A Case Report of Chemotherapy-Induced Peripheral Neuropathy Treated with Modified Guibi-tang (귀비탕가미방으로 호전된 항암화학요법 유발 말초신경병증 치험 1례)

  • Objective: The purpose of this study was to report the effectiveness of modified Guibi-tang in a patient suffering from chemotherapy-induced peripheral neuropathy (CIPN). Methods: A 54-year-old Korean female patient diagnosed with recurrent ovarian cancer had CIPN with other symptoms, such as anorexia, dyspepsia, insomnia, etc. She was diagnosed with Simbiyangheo and hence treated with a modified Guibi-tang. Neuropathic symptoms were assessed using a numerical rating scale (NRS) and a sensory score. Quality of life was assessed using the Functional Assessment of Cancer Therapy/Gynecologic Oncology Group Neurotoxicity (FACT/GOG-Ntx). Results: After 14 days of treatment, the patients showed a decrease in NRS for bilateral limb pain and improvement in other symptoms, such as general weakness, insomnia, dizziness, and headache. Quality of life also increased. Conclusion: Modified Guibi-tang may be considered an optional treatment for CIPN if the patient is diagnosed with Simbiyangheo. Further studies are needed to confirm this finding.

A study on the current state of Korean medicine treatment in infertile women: an observational multi-center study protocol (여성 난임환자의 한의치료 현황 및 경과 관찰을 위한 전향적 다기관 관찰연구 프로토콜)

  • Objectives: The purpose of this study is to investigate the characteristics of female infertility patients who come to Korean medicine clinics and analyze the trends of Korean medicine treatment methods and the effects of female infertility by registry data. Methods: We are recruiting study participants from Korean medicine hospitals and clinics. If female infertility patients come, we will ask them to register for this study. After enrollment, we will collect demographic information, treatment methods, pregnancy results, adverse events, and treatment costs. Result: First, we will analyze the characteristics of patients, the status of the use of Korean medicine treatment interventions, the cost status of Korean medicine infertility treatment, and the delivery characteristics of pregnant women after Korean medicine treatment. Second, we will analyze the effectiveness of Korean medicine treatment. The primary outcome is clinical pregnancy rate, and secondary outcomes are 12-week pregnancy maintenance rate, degree of dysmenorrhea and premenstrual syndrome, stress level, and health-related quality of life score. Discussion: This study is the first observational multi-center study in Korea for female infertility. By establishing the registry, we are creating a resource that contains patient-reported outcome measures for female infertility. The registry is expected to provide valuable data for developing Korean medicine Clinical practice guidelines for female infertility.

  • Mental status examination is an important stage in the assessment process because it serves as the foundation for establishing nursing diagnosis and intervention. Until now many students still feel difficult to understand the terms in the assessment of mental status. Interactive Mobile Learning in one of the media that is currently being developed. The use of this media will provide more in-depth learning opportunities, and students can practice their skills in carrying out practicals because of the mobility principle possessed by smartphones. The purpose of this study was to develop a smartphone-based app and evaluate the app's effectiveness by measuring nursing students' knowledge of mental status examination. Design: A randomized trial with a pre-and post-test design was conducted at a university in Indonesia. A total of seventy junior nursing students participated in this study. The intervention group received a smartphone-based app, and the control group received one-time lecture-based learning. We offered the experimental group the app and information about how to use it, and we encouraged them to use it. The control group received classroom instruction. Results: The intervention group scored significantly higher than the control group on knowledge score (t = 19.40, p = 0.000) and satisfaction with the learning method (t = 0.640, p = 0.021) Conclusion: These findings suggest that smartphonebased education could be an effective method in nursing education for teaching mental status examinations.

Effects of Contralateral Both Side Acupuncture on NIH Scale in Stroke Patients (건측 위주의 양측자침이 뇌졸중 환자의 NIH Scale상의 기능 회복에 미치는 영향)

  • Objective : In order to study the effect of contralateral both side acupuncture on recovery of motor disorders in stroke patients, a clinical study was performed. Methods : Thirteen patients with post-stroke hemiplegia were randomized into two groups. Six patients (test group) were treated with contralateral both side acupuncture. The other seven patients (control group) were treated with ipsilateral both side acupuncture. The activity of daily living was measured with a National Institutes of Health (NIH) scale. The therapy was performed once a day for 3 weeks. Results : In terms of score of NIH, the test group showed statistically meaningful increase after I weeks treatment, while the control group showed statistically meaningful increase after 2 weeks (P<0.05). The results showed no statistically meaningful difference after 3 weeks treatment between the groups. Conclusions : These results support that contralateral both side acupuncture therapy has almost the same effectiveness compared with ipsilateral both side acupuncture therapy in improvement of the activity of daily living of post-stroke hemiplegic patients.

Drug-Induced Dyskinesia Treated with Korean Medicine: A Case Report

  • Drug-induced dyskinesia is an involuntary muscle movement caused by various dopamine receptor-blocking drug exposure, such as antipsychotics, antidepressants, and antiemetics. Causative drug removal is the main treatment for drug-induced dyskinesia whenever possible because its pathophysiology lacks a universally accepted mechanism; however, the symptoms can persist for years or decades in many patients even after causative drug removal. Herein, we present a case of drug-induced dyskinesia in a 61-year-old female patient who consumed medication for approximately 10 years for her depression, anxiety, and insomnia. Cervical and facial dyskinesia was suggested to be related to perphenazine and levosulpiride administration. The patient received acupuncture, pharmacopuncture, herbal medicine, and chuna treatment for 81 days during hospitalization. The symptoms were evaluated using the Abnormal Involuntary Movement Scale, Toronto Western Spasmodic Torticollis Rating Scale, Tsui's score, and Numeric Rating Scale, which revealed remarkable improvement, suggesting the effectiveness of combined Korean medicine for drug-induced dyskinesia.

Exploring modern machine learning methods to improve causal-effect estimation

  • This paper addresses the use of machine learning methods for causal estimation of treatment effects from observational data. Even though conducting randomized experimental trials is a gold standard to reveal potential causal relationships, observational study is another rich source for investigation of exposure effects, for example, in the research of comparative effectiveness and safety of treatments, where the causal effect can be identified if covariates contain all confounding variables. In this context, statistical regression models for the expected outcome and the probability of treatment are often imposed, which can be combined in a clever way to yield more efficient and robust causal estimators. Recently, targeted maximum likelihood estimation and causal random forest is proposed and extensively studied for the use of data-adaptive regression in estimation of causal inference parameters. Machine learning methods are a natural choice in these settings to improve the quality of the final estimate of the treatment effect. We explore how we can adapt the design and training of several machine learning algorithms for causal inference and study their finite-sample performance through simulation experiments under various scenarios. Application to the percutaneous coronary intervention (PCI) data shows that these adaptations can improve simple linear regression-based methods.

3 Case Reports of Trigger Point Needling and Oriental Medical Treatment to Shoulder Pain Patients in Stroke Sequelae (Trigger Point 자침 및 한방치료로 호전된 중풍후유증 환자의 견비통 치험 3례 보고)

  • This study was performed to investigate the effectiveness of trigger point needling and oriental medical treatment to shoulder pain patients in stroke sequelae with three aspects of pain and range of motion, muscle strength. Trigger point needling was applied to 3 patients on supraspinatus, infraspinatus, deltoid, subscapularis muscle. For evaluating treatment, visual analog scale(VAS), passive range of motion(PROM), muscle strength evaluation(MSE) were checked. After treatment, VAS score of 1 cases were decreased. PROM of 1 case was improved and MSE of 2 cases were improved slightly. These results suggest that trigger point needling and oriental medical treatment were effective on shoulder pain patients in stroke sequelae and it is necessary to research more case with shoulder pain in stroke sequelae.

Analysis of the Effect of Deep-learning Super-resolution for Fragments Detection Performance Enhancement (파편 탐지 성능 향상을 위한 딥러닝 초해상도화 효과 분석)

  • The Arena Fragmentation Test(AFT) is designed to analyze warhead performance by measuring fragmentation data. In order to evaluate the results of the AFT, a set of AFT images are captured by high-speed cameras. To detect objects in the AFT image set, ResNet-50 based Faster R-CNN is used as a detection model. However, because of the low resolution of the AFT image set, a detection model has shown low performance. To enhance the performance of the detection model, Super-resolution(SR) methods are used to increase the AFT image set resolution. To this end, The Bicubic method and three SR models: ZSSR, EDSR, and SwinIR are used. The use of SR images results in an increase in the performance of the detection model. While the increase in the number of pixels representing a fragment flame in the AFT images improves the Recall performance of the detection model, the number of pixels representing noise also increases, leading to a slight decreases in Precision performance. Consequently, the F1 score is increased by up to 9 %, demonstrating the effectiveness of SR in enhancing the performance of the detection model.

Comparing Learning Outcome of e-Learning with Face-to-Face Lecture of a Food Processing Technology Course in Korean Agricultural High School

  • This study identified the effectiveness of e-learning by comparing learning outcome in conventional face-to-face lecture with the selected e-learning methods. Two e-learning contents (animation based and video based) were developed based on the rapid prototyping model and loaded onto the learning management system (LMS), which is http://www.enaged.co.kr. Fifty-four Korean agricultural high school students were randomly assigned into three groups (face-to-face lecture, animation based e-learning, and video based e-learning group). The students of the e-learning group logged on the LMS in school computer lab and completed each e-learning. All students were required to take a pretest and posttest before and after learning under the direction of the subject teacher. A one-way analysis of covariance was administered to verify whether there was any difference between face-to-face lecture and e-learning in terms of students' learning outcomes after controlling the covariate variable, pretest score. According to the results, no differences between animation based and video based e-learning as well as between face-to-face learning and e-learning were identified. Findings suggest that the use of well designed e-learning could be worthy even in agricultural education, which stresses hands-on experience and lab activities if e-learning was used appropriately in combination with conventional learning. Further research is also suggested, focusing on a preference of e-learning content type and its relationship with learning outcome.

Anomaly Detection via Pattern Dictionary Method and Atypicality in Application (패턴사전과 비정형성을 통한 이상치 탐지방법 적용)

  • Anomaly detection holds paramount significance across diverse fields, encompassing fraud detection, risk mitigation, and sensor evaluation tests. Its pertinence extends notably to the military, particularly within the Warrior Platform, a comprehensive combat equipment system with wearable sensors. Hence, we propose a data-compression-based anomaly detection approach tailored to unlabeled time series and sequence data. This method entailed the construction of two distinctive features, typicality and atypicality, to discern anomalies effectively. The typicality of a test sequence was determined by evaluating the compression efficacy achieved through the pattern dictionary. This dictionary was established based on the frequency of all patterns identified in a training sequence generated for each sensor within Warrior Platform. The resulting typicality served as an anomaly score, facilitating the identification of anomalous data using a predetermined threshold. To improve the performance of the pattern dictionary method, we leveraged atypicality to discern sequences that could undergo compression independently without relying on the pattern dictionary. Consequently, our refined approach integrated both typicality and atypicality, augmenting the effectiveness of the pattern dictionary method. Our proposed method exhibited heightened capability in detecting a spectrum of unpredictable anomalies, fortifying the stability of wearable sensors prevalent in military equipment, including the Army TIGER 4.0 system.
