Qualitative Study on Body Image and Appearance Behaviors in the Diet Center

  • The purpose of this study was to examine and understand body image, appearance behaviors, and eating disturbances among females in the Diet Center. Subjects for this research were 40 females enrolled at the Diet Center in Seoul. They were interviewed for this study using an interview schedule at September in 2004. As a result, most of the subjects (87%) were dissatisfied with their bodies, especially their lower body such as hip, thighs, legs, stomach. Subjects tended to use dieting and fasting (46.3%) as the most common appearance management behaviors in relation to the body parts. Make-up (32.9%) was also used as routine appearance behaviors, while 37.5% of subjects have had cosmetic surgery on eyes, nose and liposuction. Sixty percent of subjects had participated in Diet center programs more than twice. This may mean that society pressures women to have a slim body, which then will result in more self-confidence. Subjects tend to engage in unhealthy eating behaviors, such as 'fasting', 'inducing vomit', 'using diet pills or laxatives', and 'after chewing, spit out'. Based on these results, socio-cultural body image regarding ideal beauty would be discussed.

Eating Attitudes, Weight Concerns, Dietary Intake, and Menstruation Among Korean Female Elite Athletes

  • To examine the eating attitudes, weight concerns, dietary intake, and menstrual function of Korean female elite athletes, 109 subjects in seven sports disciplines (rhythmic gymnastics, synchronized swimming, badminton, volleyball, Taekwondo, field hockey, and soccer) responded to a questionnaire consisted of three parts; eating attitudes and behavior(Eating Attitude Test-26; EAT26), body weight perception (Body Dissatisfaction Index: BDI) and weight control behavior, and menstrual history and status. They also recorded three-day dietary intake. Body weight (43.6$\pm$4.3 kg) and body mass index (16.7$\pm$1.4 kg/$m^2$) in rhythmic gymnasts were lower than those in other athletes (P<0.05). EAT26 scores were not different among sport events (12.3$\pm$6.5 total), however, eating disturbances (EAT26 score$\geq$20) were highly prevalent in aesthetic athletes ($30\%$) than in others ($5\%$). More than half of the athletes perceived themselves overweight and four fifth of the athletes desired to reduce weight about 4.4 kg. The gymnasts consumed the lowest caloric intake (1028:t371) while the volleyball players did the highest (2995$\pm$342 kcal/d) (P<0.05). The BDI score was not different among sport events. Three fourth of the athletes experienced weight control, and the most frequently used weight reduction method was exercise followed by using robber suits, diet, and sauna. About $40\%$ of the subjects reported irregular menstrual cycles, but menstrual dysfunction ($\leq$6 menses/yr) was only $5\%$. Generally, the Korean female elite athletes desired to reduce weight from their current body weight. No differences in eating attitude and body dissatisfaction were noticed among athletic disciplines. However, eating disturbances were highly prevalent in aesthetic athletes who also reported much less energy intake than the recommend daily energy intake. It appeared that weight reduction methods were not properly practiced in these population. Menstrual dysfunction was minimally reported.

  • This study explored the eating attitudes, dieting habits, weight perception and exercise behavior of Korean women. Self-reported questionnaires were administered to 724 Korean college students. Compared to males, female students felt themselves overweight and dissatisfied with their body and desired to lose weight although both groups were within the range of the national standard. Female students dieted more, while males exercised more. Eighteen percent of females showed disturbed eating behavior. The score for disturbed eating behavior was highly related to weight perception in the female students. These results suggest that Korean college women have a high prevalence of eating disturbances without having actual weight problems. A majority of the women desired to lose weight which may be due to the misperception of their body weight and fatness. However, they did not adopt appropriate strategies such as doing regular exercise to reduce their body weight.

  • Objectives: Eating disorders are serious mental illnesses characterized by disturbances in eating behaviors with related thoughts and emotions. The objective of this study was to report a patient with an eating disorder who was treated using a combination of Korean medicine psychotherapy and traditional Korean medicine. Methods: Psychological assessment scales, including Beck Depression Inventory, Beck Anxiety Inventory, Bulimia Test, and Eating Attitudes Test-26, were used to evaluate the severity of the patient's psychological and behavioral symptoms. The patient underwent treatment with Korean medicine psychotherapy and traditional Korean medical therapies to alleviate both physical and psychological discomfort. Results: Following treatment, the patient's eating disorder symptoms were reduced with improvements in psychological assessment scales. Conclusions: This study demonstrated that treating a patient with an eating disorder using Korean medicine psychotherapy and traditional Korean medical treatments could lead to improvements in both eating disorder symptoms and psychological assessment outcomes.

Nutritional status of children with cerebral palsy according to their body mass index percentile classification

  • Purpose: Malnutrition in children with cerebral palsy (CP) is a significant factor affecting their adequate growth and development. This study aimed at conducting surveys and evaluating the dietary intake of children with CP according to their BMI classification and to thereby highlight the dietary factors affecting the nutritional status of these children. Methods: A total of 16 children were enrolled between the age of four and twelve. These subjects were further classified into three groups, namely underweight, normal and obese, with 6, 8, and 2 children in each group, respectively. The general characteristics, motor disturbances, body composition, feeding problems, eating habits, nutritional intake, dietary variety, and food frequency for children with CP were evaluated. Results: It was observed that motor disturbances tended to increase in underweight children with CP. A significant decrease (p < 0.05) in disturbances related to oral feeding was observed with an increase in obesity. The pattern of eating habits revealed that subjects in the underweight group consumed unbalanced meals, while those in the obese group tended to consume larger meals at a faster pace. The feeding disturbance data revealed that those in the underweight group could not prepare their meals while the obese group had the problem of overeating and consuming an unbalanced diet (p < 0.05). Conclusion: It is necessary for both children with CP, who have a high degree of disability, and their caregivers to take lessons on adequate nutrient intake to prevent malnutrition. Moreover, it is necessary for the caregivers and children with CP having a low degree of disability to take lessons on providing and consuming a balanced diet and to focus on the intake of sufficient calcium in order to prevent obesity.

식사도구 사용능력에 따른 뇌성마비아동의 식생활 (Eating Patterns of Children with Cerebral Palsy by Table Utensil-Handling Skills)

  • The purpose of this study was to investigate the eating patterns of children with cerebral palsy having motor disturbances as well as stiffness. The food habits, nutritional status, and snack intakes of 1 to 7 year-old children with cerebral palsy were examined. The subjects were grouped into three categories according to their table-utensil handling skills: superior, normal, and inferior. The children in the superior group were significantly taller and heavier compared to children in the other two groups. The %EARs of folic acid and total calorie intake were insufficient in all three groups; however, their %EARs of other nutrients were fully sufficient. When comparing the children's intake frequencies and preferences for snacks, the superior group showed a greater likelihood to consume various kinds of snacks than the inferior group. And the inferior group disliked more kinds of snacks than the other two groups. It was also shown that the inferior group had a significantly higher tendency for problems in chewing and swallowing. These results indicate that the development of table utensil-handling skills is very important for the food intake and growth of children with cerebral palsy, and the better their table utensil-handling skills the greater their physical development. Thus, considering their preference and intake frequency, it seems necessary that children in the inferior group be provided a greater variety of snacks and foods to receive more calories.

Gap arthroplasty with active mouth opening exercises using an interocclusal splint in temporomandibular joint ankylosis patients

  • Background: Temporomandibular joint (TMJ) ankylosis during early childhood may lead to disturbances in growth and facial asymmetry and to serious difficulties in eating as well as in breathing during sleep. The purpose of this study is to describe the effectiveness of an interocclusal splint (IOS) for active mouth opening exercises in the treatment of TMJ ankylosis. Methods: A total of nine patients with 13 instances of TMJ ankylosis from 2008 to 2010 were included in this study, of which five patients were male and four patients were female. Five patients demonstrated unilateral ankylosis, while five patients showed bilateral symptoms. Ankylosed mass resection with coronoidectomy, fibrotic scar release, and resection of stylohyoid ligament calcification was performed with gap arthroplasty without an interpositional graft, and all patients were assessed for maximum mouth opening (MMO) during a mean 6.6-year follow-up period. Results: All patients were subjected to postoperative mouth opening exercises from the day of the operation with the help of an IOS, which was based on an impression taken during surgery. All patients were sufficiently comfortable moving their mandible according to the IOS's guiding plane and impingement, and satisfactory results were achieved, in which MMO was improved by 35 mm more than 6 years after surgery. Conclusions: Complete and adequate resection of the ankylosed mass and postoperative active mouth opening exercises are essential in the treatment of TMJ ankylosis. Moreover, a more comfortable mouth opening guide and interdigitation can be achieved using an IOS, and newly organized fibrosis in the gap space between the newly made resected condylar head and temporal fossa can be suggested.

임신 및 산후 우울증 (Depression during Pregnancy and the Postpartum)

  • 임신과 출산은 일부 여성들을 주요우울증 발병에 취약하게 한다. 산모의 10%에서 출산 후 수주 이내에 산후 우울증이 발병하며 이는 장차 아동의 인지적, 정서적 발달에 부정적 영향을 줄 수 있다고 알려졌다. 임신 중 우울증은 상대적으로 간과되어 왔지만 산후 우울증과 마찬가지로 흔하며 임산부의 정신병리가 태아의 생리 발달에 영향을 미친다는 증거가 축적되고 있어 주목을 받고 있다. 임신 및 출산과 관련된 우울증의 원인을 이해하기 위해서는 다양한 정신사회학적, 생물학적 위험 요인을 포괄적으로 고려할 필요가 있다. 임신과 관련된 우울증을 치료하기 위해 정신과 의사는 임신 및 치료 방법과 관련된 여러 가지 상황의 위험 혹은 이득을 포괄적으로 분석해야 하며 치료 방법을 능숙하게 결정할 수 있어야 한다. 임신 중 우울증에는 심리치료, 약물치료, 전기 충격 요법 등이 효과적이라고 알려져 있다. 산후 우울증을 치료하는 데에는 생물학적, 심리학적, 환경적 개입 등의 방법이 있으며, 구체적으로는 세로토닌 재흡수 억제제 등의 항우울제, 지지적 상담 등의 효과가 증명되었고, 증상이 심할 경우에는 Estrogen이 효과적이라고 알려져 있다. 한편, 임신 및 출산과 관련된 우울증의 신경내분비학적, 심리사회학적 병인을 규명하기 위한 연구들이 진행되고 있다. 향후 임산부의 산전 및 산후 관리에 있어서 임상각과 간의 통합적인 접근에 의한 우울증의 조기 진단 및 치료가 필요할 것으로 보인다.

간질환자(癎疾患者)에 대(對)한 보건학적(保健學的) 조사연구(調査硏究) (A Survey on Epilepsy Patients from Public Health Aspects)

  • Two interview surveys (1976 for 800 patients, 1978 for 200 patients) and an inventory survey through medical records(1978) for epileptic patients who have registered with the Korean Epilepsy Association (Rose Club) since 1971 were carried out by trained health workers in advance of survey. The data obtained from the analysis showed as follows: 1) 35.2% of patients were born in Seoul and 70. 6% of patients born elsewhere have lived in Seoul. 2) 50-60% of patients were 15-30 years cid. 3) 33.4%, 24,6 and 24.6 of all pupils and students went to elementary, junior and senior high schools respectively. 4) 21.2% of all pupils and students had dropped out of school and 51.4% of them were away from school because of epilepsy. 5) 3.1% of all patients had no job at all and students comprised 20.9% of patients followed by clerical work, commercial business and farming with about 6% in each group.6) Reasons given for unemployment such as dismissal (4.3%), quit (27.7%), hesitation to employ (42.5%)and discontinuance of job (25.5%) were basically due to epilepsy. 7) About half(46.2%) of all patients have become Christian since the Rose Club was a voluntary agency which has been sponsored by Christians. 8) 82. 6% of patients were diagnosed as having grand mal as the most. 9) 29.4% of patients explained aura with psychomotor disturbances and 13.8% with sensory disorders. 10) 46.3% of patients were attacked with seizures when they were tired and others(11.6% and 4.9%) after excessive eating and hunger. 11) Patients suffered more seizures in spring and summer rather than in autumn and winter and most patients had attacks 1-5 times a month. 12) For etiologic reasons of epilepsy, 35.5% of patients considered it was caused by psychological stress and 11.5% by trauma. Only 1.1% of patients considered it as having hereditary components. 13) 51% of patients were slow in caring for their own illnesses. They started to reat epilepsy after spending 5 years of time from the initial seizure. Only 5.4% of patients had received the modern anti-epileptic therapy right after the nitial seizure. 14) 62.1% of patients had no therapy or irregular or incomplete treatment before registration at the Rose Club Clinic. 15) Before registration at the Rose Club, 42.4% of patients received medical care. On the other hand, 25.6% went to herb doctors and 12.5% used to go to the drugstore in order to get anti-epileptic drugs. 16) 41. 6% of patients who took anti-epileptic drugs had more or less side-effects. Indigestion was the most common. 17) For continuation of treatment, 30.3% have received treatment for more than 5 years and the evident showed that epilepsy took a longer time to be cured. 18) Regarding the medical care received 44.2% of patients were very satisfied with effective care and 26.5% felt as good. 19) For attitudes toward epilepsy. 27.0% of patients and 68.2% of patients family were pessimistic. 20) 65.9% of patients had optimistic attitudes toward effectiveness of medical care of epilepsy. 21) 64.8% of wives and husbands had better understanding and cooperative for their spouses who had epilepsy. 22) 33.3% of patients were under-treated at the place of work. 23) 70.2% of patients wished to marry when they reach childbearing age and 63% wished to have children. Through the above results it is recommended for nation-wide epilepsy control that the sound and correct health education not only from health aspect but also from welfare aspect should be planned and implemented as soon as possible.
