Seventy neonates with congenital intestinal atresia and stenosis who were treated at pediatric surgical service. Hanyang University Hospital from September 1979 to December 1996 were analyzed retrospectively. The lesion occurred in 27 cases at the duodenum, in 26 cases at the jeiunum, in 13 cases at the ileum and in 2 cases at the pylorus and colon each. There were 10 multiple atresias and 7 apple-peel anomaly cases. The atresia predominated over the stenosis by the ratio of 4 : 1. Male to female ratio was 1.3 : 1. The average gestational age was 38 weeks, and the average birth weight was 2,754 grams. Though 22.9 % were borne prematurely and 34.3 % had low birth weight, 92.3 % of them had a weight appropriate for gestational age. Polyhydramnios(40 %) was more frequently observed in duodenal and jeiunal atresia while microcolon in ileal atresia(58.3 %). Weight loss and electrolyte imbalance occurred more frequently in the duodenal stenosis cases because of delayed diagnosis. Twenty(55.6 %) of 37 jeiunoileal atresia cases had evidence of intrauterine vascular accident : 4 intrauterine intussusception, 3 intrauterine volvulus and 3 strangulated intestine in gastroschisis, and 10 cases of intrauterine peritonitis. There were one or more associated anomalies in 45 patients (64.3 %). Preoperatively proximal loop volvulus developed in 3 cases and proximal loop perforation in 5 cases and one case each of distal loop perforation, duodenal perforation and midgut volvulus occurred in the jeiunoileal atresia. Overall mortality rate was 20 %.
An 18-year-old woman with annular pancreas and duodenal duplication presented with recurrent acute pancreatitis and underwent a resection of duodenal duplication. However, the patient experienced recurrent abdominal pain after resection. Abdominal computed tomography and magnetic resonance imaging showed a dilatation of the peripheral pancreatic duct and stenosis and malformation of both the Wirsung's and Santorini's duct due to multiple stones. The modified puestow procedure was performed. The main pancreatic ducts in the body and tail were opened, and the intrapancreatic common bile duct was preserved. A Roux-en-Y pancreatico-jejunostomy was performed for reconstructing the pancreaticobiliary system after removing the ductal protein plug. The patient experienced no abdominal pain, no significant elevation of the serum amylase and lipase levels, and no stone formation during the 2 years of follow-up. This procedure is considered to be beneficial for pediatric patients with chronic pancreatitis due to annular pancreas and duodenal duplication.
Annular pancreas is a rare congenital anomaly with the descending duodenum encircled by a ring of pancreatic tissue, which may cause partial or complete obstruction of the duodenum. In newborn, the symptoms can be those of duodenal stasis resulting from partial intestinal obstruction secondary to some degree of duodenal stenosis. A male newborn weighing 2.0 Kg was born by C-section delivery at 37 weeks' gestation to a 27-year-old mother who had a hydramnios. He was in relatively good condition at birth except regurgitation of saliva and intermittent apnea. A plain film of the abdomen showed the double-bubble of gas filled stomach and proximal duodenum, and upper gastrointestinal series showed a dilated proximal duodenum, with a complete obstruction of the descending duodenum. Intraoperative finding revealed encirclement of the duodenal second portion by pancreatic tissue. Duodenojejunostomy was performed. After the operation, he had developed two serious complications, neonatal septicemia by Enterobacter cloacae on postoperative day 12 and systemic candidiasis on postoperative day 19, and been managed with ventilatory support, antibiotics, and antifungal agents with recovery.
Advanced malignant hilar biliary obstruction (MHBO) with inaccessible papilla poses a significant challenge to endoscopists, as drainage of multiple liver segments may be warranted. Transpapillary drainage may not be feasible in patients with surgically altered anatomy, duodenal stenosis, prior duodenal self-expanding metal stent, and after initial transpapillary drainage, but require re-intervention for draining separated liver segments. Endoscopic ultrasound-guided biliary drainage (EUS-BD) and percutaneous trans-hepatic biliary drainage are the feasible options in this scenario. The major advantages of EUS-BD over percutaneous trans-hepatic biliary drainage include a reduction in patient discomfort and internal drainage away from the tumor, thus reducing the possibility of tissue or tumor ingrowth. With innovations, EUS-BD is helpful not only for bilateral communicating MHBO but also for non-communicating systems with bridging hilar stents or isolated right intra-hepatic duct drainage by hepatico-duodenostomy. EUS-guided multi-stent drainage with specially designed cannulas and guidewires has become a reality. A combined approach with endoscopic retrograde cholangiopancreatography for re-intervention, interventional radiology, and intraductal tumor ablative therapies has been reported. Stent migration and bile leakage can be minimized with proper stent selection and technique, and stent blocks can be managed with EUS-guided interventions in a majority of cases. Future comparative studies are required to establish the role of EUS-guided interventions in MHBO as rescue or primary therapy.
1983년 5월부터 1987년 6월까지 영남대학교 영남의료원에서 선천성 장관폐쇄증으로 내원한 25예를 대상으로 각각의 임상소견 및 방사선학적 소견을 분석하여 다음과 같은 결과를 얻었다. 1. 비후성유문협착증 6예, 중장이상회전증 4예, 선천성 거대결장 8예, 쇄항 5예, 십이지장폐쇄증 및 회장폐쇄증이 각각 1예였다. 2. 남여비는 16:9였으며, 특히 비후성유문부협착증은 6예중 5예가 남아였다. 3. 비후성유문부협착증은 전예에서 상부위장관조영에서 특징적인 String sign 및 beak sign, shoulder sign 등을 나타냈다. 4. 1예의 십이지장폐쇄증은 double bubble sign을 보였고, 회장폐쇄증은 단순복부 사진상 폐쇄성소장공기팽창과 대장조영술에서는 microcolon을 나타내었다. 5. 4예의 중장이상회전증에서는 대장조영술상 맹장이 우측 상복부에 있었으며 2예는 Ladd's band에 의한 십이지장폐쇄를 보였다. 6. 선천성거대결장은 8예가 전부가 직장 및 하부 S자결장에 국한되어 나타났으며, 1예에서는 장염을 동반하였다. 7. 쇄항은 5예중 3예가 low type, 2예가 high type였고, 4예에서 회음부 및 비뇨생식기에 루관을 형성하였다. 위와 같은 결과와 함께 각각의 발생학적 기전과 방사선학적 소견의 문헌 고찰을 통하여 보다 신속하고 정확한 진단을 내리는데 도움을 주고자 하였다.
Infantile hypertrophic pyloric stenosis (HPS) is a relatively common entity. A number of studies for the postoperative feeding schedule has been studied to allow for earlier hospital discharge and improve cost-effectiveness in the treatment of HPS. The purpose of this study was to compare 3 feeding-methods and to evaluate the usefulness of ad lib feeding for HPS. The authors retrospectively reviewed the records of 116 patients who underwent pyloromyotomy for HPS from 1995 to 2004. Three cases were excluded because of the duodenal perforation during pyloromyotomy. Three feeding-methods were defined as: Conventional feeding (>10 hours nothing by mouth and incremental feeding every 2 hours, C), Early feeding(for 4 to 8 hours nothing by mouth and incremental feeding every 2 hours, E), and Ad lib feeding (for 4 hours nothing by mouth and ad lib feeding, A). Time to normal feeing in C, E and A were $51{\pm}24$, $34{\pm}12$ and $24{\pm}6$ hours, respectively. Hospital-stay in C, E and A were $72{\pm}17$, $55{\pm}13$ and $43{\pm}12$ hours, respectively. There were statistically significant differences according to the method of feeding. Frequency of postoperative emesis in C, E and A were 38 %, 47 % and 53 %, but was not significant statistically. Ad lib feeding decreased time to normal feeding and hospital stay, and did not increase postoperative emesis. We conclude that ad lib feeding is recommended for patient with pyloromyotomy in HPS.
Gastrointestinal (GI) emergencies in neonates and infants encompass from the beginning to the end of the GI tract. Both congenital and acquired conditions can cause various GI emergencies in neonates and infants. Given the overlapping or nonspecific clinical findings of many different neonatal and infantile GI emergencies and the unique characteristics of this age group, appropriate imaging is key to accurate and timely diagnosis while avoiding unnecessary radiation hazard and medical costs. In this paper, we discuss the radiological findings of essential neonatal and infantile GI emergencies, including esophageal atresia and tracheoesophageal fistula, hypertrophic pyloric stenosis, duodenal atresia, malrotation, midgut volvulus for upper GI emergencies, and jejunoileal atresia, meconium ileus, meconium plug syndrome, meconium peritonitis, Hirschsprung disease, anorectal malformation, necrotizing enterocolitis, and intussusception for lower GI emergencies.
Chronic silica nephropathy has been associated with tubulointerstitial disease, immune-mediated multisystem disease, chronic kidney disease, and end-stage renal disease. On the other hand, acute intentional exposure is extremely rare. The authors' experienced a 44-year-old man who took rapid cement hardener (sodium silicate) in a suicide attempt whilst in a drunken state. He visited the emergency department approximately 1 hour after ingestion. Information on the material was obtained after 3 L gastric lavage. The patient complained of a sore throat, epigastric pain, and swollen to blood tinged vomitus. Proton pump inhibitors, hemostats, steroid, and fluids were administered. Nine hours after ingestion, he was administered 200 mL hematochezia. Immediately after, a gas-troenterologist performed an endoscopic procedure that revealed diffuse hyperemic mucosa with a color change and variable sized ulceration in the esophagus, whole stomach, and duodenal $2^{nd}$ portion. Approximately 35 hours later, persistent oligouria and progressive worsening of the renal function parameters (BUN/Cr from 12.2/1.2 to 67.5/6.6 mg/dL) occurred requiring hemodialysis. The patient underwent 8 sessions of hemodialysis for 1 month and the BUN/Cr level increased to 143.2/11.2 mg/dL and decreased to 7.6/1.5 mg/dL. He was discharged safely from the hospital. Follow up endoscopy revealed a severe esophageal stricture and he underwent endoscopic bougie dilatation. Acute cement hardener (sodium silicate) intoxication can cause renal failure and strong caustic mucosal injury. Therefore, it is important to consider early hemodialysis and treatment to prevent gastrointestinal injury and remote esophageal stricture.
목적 : 총담관 확장을 유발하는 원인 중 결석과 종양을 제외한 여러 원인을 평가하는데 있어서 MRCP의 진단적 유용성을 알아보고자 하였다. 대상 및 방법 : ERCP와 MRCP를 모두 시행받은 환자 26명 (남:여=15:11, 평균연령 62세)을 대상으로 하였으며, 이들 중 12명은 역동적 MRCP를, 10명은 조영증강 MRI를 추가로 시행하였다. 총담관, 간내 담도, 췌관 등의 확장 정도, 간내 담도 결석, 췌장 가성낭종, 유두부의 부종이나 결절성 종대 등을 관찰하였다. 총담관은 T2-강조 관상영상에서 담낭절제술을 받지 않은 경우 7 mm 이상, 절제술을 받은 경우 10 mm 이상을 비정상적인 확장으로 판정하였다. 결과 : 담낭절제술을 받지 않은 환자의 총담관 평균 직경은 12.7mm(n=21, 9-19 mm)였고, 절제술을 받은 환자의 평균 직경은 13.0 mm(n=5, 10-15mm)였다(p〉0.05). MRCP에서 담관염 11명(42.3%), 만성 췌장염 8명(30.8%), 원위부 총담관 협착 6명(23.1%), 팽대부 주위 게실 3명(11.5%), Vater 팽대부 협착, 오디 괄약근 기능장애, 췌장 두부의 국소적 췌장염이 각각 2명 (7.7%), 유두염, 췌장 두부 가성낭종에 의한 총담관 확장, 총담관 내 회충이 각각 1명(3.8%) 있었다. 췌관의 확장은 10명(38.5%)에서 보였고, 십이지장 게실은 3명(11.5%)에서 관찰되었다. 역동적 MRCP(n=12)에서 2명(16.7%)은 일반 MRCP에서 보이지 않던 원위부 총담관이 관찰되었고, 조영증강 MRI를 시행한 10명 중 1명 (10.0%)만이 유두염의 소견을 보였다. 결론 : 비결석, 비종양성 총담관 확장의 원인을 찾는데 있어서 MRCP는 총담관 원위부 협착 및 만성 췌장염, 역동적 MRCP를 이용한 생리적 총담관의 확장, 조영증강 MRI 를 통한 담관염 및 담관 외 병소의 발견 등에 있어서 도움이 되는 것으로 생각된다.
관상동맥우회술의 경험이 축적됨에 따라 관상동맥우회술의 재수술은 증가하는 추세이나 아직까지 국내 보고는 미흡한 수준이다. 저자들은 관상동맥 재수술의 사망률, 유병률 등의 임상경험을 정리하고 분석하였다. 대상 및 방법: 1994년 1월부터 2002년 12월까지 관상동맥우회술의 재수술을 시행 받은 14명의 환자를 대상으로 하였다. 최초 수술 후 재수술까지의 기간은 평균 66$\pm$56 (3∼157)개월이었고 재수술 시 평균연령은 62.8$\pm$8.7 (51∼78)세였다. 2003년 6월 30일을 추적관찰 종료시점으로 하여 평균추적관찰 기간은 43$\pm$29 (8∼105)개월이었다. 재수술의 적응증은 이식편의 협착 또는 완전폐쇄가 11예(78.6%), 관상동맥질환의 진행이 1예, 그리고 두 가지 병리가 혼재한 경우가 2예였다. 결과: 입원기간 중 사망은 2예(14.3%)가 발생하였고 사망원인은 저심박출증이었다. 술 후 합병증은 총 7명(50.0%)의 환자에서 8예가 발생하였는데, 수술 전후 심근경색이 2예(14.3%), 종격동염이 1예(7.2%), 십이지장 궤양천공 1예, 허혈성 하지괴사 1예, 장간막경색에 따른 위장관 천공 1예, 지연 뇌경색 1예, 비장출혈 1예 등이었다. 조기사망 2예를 제외한 12명의 환자를 추적 관찰한 결과 만기사망은 1예가 발생하였고 추적관찰 종료시점에서 협심증을 호소하는 환자는 없었으며 모든 환자가 Canadian협심증분류 1급 또는 2급의 양호한 경과를 보였다. 결론: 관상동맥 재수술은 비교적 높은 유병률과 사망률을 보였으나 퇴원 후 환자상태는 양호하였으며 향후 경험의 축적과 노력이 요망된다.
