지방우세 딕슨기법을 이용한 슬관절 자기공명영상 지방신호분율과 골밀도 간의 상관관계 분석: 예비 연구 (Correlation Analysis between Fat Fraction and Bone Mineral Density Using the DIXON Method for Fat Dominant Tissue in Knee Joint MRI: A Preliminary Study)

  • 목적 MRI Dixon method (이하 DIXON) 기법을 이용하여 슬관절 지방우세 골조직의 지방신호분율(fat signal fraction; 이하 FF)을 구하고 골밀도(bone mineral density; 이하 BMD)와 상관관계 분석을 시행하였다. 대상과 방법 본원 슬관절 DIXON 영상을 촬영한 환자들 중 일 년 이내의 dual energy X-ray absorptiometry가 있는 환자 93명을 후향적으로 분석하였다. DIXON 영상에서 원위부 대퇴골 골간단(femur metaphyseal; 이하 Fm)과 근위부 경골 골간단(tibia metaphyseal; 이하 Tm)의 FF를 계산하였다. FF와 BMD 간 상관관계를 분석하였다. 또, 요추(lumbar spine; 이하 L), 대퇴경부(femoral neck; 이하 FN), 총대퇴골(femur total; 이하 FT)의 BMD 별로 나누고 FF에 대하여 Kruskal-Wallis H test를 시행하였다. 결과 전체 환자군에서 TmFF와 FN-BMD 사이에 유의미한 음의 상관관계를 보였다(r = -0.26, p < 0.05). 여성 환자에서는 TmFF가 FN-BMD, FT-BMD 및 L-BMD와 음의 상관관계를 보였다(r = -0.38, -0.28과 -0.27, p < 0.05). 남성 환자에서는 FmFF가 FN-BMD 및 FT-BMD와 음의 상관관계를 보였다(r = -0.58과 r = -0.42, p < 0.05). 여성 환자들을 세 군으로 나누었을 때 TmFF에 유의한 차이를 보였다(p < 0.05). 남성 환자들을 두 군으로 나누었을 때, FmFF이 군간에 유의미한 차이를 보였다(p < 0.05). 결론 슬관절 주변의 FF와 BMD는 음의 상관관계를 보였으며, DIXON을 이용한 FF 측정이 BMD 스크리닝에 사용될 수 있는 가능성을 보였다.

  • Objectives : The effects of exercise on bone density have been found to be inconsistent in previous studies. We conducted a cross-sectional study in premenarcheal girls to test two hypotheses to explain these inconsistencies. Firstly,'the intensity of mechanical strain, in terms of the ground reaction force(GRF), has more important effects on the bone mass at a weight-bearing site', and secondly, 'calcium intake modifies the bone response to exercise'. Methods : The areal bone mineral density was measured at the Os calcis, using peripheral dual energy X-ray absorptiometry, in 91 premenarcheal girls aged between 9 and 12 years. The intensity of mechanical strain of exercise was assessed by a self-report questionnaire and scored by the GRF as multiples of body weight, irrespective of the frequency and duration of exercise. The energy and calcium intake were calculated from the 24-hour dietary recall. An analysis of covariance(ANCOVA) was used to determine the interaction and main effects of exercise and calcium on the bone density, after adjusting for age, weight, height and energy intake. Results : The difference in the bone density between moderate and low impact exercise was more pronounced in the high than low calcium intake group. The bone density for moderate impact exercise and high calcium intake was significantly higher than that for low impact exercise (p=0.046) and low calcium intake, after adjusting for age, weight, height and energy intake. Conclusions : Our study suggests that the bone density at a weight-bearing site is positively related to the intensity of mechanical loading exercise, and the calcium intake may modify the bone response to exercise at the loaded site in premenarcheal girls.

폐경후 여성들의 골밀도에 따른 영양섭취상태와 생활습관과의 관련성 연구 - 충남일부 지역을 중심으로 - (Association of Bone Mineral Density with Nutrient Intake and Lifestyles of Postmenopausal Women in Chungnam)

  • This study was peformed to assess the relationships among bone mineral density(BMD), nutrient intake and lifestyle factors in postmenopausal women. A trained nutritionist examined the interviewed usual dietary intake and physical activity of 85 subjects in Chungnam with a questionnaire interview, and BMDs of the lumbar spines($L_2-L_4$), femoral necks(FN), ward's triangles(WT) and trochanters(TR) were measured by dual energy X-ray absorptiometry (DEXA). The BMDs(T-score) of $L_2-L_4$, FN, and WT were $0.996g/cm^2(-1.601),\;0.697g/cm^2(-1.697)$, and $0.793g/cm^2(-1.512)$, respectively, which were assessed as osteopenia by T-score,. and TR was normal at $0.718g/cm^2(-0.675)$. Subjects included 85 in Chungnam were divided into three groups according to the BMD measurement of the $L_2-L_4$ and FN assessed by T-score. The percentages of the osteoporosis, osteopenia, and normal groups were $32.9\%,\;42.4\%$ and $24.7\%$ respectively. The average age was significantly the highest of the osteoporosis group than in the other two osteopenia or normal groups(P<0.001). Among lifestyle factors, the BMD of lifestyles was significantly related with exercise but it was not significant with medication., salt. intake, bone fracture, coffee consumption, drinking and smoking. The nutrient intake of the subjects was most nutrient intake adequate to the Korean RDA level for most nutrients excepting energy intakes and calcium. The BMD of lumbar spines $L_2-L_4$ showed significant positive association with the intake of most nutrients except carbohydrate, especially in the normal group. The Higher BMD levels were found for those with high intake of protein and vitamin B1. The BMDs of $L_2-L_4$, and FN were positively correlated with showed energy expenditure, exercise, miscellaneous activity, per weekend and week(P<0.05) in normal group but that was showed negatively correlated in the osteopenia group. Therefore, this study confirmed that one of the most effective ways to minimize bone less in postmenopausal women is to maintain an adequate intake of calcium and other nutrients and regular physical activity.

Effect of feed restriction on the maintenance energy requirement of broiler breeders

  • Objective: This study aimed to evaluate the effect of the ad libitum and restricted feeding regimen on fasting heat production (FHP) and body composition. Methods: Twelve Hubbard broilers breeders were selected with the same body weight and submitted in two feeding regimes: Restricted (T1) with feed intake of 150 g/bird/d and ad libitum (T2). The birds were randomly distributed on the treatments in two runs with three replications per treatment (per run). The birds were adapted to the feed regimens for ten days. After that, they were allocated in the open-circuit chambers and kept for three days for adaptation. On the last day, oxygen consumption (VO2) and carbon dioxide production (VCO2) were measured by 30 h under fasting. The respiratory quotient (RQ) was calculated as the VCO2/VO2 ratio, and the heat production (HP) was obtained using the Brower equation (1985). The FHP was estimated throughout the plateau of HP 12 hours after the feed deprivation. The body composition was analyzed by dual-energy X-ray absorptiometry scanning at the end of each period. Data were analyzed for one-way analysis of variance using the Minitab software. Results: The daily feed intake was 30 g higher to T2 (p<0.01) than the T1. Also, the birds of the T2 had significatively (p<0.05) more oxygen consumption (+3.1 L/kg0.75/d) and CO2 production (+2.2 L/kg0.75/d). That resulted in a higher FHP 359±14 kJ/kg0.75/d for T2 than T1 296±17.23 kJ/kg0.75/d. In contrast, the RQ was not different between treatments, with an average of 0.77 for the fasting condition. In addition, protein and fat composition were not affected by the treatment, while a tendency (p<0.1) was shown to higher bone mineral content on the T1. Conclusion: The birds under ad libitum feeding had a higher maintenance energy requirement but their body composition was not affected compared to restricted feeding.

골다공증 여성에서 요추골 밑 대퇴골 부위의 골밀도와 종골 음속 사이의 상관관계 (Correlations of Lumbar and Femoral Bone Mineral Densities with Calcaneal Speed of Sound in Osteoporotic Woman)

  • 본 in vivo 연구에서는 36명의 골다공증 여성을 대상으로 요추골 및 대퇴골 부위의 골밀도와 종골 음속 사이의 상관관계를 조사하였다 2-4번 요추골 및 우측 대퇴골 경부의 단위면적당 골밀도는 이중에너지 X-선 흡수계측법을 이용하여 측정되었다. 우측 종골의 음속은 초음파 골밀도 측정기를 이용하여 측정되었다. 각 측정값 사이의 상관관계를 분석하기 위하여 Pearson 상관계수 (r) 및 유의수준 (p)이 이용되었다. 요추골 골밀도와 대퇴골 골밀도 사이에는 매우 높은 양의 상관관계가 나타났다 (r=0.81). 요추골 및 대퇴골 골밀도는 연령과 음의 상관관계를 보였다 (r=-0.52 및 r=-0.55). 종골 음속과 연령 사이에도 음의 상관관계가 나타나는 것을 알 수 있었다 (r=-0.45). 종골 음속은 요추골 및 대퇴골 골밀도와 비교적 높은양의 상관관계를 보였으며, 요추골 골밀도보다 대퇴골 골밀도와 더 높은 상관관계를 나타냈다 (r=0.54 및 r=0.62). 그러나 이는 종골 음속이 종골 골밀도와 갖는 상관관계에 비하여 낮으므로 종골 음속은 골절률이 가장 높은 요추골 및 대퇴골부위의 골밀도를 평가하기에 최적의 지표가 아님을 알 수 있었다. 그러므로 요추골 및 대퇴골 부위의 골밀도를 보다 정확히 평가하기 위하여 이 부위의 음향특성을 직접 측정할 수 있는 정량적 초음파 기술을 개발할 필요가 있다.

방사선 입체조형치료용 다엽콜리메이터의 특성과 조직내 선량분포 측정 (Dose Distribution and Characterization for Radiation Fields of Multileaf Collimateor System)

  • 목적 : 다엽콜리메이터를 이용하여 부정형의 치료부위를 편리하고 정확히 차폐하며 삼차원 입체조형치료를 원활히 수행하기 위하여 다엽콜리메이터의 차폐특성을 계측하고 치료계획을 위한 최적방법을 확정한다. 대상 및 방법 : 차폐폭이 1cm이고 길이가 15cm인 52개의 금속차폐엽들이 양방향 각각 26개로 구성되어 컴퓨터계획에 의거 여러모양의 차폐면을 구사할 수 있는 다엽콜리메이터(Multileaf collimator)를 대상으로 방사선 치료에 요구되는 고에너지 방사선의 출력선량율, 차폐 및 투과효과, 반음영, 조직내 선량분포등을 전리함측정기와 열형광측정기 및 필름 선량측정방법을 이용하여 계측하였다. 다엽콜리메이터의 계단식 반음영 측정은 고체팬텀과 비디오밀도측정기를 이용하여 실효반음영(Effective penumbra)과 일반적반음영(General penumbra)으로 구분 평가하였다. 결과 : 다엽콜리메이터의 출력선량율은 표준차폐조리개에 비교하여 출력 선량율이 $1-2\%$ 증가 되었으며 치료오차 이내로 평가되었다. 다엽콜리메이터의 방사선투라 선량율은 6-10MV X선에서 평균 $2\%$로 표준차폐조리개보다 컸지만 혼합차폐벽돌보다 적었다. 원형으로 형성된 차폐엽 끝면의 조사선량은 약 $20\%$증가 되었으며 엽과 엽사이의 선량은 6mm 폭의 $5\%$증가로 심부치료에 영향이 없었다. 다엽콜리메이터의 실효반음영($20-80\%$)은 약 6mm로서 혼합차폐벽돌과 거의 같았으나 중심선에서 벗어날수록 $2-3\%$증가되었다. 다엽콜리메이터에 의한 계단식 모양의 반음영은 6-10MV x-선에서 차폐면 경사각이 45도일 때 약 11mm로 가장 컸으며 방사선속에 평행히 절단된 차폐벽돌보다 약 5mm 더 증가되었다. 결론 : 다엽콜리메이터는 1cm폭의 차폐엽들로 구성되어 계단식 차폐면에 대한 반음영이 다소 증가되고 있지만 조직내부의 다문조사 경우 반음영이 완화되므로 부정형 조사면 차폐에 적당하며 방사선 입체조형치료를 수행할 수 있는 가장 적당한 차폐기구로 생각된다.

Some Factors Affecting Bone Mineral Status of Premenopausal Women

  • This study was conducted to investigate dietary and other factors affecting bone mineral density (BMD) in Korean premenopausal women. Seventy-eight premenopausal women who visited health promotion center for health examinations volunteered to participate in this study and they were divided into two groups according to the bone status by T-score : normal or osteopenic group and osteoporotic group. The demographic and general characteristics, and dietary intake were surveyed using the questionnaire. BMDs of the lumbar spine and femoral neck of subjects were measured by dual energy X-ray absorptiometry. Serum samples were measured for Lipid concentration, and calcium, phosphorus, alkaline phosphatase as bone formation indicators. Urine was analysed for creatinine as bone resolution indicators. The results are as follows:The mean BMDs of the lumbar spine and femoral neck were 1.21 0.02g/cm$^2$ and 0.97 0.04g/cm$^2$, respectively and the BMD levels of osteoporotic group were significantly lower than that of normal or osteopenic group (p<0.001, respectively). Height in osteoporotic group was significantly lower than that of normal or osteopenic group (p<0.01), and bodyweight did not show any significant difference but had a lower tendency. Mean daily intake of energy was 1720 52ka1. When nutrient intake was compared with recommended dietary allowances (RDA) of subjects, calcium, Fe, vitamin A and riboflavin intakes showed means lower than RDA. The nutrient intake did not show any significant difference between normal or osteopenic group and osteoporotic group except intakes of protein fat and niacin. Serum and urine levels did not show any significant differences between normal or osteopenic group and osteoporotic group and all were within normal range, however, serum alkaline phosphatase level of osteoporotic group was significantly higher than that of normal or osteopenic group (p<0.001). Height showed positive correlations with lumbar spine bone mineral density (LBMD, r=0.332, p<0.01), no correlation was found with femoral neck bone mineral density (NBMD). Age, age at menarche, bodyweight, body mass index (BMI) and obesity showed no correlation with BMD. The BMD of the lumbar spine was significantly and positively related to the intake of niacin and vitamin C (r=0.236, p<0.05; r=0.274, p<0.05). Serum levels of calcium and phosphorus showed a negative correlation with LBMD (r=-0.698, p=0.0001; r=-0.503, p=0.0001, respectively). The results suggested that the BMDs of the lumbar spine was positively related to the intake of niacin and vitamin C in premenopausal women. Therefore, this study confirmed that one of the most effective way to minimize bone loss would be higher intake of niacin and vitamin C rich foods and habitual physical activity may have a beneficial effect on BMD in premenopausal period.

성인여성의 식습관과 영양섭취상태와 골밀도 및 골무기질함량과의 관계 (The Relationship between Food Habit, Nutrient Intakes and Bone Mineral Density and Bone Mineral Content in Adult Women)

  • Bones are important parts in sustaining the shape of the body, but they are also metabolic organs which undergo bone remodeling by constant bone resorption and formation. Osteoporosis, the typical metabolic bone disease, is characterized by a reduction in bone mineral density (BMD). Women more than men are at risk fir osteoporosis-related fractures, especially in the lumbar spine, wrist, and hip region. Risk of fracture depends on one's BMD, which open determined by the peak bone mass value achieved at skeletal maturity and followed by subsequent age-and menopause-related bone loss. Genetic and environmental factors are known to play a key role in bone metabolism and diet is considered as one of the important environmental factors. The purpose of the present study was to assess the status of BMD and bone mineral content(BMC) to clarify the relationships between dietary intakes and the risk of osteoporosis in adult women in Taegu. Subjects were 130 healthy females in between 20 and 69 years of age. BMD and BMC of the lumbar spine(venebrae L2-4) of the subjects were measured by dual energy X-ray absorptiometry. The average age of the subjects was 47.4${\pm}$11.7 years old, the average weight was 57.2${\pm}$8.4kg, the average age of menarche was 16.6${\pm}$1.9 years old and the average age of menopause was 48.4${\pm}$5.3 years old. The nutrient intakes of the subjects measured by the convenient method were generally lower than the level of RDA. The result of nutrient intake assessed has shown that the average energy intake was 1701${\pm}$316kca1 which is 85.1% of the RDA and the average calcium intake was 485.4${\pm}$172.3mg which is 69.3% of the RDA. The intakes of protein, vitamin A, vitamin B$_1$, niacin were greater than the RDA, whereas the remaining nutrient intakes were lower than the RDA. The average BMD of the subjects was shown to be 1.06${\pm}$1.09g/$\textrm{cm}^2$. The highest BMD of 1.24${\pm}$0.14g/$\textrm{cm}^2$ was noticed in the subjects of 30s compared to 20s, 40s, 50s, 60s. The BMD values were compared by the relative body weight(R3W) of the menopause subjects, and it was found that the underweight group had significant lower BMB while the rest of the groups did not have any differences in BMD. The most strongly correlated nutrient with BMD among the menopause subjects appeared to be calcium. The women whose Ca intakes were higher than 500mg showed the significantly higher BMD than those with Ca intakes lower than 500mg. This study suggests that the most effective way to prevent osteoporosis and to reduce the incidences of fractures seems to be minimizing bone loss through the adequate intake of calcium as well as avoiding underweight, especially in menopausal women. 여대생의 우유와 유제품 섭취실태 및 골밀도와의 관계 연구 (Relation between Milk Consumption and Bone Mineral Density of Female College Students in Korea)

  • The purpose of our study was to evaluate an association between milk consumption and bone mineral density (BMD) of female college students in Korea. The dietary data of 1,410 Korean college students living in the vicinity of Seoul area were collected through the Food Frequency Questionnaire. The mean age, height, weight and 8MI of the subjects were 20.4 years, 162.1 cm, 52.7 kg, and $20.0\;kg/m^2$ respectively. The mean intakes of energy, protein, and phosphorus were 2,398.7 kcal, 95.2 g, and 1,302.9 mg respectively, which were above their EER or Recommended Intake (RI), but the mean intake of calcium was 657.8 mg which was below its RI, and 44.0% of the subjects did not meet Estimated Average Requirement (EAR) of calcium. The daily milk consumption was 131.1 g, and the total consumption of milk and dairy product was 228.7 g. Forty five percent of the subjects consumed $50{\sim}200\;g$ of milk or dairy products. After the analysis of their dietary intakes, the subjects were divided into quintiles of total consumption of milk and dairy product, which were considered as major source of dietary calcium. Ninety two subjects from the upper quintile (UQ) and 85 subjects from the lower quintile (LQ) were selected and their bone mineral densities (BMD) of lumbar spine (L1-L4) and the femur were examined by the dual energy X-ray absorptiometry. The BMD $(g/cm^2)$of L1, L2, L3, and average of femur were 1.065, 1.150, 1.187, and 0.988 for the UQ subjects, which were significantly higher than those of the LQ subjects, 1.010, 1.090, 1.146, and 0.942 respectively. Moreover, the T-scores of the UQ were significantly higher than those of the LQ. The prevalence rates of osteopenia classified according to WHO's criterion were 12.0% in the UQ. and 20.0% in the LQ group which showed non significant. Blood levels of calcium, and phosphorus of UQ and LQ group were both within the normal range, and were not significantly different. Our results showed that UQ group presented the better status of bone mineral density and bone health. Therfore our results suggest that continuous consumption of more than one and a half cups of milk per day, which corresponds to the least amount of UQ group's consumption, would affect positively on the bone health of Korean adults.

폐경 후 여성의 골밀도에 따른 영양소 섭취상태와 혈청 구리, 아연, 망간 함량에 관한 연구 (A Study on Nutrient Intakes and Serum Levels of Copper, Zinc and Manganese in Korean Postmenopausal Women with Different Bone Mineral Density)

  • The purpose of this study was to investigate the relationships among nutrient intakes, BMD, serum copper, zinc and manganese contents in Korean postmenopausal women residing in Seoul and Kyunggi-do. Nutrient intakes analyses and anthropometric measurements were performed for 64 postmenopausal women. Also, fasting blood samples were collected. The BMDs of the lumbar spine (L2-L4) and femoral neck were measured by dual energy X-ray absorptiometry (DEXA). Subjects were classified into three groups: osteoporosis (T-score ${\leq}$ -2.5, n = 20), osteopenia (-2.5 < T-score ${\leq}$ -1, n = 24), and normal (T-score) -1, n = 20). The results are summarized as follows; the mean age, height and weight were 62.1 yrs, 153.8 em, and 56.1 kg, respectively, with no significant difference among three groups. On the other hand, the mean BMI $(23.7\;g/m^2)$ of normal group was significantly higher than those of other two groups. The mean energy intake was 1,395.3 kcal (77.5% of Dietary referance intakes for koreans (KDRls)). The average daily intakes of vitamin $B_2$, calcium and zinc did not reach KDRIs. The mean intakes of iron, copper, zinc and manganese were 10.9 mg, 1.6 mg, 7.8 mg and 5.1 mg respectively, with no significant difference among three groups, The mean serum levels of copper, zinc and manganese were 96.4 mg/dl, 91.5 mg/dl and 39.5 ng/dl, respectively with no significant differences among three groups. The BMD of lumbar spine showed a significantly negative correlation intakes of animal protein, fat and iron intakes after adjusting for age, Bill. However, the BMD of lumbar spine was not significantly correlated with animal iron intakes after adjusting for animal protein, and fat intakes, indicating animal food intake is a possible confounding factor. Proper intakes of vitamin $B_2$, vitamin C are required to protect osteoporosis in postmenopausal women. These results suggest that excessive animal food intakes may have adverse effect on bone mineral density. Further studies arc necessary to elucidate the role of serum minerals in BMD.
