순환정지 후 장기기증의 의료윤리적 문제 (Medical ethical problems of organ donation after circulatory death)

  • It is necessary to expand organ donations of the deceased, as more ethical problems are emerging from the increase in organ donations from the living. As the legal and ethical discussions on donation after circulatory death (DCD) as a field with the potential to expand such domestic organ donation are being held, the need for a social consensus is increasingly highlighted. Organ DCD refers to the donation of an organ from a patient whose spontaneous blood circulation has stopped due to cardiac death. In this study, we aimed to examine whether there are legal and ethical obstacles or medical uncertainties for expanding the practice of DCD. By law, death is recognized as cardiac death, and brain death is recognized as an exception for organ transplantation. Although circulatory death precedes brain death, this paper presents a discussion to begin the process of reaching an ethical and legal agreement on whether or not circulatory death can be recognized as death. Successful implementations of DCD, including Category III DCD, in other countries are currently being reviewed to introduce Category III DCD in Korea.

Role of Nuclear Factor-κB in female Breast Cancer: A Study in Indian Patients

  • Introduction: The nuclear factor ${\kappa}B$ (NF-${\kappa}B$) is a super family of transcription factors which plays important roles in development and progression of cancer. The present investigation concerns NF-${\kappa}B$ /p65 activity in human breast cancers with overexpression of ER, PR, HER-2/neu, as well as the significance of p65 expression with regard to menopausal status, stage, grade, tumor size, nodal status, and NPI of invasive ductal carcinomas in Eastern India. Materials and Methods: In this hospital based study 57 breast cancer patients attending a Breast Clinic of a reputed institute of Eastern India were assessed for p65 protein expression in breast tumor tissue samples by Western blotting. ER, PR and HER-2/neu expression was determined by immunohistochemistry. Results: NF-${\kappa}B$/p65 was significantly associated with advanced stage, large tumor size (${\geq}5$ cm), high grade, negative ER, negative PR, and positive HER-2/neu. High NF-${\kappa}B$/p65 expression was more frequent in patients with a high NPI ($NPI{\geq}5.4$, 84.6%) compared with low NPI (<5.4, 44.4%) and this association was statistically significant (p = 0.002). Conclusion: NF-${\kappa}B$/p65 overexpression was associated with advanced stage, large tumor size, high grade, and high NPI which are poor prognostic factors linked to enhanced aggressiveness of the disease. NF-${\kappa}B$/p65 expression implies aggressive biological behavior of breast cancer and this study validates significant association of NF-${\kappa}B$ /p65 overexpression with negative estrogen and progesterone receptor status and overexpression of HER-2/neu oncoprotein. In our good clinical practice, patients with NF-${\kappa}B$ positive tumors need to be treated aggressively.

근대시기 한국의 여의사 양성과정 성립 연구 (A Study on the Establishment of the Korean Women Doctor's Training Course in the Modern Period)

  • The Gyeongseong Women's Medical Training Center was created as the result of the efforts of our internal visionaries with meaningful foreign missionaries to cultivate female doctors, yet the systematic structure of the institution developed primarily out of Korean efforts. Koreans have tried hard to cultivate their descendants and the skills of the Korean people within this framework, challenging the oppression of the ruling class in a given environment, and the results have continued to this day. First, during the Early period (1890-1909), Korea began to establish women's education and the first female doctors were trained with the help of foreign missionaries. Second, during the Growth period (1910-1919), while it was difficult for women's education to be easily expressed during Japanese colonial era, the need for women's education was growing as part of the patriotic enlightenment movement, and female students who wanted to become doctors began to go abroad. In addition, during this period, the means to train female doctors in Korea was available, but this system was not recognized by the Japanese colonial government. Third, during the Preparatory period (1920-1928), the Gyeongseong Women's Medical Class, which gave practical training to female doctors, was established and centered on Rosetta Hall and female doctors who studied abroad. Fourth, a women's medical school was established during the Establishment period (1929-1938), which created a foundation for stable supply of professional women's medical personnel. In this article, we studied the process of women who were marginalized in education until they were trained as professional intellectuals, and we hope that it will help them understand the current women's education in Korea and draw directions in the future.

한의학교육에서 통합교육과정의 도입과 운영을 위한 고려 사항 (Considerations for the Introduction and Operation of an Integrated Curriculum in Traditional Korean Medicine Education)

  • This study considers the stages of curriculum development for the integrated curriculum of Pusan National University Graduate School & Hospital of Korean Medicine, and specifically the KAS2021 (announced in 2019), improvement measures for the curriculum of the College of Traditional Korean Medicine, and the case of the College of Medicine. The introduction of integrated curriculum in the College of Traditional Korean Medicine starts from the members (doers)' agreement. In the process of development, the organization that represents the members, the organization that sets up a goal and designs the curriculum, and the organization that executes them should fulfill their own roles. The stage of development and operation should have the support system for manpower, institution, administration, and finance. The curriculum (draft) should be concrete enough to be operated in reality. For the smooth operation of integrated education, it is necessary to secure more full-time teachers than before, and it is also necessary to have an organization fully in charge of monitoring and improving the operation. For the introduction and operation of integrated curriculum in Traditional Korean Medicine education, the members' agreement, institutional change, support system, and the cultivation of manpower for the operation/evaluation/development of curriculum should be considered.

의안(醫案)의 교육적 활용 방안에 대한 고찰 - PBL(Problem Based Learning)과의 접목 가능성을 중심으로 - (A Study on the Educational Utilization of Eui'an (醫案) - the Possibility of Combination between PBL (Problem Based Learning) and Eui'an -)

  • This study tries to review the possibility of combination between PBL (Problem Based Learning) and Eui'an (醫案). Recently in the field of Korean Medicine, people have been raising issues related to education of Korean Medicine. The point of issue is 'competence centered education of Korean Medicine' which has set the goal of education of Korean Medicine. To achieve the goal, PBL is considered as a very useful tool. But it is considered that there are limitations to direct application of PBL to Korean Medicine. This study aims to lay out the specific reason for the limitations and to search for solutions to overcome the limitations when applying PBL to Korean Medicine. This study suggests that the combination between PBL and Eui'an might be a sound solution to the aforementioned problem.

교수학습방법에 따른 의학전문대학원생의 HIV/AIDS 교육프로그램 효과 (The Effect of HIV/AIDS Education Program for Professional Graduate Medical School Students by Teaching-Learning Methods)

  • 본 연구는 교수학습방법에 따른 의학전문대학원생의 HIV/AIDS 교육프로그램의 효과를 확인하기 위해서 실시되었다. 이를 위해 의학전문대학원 학생을 대상으로 개발된 HIV/AIDS 교육 프로그램을 토의식 교수학습방법과 강의식 교수학습방법으로 각각 적용 한 후, HIV/AIDS 관련 지식과 태도 그리고 교육만족도에 미치는 효과를 확인하였다. 본 연구의 설계는 비동등성 사전 사후 유사 실험연구이며, 연구대상은 일개 대학 의학전문대학원 학생 총 116명으로, 각각 토의군 60명, 강의군 56명이다. 자료 수집은 2015년 8월부터 12월까지 실시되었으며, 수집된 자료는 SPSS WIN 19.0 프로그램을 이용하여 기술통계, ${\chi}^2$-test, One-tailed independent t-test, One-tailed Mann-Whitney U-test, One-tailed Wilcoxon signed-rank test, One-tailed paired t-test로 분석하였다. 본 연구결과 HIV/AIDS 지식, 태도 그리고 교육 만족도에 대하여 강의군과 토의군 두 그룹간의 차이는 통계적으로 유의하지 않았으나 두 그룹 모두 교육 전 보다 교육 후에 HIV/AIDS 지식과 태도점수가 통계적으로 유의하게 상승하였다. HIV/AIDS 지식과 태도를 효과적으로 개선시키기 위하여 교육 대상 및 목적에 적합한 교수학습방법을 선택할 필요가 있다.

가정전문간호사의 환자·가족교육 수행정도와 저해요인 (Performance and Impediments to Patient and Family Education among Home Health Care Nurses)

  • Purpose: This study aimed to provide basic data necessary to develop education programs and educational services for home care by investigating the degree of patient and family education among home health care nurses. Methods: Data collection was carried out with 145 people from 47 institutions that agreed to participate in the research. A total of 128 questionnaires were received, of which 122 were analyzed. Data were analyzed using SPSS 12.0. Results: The item on which education was most frequently delivered was intravenous injection speed control (66.4%), whereas the item requiring the longest teaching period was pressure ulcer care. The average degree of impediment perceived by home care nurses was 2.82 out of 5. Conclusion: Medical institutions should develop educational materials and programs that reflect the characteristics and degree of home health care needed. Repeated research, including that by medical institutions, on the degree and impeding factors related to patient and family education performance of home health care nurses should be conducted. Moreover, medical institutions should investigate the nursing and educational needs of patients and families who received home health care service.

종사상 지위와 가구 균등화 소득이 치과 의료 미충족에 미치는 영향 : 2018 한국의료패널 자료를 활용한 분석 (The effect of employment status and household equalization income on unmet dental and medical care needs : a study on the application of Korea Medical Panel Data 2018)

  • Objectives: This study used the 2018 Korea Health Panel Survey data to analyze factors affecting employment status and income relating to unmet dental and medical care needs. Additionally it investigated measures to reduce oral health inequality among various socioeconomic classes. Methods: Descriptive statistics for the subjects' unmet dental and medical care needs were calculated through chi-square test analysis, and multivariate logistic regression analysis was applied to identify factors affecting the unmet dental and medical care needs. The odds ratio and 95% confidence interval were calculated for each level. These data were analyzed using STATA 17.0 SE (64-bit) version, and the statistical significance level was set to p<0.05. Results: As a result of unmet dental and medical care needs according to general characteristics, the lower the education level (p<0.001), the higher the age (p<0.001) and the lower the household equalization income (p<0.024) and the smoker status (p<0.003) were, respectively. Factors that have a statistically significant impact on unmet dental and medical care needs were divorce, separation and bereavement (p<0.001) in individuals than in married persons, and being smokers than non-smokers (p<0.009). The frequency of unmet dental and medical care needs were found to be lower in the cases of a high school graduate than an elementary school graduate (p<0.018), and of higher household equalization income (p<0.001) than the lowest household equalization income, respectively. Conclusions: It was found that various factors such as age, education level, household equalization income, employment status, type of working hours type, and smoking status affect unmet dental and medical care needs.

의학교육 시기에 따른 의과대학생들의 정서 변화에 대한 현상학적 연구 (A phenomenological study on the emotional changes of medical students according to the phase of medical education)

  • The study aimed to understand medical students' experiences of emotional changes, including their method of adapting to experiences, and the effect of the experiences in shaping their identities. We interviewed 12 medical students who were finishing their 1-year clinical internship in 2016. Data on their opinions and reasons for emotional changes during their school life were obtained. The descriptive phenomenological approach was applied to analyze the interviews. Their stress came from disappointment in themselves, competitive environment, observing a change in their personalities, meeting their parents' expectations, and interpersonal relations. The interviewees adjusted to the medical study by exercising self-control in their studies and daily lives, by practicing self-acceptance and observing their state of mind, and by breaking free from the competition-driven environment and obsession with grades. In addition, they cultivated endurance and found external support. Finally, they achieved self-efficacy and were comfortable in their identity as medical students. They still had to address the stress from working relationships and the difficulty in balancing studies and life. The medical students' self-evaluation and compulsive tendencies increased during the medical course due to the burden of studies. They evolved by learning self-control and introspection and seeking ways to adapt. Understanding this growth process of medical students will improve student support in medical schools.
