Extracorporeal shock wave lithotripsy (ESWL) with adjunctive oral litholytic therapy has proven to be a useful treatment in selected patients with gallbladder stones. To study the effect of ESWL on gallbladder dynamics, $^{99m}Tc-DISIDA$ hepatobiliary scintigraphy was done for 25 patients with symptomatic gallstones and 10 normal controls. Of these 25 patients, 15 were treated with ESWL and adjunctive oral litholytic agents (ESWL group) and 10 were treated only with oral litholytic agents (UDCA group). After overnight fast and gallbladder visualization on a routine hepatobiliary scintigraphy with 7mCi of $^{99m}Tc-DISIDA$, subjects were given fatty meal and imaged with a gamma camera interfaced to a computer (1 frame/minute for 70 minutes). A gallbladder time-activity curve was generated and latent period (LP), ejection period (EP), ejection fraction (EF) and ejection rate (ER) were calculated. ESWL group were studied before, 1day after and 2weeks after ESWL, and WDCA group were studied before and 2weeks after starting oral medication. Mean basal EF was significantly reduced in patients but other parameters were not reduced. In ESWL group, mean EF and mean ER at lday after ESWL were reduced. In 3 of them, gallbladder was not visualized at all. Two weeks after ESWL, however, all parameters were recoverd to basal level. In UDCA group, all parameters were not changed significantly during medication. We can conclude that ESWL has such immediate adverse effect on gallbladder dynamics as reducing contractility and nonvisualization of gallbladder but it has no long-term effect.
The advantages and disadvantages of the various models of crushed ESWL (Extracorporeal Shock Wave Lithotripsy) and their various side effects due to pre-treatment have been reported. We look for the appropriate intensity of the shock wave in the electromagnetic ESWL treatment of patients with lower Inferior Ureter Stones and measure the total running time of C-arm saw. This study is based on the January-June 2014 launch of 65 patients of C university hospital located in Gyeongbuk, who conducted ESWL without pre-treatment. Patients are composed of 48 male and 17 female which were more common in men, while the most common age is 50s. The occurrence of lower urinary tract stones were left and they were more absent than 5mm in size in 39 of the most common. Optimal intensity is one of the suitable intensity of pain, and it is possible to switch the strength of impact to C if one appeals pain. In addition, the C-arm of the total operating time showed $241.73{\pm}30.37$ seconds, which is the size of the lower urinary tract that showed a significant difference (p <0.05). Therefore, ESWL treatment without pre-enforce treatment, the standard for the impact frequency and impact strength depending on the site of ureteral stones is required. The standard for total operating time of C-arm generated for ureteral stones is needed in order to reduce radiation exposure, and the standard for the total operating time for the criteria is needed depending on the site in order to minimize the exposure.
The aim of this study was to determine whether Hounsfield units (HUs), volume, and various void parameters can predict stone fragility in extracorporeal shock wave lithotripsy (ESWL). HU, volume, porosity, number of voids/stone volume, and void distribution of 30 struvite stones were estimated using helical computed tomography (CT) and micro-CT. The number of shock waves necessary for full fragmentation was accepted as a measure of the stone fragility in ESWL. The correlations between the number of shock waves and the HU, volume, porosity, and number of voids/stone volume were examined. The number of shock waves of the two groups according to the void distribution was also compared. Stone volume correlated with the number of shock waves. Shell-patterned struvite stones were significantly less susceptible to fragmentation in ESWL than non-shell-patterned struvite stones. Stone volume and void distribution may be predictors of the outcome of ESWL treatment.
The effectiveness of extracorporeal shock wave lithotripter for the therapy of calculus has been well known in the field of urology. There are many studies about the performance of that and the influence into human body. Among them, it is an impertant issue that cavitation is always deal with shock wave. A medium of the shock wave is related to the cavitation phenomenon. In this paper, therefore, we analysis the spectra of radiated sounds and the break efficiency on focal region due to the medium of shock wave. The results show that the cavitation bubbles produce a harmful on the break efficiency and the stability of the radiated sounds due to the ESWL.
Symptoms of caliceal diverticular stones are commonly associated with pain, recurrent urinary tract infection and hematuria. The aim of this study is to select the proper patient for the application of more successful extracorporeal shock wave lithotripsy(ESWL) as a treatment of caliceal diverticular stone. 16 patients with caliceal diverticular stones were treated with ESWL, and all patients had single caliceal diverticulum. The diagnosis of caliceal diverticulum with stones was made by intraveneous pyelography to all patients. On these intravenous pyelogram, we also classified diverticular type, whether the diverticular neck is connected with urinary tract patently, diverticular site and stone number and size. All patients were followed after ESWL by plain film of the kidneys, ureters and bladder and interviewed. Of all patients 44% was shown stone-free completely, also 83% was rendered symptom-free. All patients whose diverticular neck connected with urinary tract patently on the intraveneous pyelogram became stone- free. Of solitary stone 60% and multiple stones(more than 2) 17% became stone free. All patients rendered stone-free became symptom-free, and of patients with residual stones 44% became symptom-free. The patients with infection before ESWL 75% had residual stones, of these patients 33% had slightly flank pain, and 25% of patients with stones recurred become stone-free. We propose that more successful ESWL for patients with caliceal diverticular stones select satisfactory patients including that the diverticular neck is connected with urinary tract patently, solitary stone and no infection simultaneously.
It is an important issue that cavitation is always deal with shock wave. During operation with ESWL(Extracorporeal Shock Wave Lithotriptor), the cavitation bubbles are generated at the focal region. This phenomenon is an important problem because the cavitation bubbles have a harmful effect on the disintegration ability and the human body. In this paper, therefore, we investigate the relation between the cavitation and the radiated sound due to the ESWL.

Survey was carried out on the acoustic outputs from 12 shock wave fields produced by the 10 extracorporeal shock wave lithotriptors whose technical documents are available, among the 33 devices approved by the Ministry of Food & Drug Safety (MFDS).The results show that the acoustic outputs (P+, P-, efd, and E), critical to the therapeutic efficacy and the patient safety, are largely different between the devices. The maximum values of P+, P-, efd, and E vary up to 2.08, 3.72, 3.89, and 15.98 times, respectively. The acoustic output parameters are not thoroughly provided in the technical documents, and some of data (eg. efd) are suspected to be abnormal outside usual ranges. The large device to device differences in the shock wave outputs are likely to undermine equivalence between the ESWL devices approved for the same indication. To verify the reliability of the data in the technical documents of the approved devices and to confirm if the acoustic outputs from the devices in clinical use are the same as those in their technical documents, an authorized test laboratory should be available. A postapproval monitoring led by the regulatory agency is suggested to maintain the acoustic outputs from the ESWL devices that suffer from degrading in performance due to aging.
In this paper, the relation between the vibration and the sound radiated due to the piezoelectric ESWL (Extra-corporeal Shock Wave Lithotripter) is examined And the relation between the focus and the vibration of the objects is examined. The same experiments with the objects that can be breton are done and the relation between the vibration and the break efficiency of the phantom is experimentally investigated. These results show that the relativity between the power of the peak frequency and the break efficiency can be confirmed.

This paper investigates the information related to the radiated sounds during treatment using the extracorporeal shock wave lothotriptor(ESWL). First, we observe the vibration of the objects from the radiated sounds and extract the information about the change of the peak frequency from the relation between the vibration and the radiated sounds. Next, we observe the change of the peak frequency according to the position of the focus and demonstrate that it is more difficult to vibrate objects exactly at the focal point than above or below the focus of the ESWL device. Lastly, we analyze the relation between the cavitation and the radiated sounds and examine the benefiting condition for the object fragmentation.
