Objective : Acute cerebral infarction is often accompanied by transtentorial herniation which can be fatal. The aim of this study is to determine the timing of surgical intervention and prognostic factors in patients who present with acute cerebral infarction. Methods : We reviewed retrospectively 23 patients with acute cerebral infarction, who received decompressive craniectomy or conservative treatment from January 2002 to December 2004. We divided patients into two groups according to the treatment modalities [Group 1 : conservative treatment, Group 2 : decompressive craniectomy]. In all patients, the outcome was quantified with Glasgow Outcome Scale and Barthel Index. Results : Of the 23 patients, 11 underwent decompressive craniectomy. With decompressive craniectomy at the time of loss of pupillary light reflex, we were able to prevent death secondary to severe brain edema in all cases. Preoperative Glasgow Coma Scale and loss of pupillary light reflex were significant to the clinical outcome statistically. With conservative treatment, 9 of the 12 patients died secondary to transtentorial herniation. The clinical outcomes of remaining 3 patients were poor. Conclusion : This study confirms the value of life-saving procedure of decompressive craniectomy after acute cerebral infarction. We propose that the loss of pupillary light reflex should be considered one of the most important factors to determine the timing of the decompressive craniectomy.
Objective: The purpose of this study was to observe the effects of Cnidii Rhizoma herbal-acupuncture solution(CR-HAS) at Joksamni($ST_{36}$) on arthritis induced by Collagen II in mice. Methods : The author performed several experimental items. The severity of arthritis, changes of mouse weight, size of the spleen and the degree of stenosis, changes of cytokine level, IgG, IgM and anti-collagen II, changes of immunocyte count, histological changes of the CIA mouse joint were analyzed. Results: 1. In the CR-HA, the arthritis index, the incidence of arthritis, the degree of joint edema was significantly decreased. 2. In the CR-HA, weight, spleen size and stenosis rate was low and maintained as the normal group was. 3. In the CR-HA, cytokine level, IgG, IgM and anti-collagen II were significantly decreased. 4. In the CR-HA, on changes of immunocyte count were maintained to the levels of normal group. 5. In histological changes of the CIA mouse joint, the cartilage destruction and synovial cell proliferation were decreased. Conclusions : These results suggest that CR-HA at the $ST_{36}$ has an important role to control the immune reactions and suppress inflammatory response on the collagen induced rheumatoid arthritis. This study can be a significant supporting evidence that CR-HA will be chosen to be the principal therapy for clinical practice of the rheumatoid arthritis in the future.
The effect of meal service for home-staying, urban elderly with low income on their mineral status and prevalence of clinical symptoms was evaluated. One hundred. One hundred and eighty three subjects were assigned to meal-served(served) and non-served(non-served)groups. A meal containing approximately one half of the RDA for energy, protein, calcium and iron was served as lunch every day to served group at a welfare center. Dietary, biochemical and clinical data were collected before and after 6 months of meal service and the changes of parameters were analyzed with paired t-test. served female showed significantly increased intake of calcium. The mean hemoglobin, serum iron, TIBC and serum copper of female were significantly increase with meal service. The proportion of anemic female compared to reference data on hemoglobin, hematocrit and serum iron were 45.5$\%$, 29.1$\%$ and 16.4$\%$ respectively, but were lowered to 18.2$\%$, 7.3$\%$, and 5.5$\%$ after 6 months of meal service. served male showed significant increase in MCHC, serum copper, and urinary Na. The urinary Ca/Creatinine, which is often used as an index of bone resorption, was decreased significantly in served group. The decrease in the proportion of served women suffering from edema of ankle and diarrhea was most apparent and less women reported feeling clinical symptoms of dizziness, constipation, difficulty in hearing, and coughing after meal service. The symptoms of benumbness of hands and feet and coughing was lowered most among men after meal service. Clinical symptoms of non-served elderly did not show improvement in feeling clinical symptoms except slightly decreased frequency in coughing of female and buzzing sound in the ears and diarrhea of male.

Dysfunction of gut immune regulation is involved in mucosal damage in inflammatory bowel disease (IBD). However, there is still no efficacious immune-regulator for the treatment of IBD. Alloferon is a novel immune-modulatory peptide that was originally isolated from infected insects. It shows anti-inflammatory effects by the regulation of cytokine production by immune cells and their activities. Therefore, we investigated the effect of alloferon in a mouse model of colitis using dextran sulfate sodium (DSS). Colitis was induced by administration of DSS in drinking water for 7 consecutive days. It was confirmed by the presence of weight loss, diarrhea, hematochezia, and colon contraction. Alloferon was injected 4 days after DSS administration. We found that alloferon improved the pathogenesis of IBD based on the reduced disease activity index (DAI) and colon contraction. Edema, epithelial erosion, and immune cell infiltration were found in mice administered DSS, but the phenomena were reduced following alloferon treatment. The plasma level of IL-6, a classical pro-inflammatory cytokine in colitis, was also decreased by alloferon. Moreover, alloferon inhibited the TNF-${\alpha}$-induced degradation and phosphorylation of $I{\kappa}B$ in Colo205 colon cancer cells. Taken together, these results show that alloferon has anti-inflammatory effects and attenuates DSS-induced colitis.
Objectives : The aim of this study is to investigate anti-atopic dermatitis effect using ato-tang.Methods : Ato-tang was external treatment to NC/Nga mice for 4 weeks, where atopic dermatitis was induced by DNCB at 1% and 0.4% for 3 weeks. Atopic dermatitis index score was measured using eye observation and picture evaluation. The histopathological change of dorsal skin was observed by hematoxylin and eosin (H&E) staining. Cytokines including IL-4, IL-5 and IL-13 were measured by Luminex or reverse transcriptase polymerase chain reaction (RT-PCR) analysis and immunoglobulin E (IgE) was measured by ELISA reader.Results : The dorsal skin of Ato-tang group showed decrease in erythema, pruritus, dry skin, edema, excoriation, erosion and lichenification level through naked eye observations. Immunoglobulin cell infiltration and the thickness of epidermis were significantly decreased in the dorsal skin compared to control. Production of Th2 cytokines (IL-4, IL-5, IL-13) and IgE level in serum were all significantly decreased, in comparison with control. In addition, mRNA expression level of Th2 cytokines (IL-4, IL-5, IL-13) in spleen was decreased, in comparison with control.Conclusion : The results indicated that external treatment of ato was improved skin barrier function in the symptoms of atopic dermatitis disease. Also, atopic dermatitis factors where cytokine as well as immunoglobulin E in serum and mRNA expression were decreased, respectively, in comparison with control. Therefore, we suggest that ato could be effectively used as a external therapeutic drug based on atopic dermatitis factors.
Objectives : The aim was to study the effect of bee venom pharmacopuncture therapy with different concentration on rheumatoid arthritis rat model. Methods : We enforced a bee venom pharmacopuncture therapy with different concentration on rheumatoid arthritis rat model by the intradermal injection of chicken type II collagen emulsified. 14 days after the onset of the rheumatoid arthritis rat model, a fixed volume of bee venom was daily injected to ST-35 acupoint in the rat's knee joint for 2-3 weeks. The hind paw volume, arthritic index, arthritic flexion pain test, pain threshold, and serum analysis (CRP, $PGE_2$, ALT, AST) were analyzed, and the expression profiles of COX-2, c-fos, and substance-P at the dorsal horn region of the spinal cord and subchondral bone of the knee joint were also analyzed by using the immunohistochemistry. Results : After the treatment of rheumatoid arthritis rats with bee venom pharmacopuncture, the paw volume of edema of arthritic rats were almost restored to the level of normal group, and behavior tests were very effective. Also the evaluation on the blood serum analysis was remarkable. COX-2, c-fos, and substance-P positive cells in the immunohistological section of dorsal horn region of the spinal cord and subchondral bone of the knee joints were significantly decreased. also the bee venom pharmacopuncture was effective to alleviate their rheumatoid arthritic inflammation cytokine inhibition as regards to the behavior tests and joint histological appearance. Conclusions : Based on the results in this study, bee venom pharmacopuncture with concentrated treatment condition was very effective in low fixed quantity and progressive low increased quantity.

Inflammatory bowel disease (IBD) is caused by dysregulation of colon mucosal immunity and mucosal epithelial barrier function. Recent studies have reported that lipoteichoic acid (LTA) from Lactobacillus plantarum K8 reduces excessive production of pro-inflammatory cytokine. In this study, we investigated the preventive effects of lysate of Lb. plantarum K8 in dextran sulfate sodium (DSS)-induced colitis. Male Sprague-Dawley rats were orally pretreated with lysate of Lb. plantarum K8 (low dose or high dose) or live Lb. plantarum K8 prior to the induction of colitis using 4% DSS. Disease progression was monitored by assessment of disease activity index (DAI). Histological changes of colonic tissues were evaluated by hematoxylin and eosin (HE) staining. Tumor necrosis factor-alpha (TNF-${\alpha}$), interleukin-6 (IL-6) levels were measured using enzyme-linked immunosorbent assay (ELISA). The colon mRNA expressions of TNF-${\alpha}$, IL-6, and toll like receptor-2 (TLR-2) were examined by quantitative real-time-transcription polymerase chain reaction (qPCR). Lysate of Lb. plantarum K8 suppressed colon shortening, edema, mucosal damage, and the loss of DSS-induced crypts. The groups that received lysate of Lb. plantarum K8 exhibited significantly decreased levels of the pro-inflammatory cytokines TNF-${\alpha}$ and IL-6 in the colon. Interestingly, colonic expression of toll like receptor-2 mRNA in the high-dose lysate of Lb. plantarum K8 group increased significantly. Our study demonstrates the protective effects of oral lysate of Lb. plantarum K8 administration on DSS-induced colitis via the modulation of pro-inflammatory mediators of the mucosal immune system.
Objectives : This study demonstrates the effects of Daejo-hwan on learning and memory impairment induced by L-NAME (75 mg/kg) treatment and on cerebral ischemic damage induced by middle cerebral artery (MCA) occlusion in rats. Methods : Daejo-hwan emulsion (73.3 mg/100 g/l ml) was administered to rats along a timed study schedule. The Moms water maze was used for learning and memory test of the rats. The MCA was occluded by using the intraluminal thread method. The brain slices were stained by 2 % triphenyl tetrazolium chloride (TTC) and 1 % cresyl violet solution. Infarct size, neuron cell number and size in penumbra was measured by using computer image analysis system. Results : 1. The escape latency of the Daejo-hwan treated group decreased significantly with respect to the control group. 2.The memory score of the Daejo-hwan treated group showed increase tendency, And the swimming distance was not different between the normal, the control, and the Daejo-hwan treated group. 3. The infarct size of the Daejo-hwan treated group decreased significantly with respect to the control group. 4. The total infarct volume of the Daejo-hwan treated group showed decrease tendency. And the brain edema index of the Daejo-hwan treated group decreased significantly with respect to the control group. 5. The neuron cell number and cell size in penumbra of the Daejo-hwan treated group increased significantly with respect to the control group. Conclusions : According to the above results, it is supposed that Daejo-hwan is clinically applicable to the vascular dementia.
The objective of this study was to evaluate the potential use of ginseng leaf as a cosmetic material. In this research, we employed enzymatic treated ginseng leaf by using Ultraflo L to improve the recovery of ginsenosides from the ginseng leaf and studied the biological activities and skin safety of the enzymatic treated ginseng leaf for use as a cosmetic material. The total ginsenoside contents of the non-enzymatic treated ginseng leaf (NEGL) and Ultraflo L treated ginseng leaf (UTGL) were 271 and 406 mg/g, respectively. The level of metabolite ginsenosides (sum of Rg2, Rg3, Rg5, Rk1, compound K, Rh1, Rh2, and F2) was higher in UTGL (93.1 mg) compared to NEGL (62.4 mg) in one gram ginseng leaf extract. The increase in amounts of ginsenoside types in UTGL compared to NEGL was generally 140% to 157%. UTGL exhibited relatively higher 2,2-diphenyl-2-picrylhydrazyl hydrate ($IC_{50}$, 2.8 mg/mL) and 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) diammonium salt ($IC_{50}$, 1.6 mg/mL) radical scavenging activities compared to NEGL (4.8 mg/mL and 2.2 mg/mL). The UTGL group showed normalized hydrogen peroxide, lipid peroxidation and visual wrinkling grade induced-UVB exposure. The UTGL did not induce any adverse reactions such as erythema and edema on intact skin sites; however, some guinea pigs treated with UTGL on abraded skin sites showed very slight erythema. The primary irritation index (PII) score of UTGL was 0.05 and it was classified as a practically non-irritating material (PII, 0 to 0.5). In skin sensitization tests with guinea pigs, UTGL had a positive rate of skin sensitization at 40%, and the mean evaluation score was 0.4.

Background: Nephrotoxicity is the major side effect in cisplatin chemotherapy. Previously, we reported that the ginsenosides Rk3 and Rh4 reduced cisplatin toxicity on porcine renal proximal epithelial tubular cells (LLC-PK1). Here, we aimed to evaluate the protective effect of ginsenosides Rk3 and Rh4 on kidney function and elucidate their antioxidant effect using in vitro and in vivo models of cisplatin-induced acute renal failure. Methods: An enriched mixture of ginsenosides Rk3 and Rh4 (KG-KH; 49.3% and 43.1%, respectively) was purified from sun ginseng (heat processed Panax ginseng). Cytotoxicity was induced by treatment of $20{\mu}M$ cisplatin to LLC-PK1 cells and rat model of acute renal failure was generated by single intraperitoneal injection of 5 mg/kg cisplatin. Protective effects were assessed by determining cell viability, reactive oxygen species generation, blood urea nitrogen, serum creatinine, antioxidant enzyme activity, and histopathological examination. Results: The in vitro assay demonstrated that KG-KH ($50{\mu}g/mL$) significantly increased cell viability (4.6-fold), superoxide dismutase activity (2.8-fold), and glutathione reductase activity (1.5-fold), but reduced reactive oxygen species generation (56%) compared to cisplatin control cells. KG-KH (6 mg/kg, per os) also significantly inhibited renal edema (87% kidney index) and dysfunction (71.4% blood urea nitrogen, 67.4% creatinine) compared to cisplatin control rats. Of note, KG-KH significantly recovered the kidney levels of catalase (1.2-fold) and superoxide dismutase (1.5-fold). Conclusion: Considering the oxidative injury as an early trigger of cisplatin nephrotoxicity, our findings suggest that ginsenosides Rk3 and Rh4 protect the kidney from cisplatin-induced oxidative injury and help to recover renal function by restoring intrinsic antioxidant defenses.
