Infections involving Echinostoma hortense (E. hortense) are considered to more severe than infections caused by other heterophyids. Although parasite expulsion by host immune responses attenuates the symptoms of infection, the detailed mechanisms of the host immune response need to be determined, especially in local immune responses involving cytokine and immunoglobulin expressions. We infected BALB/c mice with E. hortense and examined recovery rates together with expressions of multiple cytokines and immunoglobulins in the villi and crypts of the small intestine using immunohistochemistry. We observed a close correlation between worm expulsion rates and cytokine/immunoglobulin expressions in E. hortense infected mice. This study contributes to an understanding of the relationship between the immune response and parasite expulsion in hosts.
Two cases of human Erhinostoma hortense infection and their probable infection source were identified by prasiqantel (Distocide) treatment of the patients and by examining two kinds of fresh water fish which were eaten raw by them. The result of the research can be summarized as follows: 1. The patients, each aged 31 and 30, were residing in the same house in Wonju City, Kangwon Province. The first case was hospitalized due to epidemic hemorrhagic fever (E.H. fever) and the second case was healthy but had slight degree of abdominal pain and diarrhea from time to time. In the stool examination, eggs of 5. hortense ($114.3{\times}71.0{\mu\textrm{m}}$) average from the first case and $119.1{\times}68.3{\mu\textrm{m}}$ average from the second) were found. By administering single dose of praziquantel (10~15mg/kg) and purgation with magnesium salt to them. sin adults of 5. hortense were collected from the diarrheal stools of the second case. 2. By examining 8:k Moroco oxycephalus and 20 Carassius carassius which were captured at the place where the two patients had captured and eaten the fresh water fish, the metacercariae of Echinostoma sp. were found from 3 (3.5%) M. oxycephalus. 3. After the experimental infection of 3 isolated metacercariae to one albino rat three adults of 5. hortense were recovered. By the present study, the two patients revealing the echinostomatid eggs in their stools were proven to be infected with 5. hortense and to be the second and third human cases of this luke infection in Korea. Mcroco oxycephalus harboured the metacercariae of E. hortense and appeared to be a new second intermediate host.
This experiment was performed to examine the in vitro and in vivo affects of the two different haplotype strains of mice, BALB/c and C3H/HeN infected with Echinostoma hortense, and the manifestation of the profiles of cytokine in the splenocytes. In the in vitro experiment, the two mice's splenocytes were divided and stimulated with antigen of crude extracts and the antigen of excretory and secretory products of an adult warm and the manifestation of cytokine mRNA was verified with RT-PCR. As a result, the two different strains of mice both strongly manifested the Th2 cytokine rather than the Thl cytokine and in the case of the Th2 cytokine, the BALB/c mice manifested more strongly than the C3H/HeN mice. In the experiment using the ELISA method, the protem cytokine manifestation had the same result as the mRNA experiment. In the in vivo experiment, the mice was infected via oral route with the metacercaria of the Echinostoma hortense and the manifestation of cytokine was verified by RT-PCR and ELISA and the results were the same as the in vitro experiment. Therefore, in the two strains of BALB/c and C3H/HeN, the C3H/HeN showed a higher susceptivity to the Echinostoma hortense.
Echinostoma hortense (Digenea: Echinostomatidae) is one of the intestinal flukes with medical importance in humans. However, the mitochondrial (mt) genome of this fluke has not been known yet. The present study has determined the complete mt genome sequences of E. hortense and assessed the phylogenetic relationships with other digenean species for which the complete mt genome sequences are available in GenBank using concatenated amino acid sequences inferred from 12 protein-coding genes. The mt genome of E. hortense contained 12 protein-coding genes, 22 transfer RNA genes, 2 ribosomal RNA genes, and 1 non-coding region. The length of the mt genome of E. hortense was 14,994 bp, which was somewhat smaller than those of other trematode species. Phylogenetic analyses based on concatenated nucleotide sequence datasets for all 12 protein-coding genes using maximum parsimony (MP) method showed that E. hortense and Hypoderaeum conoideum gathered together, and they were closer to each other than to Fasciolidae and other echinostomatid trematodes. The availability of the complete mt genome sequences of E. hortense provides important genetic markers for diagnostics, population genetics, and evolutionary studies of digeneans.
As gastroduodenoscopy performed more frequently, case reports of human echinostomiasis are increasing in Korea. A Korean woman presented at a local clinic with complaints of abdominal pain and discomfort that had persisted for 2 weeks. Under gastroduodenoscopy, two motile flukes were found attached on the duodenal bulb, and retrieved with endoscopic forceps. She had history of eating raw frog meat. The two flukes were identified as Echinostoma hortense by egg morphology, 27 collar spines with 4 end-group spines, and surface ultrastructural characters. This report may prove frogs to be a source of human echinostome infections.

Echinostoma hortense (E. hortense) is an intestinal trematode with the highest infection rate in South Korea. However, the immune response against E. hortense infection has not been explained well. In the present study, we investigated the effect of treatment with cyclosporin A (CsA) and histamine receptor antagonists on the cytokine expression and mucosal goblet cells in E. hortense-infected C3H/HeN mice. The alteration of cytokine mRNA expression ($TNF-{\alpha},\;IL-l{\beta},\;IL-4\;and\;IL-5$), intestinal worm recovery rate and goblet cell responses were measured weekly from 0 to 5 weeks post-infection (P.I.) in the control and the following three drug-treated groups: CsA, hydroxyzine and cimetidine. Compared with the control group, the expression of $TNF-{\alpha}$, IL-4 and IL-5 mRNAs decreased in the CsA- and hydroxyzine-treated groups, but only IL-4 mRNA expression did in the cimetidine-treated group. Worm recovery rate was significantly increased in the drug-treated groups. Mucosal goblet cells and their mucin response significantly decreased in the CsA-treated group (P<0.01), but significantly increased in the cimetidine- (P<0.05) and hydroxyzine- (P<0.01) treated groups. These data suggest that CsA treatment inhibits production of Th1- and Th2-type cytokines which are necessary for the worm expulsion. Histamine receptor increases goblet cells and their mucin activation, although it remains to be elucidated whether it directly affects the worm expulsion period of E. hortense in C3H/HeN mice.
The tegumental ultrastructures oi Echinostoma hortense adults were observed by scanning electron microscopy. The worms of 4 weeks of age were harvested from albino rats experimentally infected with the metacercariae obtained from the loach. The results were as follows: 1. The worms were leaf-like and their anterior end portion, including oral sucker and head crown, ventrally curved to face posteriorly. The tegument of whole body was wrinkled transversely and covered with cobblestone-like cytoplasmic processes. 2. The oral sucker had roundly swollen (type I ) sensory papillae on the ventral half of its lip and mini-ciliated knob-like (type I ) sensory papillae, arranged in 2∼3 rows, on the dorsal outer surface. 3. Aspinous ventral sucker had many of type I papillae arranged in a circular band on its outer surface. The tegument around the genital opening was of similar feature to the ventral sucker, but sensory papillae were hardly found around the former. 4. Scale-like spines with broad base and round tip were distributed densely on the tegument anterior to the ventral sucker but they became sparse in posterior half of the ventral surface, finally to disappear at posterior extremity. A few number of type I papillae were observed on the ventral surface. The results suggest that the tegument of 5. hortense is similar to that of other echinostomes etapecially 5. revolutum. But the number and arrangement of collar spines, and/or the type and distribution of sensory papillae seem characteristic features of E. hortense differed from other echinostomes.
Echinostomatid eggs were found from a 21year old man, whose native village is Munkyong-gun, Kyongsangnam-do during fecal examination in October 1983. The eggs were detected again one week later concommitantly with the eggs of Clonorchis sinensis. He had no subjective symptoms related with these fluke infections. He was treated with praziquantel ($Distocide^{\circledR}$) 20 mg/kg in single dose and purgated with magnesium salt. One echinostomatid fluke and 4 adults of C. sinensis were collected from the diarrheal stools. After morphological observation, the echinostomatid fluke was identified as Echinostoma hortense Asada, 1926. This is the first record on human E. hortense infection in Korea.
This study was aimed to investigate the enzyme-histochemical localization and characteristics of alkaline and acid phosphatase extracted from adult of Echinostoma hortense. Using the Gomori calcium stain and the Gomori lead nitrate satin method, we found that the alkaline and acid phosphatases were localized mostly in the intestine, vitellaria and pharynx of Echinostoma hortense. The three isozymes of alkaline phosphatase and two isozymes of acid phosphatase were separated from Echinostoma hortense by electrophoresis. The isozymes of alkaline phosphatase were 145.9, 207.5, 220.8 kDa and the isozymes of acid phosphatase were 179.5 and 209.4 kDa. The activity of alkaline phosphatase was denatured completely after heating at 9$0^{\circ}C$ for 12 seconds. The optimum pH and temperature for activity of alkaline phosphatase were about pH 9 and 4$0^{\circ}C$, while the optimum pH for activity of acid phosphatase was about pH 5. The maximum activity of alkaline phosphatase was at 189 unit, but maximum activity of acid phosphatase was at 71 unit As the result from above, we observed that alkaline and acid phosphatases funtion mainly in the alimentary tract and vitellaria. Echinostoma hortense performs the parasitism in the intestine of host by using proper isozyme of phosphatase.
Eosinophils play an essential role in allergy reaction after parasite infection. To examine the immune reaction induced by eosinophils, we investigated the allergy reaction in BALB/c mice infected with Echinostoma hortense's metacercariae, as well as the effect of ketotifen, an anti-allergy drug, on eosinophil immune reaction in the villi of host intestine. The worm recovery rate was higher in ketotifen-treated mice than in untreated mice and the worms in ketotifen-treated mice survived longer than those in untreated mice. The antibody titer in the serum of ketotifen-treated mice was very low. Especially, Echinostoma hortense infection strongly increased serum IgE level and eosinophil infiltration into the villi of the mouse intestine. Ketotifen treatment suppressed eosinophil infiltration into the infected areas and inhibited IL-4 production. The reduced IL-4 production may be related with the reduction of IgE, IgG1 and IgG2 production. In conclusion, ketotifen inhibited eosinophil infiltration functioning in the allergy reaction induced by parasite infection and the expression of immunoglobulins and cytokines.
