Long-term cardiac composite risk following adjuvant treatment in breast cancer patients

  • Purpose: Cardiotoxicity is a serious late complication of breast cancer treatment. Individual treatment risk of specific drugs has been investigated. However, studies on the evaluation of the composite risk of chemotherapeutic agents are limited. Methods: We retrospectively analyzed the medical records of breast cancer patients who received adjuvant treatment and had available serial echocardiography results. Patients were assigned to subgroups based on chemotherapy containing anthracyclines (A), anthracyclines and taxanes (A+T), and radiotherapy (RT). The development of cardiac disease and serial ejection fraction (EF) were reviewed. EF decline up to 10% from baseline was considered grade 1 cardiotoxicity and EF decline >20% or absolute value <50% was considered grade 2 cardiotoxicity. The most recent medical records and echocardiography results over 1 year of chemotherapy completion were also reviewed. Late cardiotoxicity was defined as a lack of recovery of EF decline or aggravated EF decline from baseline. Results: In total, 123 patients were evaluated. A small reduction in EF was observed after chemotherapy in both chemotherapy groups. There were no significant differences between groups A and A+T in EF decline following chemotherapy. We could not find any differences in composite risk between the chemotherapy groups and the RT group during follow-up. Late cardiotoxicity was seen in 15.45% of patients. During follow-up, three patients were diagnosed with dilated cardiomyopathy. Conclusion: There was no significant composite risk elevation following adjuvant treatment of breast cancer. However, late cardiotoxicity was considerable and further research in this direction is necessary.

Reciprocal effect of ethyl formate and phosphine gas on two quarantine pests, Tetranychus urticae(Acari: Tetranychidae) and Myzus persicae(Hemiptera: Aphididae)

  • Fumigation of fruits and vegetables during quarantine and pre-shipment(QPS) treatment should be effective with a shorter fumigation time to minimize phytotoxicity. In this research study, a shorter fumigation time, 2 hours exposure which is shorter than that of the current commercial fumigation procedures using a lower dose of ethyl formate (EF) mixed with phosphine (PH3) on strawberry was investigated. The reciprocal effect between EF and PH3 against nymphs and adult Myzus persicae (Sulzer) and Tetranychus urticae (Koch) was evaluated. In addition, L(Ct)50 and L(Ct)99 of EF only and EF mixed with PH3 were analyzed at 5℃ and 20℃. The synergistic ratio (SR) of L(Ct)50 and L(Ct)99 for the nymph and adult stages of M. persicae were >1.0, which indicated a synergistic effect between EF and PH3. However, the SR values of L(Ct)50 and L(Ct)99 of the nymph and adult stages of T. urticae were ≤1.0 indicating that there was no synergistic effect between the two fumigants against T. urticae. Our results showed that the reciprocal effect between EF and PH3 has different effects on M. persicae and T. urticae. This could be attributed to the biological and physical differences between the class Arachnida and Insecta. The synergistic effect between EF and PH3 against M. persicae within a shorter exposure period and without phytotoxicity on fruits and vegetables will significantly benefit the horticultural industry.

좌심실기능에 따른 관상동맥우회술의 위험인자 (Risk Factors of Coronary Artery Bypass Grafting According to Ventricular Function)

  • .저자들은 1994년 7월부터 1996년 6월까지 관상동맥우회 술을 시행한 환자 103명을 심실구축력 40%를 기준 으로 심실기능저하군 24명과 심실기능정상군 79명으로 분류하여 환자의 술전,수술변수 18개를 분석하였다. 분석결과 환자의 특성과 수술사망에 영향을 미치는 위험인자에 대해 다음과 같은 결과를 얻었다. 1. 술전 변수중 심비대(p=0.0012), 혈청 크레아티닌 이상(p=0.0166), IABP 실시(p=0.0096)는 기능저하군이 기능 정상군보다 유의하게 높은 빈도를 보였다 2. 수술변수중 기능저하군은 수술적응증에 있어서 심근경색후 협심증의 빈도가 많았고(p=0.00003), 내흉동맥 의 사용은 유의하게 적었다(p=0.00416). 3. 조기사망률은 기능저하군이 기능정상군보다 약간 높았으나 통계적으로 유의한 차이는 없었다(기능저하군 8.3%, 기능정상군 5.1%, p=0.5492). 4. 합병증발생률은 기능저하군 50%(12/24), 기능정상군 33%(79/25)로 두군간에 유의한 차이는 없었다(p=0.1007). 5. 심실기능정상군에서는 연령(고령)(p=0.041)이 수술후 조기사망에 유의한 위험인자로, 심실기능저하군에서 는 연령(p=0.018)외에도 술전 IAaP 실시(p=0.0036), 고콜레스테롤혈증(p=0.00 7), 응급수술(p=0.0036)이 유의 한 위험인자로 작용하였다. 이상의 결과를 통해 심실기능이 저하된 관상동맥질환 환자는 심실기능이 정상인 환자에 비해 많은 술후 조기사망의 위험인자를 갖고있으나.술후 조기사망률과 합병증 발생률은 두 군간에 유의한 차이를 보이지 않 음으로써 심실기능이 저하된 환자에 있어서도 증상의 개선과 장기생존률의 향상을 얻기 위하여 좀더 적극적인 관상동맥우회 술의 실시가 필요할 것으로 생각된다.

In vitro 동물세포에서 GGEx18의 ethyl acetate 분획물에 의한 지방산 β-산화효소 유전자 발현의 조절 (Ethyl acetate fraction of GGEx18 modulates fatty acid β-oxidizing enzymes)

  • Objectives : This study was undertaken to investigate the effects of the GGEx18 ethyl acetate fraction (EF) on lipid accumulation and gene expression of fatty acid-oxidizing enzymes using 3T3-L1 adipocytes, C2C12 skeletal muscle cells, and NMu2Li liver cells. Methods : PPAR${\alpha}$, AMPK and UCPs transactivation was examined in NMu2Li hepatocytes, C2C12 myocytes, and 3T3-L1 preadipocytes using transient transfection assays. Results : 1. Compared with control, EF significantly increased the mRNA expression of VLCAD in 3T3-L1 adipocytes. 2. Compared with control, EF (0.1 ${\mu}g/ml$) significantly inhibited lipid accumulation in 3T3-L1 adipocytes. 3. EF significantly increased the mRNA expression of AMPK${\alpha}$1, AMPK${\alpha}$2 and PPAR${\alpha}$ in C2C12 skeletal muscle cells compared with control. 4. EF significantly increased the mRNA expression of genes involved in fatty acid ${\beta}$-oxidation, such as thiolase, MCAD, and CPT-1 in C2C12 skeletal muscle cells compared with control. 5. EF significantly increased the mRNA expression of UCP2 involved in energy expenditure in C2C12 skeletal muscle cells compared with control. 6. Compared with control, EF (10 ${\mu}g/ml$) significantly inhibited lipid accumulation in C2C12 skeletal muscle cells. 7. EF (10 ${\mu}g/ml$) significantly increased the mRNA expression of ACOX, HD, VLCAD and MCAD in NMu2Li liver cells compared with control. Conclusions : These results suggest that EF may prevent obesity by increasing the mRNA expression of mitochondrial fatty acid ${\beta}$-oxidizing enzymes in 3T3-L1 adipocytes, by not only regulating the fatty acid oxidation through activation of AMPK and PPAR${\alpha}$, but also increasing the UCP2 mRNA expression in C2C12 skeletal muscle cells, and by stimulating the mRNA expression of fatty acid-oxidizing enzymes in NMu2Li liver cells.

Calmodulin of Olive Flounder Paralichthys olivaceus : Cloning and Expression Analysis

  • Calmodulin은 $Ca^{2+}$ 결합단백질로서 생체 내에서 $Ca^{2+}$ 의존적 기작을 통하여 다양한 생물학적 기능에 관여한다. 본 연구에서는 넙치 Paralichthys olivaceus의 cDNA library로부터 Calmodulin cDNA를 분리 동정하였다. 염기 서열 및 아미노산 서열을 분석한 결과, 넙치 Calmodulin cDNA는 782개의 nucleotides로 구성되어 있고, 4개의 잘 보존된 $Ca^{2+}$결합 motifs (EF-I, EF-II, EF-III, EF-IV)를 가지는 149개의 아미노산 잔기를 전사할 수 있는 open reading frame을 포함한다. 또한 번역된 아미노산 서열은 인간, 쥐, zebrafish, 개구리의 Calmodulin 아미노산 서열과 100% 동일성을 보이며 보라성게, 침팬지의 Calmodulin 아미노산 서열과 각각 97, 99%의 동일성을 보인다. 넙치 Calmodulin 전사체는 뇌와 장 조직에서 많은 양이 발현되었고, 신장, 아가미, 눈, 근육, 피부, 지느러미에서도 발현이 관찰되었다. 또한 넙치 Calmodulin 전사체는 수정 후 7일째의 발생 초기 단계 시료에서도 발현되어 수정 후 34일째까지 그 발현이 서서히 증가하였다. 이상의 결과들로부터 넙치 Calmodulin은 넙치의 발생 초기 단계에서 필요한 단백질로 생각되며 아마 항상성 유지에 중요한 역할을 할 것으로 예상된다.

오수유 메탄올 추출물이 Helicobacter pylori 병원성 관련 유전자 발현에 미치는 영향 (Effect of Evodiae fructus Methanol Extract on Virulence-Related Genes' Expression of Helicobacter pylori)

  • 위에 군집을 형성하는 그람 음성 박테리아인 Helicobacter pylori는 위암을 일으킬 수 있는 1급 발암 인자로, H. pylori의 성장을 억제하거나 제거하는 물질을 탐색하는 연구가 꾸준히 진행되어 왔다. 오수유는 운향과에 속하는 오수유의 열매를 일컫는 생약으로 한방에서는 설사 및 복통에 사용되던 약재이다. 오수유 추출물이 H. pylori의 성장을 억제한다는 보고는 있었으나 병원성 요인에 어떤 영향을 주는지에 대한 연구는 보고된 바가 없었다. 본 연구에서는 각기 다른 메탄올 농도에서 추출한 오수유 추출물의 H. pylori 성장 억제 효과를 비교하였으며 또한 오수유 추출물이 H. pylori의 병원성 인자에 미치는 영향을 연구하였다. 95% 메탄올 오수유 추출물이 가장 낮은 최소저해농도(MIC) 값을 보였으며, 이 95% 추출물을 대상으로 하여 MIC 미만 농도에서 H. pylori의 병원성 인자의 발현 변화를 확인하였다. 95% 메탄올 오수유 추출물은 H. pylori의 주요 병원성 인자인 cagA, vacA 그리고 ureB의 mRNA 및 단백질의 발현을 억제하였으나 흥미롭게도 ureA의 발현은 증가시켰다. 하지만 H. pylori의 배양액과 세포내의 암모니아의 량이 현저히 낮아졌음을 확인하였고, 이는 오수유 추출물이 ureB의 mRNA 및 단백질의 발현을 억제하여 H. pylori의 urease 활성을 억제한다는 것을 확인하였다. 오수유 추출물의 병원성 인자 조절 기전에 대해서는 좀 더 연구가 필요할 것으로 보이나, 본 연구 결과는 오수유 추출물이 H. pylori로 인한 부정적 영향을 줄여줄 것으로 생각한다.

Direct-fed Enterococcus faecium plus bacteriophages as substitutes for pharmacological zinc oxide in weanling pigs: effects on diarrheal score and growth

  • Objective: Effects of direct-fed Enterococcus faecium plus bacteriophages (EF-BP) were investigated as potential substitutes for pharmacological ZnO for weanling pigs. Methods: Dietary treatments were supplementations to a basal diet with none (NC), 3,000-ppm ZnO (PC), 1×1010 colony-forming units of E. faecium plus 1×108 plaque-forming units (PFU) of anti-Salmonella typhimurium bacteriophages (ST) or 1×106 PFU of each of anti-enterotoxigenic Escherichia coli K88 (F4)-, K99 (F5)-, and F18-type bacteriophages (EC) per kg diet. In Exp 1, twenty-eight 21-day-old crossbred weanling pigs were individually fed one of the experimental diets for 14 days and euthanized for histological examination on intestinal mucosal morphology. In Exp 2, 128 crossbred weanling pigs aged 24 days were group-fed the same experimental diets in 16 pens of 8 piglets on a farm with a high incidence of post-weaning diarrhea. Results: None of the diarrheal score or fecal consistency score (FCS), average daily gain (ADG), gain: feed ratio, structural variables of the intestinal villus, and goblet cell density, differed between the EF-BP (ST+EC) and NC groups, between EF-BP and PC, or between ST and EC, with the exception of greater gain: feed for EF-BP than for PC (p<0.05) during days 7 to 14 (Exp 1). In Exp 2, ADG was less for EF-BP vs PC during days 0 to 7 and greater for EF-BP vs NC during days 7 to 14. FCS peaked on day 7 and declined by day 14. Moreover, FCS was less for EF-BP vs NC, did not differ between EF-BP and PC, and tended to be greater for ST vs EC (p = 0.099). Collectively, EF-BP was comparable to or slightly less effective than PC in alleviating diarrhea and growth check of the weanling pigs, with ST almost as effective as PC, when they were group-fed. Conclusion: The E. faecium-bacteriophage recipe, especially E. faecium-anti-S. typhimurium, is promising as a potential substitute for pharmacological ZnO.

생산 가능한 토지면적산정기법을 이용한 지속성 평가에 대한 연구: 제주도 지역을 중심으로 (The Assessment of the Sustain ability on Human Activities through Ecologically Productive Land Calculated by the Ecological Footprint of Cheju Island)

  • The object of this study is the introduction and the application of Ecological Footprint(EF) for sustainability of regional people activities. It is a tool for the evaluation of specific projects through various lifestyles or consumption to area of broad policy and budgets. But in Korea there is no assessment for consumption level of human activities by EF. Therefore this study try to analyse ecologically productive land for human activities of Cheju-island in order to assess the sustainability in Cheju and compares with another industrized countries. We analyze the human activities level of Cheju-island then the EF is about 0.9(ha/cap). This value is not more than another countries like Canada and United States. EF assists in choosing technologies, policies and Life Cycle Assessment(LCA) which can perform a certain task with the smallest ecological footprint. It will help society to avoid collapse and move towards sustainability and ecological efficiency.

In vitro Antioxidant and Cytoprotective Activities of the Extract of Dangyuja (Citrus grandis Osbeck) Leaves

  • The antioxidant activities of the extracts of dangyuja (Citrus grandis Osbeck) leaves were evaluated. The highest phenolic content was obtained from the ethyl acetate fraction (EF) (202.1$\pm$0.8 mg GAE/g dried extract) and it exhibited the highest 1,1-diphenyl-2-picrylhydrazyl (DPPH) free radical scavenging activity. The cytoprotective effects of EF on oxidative damage induced by tert-butyl hydroperoxide (t-BHP) in a human hepatoma cell line, HepG2 cells, were investigated to understand the intracellular antioxidant mechanisms. Treatment of HepG2 cells with EF prior to oxidative stress was found to inhibit reactive oxygen species (ROS) generation, lipid peroxidation, and DNA damage in a dose-dependent manner. Gas chromatography-mass spectrometry (GC-MS) studies on EF resulted in tentative identification of 19 compounds representing 94.3% of the total content. Taken together, these results demonstrated that EF has excellent antioxidant activities and thus dangyuja leaves have great potential as a source for natural antioxidant which can be applied in food products.

여성의 무릎관절 기능 영향요인 (The Factor of Knee Joint Function in Women)

  • Purpose: The purpose of this is to identify the relationship among the age, body mass index(BMI) and exercise frequency(EF) with knee joint position sense in korean healthy women. Methods: Healthy women of 328 who participated in this study were tested knee joint position sense; reposition error was measured with a Myrin goniometer. Each reposition error was analyzed using descriptive statics, pearson correlation coefficients, and stepwise multiple regression. Results: The mean reposition error by age groups was significant decrease getting older. The mean reposition error by BMI groups was significant decrease getting higher. And the mean reposition error by EF groups was significant decrease getting lower. The knee joint position sense showed a significantly correlation with age(r=0.36, p=.00), BMI(r=0.34, p=.00) and EF(r=-0.50, p=.00). The most powerful predictor of knee joint position sense was EF. The reposition error according to stepwise multiple regression is $3.36+(-2.64){\times}EF+0.13{\times}age$, and account for 46%($R^2=0.46$) of the variance in the knee joint position sense. Conclusion: The older the women are, for prevent of knee injuries due to decreased joint position sense, regular exercise is essential factor.
