A Study on Failure Mode and Effect Analysis (FMEA) for Preoperative Risk Prevention (오류유형 영향분석(FMEA)을 적용한 수술준비 위험예방활동의 효과)

  • Purpose: The purpose of this research was to provide patients with safe preoperative preparatory procedures by removing any risk factors from the preparatory procedures by using failure mode and effects analysis, which is a prospective risk-managing tool. Methods: This was a research design in which before and after conditions of a single group were studied, Failure mode and effects analysis were applied for the preparatory procedures done before operations. Results: The preparation omission rate before the operation decreased from 2.70% to 0.04%, and operation cancellation rate decreased from 0.48% to 0.08%. Conclusion: Failure mode and effects analysis which remove any risk factors for patients in advance of the operation is effective in preventing any negligent accidents.

Investigation of short column effect of RC buildings: failure and prevention

  • If an infill wall in a reinforced concrete frame is shorter than the column height and there is no initial gap between the column and the infill wall, the short column effect can occur during an earthquake shaking. This form of damage is frequently observed in many earthquake-damaged buildings all around the world and especially in Turkey. In this study, an effective method, which consists of placing additional infill wall segments surrounding the short column, to prevent this type of failure is examined. The influence of adding infill wall in the reduction of the shear force in the short column is also investigated. A parametric study is carried out for one-storey infilled frames with one to five bays using the percentage of the additional infill wall surrounding the short column and the number of spans as the parameters. Then the investigation is extended to a case of a multistorey building damaged due to short column effect during the 1998 Adana-Ceyhan earthquake in Turkey. The results show that the addition of the infill walls around the potential short columns is an effective way to significantly reduce the shear force.

  • The strengthening of concrete structures in situ with externally bonded carbon fiber is increasingly being used for repair and rehabilitation of existing structures. Carbon fiber is attractive for this application due to its good tensile strength, resistances to corrosion, and low weight. Generally bond strength and behavior between concrete and carbon fiber mesh(CFM) is very important, because of the enhanced bond of CFM. Therefore if bond strength is sufficient, it will be expected to enhance reinforcement effect. If insufficient, reinforcement effect can not be enhanced because of bond failure between concrete and CFM. This study is to investigate the bond strength of CFM to the concrete using direct pull-out test and tensile-shear test. The key variables of the experiment are the location of clip, number of clips and thickness of cover mortar. The general results indicate that the clip anchorage technique for increasing bond strength with CFM appear to be effective to maintain the good post-failure behavior.

Semiparametric support vector machine for accelerated failure time model

  • For the accelerated failure time (AFT) model a lot of effort has been devoted to develop effective estimation methods. AFT model assumes a linear relationship between the logarithm of event time and covariates. In this paper we propose a semiparametric support vector machine to consider situations where the functional form of the effect of one or more covariates is unknown. The proposed estimating equation can be computed by a quadratic programming and a linear equation. We study the effect of several covariates on a censored response variable with an unknown probability distribution. We also provide a generalized approximate cross-validation method for choosing the hyper-parameters which affect the performance of the proposed approach. The proposed method is evaluated through simulations using the artificial example.

The effect of non-persistent joints on sliding direction of rock slopes

  • In this paper an approach was described for determination of direction of sliding block in rock slopes containing planar non-persistent open joints. For this study, several gypsum blocks containing planar non-persistent open joints with dimensions of $15{\times}15{\times}15cm$ were build. The rock bridges occupy 45, 90 and $135cm^2$ of total shear surface ($225cm^2$), and their configuration in shear plane were different. From each model, two similar blocks were prepared and were subjected to shearing under normal stresses of 3.33 and $7.77kg/cm^{-2}$. Based on the change in the configuration of rock-bridges, a factor called the Effective Joint Coefficient (EJC) was formulated, that is the ratio of the effective joint surface that is in front of the rock-bridge and the total shear surface. In general, the failure pattern is influenced by the EJC while shear strength is closely related to the failure pattern. It is observed that the propagation of wing tensile cracks or shear cracks depends on the EJC and the coalescence of wing cracks or shear cracks dominates the eventual failure pattern and determines the peak shear load of the rock specimens. So the EJC is a key factor to determine the sliding direction in rock slopes containing planar non-persistent open joints.

Experimental research on masonry mechanics and failure under biaxial compression

  • This study aimed to develop a simple and effective method to facilitate the experimental research on mechanical properties of masonry under biaxial compressive stress. A series of tests on full-scale brick masonry panels under biaxial compression have been performed in limited principal stress ratios oriented at various angles to the bed joints. Failure modes of tested panels were observed and failure features were analyzed to reveal the mechanical behavior of masonry under biaxial compression. Based on the experimental data, the failure curve in terms of two orthotropic principal stresses has been presented and the failure criterion of brick masonry in the form of the tensor polynomial has been established, which indicate that the anisotropy for masonry is closely related to the difference of applied stress as well as the orientation of bed joints. Further, compared with previous failure curves and criteria for masonry, it can be found that the relative strength of mortar and block has a considerable effect on the degree of anisotropy for masonry. The test results demonstrate the validity of the proposed experimental method for the approximation of masonry failure under biaxial compressive stress and provide valuable information used to establish experimentally based methodologies for the improvement of masonry failure criteria.

Behavior of Failure on Agricultural Reservoirs Embankment by Riprap Reinforcement Method (Riprap으로 보강된 농업용 저수지 제체의 붕괴거동)

  • In this study, the large scale test was performed to investigate the behavior of failure on the embankment and spillway transitional zone by overtopping. The pore water pressure, earth pressure, settlement and failure behaviors according to several reinforcing method were compared and analyzed. The pore water pressure showed a small change in the spillway transition zone and core, indicating that the riprap and geotextile efficiently reinforced the embankment, but non-reinforcement showed a largely change in pore water pressure. The earth pressure by riprap and geotextile at upstream slope and bottom core increased rapidly with the infiltration of the pore water by overtopping. And the earth pressure at crest showed a smally change due to effect of the inclined core. A settlement by riprap showed a small change and the geotextile decreased a rapidly due to failure of crest. The width of failure by riprap at intermediate stage (50 min) showed a largely due to sliding of crest. But, the width and depth of the seepage erosion after the intermediate overtopping period (100 min) were very small due to the effect of riprap than geotextile and non-reinforcement which delayed failure. It has the effect that protect reservoir embankment from erosion in the central part. The pore water pressure at the spillway transition zone due to overtopping increased a rapidly in the case of non-reinforcement, but the reinforced methods by geotextile and riprap showed a smally change. Therefore, the reinforced method by riprap and geotextile was a very effective method to protect permanently and the emergency an embankment due to overtopping, respectively.

No Effect of IV cyclophosphamide in Children with Steroid Resistant Nephrotic Syndrome (스테로이드 저항성 신증후군 소아 3례에서 Cyclophosphamide 정맥투여요법의 치료실패)

  • Steroid Resistant Nephrotic Syndrome(SRNS) in children has poor prognosis and no effective therapy. In 1994, Ravi Elhence have reported that IV cyclophosphamide therapy was effective against SRNS of children. So, we evaluated the efficacy of IV cyclophosphamide in 3 children with biopsy proven steroid-resistant MCNS. And the result was the rapeutic failure. In conclusion, IV cyclophosphamide therapy wass not effective against SRNS of children.

The Effects of Service Failure Magnitude on Perceived Justice, Positive Emotion, and Revisit Intention in Restaurants -A Comparison of Korean and American Customer Perceptions- (레스토랑에서의 서비스 실패 정도가 회복 공정성, 긍정 감정, 재방문 의도에 미치는 영향 -한국 고객과 미국 고객 비교를 중심으로-)

  • The purpose of this study was to compare the effects of service failure magnitude on perceived justice, positive emotion, and revisit intention at restaurants between a collectivist culture(Korea) and an individualistic culture(United States). The results showed that the higher a customer's perception of service failure magnitude, the lower the perceived recovery justices in both cultures. The distributive, procedural, and interactional justices had a favorable effect on positive emotions in both cultures. Other findings revealed that interactional justice was the best predictor of revisit intention in Korea, while distributive justice was the best determinant of revisit intention in the United States. This research suggests that global restaurant managers need to understand how cultural orientation influences consumer perceptions of service failures in order to develop a highly targeted and effective service recovery.

Effect of Hemodialysis on Dogs with Acute Renal failure Induced by Bilateral Ligation of the Ureter (양측 요관 결찰에 의해 유발된 급성신부전증 개에 대한 혈액투석 효과)

  • The present study was performed to clarity the effect of hemodialysis(HD) on acute renal failure induced by bilateral ligation of the ureter. HB was applied on 48 hours(1st HD) and 72 hours(2nd HD) after ligation of the ureter, respectively. Clinical signs including nausea, vomiting, diarrhea and depression were observed in all cases(6 heads), however, those symptoms were much improved after HD. Hematological values including WBC, PCV and PLT were increased after ligation of the ureter, however, those values were decreased by HD without significance, compared with those of before HD. In addition, serum BUN, creatinine, Ca and P levels were increased after ligation of the ureter, however, BUN(p<0.05), creatinine(p<0.05) and P(p<0.05) were significantly decreased by HD, compared with before HD, respectively. Considering above findings, it was thought that HD was effective for improvement of clinical symptoms of dogs with induced acute renal failure.
