Professional Nursing Quality Assurance (전문직 간호의 질적보장)

  • 질적 보장은 전문직 종사자에 의하여 수혜받는 대상자를 보호하기 위하여 전적으로 필요한 프로그램이다. 이러한 질적보장을 통하여 전문직의 자율성과 책임을 튼튼히 할 수 있는 의무는 전문직 종사자 모두의 관심사가 되어야 한다. 미국에 있어서 질적 간호에 대한 관심은 이제 새로운 이유가 아니다. Beyers(1988)는 건강체계 지도자 사이에 가장 중요한 문제로 강조하는 것이 수혜자의 이용의 용이성(Access), 숫가(Cost) 그리고 질적인 제공(Quality)이 라고 피력 하였다. 미국 건강사업 평가자와 조정자(Regulator)들은 질(Quality)을 위한 자율적인 프로그램에 많은 에너지를 투입하여 전반적인 건강사업 즉 의료계의 질보장, 더 나아가서는 간호의 질을 향상 시키기 위한 많은 프로그램을 분석 연구한다. 이 논문은 간호에 있어서 질적 보장을 위한 내용으로 주요점은 질적 보장에 사용되는 용어, 역사적 배경, 질적 보장을 강조하는 이유, 질적 보장제도의 개발, 질 사정(euali assessment)과 질적 보장에 있어서 간호 전문직 개입을 차례대로 설명하였다. 구미의 경우 1970년대부터 간호직에 있어서는 질적보장제도개발에 우선순위를 두게되어 미 간호협회에서는 Professional Standards Review Organizations (PSRO)에 근거하여 간호실무를 위한 표준을 내어놓았다. 질적보장을 위한 평가방법으로 구조, 과정, 결과 그리고 수혜자가 간호계획을 세우는데 사정에 참여하는 방법 등 4가지를 간략하게 설명하였다. 질적 보장을 향상시키기 위해서는 전문직과 공공의료기간이 서로 협동체가 되어 중추역할을 해야함을 강조하고 이렇게 서로 협력하므로써만이 비로서 우리의 목적을 성취할 수 있다고 생각된다.구 등이 이용된다. $^{166}$ Ho, $^{198}$ Au, $^{32}$P, $^{90}$ Y, $^{169}$ Er, $^{186}$ Rc, $^{131}$ I, $^{211}$ At 등 의 방사성 핵종의 교질, 미소구 또는 단세포군 항체표지 형태로 직접 종양내 또는 공동이나 체강에 투여하는 치료법이 있다. 류마치스 관절염의 슬관절에 $^{165}$ Dy colloid를 주사하는 $^{166}$ Ho-MAA도 활발히 이용되고 있다. and computed groundwater levels. 본 논문에서는 가파른 산사면에서 산사태의 발생을 예측하기 위한 수문학적 인 지하수 흐름 모델을 개발하였다. 이 모델은 물리적인 개념에 기본하였으며, Lumped-parameter를 이용하였다. 개발된 지하수 흐름 모델은 두 모델을 조합하여 구성되어 있으며, 비포화대 흐름을 위해서는 수정된 abcd 모델을, 포화대 흐름에 대해서는 시간 지체 효과를 고려할 수 있는 선형 저수지 모델을 이용하였다. 지하수 흐름 모델은 토층의 두께, 산사면의 경사각, 포화투수계수, 잠재 증발산 량과 같은 불확실한 상수들과 a, b, c, 그리고 K와 같은 자유모델변수들을 가진다. 자유모델변수들은 유입-유출 자료들로부터 평가할 수 있으며, 이를 위해서 본 논문에서는 Gauss-Newton 방법을 이용한 Bard 알고리즘을 사용하였다. 서울 구로구 시흥동 산사태 발생 지역의 산사면에 대하여 개발된 모델을 적용하여 예제 해석을 수행함으로써, 지하수 흐름 모델이 산사태 발생

Electrochemical Studies on the Heavy Lanthanide Complexes (무거운 란탄족 이온 착물의 전기화학적 연구)

  • Electrochemical behavior of the heavy lanthanide complexes of alizarin red S(ARS) has been investigated by d. c. polarography, differential pulse polarography and cyclic voltammetry. The reduction mechanism at a mercury electrode of alizarin red S as a complexing ligand showed a one step of two-electron transfer and the electron process is found to be reversible. Alizarin red S forms a 3:1 adsorptive complexes with lanthanides and the complexes are reduced via one step of two-elctron. The reduction potential of complex wave($P_2$) shifted more negatively than the ligand wave($P_1$). The linear calibration curves of the decreacing $P_1$ and increasing $P_2$ is obtained when the lanthanide concentration varies from $2.0{\times}10^{-6}M$ to $6.4{\times}10^{-5}M$ under the condition of pH 9.5, 0.1M LiCl and $1{\times}10^{-3}M$ ARS.

Effect of Fermented Herbal Extracts, HP-1 on Enzyme Activities and Gene Expressions Related to Alcohol Metabolism in Ethanol-loaded Rats (발효한약추출물 HP-1이 알코올을 투여한 쥐의 알코올 대사에 미치는 영향)

  • Recently, much attention has been paid to developing various kinds of fermented herbal extracts, a new type of traditional herbal medicine, in the field of Korean traditional medicine. The fermentation of medicinal herbs is intended to exert a favorable influence on digestability, bioavailavility and pharmacological activity of herbal extract in the gastrointestinal tract. It also produces a number of fermentation products that intensify the nutritional and pharmacological aspects of the medicinal herbs. In order to develop a functional beverage of alleviating the aftereffects of the previous drinks, the extracts (HP-1) of fermented herbal mixture, including Artemisia capillaris Thunb., Lonicera japonica Thunberg, and Hovenia dulcis Thunb., were prepared and the medicinal effect as a hangover cure was evaluated in ethanol-loaded rats. The enzyme activities of alcohol dehydrogenase (ADH) and acetaldehyde dehydrogenase-2 (ALDH2) were analyzed by measuring the concentration of blood ethanol and acetaldehyde. The mRNA expression of ADH and ALDH2 was also investigated through RT-PCR analysis. In the HP-1-treated group, the concentration of blood ethanol was significantly reduced at one hour after loading of ethanol, as compared to that in the saline-treated group. The reduced ethanol was converted to acetaldehyde, which resulted in rapid increase in acetaldehyde concentration in an hour. Acetaldehyde was started to decrease at 5 hours after ethanol loading. It implies that HP-1 is highly effective to stimulate the activities of ADH and ALDH2. The HP-1 treatment also activated the mRNA expression of ADH and ALDH. This study suggests that fermented herbal extract, HP-1 can be used as a functional beverage of alleviating the alcohol-induced hangover symptoms by stimulating the activities and gene expression of hepatic alcohol metabolizing enzymes.

Rare Earth Dispute and Trend in Development of NdFeB Anisotropic Bonded Magnets (희토류 자원분쟁과 NdFeB계 이방성 본드자석 개발동향)

  • NeFeB anisotropy bonded magnet has proposed a new paradigm of weight reduction of small motors by replacing the conventional ferrite permanent magnets with its high magnetic property of 25 MGOe during last five years. It has also advanced by leaps and bounds in the field of motor industry for automobiles and electric power tools. And it has led a new innovation of fifty percent weight lightening compared to its current motors by correctly focusing on fuel performance improvement through weight lightening that automobile industry chased. There was, however, another price skyrocketing in 2011 after China had announced its export regulation in rare earth materials in July, 2010. And this price change has an extensive impact on the industries that consume rare earth magnets. This environmental change has caused technical challenge to improve the performance by using least amount of rare earth elements in NdFeB anisotropy bonded magnets, and led to make a new technical approach to a new applied field. In this article, we will show how each nation deals with this industrial issue, and introduce development trend and application of anisotropic NdFeB bonded magnets, so-called MAGFINE made by Aichi Steel Corp.

Studies on the Hemodilution Perfusion with Rygg-Kyvsgaard Oxygenator (혈액희석 체외순환법에 관한 임상적 관찰 -상온하 Rygg-Kyvsgaard 산화기 및 Sigmamotor pump 사용예를 중심으로-)

  • Clinical perfusion data on 16 cases of cardiopulmonary bypass using Sigmamotor pump and RyggKyvsgaard Oxygenator which performed at Seoul National University Hospital during the period of Aug. 1968 to Aug. 1970 was analized. AIl cases were hemodiluted and the perfusion was carried out under the normothermic condition. The age of the patients ranged between 6 and 43 years. The b:dy weight varied between 18.3 and 54.0 kg and the body surface area between 0.78 and 1. 59$M^2$. The priming solution was consiste:I with fresh ACD blood. Hartmann solution and Mannitol. The average amount of priming was approximately 2242 ml. The average hemodilution rate was 17%. The flow rate ranged from 1.7L to 3.5L/Min/$M^2$ and averaged 2.4L/Min/$M^2$ or 78mI/Min/kg. The duration of perfusion varied from 22 to 110 min with average of 56.9 minutes. Some hemodynamic responses were observed. The arterial pressure dropped immediately after the initiation of partial perfusion and was more marked after the total perfusion foIlowed by gradual increase to the safety level. The central venous pressure reflected the reduced blood volume especially in the cases of prolonged perfusion which lasted over 60 min. In most of the cases, red blood cell count decreased and white blood ceIl count increased after the perfusion. Hemoglobin level was decreased, averaging of 12.5mg%, Hct 3.3% and platelets count of 18% postoperatively. Plasma hemoglobin increased mildly, from pre-perfusion average value of 4. 06mg% to postperfusion value of 22.5mg%. Serum potassium was 4.4mEq/L pre-operatively and was decreased to 3.7mEq/L postoperatively. Five cases showed definite hypopotassemia immediately after the operation. Sodium and chloride decreased mildly. These electrolyte changes are thought to be related with hemodilution. diuretics and reduced blood volume during and after the perfusion. Arterial blood pH value revealed minimal to moderate elevation from preperfusion average value of 7.376 to 7.461 during perfusion and then 7.365 after perfusion. The pC02 and hicarbonate showed minimal to moderately lowered values. The total CO2 was decreased. Buffer base decreased during perfusion (Av. 42.6mEq/L) and further decreased after the perfusion (Av. 40.8mEq/L). These arterial blood acid base changes suggested that the metabolic acidosis was accompanied by respiratory alkalosis during and immediately after the perfusion. Authors belived that the acidosis could more effectively be corrected with the more additional dose of bicarbonate than we used by this study. The chest tune drainage during the first 24 hours following operation was 1158 ml in average. One case (Case No. 15) showd definite bleeding tendency and it was believed that the cause might be due to the defect of heparin and protamine titration. The average urinary out put during 24 hours post-perfusion was 1291ml. One case (Case No. ]) showed definite post perfusion oliguria. As conclusion hemodilution using fresh ACD blood. Hartmann and Mannitol solution added with Bivon and high flow rate unler normothermia. was thought to amelioratc the severity of mctabolic acidosis during and after perfusion with relatively satisfactory effect on the diuresis and bleeding tendency.

Effects of Geiji-Bokryung-Hwan on eNOS, nNOS, Caveolin-1 and bFGF Protein Expressions and the Endothelial Cells of the Corpus Cavernosum in Hypercholesterolemic Rat

  • We examine the effect of Geiji-Bokryung-Hwan(GBH) on erectile function in a rat model of hypercholesterolemic erectile dysfunction. GBH, a drug preparation consisting of five herbs of Cinnamomi Ramulus (Geiji), Poria Cocos (Bokryun), Mountan Cortex Radicis (Mokdanpi), Paeoniae Radix (Jakyak), and Persicae Semen (Doin) is a traditional Korean herbal medicine that is widely used in the treatment of atherosclerosis-related disorders. In this study, 3-month-old Sprague-Dawley rats were used. The 6 rats control animals were fed a normal diet and the other 18 rats were fed 1% cholesterol diet for 3 months. After 1 months, GBH was added to the drinking water of the treatment group of 12 rats but not the cholesterol only group of 6 rats. Of the 12 rats 6 received 30 mg/kg per day (group 1) and 6 received 60 mg/kg per day (group 2) of GBH. At 3 months erectile function was evaluated with cavernous nerve electrostimulation in all animals. Penile tissues were collected for electron microscopy, and to perform Western blot for endothelial nitric oxide synthase (eNOS), neuronal nitric oxide synthase (nNOS), basic fibroblast growth factor (bFGF) and caveolin-1. Systemic arterial pressure was not significantly different between the animals that were fed the 1% cholesterol diet and the controls. Conversely erectile function was not impaired in the herbal medicine treated rats. Electron microscopy showed many caveolae with fingerlike processes in the cavernous smooth muscle and endothelial cell membranes in control and treated rats but not in the cholesterol only group of rats. Western blot showed differences among groups in protein expression for eNOS, nNOS, caveolin-1 and bFGF protein expression in penile tissue. Increased eNOS and nNOS protein expressions dy high cholesterol diet were significantly decreased in group 1 and group 2. Interestingly, caveolin-1 and bFGF protein expression was significantly higher in groups 1 and 2 than in the cholesterol only and control groups.

Solvent Extraction of Heavy Rare Earth Elements Using PC88A from Synthesis Solution of Low Concentration Rare Earth Solution (저농도 희토류 용액으로부터 PC88A를 이용한 중희토류의 용매추출 연구)

  • The behavior on the solvent extraction of heavy rare earths (HRE) by using PC88A was confirmed to demonstrate the possibility of recovery on the HRE from industrial wastewater, which consist of low concentration rare earth. We verified the extraction behavior of the HRE through a change of equilibrium pH, extractant concentration and A/O ratio, and also confirmed the stripping behavior depending on the type of mineral acids. At equilibrium pH 1.0, extraction of rare earth (RE) was completed from 95% to 100%. In all extraction conditions, it tend to be extracted in order of high atomic number. When A/O ratio was 10/1, Yb and Tm were concentrated at the maximum and increased 6-fold and 3-fold compared to initial concentration, respectively. To confirm the stripping behavior of the RE, three mineral acids were applied to the organic phase and consequently rate of stripping was increased in order of $HNO_3$, $H_2SO_4$ and HCl.

The effect of brain education-based exercise and KPEM manual therapy integrated program on sleep and quality of life in cancer patients (뇌교육 기반 운동 및 KPEM도수치료 통합 프로그램이 암환자의 수면과 삶의 질에 미치는 영향)

  • Background: The purpose of this study was to investigate the effect of brain education-based exercise and KPEM manual therapy integrated program on the sleep and quality of life of cancer patients. Design: Seventy subjects who were diagnosed with cancer and were undergoing treatment volunteered to participate in this study. All subjects used a nonequivalent control group pretest-posttest design for either the experimental group or the control group. In the final analysis, there were 25 subjects in the experimental group and 18 subjects in the control group. Methods: For 12 weeks, the experimental group performed brain education-based exercise (20 minutes) and KPEM manual therapy (50 minutes), and the control group performed basic physical therapy and autonomous exercise. For evaluation, the Korean version of the Pittsburgh Sleep Quality Index (PSQI-K) and the quality of life index were measured after intervention using the European Organization for Research and Treatment of Cancer (EORTC-3.0Ver). Effect between groups, time effect over time, and group*time interaction were analyzed through a pre-test before and after the 12-week intervention period, and repeated measure ANOVA after 12 weeks of the integrated program intervention. All statistical significance levels were set at α=.05. Results: The PSQI in the time effect (p=.001) and the group*time interaction (p<.001) were statistically significant. In terms of EORTC, QL2 and PF2 were significant in time effect (p=.024; p=.021) and group*time interaction (p=.007; p=.021), whereas in RF2, significance was only found in group*time interaction (p=.028). In symptom indicators, time effect was the only significant factor in FA, SL, AP, and CO, respectively (p=.002; p=.028; p=.041; p =.005) and in DY, there were significant differences in the time effect (p=.016) and group*time interaction (p=.002). Conclusion: The brain education-based exercise and KPEM manual therapy integrated program effectively improves the sleep and quality of life of cancer patients. It is considered that this exercise and therapy can be actively used as a psychological, emotional, and physically complementary physical therapy intervention to improve the quality of life of cancer patients.

  • Recently, the government has emphasized the importance of establishing an innovation system for SMEs as part of an innovative growth strategy for national economic development. SMEs in Korea are experiencing many difficulties due to the loss of industrial competitiveness due to the powerful wave of the 4th industrial revolution along with the pandemic of COVID-19 that started in 2020. In relation to environmental changes, it is emerging as an important issue for SMEs to pursue open innovation for survival and growth, as well as to possess excellent technological innovation capabilities. In accordance with the need for such research, this study analyzed the factors that affect business performance or technological performance of small and medium-sized manufacturers, and identified the relationship between these influencing factors and corporate performance. The results of this study suggest that it is important for SMEs to effectively utilize R&D services and technology innovation capabilities and resources possessed by SMEs to secure a competitive advantage. The results of this stu dy are considered to be able to provide basic data and information necessary for SMEs to secu re a competitive edge and enhance industrial competitiveness through government support systems and open innovation.

Effects of Radiofrequency Induced Local Hyperthermia on Normal Canine Liver (라디오 파를 이용한 국소 온열치료 : 정상 개의 간에 미치는 영향)

  • In order to assess the effects of radiofrequency-induced local hyperthermia on the normal liver, histopathologic findings and biochemical changes after localized hyperthermia in canine liver were studied. Hyperthermia was externally adminsitered using the Thermotron RF-8 (Yamamoto Vinyter Co., Japan; Capacitive type heating machine) with parallel opposed electrodes. Thirteen dogs were used and allocated into one control group (N=3) and two treatment groups according to the treatment temperature. Group I (N=5) was heated with $42.5\pm0.5^{\circ}C$ 30 minutes, and Group II (N=5) was heated with $45\pm0.5^{\circ}C$ for 15-30 minutes. Samples of liver tissue were obtained through a needle biopsy immediately after hyperthermia and T,14, and 28 days after treatment. Blood samples were obtained before treatment and W, 3,5, 7,14 and 28 days after treatment and examined for SGOT, SGPT and alkaline phosphatase. Although SGOT and SGPT were elevated after hyperthermia in both groups (three of five in each group), there was no liver cell necrosis or hyperthermia related mortality in Group 1. A hydropic swelling of hepatocytes was prominent histologic finding. Hyperthermia with $45^{\circ}C$ for 30 minutes was fatal and showed extensive liver cell necrosis. In conclusion, liverdamage dy heat of $42.5\pm0.5^{\circ}C$ for 30 minutes is reversible, and liver damage by heat of $45\pm0.5^{\circ}C$ for 30 minutes can be fatal or irreversible. However, these results cannot be applied directly to human trial. Therefore, in erder to apply hyperthermic treatment on human liver tumor safely, close obsewation of temperature with proper thermometry is mandatory. Hyperthermic treatment should be confined to the tumor area while sparing a normal liver as much as possible.
