A Clinical Case Study on the Tinnitus with Sudden Sensorineural Hearing Loss (돌발성 난청을 동반한 이명 환자 1례에 대한 증례보고)

  • Tinnitus is noise from ear or head without any external sound stimulation and can cause hearing difficulties, psychologenic disturbances or many difficulties in everyday life. the etiology has yet been discussed and the management is also very difficult. Sensorineural hearing loss is defined as a sudden hearing impairment which was develope over a period of hours to days. I report one tinnitus case which accompanied with sudden sensorineural hearing loss. In the early and acute stage which judged as Dam-Wha, herbal treatment with Tong-myeong-ri-gi-tang was efficacious on the tinnitus. In the convalescent stage and judged as Sin-heo, herbal treatment with Ja-sin-tong-i-tang was efficacious on the tinnitus that accompanied with sudden sensorineural hearing loss.

  • Maxillary first molar is the key in normal occlusion. Mesial drifting of maxillary first molar result form early loss of second deciduous molar. Mesial drifted maxillary first molar was treated by headgear, Hawley appliance with screw, brasswire, etc. But, these appliance should be necessary for patients cooperation. Recently, several appliance for molar distalizing without patients cooperation has been introduced. We are reporting in this paper about distalizing of mesial drifted maxillary first molar because of early loss of deciduous second molar by open coil jig. Distalization of molar by open coil jig is predictable, rapid, painless method without mecesscity of patient cooperation.

A Study of Nutritional Assessment and Dietary intake after Gastrectomy of Gastric Cancer patients (위암환자의 위절제술 후 영양상태 평가 및 식사섭취도에 관한 연구)

  • The purpose of this study is to investigate the nutritional status and dietary intake of gastrectomized cancer patients. For this study, from 1993. 1 to 1993. 8, 50 postoperative gastric cancer patients were selected to examine anthropometric and laboratory data(Body Weight, Body Fat, serum Albumin, Total Lympocyte count), and dietary intake related symptoms. The results were 1) All anthropometric and laboratory data were significantly deteriorated by gastrectomy(s-Albumin, TLC. Body Fat : p<0.001). Weight loss of gastrectomized patients was 8.23$\pm$3.72% from admission to discharge. 2) In many gastrectomized cancer patients, preoperative dietary intake was decreased by abdominal discomfort, indigestion, early satiety, and anorexia. 3) Postoperative energy intake was 602$\pm$158㎉, and it is correspond to 31.18$\pm$.90% of daily energy requirement(1918$\pm$236㎉). The cause of poor oral intake is mostly fear, abdominal distension and fullness, and early satiety. In consideration of the fact that an inadequate energy intake was the main cause of the decreasing nutritional status, a careful nutritional care and dietary education is necessry after gastrectomy.

A Study on the Temperature Dependency Affecting Setting and Strength Development of Concrete Using Mineral Admixtures (혼화재 사용 콘크리트의 응결 및 강도발현에 미치는 온도의존성에 관한 연구)

  • This experimental study investigate temperature dependency affecting setting and strength development of concrete using mineral admixtures such as CKD, FA and BS. For the properties of setting at $5^{\circ}C$, setting time of concrete with mineral admixture was delayed about $3\~14$ hour compared with that of plain concrete. Use of CKD had a desirable effect on reducing setting retard under $5^{\circ}C$ because of $CaCO_3$ of CKD while use of FA and BS retarded setting time greatly. For compressive strength under $5^{\circ}C$, concrete with CKD had the most compressive strength in early age compared with the other mineral admixtures but exhibited slight strength loss in $-5^{\circ}C$ at 28days. Especially, concrete with FA and BS was observed in early stage at low curing temperature because of strength loss remarkably in $-5^{\circ}C$.

  • In this study, it is examined the properties of early strength of concrete mixed with polycarboxylate superplasticizer. For this experiment, it is analyzed that the slump and strength properties according to the mixture factors, compared with cements and superplasticizers of each company and curing temperature($15,\;20^{\circ}C$). (1) The slump loss of concrete used polycarboxylate superplasticizer(rapid strength type) showed $0.5\~1.5cm$, it is judged that slump loss according to the time lapse can be minimized. (2) The performance of polycarboxylate superplasticizer kept up consistency and accelerated strength development. it is possible to reveal 12MPa within $18\~20$hours at $20^{\circ}C$ curing, but impossible within 24hours at $15^{\circ}C$. (3) It is necessary to studies about rapid strength development in the low temperature.

Storage Life Investigation of Diverse Grape Cultivars (포도품종에 따른 저장성 연구)

  • Storage life of grape cultivars including Kyoho(Inzuka), which were imported from Japan and are cultivated after adoption as farming application, were examined. The stu요 was accomplished with grapes packed by PE film under storage temperature $0^{\circ}C$ and humidity 90%RH loss of weight, Weight loss and occurrence of abnormal fruit increased in proportion to storage days. Abnormal fruits of Kyoho(izuka), North Black , and Seto giants increased rapidly after 60 days of storage, resulting in slight deterioration of commercial value. Similarly, this phenomenon of Campbell early , Takasumi , and Aki Queen occurred after 70 days of storage. External appearance decreased rapidly after 60 days of storage, resulting in 7.0 index of Takatsuma and Aki Queen , 3.0-3.6 of North Black and Seto Giants, and 5.0 of Campbell Early, kyoho (izuka0, and takasumi after 80 days of storage. Soluble solide content and titratable acidity showed a trend to be decreased with lapse of storage. Sugar/acid ratio shwoed much fluctuation between different grapes. Possible storage days, in view of more 70% merchandise berry ratio, were 90 days of Takatsuma and Aki Queen , 70 days of Campbell Early, 60 days of Takasumi , kyoho(Iizuka), and Seto giants, below 60 days of North Black.

Investigation of Likelihood of Cracking in Reinforced Concrete Bridge Decks

  • One of the biggest problems affecting bridges is the transverse cracking and deterioration of concrete bridge decks. The causes of early age cracking are primarily attributed to plastic shrinkage, temperature effects, autogenous shrinkage, and drying shrinkage. The cracks can be influenced by material characteristics, casting sequence, formwork, climate conditions, geometry, and time dependent factors. The cracking of bridge decks not only creates unsightly aesthetic condition but also greatly reduces durability. It leads to a loss of functionality, loss of stiffness, and ultimately loss of structural safety. This investigation consists of field, laboratory, and analytical phases. The experimental and field testing investigate the early age transverse cracking of bridge decks and evaluate the use of sealant materials. The research identifies suitable materials, for crack sealing, with an ability to span cracks of various widths and to achieve performance criteria such as penetration depth, bond strength, and elongation. This paper also analytically examines the effect of a wide range of parameters on the development of cracking such as the number of spans, the span length, girder spacing, deck thickness, concrete compressive strength, dead load, hydration, temperature, shrinkage, and creep. The importance of each parameter is identified and then evaluated. Also, the AASHTO Standard Specification limits liveload deflections to L/800 for ordinary bridges and L/1000 for bridges in urban areas that are subject to pedestrian use. The deflection is found to be an important parameter to affect cracking. A set of recommendations to limit the transverse deck cracks in bridge decks is also presented.

Management of Premature Loss of Primary Molars with Flexible Denture (탄성의치를 이용한 유구치 조기 상실의 치료)

  • Early loss of a primary second molar can cause mesial drift or tilting of the permanent first molar. We present a case of the early loss of the mandibular primary molars in a 5-year-old girl. The patient required extraction of the mandibular left primary first and second molars due to folliculitis on the successors. A flexible denture was used for eruption guidance of the permanent first molar and the recovery of masticatory function simultaneously.

Evaluation of Three Pork Quality Prediction Tools Across a 48 Hours Postmortem Period

  • Numerous reports have evaluated the predictive ability of carcass probes for meat quality using measurements taken early postmortem or near 24 h. The intervening time period, however, has been largely ignored. In this study, the capacity of three probes [pH, electrical conductivity (EC), and grading probe light reflectance (GP)] to predict pork longissimus muscle quality (drip and cooking losses, Warner-Bratzler shear, $L^*$, n = 30) was evaluated at 45 min, 90 min, 3, 6, 12, 24, and 48 h postmortem. The strongest relationships were observed between cooking loss and 6 h EC and GP ($R^2$ = 0.66, 0.72), and $L^*$ and GP ($R^2$ = 0.57-0.66, 12-48 h). pH was most valuable early postmortem ($R^2$ = 0.63, 90 min with cooking loss). GP at 6 h most effectively ($R^2$ = 0.84) predicted a two factor (cooking loss+$L^*$) meat quality index. Results emphasize the predictive value of measures taken between 3 and 12 h postmortem.

Weight Variation in Term Newborns Hospitalized during Early Postnatal Period (고위험만삭아의 생후 초기 체중변이)

  • Purpose: This study was performed to explore the weight variations in high-risk term newborns hospitalized during the early postnatal period. Methods: A retrospective explorative study was performed with 64 term newborns who were hospitalized in the NICU after birth. Data on daily weight, birth information, and clinical features such as phototherapy, placements, nutritional status were reviewed through medical records for 14 days of life. General Linear Model, GLM was applied to analyze the weight variation by clinical features of these high-risk term newborns for 14 days of life. Results: Newborns at 40 weeks of gestation showed little weight loss during the few days after birth then steadily gained weight to 7.6% at the 14th day. Infants born at 37-39 gestation showed little weight gain for 14 days though the weight loss itself was not apparent. As well, return to birthweight was not observed in newborns with phototherapy, infants placed on a warmer or infants having gastrointestinal dysfunction for 14 days of life. Conclusion: Even for term newborns, physiologic weight loss may not be warranted even if newborn is born at less than 40 week of gestation, or with high-risk conditions that warrant admission to NICU.
