic prescriptions (OR = 2.394, 95% CI = 1.766-3.244, p < 0.001). Stroke or cerebral infarction was associated with decreased odds of opioid analgesic prescriptions (OR = 0.636, 95% CI = 0.471-0.858, p = 0.003). Conclusion: A significant proportion of hospitalized elderly patients use analgesics. Health care professionals should consider factors associated with analgesic use in this population to improve pain management.
본 연구는 요양병원 입원노인의 간호만족도, 자아존중감 및 우울이 적응에 미치는 영향을 규명하기 위한 서술적 조사연구이다. 대상자는 B시와 K시 소재 15개 요양병원 입원노인 116명이며, 자료수집기간은 2015년 2월14일에서 3월31일 까지였다. 수집된 자료는 t-test, ANOVA, Scheff's test, Pearson's correlation coefficients와 multiple regression으로 분석하였다. 연구결과, 대상자의 간호만족도 정도는 평균평점이 $3.92{\pm}0.60$으로 비교적 높게 나타났다. 자아존중감 정도는 평균평점이 $2.83{\pm}0.47$로 중간보다 높게 나타났으며, 우울정도는 평균평점이 $0.38{\pm}0.25$로 비교적 낮게 나타났다. 대상자의 적응정도는 평균평점이 $3.45{\pm}0.64$로 비교적 높게 나타났다. 대상자의 적응 정도는 제 특성 중종교, 배우자 유무, 지각된 경제상태, 지각된 힘든 문제에 따라 통계적으로 유의한 차이가 있었다. 간호만족도와 적응과의 관계와 자아존중감과 적응과의 관계는 각각 중간정도의 양의 상관관계가 있었고 통계적으로 유의하였다. 우울과 적응과의 관계는 강한 음의 상관관계가 있었고, 통계적으로 유의하였다. 적응을 설명하는 유의한 변수는 우울, 간호만족도, 자아존중감, 종교 순이었으며, 전체 설명력은 53.5%이었다. 따라서 요양병원 입원노인의 적응을 높이기 위해서는 우울을 감소시키고, 간호만족도와 자아존중감을 높일 수 있는 간호중재의 개발을 위한 구체적인 노력이 필요하다.
Objectives: The purpose of the study was to investigate the relationship between oral health perception and oral health-related quality of life-14(OHIP-14) in a geriatric hospital. Methods: A self-reported questionnaire was completed by 230 elderly people in a long-term care institution. The questionnaire was consisted of items on general characteristics, oral health perception and OHIP-14. Except incomplete answers, 226 data were analyzed statistically with one-way ANOVA, t-test, Pearson's correlation coefficient and post hoc Tukey test and ANOVA when significance was set at 0.05. Results: The OHIP-14 showed a negative linear relationship(p<0.01) of r=-0.272 with the oral health perception. Among the subgroups of the OHIP-14, quantitative linear relationship(p<0.01) was shown in functional limitation(r=0.822), physical pain(r=0.825), psychological discomfort(r=0.568), physical disability(r=0.895), psychological disability(r=0.652), social disability(r=0.804) and handicap(r=0.818). Conclusions: In order to improve the oral health perception and OHIP-14 in a geriatric hospital, it is necessary to develop continuously a variety of oral health education and systematic oral health promotion program.
The purpose of this study is to find a solution to effectively nurse elderly people in the aging society of next generation by analyzing relationship among the social problem of elderly people in the future society, their preferred life style, and their understanding level of an asylum for the aged. Data for this study were collected by questioning 316 persons(257 visitors at C University hospital and 59 residents in Chun - Ra Nam Do) from December 1996 to March 1997. The results were as follows: The assumption that an individual's preferred life style at old age depends on his /her understanding level of the social problem of elderly people was verified ($X^2=9.75$, p<0.00). The assumption that one's understanding of an asylum for the aged depends on his /her understanding level of the social problem of elderly people was verified ($X^2=4.40$, p<0.05). The assumption that one's understanding of an asylum for the aged depends on his/her pre-ferred life style at old age was verified($X^2=34.51$, p<0.01). Among those who want to live together with children when they get old, the following differences in their conception were noted: Elderly people who has lost his /her wife /husband should marry again(p<0.01). Children should take care of their parents(p<0.01). Children should live with their parents(p<0.01), A desirable living style for an elderly people. An undesirable living style for an elderly people. Among those who want to live alone when they get old, the following differences were observed: It is okay for children to live separately from their parents as long as they provide financial support(p<0.01). Any child, regardless of the distinction of sex or the order of birth among siblings, can take care of his/her parents(p<0.05), It is desirable for an elderly people to live alone (p<0.01). It is undesirable for an elderly people to live in an asylum for the aged(p<0.05). It was verified that those who view an asylum for the aged in a negative way believe that children should take care of their parents. Also, it was verified that those who view an asylum for the aged in a positive way believe that children should provide financial support to their parents even though they do not live together(p<0.01). Regardless of the subjects' opinions on seriousness of the social problem of elderly people, the assumption that any child, regardless of the distinction of sex or the order of birth among siblings, can take care of his/her parents was verified(p<0.01) (p<0.05). Regardless of the subjects' view on asylum for the aged, the assumption that it is undesirable place for an elderly people to live was verified (p<0.01). The assumption that one's understanding of an asylum for the aged depends on the distinction of sex among general characteristics such as sex, age, educational background, occupation, and religion was verified(t=-2.82, p<0.01). Purpose: The purpose of this study is to determine the relationship between nursing students' level of knowledge of and attitude toward the elderly and their behavior towards the elderly. Methods: For this study, 223 nursing students participated. The data was analyzed with PASW statistics program version 18.0 using descriptive statistics, a t-test, one-way ANOVA, Pearson's Correlation coefficient and multiple regression analysis. Results: The findings of the study displayed a score of 14.5 out of 25 for the level of knowledge of the elderly, a score of 3.9 out of 7 on the attitude toward the elderly, and 1.3 points on a 4-point scale on behavior towards the elderly. There was a positive correlation (r=.410, p<.001) between attitude and behavior towards the elderly, thus confirming that attitude towards the elderly influences behavior towards them. Conclusion: The most influential variable on the behavior towards the elderly emerged to be attitude. Thus, although knowledge and attitude cannot be neglected in manifesting positive behaviors in nursing students when they care for the elderly, knowledge training and elderly nursing practical sessions need to take place in order to strengthen attitude, which displayed a positive correlation with behavior.

The study attempted to suggest improvements to the standards of evacuation behavior model for occupants of domestic and overseas long-term care hospitals. As a result of the study, patients living in long-term hospitals have a problem that is difficult to evacuate on their own. It is judged that there is a need to present an evacuation behavior model database by setting evacuation priorities and evacuation plans. 우리나라의 급속한 고령화 현상은 노인요양병원의 확충으로 인한 인력수급의 어려움을 겪고 있다. 따라서 노인요양보호 서비스의 인력충원을 위해 외국 인력의 고용이 증가하고 있다. 본 연구에서는 노인요양병원의 외국인 간병인의 돌봄서비스 질이 기관에 대한 신뢰, 만족, 재이용의도에 미치는 영향에 대해 살펴보고자 한다. 본 연구는 서울 경기 지역 6개의 노인요양병원에서 중국동포 간병인의 돌봄서비스를 받고 있는 노인 249명을 조사하였다. 자료분석은 SPSS와 AMOS를 이용하여 분석을 실시하였다. 분석결과, 첫째, 노인요양병원 간병인의 돌봄서비스 질인 신뢰성, 대응성, 공감성, 유형성, 전문성은 기관에 대한 신뢰와 만족, 재이용의도와의 관계에서 모두 정적인 상관관계를 보이는 것으로 나타났다. 둘째, 노인요양병원의 중국동포 간병인의 돌봄서비스 질이 간병인의 신뢰에 미치는 요인으로는 대응성, 유형성, 전문성이며, 만족에 미치는 요인으로 돌봄서비스의 신뢰성, 유형성, 전문성 요인이 만족에 유의한 정의 영향을 미치는 것으로 나타났다. 또한 기관의 신뢰와 만족은 모두 재이용의도에 정의 영향을 미치는 것으로 나타났다. 셋째, 노인요양병원의 중국동포 간병인의 돌봄서비스의 질과 만족도의 관계에서 신뢰에 대한 간접효과는 돌봄서비스의 질 중 대응성을 제외하고 모든 독립변수에서 나타났으며, 신뢰와 재이용의도의 관계에서 만족도는 간접효과가 있는 것으로 나타났다. 본 연구의 결과는 노인요양병원의 중국동포 간병인 돌봄서비스 질이 이용자의 재이용률을 높이기 위해서 양질의 돌봄서비스 질을 통해 신뢰와 만족을 높일 수 있는 방안을 제공하여야 할 것을 시사한다.
In 2015, the population of elderly people in Thailand was 16% of the total population and is predicted to be over 20% by 2021 and nearly 28% by 2031. The increase of the elderly population in Thailand has also increased the proportion of dependent elderly people, and caring for them poses many challenges for both families and the government. This descriptive method research aimed to survey the health status of dependent older people in the rural community of Lampang province in northern Thailand. The participants consisted of 62 older people and 62 primary family caregivers from Hong Ha Health Promoting Hospital, Lampang, Thailand (totaling 124 people). The researchers assessed the health status of older people and their activities of daily living (ADL). In addition, researchers assessed the health status and stress of caregivers. All the participants were interviewed about their experiences with caregiving. The results showed that most of the older respondents were female with an average age of 78.15 years. Based on the ADL assessment, 50 of the 62 older persons were homebound while the rest were bedridden. The majority of older people had chronic or long-term conditions that required hospitalization from time to time. Their frequent health problems included oral disorders such as tooth decay or caries/gum disease/no teeth, reduced sight, psychological disorders, knee pain and risk of falling, low BMI, risk of malnutrition, and urinary leakage and incontinence (58.06%, 66.13%, 62.90%, 70.97%, 38.71%, 66.13%, and 37.10%, respectively). Usual care provided by the family members included personal hygiene care, food preparation and feeding, medication management, housekeeping and organizing necessary equipment, supply of needed equipment, prevention of falls, helping with travel for medical checkups and treatment, and providing companionship. Families experienced shortages of medical supplies, daily use equipment, lack of employment, inadequate income, and difficulty accessing health care services due to lack of transportation. Some caregivers experienced caregiving stress related to a lack of social interactions as well as routine caregiving activities. Families need different types of support in order to promote the well-being of older people and caregivers. This highlights the need for a community participation model for the care of older people in order to reflect sustainable long-term outcomes.
Purpose: As an interest in the elderly medical expenses increases, elderly people are increasingly purchasing indemnity private health insurance. Authors tried to investigate factors of having the indemnity private health insurance among the elderly people aged 65-75 years. Methods: We conducted panel logit regression analysis on 2,465 subjects as of 2016 using Korean Health Panel from 2010 to 2016. The dependent variable was whether to enroll in the indemnity private health insurance. The explanatory variables included socio-demographic characteristics, economic factors, health status, and health behaviors. Findings: As a result of the analysis of factors of purchasing indemnity private health insurances, it was analyzed that people with larger family, educated, pensioner, high household income or no disability were more likely to have indemnity private health insurance. Practical Implications: Considering the results of this study, the factor of purchasing indemnity private health insurance among elderly people were more likely to be their economic than demographic characteristics such as sex, age, and marital status. Policy makers should make efforts to reduce the burden on the elderly medical expense and to improve equity of medical use through institutional improvement such as raising age limit and lowering premium of indemnity private health insurance and expansion of public health insurance.
