뇌졸중 후 중추성 통증 환자에 대한 동서협진이 진통과 재활에 미치는 영향 (Effect of East-West pain treatment for Central Poststroke Pain on alleviation of pain and Rehabilitation)

  • Purpose : In order to study the effectiveness of East-West pain treatment on central poststroke pain(CPSP), we evaluated its effect on alleviation of pain and rehabilitation of CPSP patients who were treated with eletroacupuncture and west pain treatment for four weeks. Methods : Twenty four patients diagnosed by their pain characteristics of central pain form stroke were treated with sympathetic nerve block, gabapentin, amitriptyline, and electroacupuncture for four weeks. Pain intensity through the visual analogue scale(VAS), and improvements of mobility and rehabilitation through the modified Barthel index(MBI) and Rankin scale(RS), respectively, before and after pain treatment were also assessed. Results : VAS pain scores were significantly improved from $7.7{\pm}1.7$ to $4.4{\pm}2.0$ with pain treatment(p<0.05). In accordance with improvement of ain scores, RS and MBI scores ere also improved from $2.88{\pm}0.95$ to $2.13{\pm}1.01$ and from $83.0{\pm}16.9$ to $94.7{\pm}9.5$(p<0.05), respectively, with pain treatment(p<0.05). Conclusions : It was suggested that the active pain treatment was contributed to the rehabilitation of CPSP patients, resulting in improvement of quality of life of CPSP patients. Futhermore, East pain treatment in combination with West pain treatment may be useful modality to alleviate CPSP.

Cancer Pain Control for Advanced Cancer Patients by Using Autonomic Nerve Pharmacopuncture

  • Objectives: The purpose of this study is to report a case series of advanced cancer patients whose cancer pain was relieved by using autonomic nerve pharmacopuncture (ANP) treatment. ANP is a subcutaneous injection therapy of mountain ginseng pharmacopuncture (MGP) along the acupoints on the spine (Hua-Tuo-Jia-Ji-Xue; 0.5 cun lateral to the lower border of the spinous processes of vertebrae) to enhance the immune system and to balance autonomic nerve function. Methods: Patients with three different types of cancer (gastric cancer, lung cancer, colon cancer with distant metastases) with cancer pain were treated with ANP. 1 mL of MGP was injected into the bilateral Hua-Tuo-Jia-Ji-Xue on the T1-L5 sites (total 12-20 mL injection) of each patient's dorsum by using the principle of symptom differentiation. During ANP treatment, the visual analogue scale (VAS) for pain was used to assess their levels of cancer pain; also, the dosage and the frequency of analgesic use were measured. Results: The cancer pain levels of all three patients improved with treatment using ANP. The VAS scores of the three patients decreased as the treatment progressed. The dosage and the frequency of analgesics also gradually decreased during the treatment period. Significantly, no related adverse events were found. Conclusion: ANP has shown benefit in controlling cancer pain for the three different types of cancer investigated in this study and in reducing the dosage and the frequency of analgesics. ANP is expected to be beneficial for reducing cancer pain and, thus, to be a promising new treatment for cancer pain.

견비통의 한${\cdot}$양방 진료 및 협진의 경제성 평가 (Economic Evaluation of Eastern, Western and Collaborative Treatments for Patients with Frozen Shoulder Pain)

  • Objectives : The purpose of this study was to evaluate and compare the cost-effectiveness of Eastern, Western, and collaborative treatments and suggest a cost-effective approach for patients with frozen shoulder pain. Methods : Using the data of fifty-two patients, treatment effectiveness was measured by CSA, SPABI, and ROM scales and changes from the baseline score were evaluated. Data source for cost estimation was based on the national health insurance (NHI) payment system. Because the price in NHI was differentiated by health care institutions, five collaborative types were considered in assessing costs. Cost-effective ratios were computed for economic evaluation. Results : Compared with Eastern treatment, collaborative and Western treatments showed better effects on CSA scale after 4 weeks' treatment. The collaborative approach was also the most effective treatment on SPADI and ROM scales. The direct cost per patient receiving Eastern treatment was less than other treatments. In general, collaborative treatment dominated Eastern and Western treatments in cost-effectiveness an analysis. However, the cost-effectiveness ratio of Eastern treatment resulted in \9,000 compared to \29,000 of collaborative treatment on SPADI. Four different indicators of ROM scales resulted in different approaches as the cost-effective treatment. Conclusions : Considering cost-effectiveness ratios, collaborative treatment was the best treatment on CSh and SPADI scales after 4 weeks' treatment. As for ROM scales, the recommended alternatives were Eastern treatment for patients with abduction and adduction disabilities, Western treatment for those with flexion disability, and collaborative approach fir those with extension disabiliry.

대장암 환자의 암성 통증에 사암침법을 적용하여 통증 감소를 보인 증례 1례 (Cancer Pain Relieved by Acupuncture on a Patient with Colorectal Cancer: Single Case Report)

  • Most of patients who have cancer still suffer from various forms of pain that significantly impact their quality of life. Cancer pain is a difficult problem for clinicians because analgesic drugs do not always procure complete relief. Acupuncture has been used as complementary treatment of cancer pain. We report a patient with colorectal cancer who suffer from cancer pain. The patient underwent chemotherapy, radiotherapy and opioid therapy. However, the patient insisted on being treated for his continuous pain with oriental medical treatment. The patient was treated with acupuncture(especially SAAM acupuncture) for 2 months, additional herbal medication was administered. After those treatments, the patient showed remarkable improvement in terms of intensity and severity of cancer pain. Further study will be needed in order to determine the long-term efficacy of acupuncture on cancer pain.

유착성 관절낭염 치료에 있어서 한방 치료와 양방 치료의 임상적 고찰 - 동서협진 모델 개발을 위한 기초 연구를 중심으로 - (Clinical Observation of Western Medical Treatment and Acupuncture Treatment on Frozen Shoulder Patients)

  • Objectives : To observe the effect of acupuncture treatment and western medical treatment on frozen shoulder patients. Methods : 39 voluntary patients were randomly assigned to the Eastern treatment group(E group, n=22) and the Western treatment group(W group, n=17). The E group received acupuncture treatment on LI15, TE14, GB21 and Master Dong's acupuncture points, Shin-gwan and Gyun-joong, twice a week for 4 weeks. The W group received suprascapular nerve block, subacromial injection and trigger point injection, twice a week for 4 weeks. Both groups were instructed to practice self exercise during their daily lives. Evaluations were made before treatment and after treatment using Constant Shoulder Assessment(CSA), Shoulder Pain and Disability Index(SPADI) and the patient's satisfaction concerning the treatment was measured by Visual Analogue Scale(VAS). The obtained data were analyzed and compared. Results : The E group showed significant improvement(p<0.05) according to the CSA and SPADI. The patient's satisfaction scored 5.67 on a scale of 10. The W group showed significant improvement(p<0.05) according to the CSA and SPADI. The patient's satisfaction scored 7.73. But the difference between the two groups were insignificant according to CSA and SPADI. Conclusion : Both acupuncture and nerve block treatment significantly improved frozen shoulder. But the difference of the two treatments was insignificant.

  • Background : Central poststroke pain(CPSP) can occur as a result of lesion or dysfunction of the brain from stroke and may cause many difficulty in the social activities and daily life. In this study, we evaluate the clinical effectiveness of east-west medical management for CPSP through VAS(visual analogue scale), infrared themography, MBI(Moderfied Barthel Index) and Rankin scale. Methods : We treated thirty patients with oriental medical treatment method and western & oriental medical treatment method. Each group has fifteen patients of the CPSP. We evaluated their pain(characterizes tingling and burning sensation, aching, hyperalgesia, and allodynia) through VAS(visual analog scale) pain score, the skin temperature of pain site by infrared thermography and assessed their mobility & rehabilitation ability through MBI(Moderfied Barthel Index), Rankin scale before and after pain treatment. Results : The skin temperature of pain site was lower than non-pain site. The difference of skin temperature improved from $0.65{\pm}0.45^{\circ}C$ to $0.39{\pm}0.25^{\circ}C$ after oriental medical treatment and $0.68{\pm}0.54^{\circ}C$ to $0.27{\pm}0.24^{\circ}C$ after western & oriental medical treatment VAS scores improved from $7.9{\pm}1.4$ to $4.7{\pm}1.6$ after oriental medical treatment and $8.1{\pm}1.3$ to $4.6{\pm}1.2$ after western & oriental medical treatment. MBI scores improved from $61.40{\pm}13.58$ to $85.00{\pm}13.85$ after oriental medical treatment and $52.26{\pm}13.52$ to $77.13{\pm}12.04$ after western & oriental medical treatment. And Rankin scale scores improved from $3.33{\pm}0.72$ to $2.46{\pm}0.74$ after oriental medical treatment and $3.60{\pm}0.82$ to $2.66{\pm}0.81$ after western & oriental medical treatment Conclusion : The difference of skin temperature and Rankin scale scores more significantly improved after western & oriental medical treatment than oriental medical treatment. According to the results, we thought east-west medical management is very useful treatment for CPSP and rehabilitation of the patients with stroke.

침 치료가 오십견(五十肩) 환자의 적외선 체열촬영에 미치는 영향 (Clinical Study : Effect of Acupuncture on Digital Infrared Thermographic Imaging(DITI) in Frozen Shoulder Patients)

  • Objectives : To observe the effect of acupuncture treatment on Digital Infrared Thermographic Imaging(DITI) in frozen shoulder patients. Evaluations were made before treatment, after 1 week of treatment, after 2 weeks, 3 weeks and after 4 weeks of treatment. Constant Shoulder Assessment(CSA), Shoulder Pain and Disability Index(SPADI), Range of Motion(ROM) and the patient's satisfaction concerning the treatment was measured by Visual Analogue Scale(VAS). DITI was measured before treatment and after 4weeks of treatment. The obtained data was analyzed. Results : CSA, SPADI, VAS, adduction and extension showed significant(p<0.05) improvement. Abduction and flexion both improved after 4 weeks of treatment, but the improvement was statistically insignificant(p>0.05). DITI showed improvement but the improvement was insignificant(p>0.05). Conclusion : 4 weeks of acupuncture treatment significantly improved CSA, SPADI, VAS, adduction and extension in frozen shoulder patients(p<0.05). The improvement of abduction and flexion after 4 weeks of acupuncture treatment was insignificant(p>0.05). DITI results improved after 4 weeks of acupuncture treatment. But the change of thermal difference was insignificant(p>0.05).

항암화학요법과 병행한 한의기반 통합암치료를 통한 전이성 비소세포폐암 환자의 암성 통증 호전 증례보고 (A Case Report on the Improvement of Cancer Pain in a Patient with Metastatic Non-Small Cell Lung Cancer Through Herbal Medicine-based Integrative Cancer Treatment with Chemotherapy)

  • Objective: To demonstrate an improvement in metastatic cancer pain and a decrease in tumor size in a patient with non-small cell lung cancer. Method: A 53-year-old female patient diagnosed with metastatic non-small cell lung cancer in August 2022 underwent integrative cancer treatment (ICT) for two months to decrease the tumor size and improve back pain from bone metastasis. The patient underwent chemotherapy with ICT. Radiologic outcomes were assessed by chest, abdomen, and pelvis computed tomography based on the Response Evaluation Criteria in Solid Tumors (RECIST) protocol. Clinical outcomes were assessed using National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), Eastern Cooperative Oncology Group (ECOG), and a numeric rating scale (NRS). Result: During the two months of treatment, the NRS scores for back pain were improved, and the ECOG score improved from grade 2 to 1. The size and metabolic activity of the primary lung tumor decreased and underwent partial remission based on RECIST. No serious side effects of grade 3 or higher were noted on the NCI-CTCAE test. Conclusion: This case suggests that ICT may have a therapeutic effect for cancer pain and a synergetic effect with chemotherapy for metastatic non-small cell lung cancer.

쑥뜸치료(Moxibustion)로 호전된 비소세포성 폐암 환자의 암성 통증 1례 (A Clinical Case of Patient with Cancer Pain Treated by Moxibustion)

  • Background & Objective: Regardless of types of cancer, cancer pain means the pain occurred by tumor itself, caused by complication or side effects during treatment and which is neither directly related to tumor nor treatment. This case was performed to relieve the cancer pain and study the effect of moxibustion to the cancer pain. Method: For 27 days, the patient was treated by moxibustion($MANINA^{(R)}$), acupuncture, herbal medicine and narcotic analgesics. We planned to maintain moxibustion on 8 points of Bladder meridian line(BL13, BL17, BL42, BL43) for 20 minutes. The improvement of the symptom was evaluated by BPI/VAS, and the amount of narcotic analgesics applied. Results: The BPI/VAS was declined and amount of applying narcotic analgesics was decreased. Conclusion: It is suggested that moxibustion on BL13, BL17, BL42, BL43 is useful to relief of cancer pain in left flank area.

Bowel Perforation Treated with Acupuncture and Gami-Gamchogungang-tang: A Case Report

  • Objective: This case report details the successful management of bowel perforation through traditional Korean medicine. Often, emergency surgery is required due to potential complications, such as peritonitis. In this case, the patient had previously undergone a total colectomy, making surgical treatment complicated. Methods: The patient revealed persistent abdominal pain and over 20 instances of diarrhea per day. During the course of treatment, which included two hospitalizations and one outpatient visit, acupuncture treatment and herbal medicine were administered. Throughout the treatment period, the intensity of abdominal pain and the frequency of diarrhea gradually decreased. Results: At the end of treatment, a follow-up abdominal computed tomography (CT) scan showed no evidence of perforation. Additionally, blood tests revealed no abnormalities in liver or kidney function, confirming the safety of the treatments.
