Background: Massive, irreparable rotator cuff tears (RCTs) are a challenging clinical problem in young patients. In recent years, arthroscopic superior capsular reconstruction (ASCR) is a popular treatment in the massive, irreparable RCTs. However, studies reporting clinical results of ASCR are rare in the literature. Methods: Between 2013 and 2015, six patients underwent ASCR. One patient treated with dermal allograft, while five patients with autogenous fascia lata graft. Demographic data, as well as preoperative and last follow-up clinical data including pain, range of motion (ROM), strength, American Shoulder and Elbow Surgeons system, the Constant system, the University of California at Los Angeles system, the Simple Shoulder Test, and the Shoulder Pain and Disability Index system were obtained. Acromiohumeral distances and Hamada classification were measured on standard anteroposterior x-ray. Results: All patients were men, and the average age was $59.5{\pm}4.18years$ (range, 53-65 years). The minimum follow-up was 18 months with a mean follow-up was $27.33{\pm}7.58months$ (range, 18-36). All patients had postoperative improvement in pain scores and functional scores. The ROM and strength did not improve after surgery. The Hamada score progressed of radiographic stage in 2 patients. In the case of dermal allograft, there was graft failure 6 weeks after ASCR. Conclusions: Our results support the ASCR as a viable treatment for surgical salvage in massive, irreparable RCTs. This treatment option may provide patients with decreased pain and increased function. And studying our case of dermal allograft failure provides opportunities to decrease graft failure in ASCR using dermal allograft.
Background: This study introduces a surgical technique with good clinical outcome useful in the treatment of osteoporotic displaced 3- or 4-part proximal humeral fractures. Methods: From May 2014 to February 2016, 16 patients with displaced 3- or 4-part proximal humeral fractures were treated by application of a locking plate with an endosteal strut allograft via a deltoid splitting approach with a minimum follow-up of 12 months. The allograft was inserted through a fractured gap of the greater tuberosity to support the humeral head and then fixed by a locking plate with meticulous soft tissue dissection to protect the axillary nerve. Surgical outcomes were evaluated by the American Shoulder and Elbow Surgeons (ASES) and visual analogue scale (VAS) scores, radiological imaging, and clinical examination. Fixation failure on radiographs was defined as a >$5^{\circ}$ loss of neck shaft angle (NSA) compared to that on an immediate postoperative radiograph. Avascular necrosis (AVN) of the humeral head was also evaluated. Results: In all cases, complete union was achieved. The ASES and VAS scores were improved to $85.4{\pm}2.1$ and $3.2{\pm}1.3$, respectively. Twelve patients (75.0%) had greater than a $5^{\circ}$ change in NSA; the average NSA change was $3.8^{\circ}$. Five patients (31.3%) had unsatisfactory ranges of motion exhibiting a <$100^{\circ}$ active forward flexion. No axillary nerve injuries or AVN were observed at the last follow-up. One patient was converted to reverse total arthroplasty due to severe pain and functional deficit. Conclusions: Minimally invasive fixation via a locking compression plate and an endosteal fibula strut allograft in Neer classification 3-or 4-part fractures with severe osteoporosis in elderly patients can achieve good clinical results.
Background: In a previous study undertaken to quantify capsular volume in rotator cuff interval or axillary pouch, significant differences were found between controls and patients with instability. However, the results obtained were derived from two-dimensional cross sectional areas. In our study, we sought correlation between three-dimensional (3D) capsular volumes, as measured by magnetic resonance arthrography (MRA), and multidirectional instability (MDI) of the shoulder. Methods: The MRAs of 21 patients with MDI of the shoulder and 16 control cases with no instability were retrospectively reviewed. Capsular areas determined by MRA were translated into 3D volumes using 3D software Mimics ver. 16 (Materilise, Leuven, Belgium), and glenoid surface area was measured in axial and coronal MRA views. Then, the ratio between capsular volume and glenoid surface area was calculated, and evaluated with control group. Results: The ratio between 3D capsular volume and glenoid surface area was significantly increased in the MDI group ($3.59{\pm}0.83cm^3/cm^2$) compared to the control group ($2.53{\pm}0.62cm^3/cm^2$) (p<0.01). Conclusions: From these results, we could support that capsular volume enlargement play an important role in MDI of the shoulder using volume measurement.
Purpose: We investigated the relationship between the muscle activity and kinematic variables of the upper extremity during a push-up task on stable and unstable surfaces. Methods: We recruited 15 healthy subjects. Subjects completed the push-up task on stable and unstable surfaces. Surface electromyograms were recorded from the serratus anterior, upper trapezius, latissimus dorsi, infraspinatus to monitor changes in muscle activity. Markers for kinematic changes of elbow flexion, shoulder extension, shoulder retraction and scapular adduction were attached at C7, the T7 spinous process, both acromions, the scapula superior and inferior angle, the humerus lateral epicondyle, and the ulnar styloid process. Correlation coefficients between muscle activity and kinematic variables were analyzed by SPSS for Windows, version 15.0. Results: On the unstable surface, elbow flexion and shoulder extension increased with increasing muscle activity of serratus anterior, upper trapezius and infraspinatus. On the stable surface, shoulder retraction decreased with increasing muscle activity of serratus anterior and infraspinatus. Scapular adduction decreased with all types of increasing muscle activity, regardless of the support surface. Conclusion: Correlations between muscle activity and kinematic variables were observed on stable and unstable surfaces during a push-up task, while correlations between right and left variables were not clear. These finding suggest that it may be used to develop a rehabilitation program which could be effective in improving shoulder function in patients with shoulder problems.
A simulation is performed to investigate the effect of the pipe supports on the change of the natural frequencies of curved pipe systems containing fluid flow, for different elbow angles and geometry of the pipe systems. Based upon the Hamilton's principle, the equations of motions are derived, and the finite element equation is constructed to solve the corresponding eigenvalue problem. The angles of elbows do not affect the change of the fundamental natural frequency, but affect the change of the third or higher natural frequencies. Without any support, the change of the fundamental natural frequency due to the geometric change is smaller than the change of the second or higher natural frequencies. The more curve parts exist in the pipe system, the less change of lower frequency range, compared with the change of higher frequency range, is observed. Spring supports can be used to reduce the fundamental natural frequency, without change of the second or higher natural frequencies. To avoid resonance, which is critically dangerous from the view point of structural dynamics, the mechanical properties such as stiffness or the location of pipe supports are need to be changed to isolate the natural frequencies from the frequency range of dominant vibration modes.
Purpose: This study is a descriptive survey to find out musculoskeletal symptoms in care workers working at medical welfare facilities for elders and factors affecting such symptoms. Methods: Data were collected from 115 care workers selected through convenient sampling from 6 medical welfare facilities in Seoul and Gyeonggi-do during the period from May 15th to May 19th. 2006. The Korean version of Job Content Questionnaire (JCQ) were used. Result: Of the subjects, 81.7% complained of musculoskeletal symptoms in two or more parts of their body. The frequency of body parts with musculoskeletal symptoms was high in order of shoulder, leg/foot, waist, neck/hand/wrist/finger and arm/elbow. The average job insecurity instability in the age group of 50-59 was 9.19, the average degree of regular exercise was 59.68. and the average job demand in those diagnosed with musculoskeletal diseases was 47.06, and the average job demand in those wounded during exercise or by an accident was 47.78, and all these were statistically significant. The heavier physical load in their work was, the higher their complaint of musculoskeletal symptoms was. In the lower social support group, the degree of complaint on musculoskeletal symptoms was remarkably high. Conclusion: The physical load of their duty and social support worked as the factors affecting musculoskeletal symptoms in care workers.
Objectives The purpose of this study is to know the effect of acupuncture on brachial plexus palsy due to birth injury. Methods We decided to treat one week of Vojta therapy and occupational therapy, but other were combined, and then add three weeks of acupuncture treatment as well. Results Right clavicle fracture at birth brachial plexus injury due to decreased locomotion of the right upper extremity, muscle weakness in children aged 7 months to Vojta therapy, occupational therapy and acupuncture, when performed in conjunction mobility increase of the shoulder joint, elbow support improvement, recovery of motor function of the hand grip, etc. to obtain a significant motor function recovery improvement. Conclusion Acupuncture was a good treatment of choice for the better shoulder, joint and hands movements.
Cause and the elimination method for the chattering phenomena were investigated the check valve attached exit of the auxiliary cooling water pump at a korean nuclear powerplant. From the site experiment and numerical calculation the incident angle of the disk was so small that it was not able to produce the lifting force to overcome the component of disk weight. Moreover, it turned out that the installed position was not symmetric for the secondary vortical flow generated inside the elbow, so that the flow structure had strongly unstable flow characteristics. From this technical support, the tapping noise and the chattering phenomena were eliminated exactly by changing the incidence angle of the valve disk and installed position of the check valve.
Objectives : This study was to investigate the relationship analysis in VDT(visual display terminal) of work hours. Methods : Questionnaires were completed by 41 women in VDT operations. The information was used to estimate multiple regression for the VDT of work hours related factors association. Results : The prevalence of the VDT of work hours was significantly higher in the shoulder which followed by the neck, the hand and fingers, the arms, the lowback, the back. The VDT of work hours decreased with aging. The VDT of work hours increased with work duration, single, lower among habitual exercise. The desk height, the seat pan height, the posture on the sitting, the elbow angle all did not meet the recommended criteria. Conclusions : Data from this study support a statistically significant association with satisfaction for occupation.
