Dislocation after Revision Total Hip Arthroplasty: A Comparison between Dual Mobility and Conventional Total Hip Arthroplasty

  • Purpose: The objective of this study was to analyze the results from a cohort of patients who underwent a revision total hip arthroplasty (THA) using a dual mobility cup (DMC) implant. Materials and Methods: A retrospective review of revised THAs was conducted using the database from a single tertiary referral hospital. A total of 91 revision THAs from 91 patients were included in the study. There were 46 male hips and 45 female hips. The mean age was 56.3±14.6 years, and the mean follow-up period was 6.4±5.9 years. In performance of revision THAs, the DMC implants were used in 18 hips (19.8%), and the conventional implants were used in 73 hips (80.2%). Results: During the follow-up period, three dislocations were identified, and the overall dislocation rate was 3.3%. Early dislocation (at one month postoperatively) occurred in one patient, while late dislocation (at a mean of 7.5 years) occurred in two patients. There was no occurrence of dislocation in the DMC group (0%), and three dislocations were detected in the conventional group (4.1%). However, no significant difference in the rate of dislocation was observed between the two groups (P=0.891). Conclusion: Although the rate of dislocation was higher in the conventional group, there were no statistically significant differences between the two groups due to the small number of patients. Nevertheless, we believe that the dual mobility design is advantageous in terms of reducing dislocation rate and can be recommended as an option for a revision THA.

EEDARS: An Energy-Efficient Dual-Sink Algorithm with Role Switching Mechanism for Event-Driven Wireless Sensor Networks

  • Energy conservation is a vital issue in wireless sensor networks. Recently, employing mobile sinks for data gathering become a pervasive trend to deal with this problem. The sink can follow stochastic or pre-defined paths; however the controlled mobility pattern nowadays is taken more into consideration. In this method, the sink moves across the network autonomously and changes its position based on the energy factors. Although the sink mobility would reduce nodes' energy consumption and enhance the network lifetime, the overhead caused by topological changes could waste unnecessary power through the sensor field. In this paper, we proposed EEDARS, an energy-efficient dual-sink algorithm with role switching mechanism which utilizes both static and mobile sinks. The static sink is engaged to avoid any periodic flooding for sink localization, while the mobile sink adaptively moves towards the event region for data collection. Furthermore, a role switching mechanism is applied to the protocol in order to send the nearest sink to the recent event area, hence shorten the path. This algorithm could be employed in event-driven and multi-hop scenarios. Analytical model and extensive simulation results for EEDARS demonstrate a significant improvement on the network metrics especially the lifetime, the load and the end-to-end delay.

  • We have proposed and fabricated a dual gate AlGaN/GaN high electron mobility transistor (HEMT), which exhibits the low leakage current and the high breakdown voltage for the high voltage switching applications. The additional gate between the main gate and the drain is specially designed in order to decrease the electric field concentration at the drain-side of the main gate. The leakage current of the proposed HEMT is decreased considerably and the breakdown voltage increases without sacrificing any other electric characteristics such as the transconductance and the drain current. The experimental results show that the breakdown voltage and the leakage current of proposed HEMT are 362 V and 75 nA while those of the conventional HEMT are 196 V and 428 nA, respectively.

Total Hip Arthroplasty with Cemented Dual Mobility Cup into a Fully Porous Multihole Cup with Variable Angle Locking Screws for Acetabular Fractures in the Frail Elderly

  • Purpose: The purpose was to examine the clinical and radiological outcomes after surgical treatment of acetabular fractures with total hip arthroplasty with a dual mobility cup cemented into a porous multihole cup in the population of frail elderly patients. Materials and Methods: A retrospective review of 16 patients who underwent surgery (mean age, 76.7 years) with a mean follow-up period of 36.9 months was conducted. Following surgery, patients underwent postoperative follow-up at six weeks, three, six, and 12 months and clinical and radiological examinations were performed. Results: Classification of fractures was based on the Letournel classification. Following surgery, all patients were allowed weight-bearing as tolerated immediately postoperative. Fourteen patients showed maintenance of preoperative mobility status at one year. The mean Harris hip score was 64.8 (range, 34.7-82.8) and 80.0 (range, 60.8-93.8) at three months and one year, respectively. The mortality rate was 12.5% at one year (2/16). Complications included heterotopic ossification (2/16), deep venous thrombosis (1/16), heamatoma (1/16), and femoral revision due to a Vancouver B2 fracture (1/16). No case of deep infection, dislocation, or implant loosening was reported. Conclusion: Total hip arthroplasty using a dual mobility cup cemented into a porous multihole cup with locking screws resulted in a stable construct with a capacity for immediate weight-bearing as tolerated with rapid relief of pain. The findings of this study suggest that this procedure can be regarded as a safe method that has shown promising clinical and radiological outcomes for treatment of patients with medical frailty.

  • In this paper, we consider a group mobility model to formulate a clustering mechanism called Dual Coalescent Energy-Efficient Algorithm (DCEE) which is scalable, distributed and energy-efficient for wireless mesh network. The differences of the network nodes will be distinguished to exploit heterogeneity of the network. Furthermore, a topology control, that is, adjusting the transmission range to further reduce power consumption will be integrated with the cluster formation to improve network lifetime and connectivity. Along with network lifetime and power consumption, clusterhead changes will be measured as a performance metric to evaluate the. effectiveness and robustness of the algorithm.

Electrical Characteristics of Solution Processed DAL TFT with Various Mol concentration of Front channel

  • In order to investigate the effect of front channel in DAL (dual active layer) TFT (thin film transistor), we successfully fabricated DAL TFT composed of ITZO and IGZO as active layer using the solution process. In this structure, ITZO and IGZO active layer were used as front and back channel, respectively. The front channel was changed from 0.05 to 0.2 M at fixed 0.3 M IGZO of back channel. When the mol concentration of front channel was increased, the threshold voltage (VTH) was increased from 2.0 to -11.9 V and off current also was increased from 10-12 to 10-11. This phenomenon is due to increasing the carrier concentration by increasing the volume of the front channel. The saturation mobility of DAL TFT with 0.05, 0.1, and 0.2 M ITZO were 0.45, 4.3, and $0.65cm2/V{\cdot}s$. Even though 0.2 M ITZO has higher carrier concentration than 0.05 and 0.1 M ITZO, the 0.1 M ITZO/0.3 M IGZO DAL TFT has the highest saturation mobility. This is due to channel defect such as pores and pin-holes. These defect sites were created during deposition process by solvent evaporation. Due to these defect sites, the 0.1 M ITZO/0.3 M IGZO DAL TFT shows the higher saturation mobility than that of DAL TFT with front channel of 0.2 M ITZO.

  • The Mobile IPv6(MIPv6) has a well-known time gap of packet loss between link down and binding update. To minimize such packet loss, this paper proposes a MIPv6-based dual tunneling mechanism, which keeps exploiting the old tunnel while creating a new tunnel. Superiority of the proposed mechanism is evaluated by quantitative analysis on the lost packets and computer simulation, based on two overlay heterogeneous access networks, such as the UMTS and a WLAN.

Dual-mobility versus Fixed-bearing in Primary Total Hip Arthroplasty: Outcome Comparison

  • Purpose: Use of dual mobility (DM) articulations can reduce the risk of instability in both primary and revision total hip arthroplasty (THA). Knowledge regarding the impact of this design on patient-reported outcome measures (PROMs) is limited. This study aims to compare clinical outcomes between DM and fixed bearing (FB) prostheses following primary THA. Materials and Methods: All patients who underwent primary THA between 2011-2021 were reviewed retrospectively. Patients were separated into three cohorts: FB vs monoblock-D vs modular-DM. An evaluation of PROMs including HOOS, JR, and FJS-12, as well as discharge-disposition, 90-day readmissions, and revisions rates was performed. Propensity-score matching was performed to limit significant demographic differences, while ANOVA and chi-squared test were used for comparison of outcomes. Results: Of the 15,184 patients identified, 14,652 patients (96.5%) had a FB, 185 patients (1.2%) had a monoblock-DM, and 347 patients (2.3%) had a modular-DM prosthesis. After propensity-score matching, a total of 447 patients were matched comparison. There was no statistical difference in the 90-day readmission (P=0.584), revision rate (P=0.265), and 90-day readmission (P=0.365) and revision rate due to dislocation (P=0.365) between the cohorts. Discharge disposition was also non-significant (P=0.124). There was no statistical difference in FJS-12 scores at 3-months (P=0.820), 1-year (P=0.982), and 2-years (P=0.608) between the groups. Conclusion: DM bearings yield PROMs similar to those of FB implants in patients undergoing primary THA. Although DM implants are utilized more often in patients at higher-risk for instability, we suggest that similar patient satisfaction may be attained while achieving similar dislocation rates.

Pentacene Thin-Film Transistors with Polyimide/$SiO_2$ Dual Gate Dielectric

  • Relationships between field effect mobility and grain size on pentacene thin-film transistors with $polyimide/SiO_2$ gate dielectrics have been studied. 6 kinds of polyimide were used as surface treatment gate dielectric layer. Grain size of the pentacene thin film were between 5 and $30\;{\mu}m$ and depended on the polyimide. The field effect mobility were also depended on the polyimide and the those values were from 0.027 to $0.69\;cm^2/(Vs)$. The field effect mobility tends to increase with increasing the grain size. Precursor type polyimide containing polyamic acid show better mobility of $0.69\;cm^2/(Vs)$ than soluble type polyimide. Bias stress characteristics in air are discussed in the basis of the grain size.

  • Electron mobility which is a basic parameter to represent gas state, is calculated for 38mm 20W fluorescent lamp. 2 Electron Group Model is u&ed for electron distribution function. To prove this result electron mobility in 38mm 20W fluorescent lamp is measured at various wall temperatures. In order to obtain electric field strength of positive column, the sum of anode and cathode fall voltages assumed to be 15(VJ) and dual slit is used to measure positive column length. The measured and calculated result is potted under $10-80^{\circ}C$ wall temperature.
