A Study on the Management and Living Environment of the Group-homes for the Elderly -Focusing on Jeonju and Kwangju- (노인그룹홈의 운영과 생활환경에 관한 연구 -전주시와 광주시를 중심으로-)

  • The main objective of this study is to examine the situations of group-homes for the elderly and clarify the problems of them. This study investigates the present conditions of welfare facilities and supply conditions. 4 group-homes were surveyed in Jeonju and Kwangju cities. Data was collected through various ways including the survey conducted by facility operators, the measurement of living space, and photo-takings. The major results of this study are as follows: 1) With the introduction of nursing care insurance in 2008, the increase of the demand and supply to group-home will be expected, due to less cost for recuperation. The care of elderly in homelike atmosphere is effective to the elderly suffering from dementia. Therefore, it is proper that the supply of group-homes for dementia will be needed. 2) Group-homes for the elderly are being supplied under the standards of welfare facilities in the present. Separate supply standards, however, are needed because they are not suitable for small scale group-homes. 3) To supply Group-homes for the elderly on a one-unit basis would cause various problems such as economical unreality, personnel distribution and so on. It is necessary to supply more than two units for more efficient management. 4) The management of group-homes for the elderly is financially unstable because the operating funds heavily depend on the users or people who are connected with the facilities through private networks. Therefore, additional support is needed to settle the financial problem. It would be helpful to build the public networking that provides information about the group-home for the elderly for the public. 5) In case of the increase in the supply of the group-home in a residential area, it could be helpful to promote the group-home to the public. 6) The operators state that the main problems of group homes for the elderly lie in financial difficulties and the shortage of professionals and volunteers. The financial, personnel, and material support for personnel and the supervision of management are required for the transparency and effectiveness of the facility management. In addition, the closer network and cooperation system with the community is required. 7) The present living environments for group-homes for the elderly leave much room for improvement, considering the physical condition of the elderly. The more careful selections in the color, shape, material, and equipment of the facilities should be made for the convenience of the elderly users.

A Study on Sexual Desire Outburst of the Male Elderly Residents Experienced by the Workers in Living Facilities (생활시설 종사자가 경험한 남성노인의 성적욕구 표출에 관한 연구)

  • This study has been performed to explore verbal and behavioral expression of sexual desires among male elderly residents who have been living in long tenn care facilities. There are three topics covered in this study; first, in what situations and how seriously do care workers encounter expression of sexual desires of the elderly residents? Second, what kind of negative consequences do they believe those sexual behaviors will lead to? Third, how can we implement defensive measures against the sexual behaviors? In this study, twenty three care workers working full time in five retirement and care facilities were asked about their experience and perception of the above study agendas, and answers of the qualitative interview were drawn as follow; first, the elderly residents apparently show a variety of sexual harassment and provoking behaviors such as sticking to specific women, physically touching and attacking, and induce obscenely activities against female residents, care workers, and volunteer visitors. Second, their sexual behaviors are often influenced by their isolated and abandoned emotionality as well as living situation in rural areas. Third, their sexual behaviors often critically affect care work plan and facility managements by severely discouraging female care givers and community supporters. Therefore in this study, suggestions and defensive measures were made as follows: first, education and counselling programs toward female workers and volunteers need to be developed, and the programs should cover psychological and behavioral mechanism of sexuality in later life. Second, self control plans need to be empowered toward the elderly residents; in the plans, the elderly residents shall be encouraged to evaluate primary cause and proper solutions of sexual behaviors of their peering residents. Third, combination of healthy housing and care facilities for frail elderly need to be integrated in a neighboring location, so that when residents and workers encounter extreme episodes of sexuality of healthy residents in a housing facility, the problematic sexual residents are partially transferred into a neighboring care facility and thereafter other residents and cafe givers are relieved from stressful contacts with the extremely sexual residents.

An Integrated Review on Main Caregiver's Burden of Elderly in Korean Nursing Home (한국 요양시설 노인의 주 돌봄자 부담감에 대한 통합적 고찰)

  • The purpose of this study is to investigate the burden of caring for the care-givers of the elderly in Korea by using an integrated literature review method. A total of 23 studies were analyzed using a search database. When care-givers had higher sense of filial and guilt or more than two diseases, they showed high level of feeling of burden. Also, they had high level of feeling of physical burden by their oldness, service period (especially at the period of 1-3 years). In the feeling of economic burden, they had high level of feeling of burden by their oldness, or elderly's disease periods. The feeling of burden by psychological condition was found in elderly in aged and the beginning of admission of nursing home. The feeling of burden by environment situation was found when the functional status of the elderly was bad. Therefore, we need to concern care-givers's feeling of burden with elderly people in the nursing home. In the future, I believe that the findings of this study will be helpful for development of the intervention program for alleviate burden for the care-giver.

Physical Functioning and Related Factors in the Elderly People Admitted Long-term Home Care Insurance (재가급여 장기요양인정 노인들의 신체기능 상태 및 관련요인)

  • This study was performed to determine the levels of physical functioning and to reveal its association with socio-demographic characteristics, health status and health related behaviors in the elderly people admitted long-term home care insurance. The interviews were performed, during the period from December 1, 2011 to March 31, 2012, to 618 home care elderlies admitted long-term care insurance dwelling in Daejeon city. As a results, the distribution of impaired ADL and IADL group of all subjects were 45.6% in ADL, and 48.4% in IADL. in consequently, the rate of impaired IADL group was higher than that of impaired ADL group. In logistic regression analysis, the adjusted odds ratio of the impaired ADL and IADL group were significantly increased in the group of male, living with spouse or family, bear for living expenses with son and daughter, health insurance, irregular exercise, seldom with frequency of going out, seldom with activity of hobbies, Subjective health status is good, and disability of body, visual acuity, hearing ability and mastication ability is good, and without urinary incontinence and amnesia. In conclusion, the level of physical functioning in the elderly people admitted long-term home care insurance was decreased in IADL than ADL. And the level of ADL and IADL were related with the various variables of socio-demographic characteristics, health status and health related behaviors.

The Effect of Choice and Service Quality on User Satisfaction among Long-Term Home Care Service Recipients (장기요양 재가서비스 서비스 품질과 선택권 실현이 이용자 만족도에 미치는 영향)

  • The purpose of this study is to examine the moderating effect of choice realization between service quality and user satisfaction to elderly home care services. For this purpose, we analyzed 258 respondents who received elderly home care service in Jeollabuk-do. Considering the assumption that the moderating effect of choice realization would be different by rural and urban areas because of the disparity of the elderly welfare infrastructure, the urban and rural areas were analyzed separately. The analyses showed that choice realization had a moderating effect on the relationship between service quality and user's service satisfaction for the service clients resided in urban areas. However, there was no significant moderating effect of choice realization for the service clients in rural areas. This result implies that the moderating effect of selection realization is different in rural and urban areas because of the poor elderly home care facilities in rural areas. In order for aged people to actively express the effect of service user choice, it is necessary to solve welfare imbalance between regions through expansion of welfare infrastructure and policy support to rural area.

An Analysis of Unit Care Characteristics of Large Skilled Nursing Facilities for the Elderly in City Areas (도심지역 대규모 노인전문요양시설의 단위공간별 특성 분석)

  • The purpose of this study was to investigate unit care characteristics of large skilled nursing facilities for the elderly, which are located in city areas, and to discuss the way of encouraging home-like atmosphere in perspective of spatial unit. First of all, the analysis of five facilities opened before 2002 was performed, based on previous studies, to compare with them opened in recent 5 years. Most of the previous facilities did not have differentiation between their residential and nursing units, and the number of residents in each unit were excessive in comparison with the standard limit(12-28). On the contrary, the facilities established in recent 5 years had systematic spatial unit structure. The residential units had basically rooms and spaces for small group and the number of elderly residents per unit was appropriate on the basis of standard limit which was suggested by Kwon(2002). The nursing units were consist of nursing station, 2-3 residential units, spaces for large group of residents, hair dressing, nurse, living assistant, bathing, storage and etc. But, there was a problem In space usage even though they have distinct unit care systems. Some spaces for groups were empty without residents, furniture, and other equipments, which were necessary for unit care.

Use of Home Nursing Therapy and Need of Home Care Equipments (가정간호환자의 치료적 간호서비스 제공과 의료기구 사용 및 요구도)

  • This study was conducted with whole home care nurses nationwide to provide secondary analyzed data to understand on their usage of medical equipments and their need of them for a month. This study found that treatments given by home care nurses were nelaton catheterization, bladder washing/urethral washing, newborn care, exchange and care for nasogastric tube and suction in that order of frequency. Second, instruments and equipments used for home care were reported to be stethoscope, patient monitor, blood pressure measuring equipment, air flotation mattresses, beds for patients, mattresses, suctioning device sets, enteral feeding equipment and dressing set in that oder of frequency. Moreover, need assessment of medical instruments and equipments showed renal dialysis was most needed and patient monitor, blood pressure measuring equipment, enteral feeding equipment, solution and other supplies for renal dialysis and beds for patient were necessary in that order. In conclusion, the results of this study investigating special treatments and medical instruments and equipments used for home care patients and analyzing patients' need, were expected to be useful for expansion of application of long-term care insurance for the elderly and health insurance as well as for quality control of home care and development of medial instruments and equipments used at home.

The Effects of Percussion Program on Depression and Self-esteem of the Elderly Living in Residential Care (타악기 연주 프로그램이 시설노인의 우울감과 자아존중감에 미치는 영향)

  • This study was to investigate the effects of percussion program on depression and self-esteem for the elderly lived in residential care. 12 older adults participated in the 40-min. percussion program for 4 weeks(10 sessions). The percussion program consisted of three steps; 1) introduction for creating rapport and relationship, 2) process for mood induction and increasing interaction, and ending for increasing self-achievement and confirming the changed mood. Korean Geriatric Depression Scale (KGDS) and Self-Esteem Scale (SES) were administered before the first session and after the completion of the program. Results showed statistical significance (p<.05) in KGDS (p=.020) and SES (p=.025). Findings imply that percussion program can induce positive changes in depression and self-esteem for elderly in the residential care, and more depressed elderly group may have better outcome in percussion playing. Future studies should consider various levels of depression among elderly, and design the program to be more sensitive to the level of functioning.

  • Korea is undergoing a change in its population structure, due to economic development, falling birth rates and the development of health care. As the population ages, the number of elderly people who have difficulty in daily life such as dementia and paralysis increases every day. The growth of nuclear families and the increased participation of women in society make it difficult to take care of elderly people who need long-term care at home. As the social problems resulting from this have emerged as serious problems, the government enacted and implemented the Long Term Care Insurance Act to improve the quality of life for the elderly subject to long-term care and to ease the burden of family support in order to solve such problems. Therefore, the present study explores ways to improve legal and institutional aspects, and seek mental and psychological measures for the stability of old life as well as the physical health of welfare.

Current status and future tasks of visiting oral health care services for elders (우리나라 노인 방문 구강건강관리 서비스의 현황과 향후과제)

  • Objectives: This study confirms the current status of visiting oral health-care services for the elderly to draw policy implications for revitalization of the visiting oral health care services in the future. Methods: First, a survey was conducted on health centers about the current status of the elderly visiting oral health-care service and how to revitalize it. Next, the number of oral hygiene services provided to the elderly was checked in the long-term care insurance system. Results: Oral health education (100%) was the most common practice in visiting oral health-care service for the elderly, and the most difficult thing in providing services was the lack of dental hygienists (38.9%). The status of oral health-care services in the long-term care insurance system for elderly revealed that the total number of service claims has been confirmed to be zero since the introduction of the system. Conclusions: Despite the existence of a system that provides elderly visiting oral healthcare services, to revitalize it, the law must be amended to secure a dental hygienist as the main agent of the activity and to further take responsibility for autonomous authority and performance.
