Disinfection of microorganisms using UV light is widely used in the field of water supply and wastewater treatment plant, In spite of high germicidal effect and relatively clean by-product, UV disinfection has fundamental defeat that is accumulation of fouling materials at the interface of water and lamp sleeve. Non-contact type of UV photoreactor which can avoid this fouling generation was developed and the experimental performance evaluation of the system was carried out in this study. Log inactivation rate of E. coli was selected as a disinfection index. The concentration of E. coli of second clarifier effluent was $8.2{\times}10^1-8.2{\times}10^3$ colony per mL and was well inactivated by the non-contact type of UV photoreactor. Under the UV intensity condition of $2.1-2.5mW/cm^2$, E. coli removal rate was observed in the range of 54 - 95% when the HRT was increased from 10 to 52 seconds. Experimental results showed that log inactivation of E. coli was proportional to UV dosage and $200mJ/cm^2$ of UV dose is expected for the 2.0 log inactivation of E. coli from the second clarifier effluent. Between the two parameters of UV intensity and contact time which are consist of UV dose, UV intensity was 4 times more effective than contact time.
From September 1989 to June 1992,22 patients with nasopharyngeal carcinoma were treated in Asan Medical Center with an external beam of 60 Gy followed by a boost dose of 15 Gy HDR brachytherapy. There were 5 females and 17 males with median age of 44 years (range: 20-69 years). All patients were histologically confirmed and staged by physical examination, CT scan and/or MRI. By the AJCC TNM staging system, there were 2 patients with stge II (T2NO), 4 with stage III (T3NO, T1-3N1), and 16 with stage IV (T4 or N2-3). Four patients received chemotherapy with 5-FU and cisplatin prior to radiotherapy. All patients were followed up periodically by a telescopic examination and radiologic imaging study of CT scan or MRI with a median follow-up time of 13 months (range: 3-34 months). Twenty one patients showed a complete response ore month after completing therapy and one patient showed a complete response after three months. At the time of this analysis, seventeen patients remain alive without evidence of disease, but four patients developed distant metastasis and one patient died a month after treatment. The local control rate was $100{\%}$ in a median follow-up time of 13 months. The two year overall and disease free survival rates by the Kaplan-Meier method were $94{\%}$ and $67{\%}$, respectively. Serious radiation sequelae have not been observed yet. Although longer follow-up is needed, this retrospective analysis suggests that HDR brachytherap. given as a boost therapy for nasoharyngeal carcinoma may improve the local control. To reduce the incidence of distant metastasis, we need to develop a more effective systemic chemotherapy.
A 3-year old, female Cocker Spaniel dog was referred to Seoul National University Hospital for Animals with depression and vomiting. The dog was diagnosed as hypoadrenocorticism based on the typical electrolyte alteration and the result of adrenocorticotropic hormone (ACTH) stimulation test. Initial treatment with oral fludrocortisone at a dose rate of 0.02 mg/kg/q24h for 6 weeks period was ineffective at maintaining serum electrolyte concentrations within normal limits. Although a dose rate of oral fludrocortisone was significantly increased up to 0.06 mg/kg/q24h during 24 weeks period, the treatment was still ineffective. Moreover, the patient showed side effects related to the glucocorticoid excess including PU/PD, weight gain and lipemia. After alternation with desoxycorticosterone pivalate (DOCP, 2.2 mg/kg, IM) every 25 day, the clinical signs was disappeared and the electrolyte balance was maintain with no side effect. Therefore, DOCP may be suggested as an effective drug in canine hypoadrenocorticism uncontrolled with oral fludrocortisone.
Background: We strived to evaluate the status of nivolumab use and associated factors on the clinical efficacy of the drug. Methods: The study was retrospectively conducted in patients who had been administered nivolumab at least once at the cancer center of Seoul National University Hospital from June 2015 to April 2017. Data were collected from electronic medical records. A medication-use evaluation was performed based on the American Society of Health-System Pharmacists mediation-use guidelines. Results: Sixty-six of the 74 patients (89.2%) showed indications approved for nivolumab use by the Korean Ministry of Food and Drug Safety (MFDS; n=55) or the US Food and Drug Administration (FDA; n=11). Approximately 73.0% of the patients were administered the approved dose of 3 mg/kg but 25.7% were administered an unapproved fixed dose of 100 mg. The overall response rate was 21.7%, and the response rate of non-small cell lung cancer patients, who accounted for the largest number of indications, was 18.8%. Adverse reactions were found in 90.1% of the patients and were mostly mild (86%). The expression of programmed death-ligand 1 (PD-L1) was analyzed as a factor affecting treatment response (p=0.028, odds ratio [OR]=11.331). Conclusion: PD-L1 expression was found to affect treatment response. However, caution is required while using an unapproved dosage and in the absence of monitoring for effectiveness and safety. Therefore, an effective protocol or instruction manual for the proper use of nivolumab should be considered.
One of the Musty and earthy smell compounds in raw water is generally attributed to 2-methylisoborneol (2-MIB). It is well known that activated carbon and oxidants such as $O_3$, Cl $O_2$, are effective ways to control 2-MIB. In isotherm equilibrium experiments, 2-MIB in distilled water was much more adsorbed to the activated carbon(A/C) than raw water containing dissolved organic carbon (DOC). The Freundlich constants(k) of distilled water and raw water were 3.36 and 0.049, and 1/n values were 0.80 and 0.42, respectively. The 2-MIB residual rate were Y = $e^{-0}$.55x/~ $e^{-0}$.54x/ with Ozone( $O_3$) dose by 5 minutes contact time at the 241 and 353 ng/L initial concentrations. The 2-MIB residual rate were Y = $e^{-0}$.32x/~ $e^{-0}$.35x/ with Chlorine dioxide(Cl $O_2$) dose by 15 minutes contact time at the 89 and 249 ng/L initial concentrations. 2-MIB was decreased from 1911 ng/L to 569ng/L by post-ozonation(70%removal efficiency) and removal efficiencies of 2-MIB by the following 4 kinds Granular Activated Carbon(GAC) process such as coal base, coconut base, wood base and zeolite+carbon base were 95.8, 89.5, 88.4, and 93.7% respectively.ely.

To evaluate the growth rate of larval and pupal parameters of silkworm Bombyx mori fed with Silver Nanoparticles (AgNps) treated $MR_2$ mulberry leaves, the following works have been considered. The AgNp was synthesized by chemical reduction method, it was diluted by different concentrations such as 25%, 50%, 75% and 100% (without dilution). Fresh mulberry leaves (Morus alba L.) were sprayed by each concentration and were fed to silkworms, from $3^{rd}$, $4^{th}$ and $5^{th}$ instar, five feedings/day. Group $T_1$ larvae received $MR_2$ mulberry leaves sprayed with distilled water and served as control, group $T_2$, $T_3$, $T_4$ and $T_5$ larvae received 25%, 50%, 75% and 100% AgNps sprayed mulberry leaves, respectively. Silkworm larvae fed on M. alba ($MR_2$) leaves sprayed with 25% concentration of AgNps (group $T_2$) was significantly increased the larvae and cocoon length, width and weight as compared to those fed on control (group $T_1$) $MR_2$ mulberry leaves and other groups ($T_3$, $T_4$ and $T_5$). Hence, 25% AgNps dose was fixed as an effective dose. It has been observed from the present study that 25% AgNps treated (group $T_2$) leaves fed by silkworms have enhanced the larval and pupal growth and quantity of silk production than control.
The number of facilities using radiation generators increases and related regulations are strengthened, the establishment of a shielding management and evaluation technology has become important. The characteristics of the radiation generator used in previous report differ from those of currently available high-frequency radiation generators. This study aimed to manufacture lead, iron, and concrete shielding materials for the re-verification of half-value layers, tenth-value layers, and attenuation curve. For a comparison of attenuation ratio, iron, lead, and concrete shields were manufactured in this study. The initial dose was measured without shielding materials, and doses measured under different types and thicknesses of shielding material were compared with the initial dose to calculate the transmission rate on 50-300 kVp X-ray. All the three shielding materials showed a tendency to require greater shielding thickness for higher energy. The attenuation graph showed an exponential shape as the thickness decreased and a straight line as the thickness increased. The difference between the measurement results and the previous study, except in extrapolated parts, may be due to the differences in the radiation generation characteristics between the generators used in the two studies. The attenuated graph measured in this study better reflects the characteristics of current radiation generators, which would be more effective for shield designing.
Background: Rocuronium bromide has been evaluated as a mydriatic agent in birds, but the species applied were limited and the dose and effect were variable. Objective: This study aims to evaluate the efficacy of topical rocuronium bromide as mydriatics in 4 species according to horizontal palpebral fissure length: Feral pigeon (Columba livia), Common kestrel (Falco tinnunculus), Northern boobook (Ninox japonica), and Eurasian eagle owl (Bubo bubo). Methods: A total of 32 birds (8 for each species) were included as pre-releasing examination. Rocuronium bromide was instilled in one randomly selected eye of each bird based on palpebral fissure length criteria (0.5 mg/50 µL for pigeons, 1 mg/100 µL for kestrels and boobook owls, and 2 mg/200 µL for eagle owls). The contralateral eye was used as control and treated with normal saline. After instillation of the drug, pupil diameter, pupillary light reflex, intraocular pressure, heart rate, and respiratory rate were evaluated at 10 min intervals up to 180 min and at 30 min intervals up to 360 min. Results: Statistically significant mydriasis was obtained in all birds (p < 0.001). However, in boobook and eagle owls, marked mydriasis persisted until 360 min. Side effects including corneal erosion and lower eyelid paralysis were common, which was observed in 26/32 birds. Blepharospasm was also noted during this study. No systemic adverse signs were observed. Conclusions: Rocuronium bromide could be a good mydriatics option for 4 species of birds, however, further studies are needed to find lowest effective dose to reduce drug-related side effects.
As the importance of intervention has recently increased, interest in the health of medical staff performing the procedure is increasing. Existing radiation shielding devices have limited the operator's movement and have not been properly used due to the risk of infection, and adequate radiation shielding of the operator's gonads and furthermore, the entire area of the procedure room has not been achieved. An auxiliary shielding device was manufactured by attaching a Bismuth to the elbow support used in the procedure, and the radiation shielding effect was measured. As a result of the measurement, the average spatial dose rate decreased by about 64.8%, and the independent sample t-test analysis showed statistically significant below the significance probability (p<0.05). The use of an auxiliary shielding device is considered to be an effective shielding method that can shield the operator's gonads and reduce the radiation spatial dose rate of the entire area of the procedure room.
As the breast cancer rate is increasing fast in Korean women, people pay more attention to mammography and number of mammography have been increasing dramatically over the last few years. Mammography is the only means to diagnose breast cancer early, but harms caused by radiation exposure shouldn't be overlooked. Therefore, it is important to calculate the radiation dose being absorbed into the breast tissue during the process of mammography for a protective measure against radiation exposure. Because it is impossible to directly measure the radiation dose being absorbed into the human body, statistical calculation methods are commonly used, and most of them are supposed to simulate the interaction between radiation and matter by describing the human body internal structure with anthropomorphic phantoms. However, a simulation using Geant4 Code of Monte Carlo Method, which is well-known as most accurate in calculating the absorbed dose inside the human body, helps calculate exact dose by recreating the anatomical human body structure as it is through the DICOM file of CT. To calculate the absorbed dose in the breast tissue, therefore, this study carried out a simulation using Geant4 Code, and by using the DICOM converted file provided by Geant4, this study changed the human body structure expressed on the CT image data into geometry needed for this simulation. Besides, this study attempted to verify if the dose calculation of Geant4 interlocking with the DICOM file is useful, by comparing the calculated dose provided by this simulation and the measured dose provided by the PTW ion chamber. As a result, under the condition of 28kVp/190mAs, the Difference(%) between the measured dose and the calculated dose was found to be 0.08 %~0.33 %, and at 28 kVp/70 mAs, the Difference(%) of dose was 0.01 %~0.16 %, both of which showed results within 2%, the effective difference range. Therefore, this study found out that calculation of the absorbed dose using Geant4 Simulation is useful in measuring the absorbed dose in the breast tissue for mammography.
