Preparation and Management of Recipients in Bovine Embryo Transfer (수정란이식에 있어서 수란우의 준비와 관리)

  • Recipients are an integral part of embryo transfer and they are expensive to maintain as a good recipient. Recipient management is one of the most important components in a successful embryo transfer program. Management includes selection and subsequent care of the animals. A good recipient is basically on "open" cows or heffers whose reproductive tract is capable of receiving one or two embryos and incubating it to term. Potential recipients should be always be healthy and cycling normally ranging from 18 to 23 days. A thorough veterinary examination is recommended for candidate of recipients and cattle for questionable health should be eliminated from the recipient herd. Age and size of recipients are particularly important considerations when heifers are used, because of most embryos available for transfer are from large dams and sires. Body condition can influence a recipient's production, reproduction and health. Obese and underconditioned cattle should be avoided for use. Transfer of fresh embryos especially requires precise synchronization of donors and recipients. For estrus synchronization, PGF$_2$$\alpha$ is injected twice 10 to 12 days apart and short4erm progestagen treatment is applied to potential recipient cattle by coil into vagina (PRID) or ear implant (Synchro-Mate-B). The highest pregnancy results are achieved in recipients at exact synchrony with donors or 12 to 24 hr earlier than donors. Estrus detection is a major factor in breeding efficiency. High accuracy can be achieved by use of heat mount detection alds or by obserbing cattle for 30-minute peroids 3 times daily. Assay progesterone in milk can be used to discrIminate between pregnant and nonprenant recipients. Rectal palpation on day 35 to 70 after is an accurate and safe method of pregnancy diagnosis. Embryonic mortality in recipients may be associated with factors such as high environmental temperature and nutritional or lactational stress in early lactation period. Achievement of short calving interval requires concentrated management activity during the first 90 days following calving. Acceptable candidate for a recipient should be routinely vaccinated for infectious diseases. Proper nutritional programs according to NRC requirements and body condition scoring system for recipient cattles are vital to the ultimate success of an embryo transfer program.r program.

Factors Influencing Osteoporosis (폐경기 전.후 여성의 골다공증 영향요인)

  • Purpose: This study was carried out to identify factors influencing osteoporosis in women at pre- and post-menopausal state. Methods: The subjects of this study were 52 pre-menopausal and 125 post-menopausal women who were assessed of bone density in one general hospital. The data were collected through review of clinical records and telephone interviews using a questionnaire. Results: In the pre-menopausal women, the factors influencing osteoporosis were regular exercise (protective factor) and age (risk factor). Regression analysis showed that the factors attributable to osteoporosis included educational level, weight, age and number of pregnancy, accounted for 41.89% of the total variance. In the post-menopausal women. the factors influencing osteoporosis were age (risk factor), low educational level (risk factor), low economical state (risk factor), high parity (risk factor), and intake of coffee (protective factor). Regression analysis also showed that factors attributable to their osteoporosis included age, educational level, number of delivery, intake of the coffee, regular exercise, number of pregnancy and duration of oral pill intake, accounted for 37.41 % of the total variance. Conclusion: In pre-menopausal women, regular exercise was one of the most powerful determinant of their bone mass. Therefore, it is necessary to participate in a regular exercise program to maintain peak bone mass density prior to the onset of menopause. In post-menopausal women, increased age was the most influencing factor of their bone mass. Therefore, it is essential to establish early diagnosis and management of osteoporosis after menopause.

  • In 2008, the Joint United Nations Programme on HIV/AIDS (UNAIDS) estimated that about 430,000 children worldwide became infected with HIV, mostly through mother-to-child transmission (MTCT) during pregnancy, labor, delivery, or breastfeeding. The MTCT prevention program proved to be feasible and effective in reduction of perinatal HIV transmission. Three babies born from HIV-infected mothers were admitted to the National Medical Center in 2009. Only two women received antiretroviral (ARV) therapy during pregnancy, labor, and after delivery, and their infants received zidovudine (AZT) for 6 weeks. The outcome, after a follow-up period of 4 months to 16 months, was favorable in all patients. Thus, we emphasize the need for expansion of antenatal HIV screening of pregnant women, implemented for early HIV diagnosis and effective ARV therapy for reduction of perinatal HIV transmission.

Simple and Sensitive Electrochemical Sandwich-type Immunosensing of Human Chorionic Gonadotropin based on b-cyclodextrin Functionalized Graphene

  • The effective detection of human chorionic gonadotropin (HCG) is considerably important for the clinical diagnosis of both of early pregnancy and nonpregnancy-related diseases. In this work, a simple and sensitive electrochemical sandwich-type immunosensing platform was designed by synthesizing b-cyclodextrin (CD) functionalized graphene (CD/GN) hybrid as simultaneously sensing platform and signal transducer coupled with rhodamine b (RhB) as probe. In brief, GN offers large surface area and high conductivity, while CD exhibits superior host-guest recognition capability, thus the primary antibody (Ab1) of HCG can be bound into the cavities of CD/GN to form stable Ab1/CD/GN inclusion complex; meanwhile, the secondary antibody (Ab2) and RhB can also enter into the cavities, producing RhB/Ab2/CD/GN complex. Then, by using Ab1/CD/GN as sensing platform and RhB/Ab2/CD/GN as signal transducer (in which RhB was signal probe), a simple sandwich-type immunosensor was constructed. Under the optimum parameters, the designed immunosensor exhibited a considerable low analytical detection of 1.0 pg mL-1 and a wide linearity of 0.002 to 10.0 ng mL-1 for HCG, revealing the developed sandwich-type electrochemical immunosensing platform offered potential real applications for the determination of HCG.

Spontaneous ovarian hyperstimulation syndrome following a thawed embryo transfer cycle

  • This article reports a case of spontaneous ovarian hyperstimulation syndrome (OHSS) following a thawed embryo transfer cycle. OHSS, a potentially life-threatening condition, is an iatrogenic complication of controlled ovarian stimulation; therefore, it is very important to prevent and treat OHSS during treatment with ovulation-inducing agents. Despite our efforts to prevent OHSS, in this case, severe spontaneous OHSS occurred, which resulted in uncontrolled preterm labor and a preterm delivery and also persisted for 6 weeks after delivery. Freezing all embryos cannot entirely prevent the development of OHSS because OHSS can occur spontaneously. Although spontaneous OHSS remains a rare event, females with a history of OHSS may have an elevated risk for spontaneous OHSS. We suggest closely monitoring cases of pregnancy following thawed embryo transfer for early diagnosis of spontaneous OHSS and the use of conservative management.

  • Gestational choriocarcinoma is a highly malignant tumor which arises from the trophoblast of human pregnancy. This tumor develops early pulmonary metastasis and the most common pattern of pulmonary metastasis is discrete multiple nodules. But occasionally solitary pulmonary metastasis occurs. Authors presented three cases of choriocarcinoma presented with different types of solitary pulmonary metastases with review of literatures. We emphasize the importance of careful obstetric history taking and screening of serum gonadotropin level in differential diagnosis of solitary pulmonary lesion especially among women who are from areas of high prevalence of trophoblastic neoplasia.

Seroprevalence of toxoplasmosis in Korean pregnant women

  • This study was performed in order to evaluate the sero-epidemiological status of toxoplasmosis in pregnant Korean women. Among 5,175 sera and 750 amniotic fluid samples obtained from pregnant women, 41 serum samples $(0.79\%)\;and\;10 (1.33\%)$ amniotic fluid samples tested positive for IgG antibodies by ELISA. Fifty one cases showing a score more than 0.25 on ELISA were tested for PCR reaction against the SAG1 gene. Only one case of the 51 ELISA positive cases exhibited a positive reaction on all tests. This case had a history of acute nephropyelitis during early pregnancy, but fortunately, had delivered a phenotypically healthy baby. In this study, the seroprevalence of toxoplasmosis in pregnant women was found to be comparatively low, consistent with previous reports from Korea. However our trials, performed with a variety of diagnostic tools, were considered to be useful for the precise diagnosis of congenital toxoplasmosis.

Bullae-Forming Pulmonary Metastasis from Choriocarcinoma Presenting as Pneumothorax

  • Gestational trophoblastic disease (GTD) is a condition of uncertain etiology, choriocarcioma, or placental-site hydatidiform moles, invasive moles, choriocarcinoma, and placental-site trophoblastic tumors. It arises from the abnormal proliferation of trophoblastic tissue and spreads beyond the uterus hematogenously. The early diagnosis of GTD is important to ensure timely and successful management and the preservation of fertility. We report the unusual case of a metastatic choriocarcinoma that formed bullae on the lung surface and presented as recurrent pneumothorax in a 38-year-old woman with elevated beta-human chorionic gonadotropin (hCG) levels. She underwent thoracoscopic wedge resection of the involved lung and four subsequent cycles of consolidation chemotherapy. No other evidence of metastatic disease or recurrent pneumothorax was noted during 22 months of follow-up. GTD should be considered in the differential diagnosis of spontaneous pneumothorax in reproductive-age women with an antecedent pregnancy and abnormal beta-hCG levels.

  • Developmental dysplasia of the hip broadly includes inadequate development of the hip joint involving the acetabulum or proximal femur, or both. Although ultrasonographic studies in neonates have greatly lowered the frequency of neglected or operatively treated cases, its sensitivity is less than desired. Hip dysplasia without subluxation is commonly diagnosed incidentally and strongly related to degenerative arthritis in females after the 4th decade. Hip dysplasia with subluxation shows symptoms through various periods, depending on its severity, especially for women with onset during pregnancy. A complete physical examination and early treatment for neonates are extremely important for obtaining satisfactory outcomes. To avoid underdiagnosis and to serve appropriate treatment on time, the authors recommend examining any suspicious hips in infants under two years of age. The study will discuss the diagnosis and primary treatment of developmental dysplasia of the hip.

Studies on use of milk progesterone EIA-kit for diagnosis of reproductive disorders and non-pregnancy in dairy cows (젖소의 번식장애 및 비임신진단을 위한 Milk Progesterone 측정 EIA-kit의 이용에 관한 연구)

  • This study was carried out to investigate the effect, range of practice, and propriety for diagnosis of early non-pregnancies and reproductive disorders by dairy cows' milk progesterone analysis used EIA-kit of home products. The results were summarized as follows : 1. During 2 to 6 months after artificial insemination, the results of milk progesterone measurement by Home-kit and Auto ELISA reader-kit with pregnant dairy cows(152 heads) certified by rectal palpation were revealed, in Home-kit, 145 heads(95.4%) of positive reaction, 7 heads(4.6%) of quasi-positive, and 0 heads(0%) of negative among 152 heads and, in Auto ELISA reader-kit, 152 heads(100%) of positive reaction among 152 heads. 2. During 19 to 22 days after artificial insemination, the results of milk progesterone measurement by Home-kit, and thereafrer during 50 to 90 days after that, the results of pregnant test by rectal palpation were summarized as follows : 147 heads(82.1%) among 179 heads of positive reaction by Home-kit and 5 heads(31.3%) among 16 heads of quasi-positive were revealed pregnant cows by rectal palpation, and 42 heads(100%) among 42 heads of negative were non-pregnant. 3. During 19 to 22 days after artificial insemination, the results of milk progesterone measurement by Auto ELISA reader-kit, and thereafrer during 50 to 90 days after that, the results of pregnant test by rectal palpation were summarized as follows : 146 heads(86.9%) among 168 heads of positive reaction by Auto ELISA reader-kit and 6 heads(28.6%) among 21 heads of quasi-positive were revealed pregnant cows by rectal palpation, and 48 heads (100%) among 48 heads of negative were non-pregnant. 4. For the accuracy of the rectal palpation, Home-kit and Auto ELISA reader-kit were used in the cows of ovarian diseases. The results were following : in the cows of reproductive disorders expected negative milk progesterone, the accuracies of rectal palpation were the same 75.5%(40 heads among 53 heads) by Home-kit and Auto ELISA reader-kit, and in the cows of reproductive disorders expected positive milk progesterone, the accuracies of rectal palpation were 82.6%(19 heads among 23 heads) and 91.3%(21 heads among 23 heads) by Home-kit and Auto ELISA reader-kit, respectively, and the general accuracies of rectal palpation were 77.6%(59 heads among 76 heads) and 80.3%(61 heads among 76 heads) by Home-kit and Auto ELISA reader-kit, respectively.
