Future Prospects of the Development of Calcium Antagonists

  • In considering the mechanism of action of the calcium antagonists, it is important to realize that there are three distinct receptor types and that the new classification divides these three drugs as members of the dihydropyridine, phenylalkylamines and benzothiazipines, respectively. The World Health Organization as well as the International Union of Pharmacology and Cardiology have adopted this classification. Unlike every other class of drugs, such as the alpha and beta adrenergic blocking agents, diuretics, etc., the calcium antagonists need to be thought of as three distinct drug classes. The reason they share some, but not all of the pharmacological profile is that they all act at specific receptor domains present in one large protein of 165 daltons present in all excitable tissue. This protein along with several other subunits make up what is known as the voltage-dependent calcium channel (the so-called "L"type, L-VDCC). The mechanism of action of the three drugs involve first a specfic binding and then an inhibition of the movement of calcium into the cell Some of these drugs, such as diltiazem, may have other interesting intracellular effects perhaps associated with protection of the mitochondria during ischemic insults. The nature of the receptor is being explored by molecular genetic techniques, and we have recently cloned two of the major subunits; some of the data will be presented.

Cornus officinalis Methanol Extract Upregulates Melanogenesis in Melan-a Cells

  • Cornus officinalis is widely distributed in Korea, and its fruit has been used to make as herbal drug for traditional medicine in Korea, Japan, and China because of its tonic, analgesic, and diuretic properties. However, the effects of C. officinalis methanol extract (COME) on melanogenesis remain poorly understood. We evaluated the melanogenic capability of COME in melan-a cells, which are immortalized mouse melanocytes. COME increased melanin synthesis in a dose-dependent manner. Treatment with $12.5{\mu}g/mL$ of COME significantly increased melanin content by 36.1% (p < 0.001) to a level even higher than that (31.6%) of 3-isobutyl-1-methyl-xanthine, a well-known pigmentation agent. COME also upregulated tyrosinase activity and its messenger RNA and protein expression. In addition, COME upregulated the expression of tyrosinase-related proteins 1 and 2 and microphthalmia-associated transcription factor-M messenger RNA expression. These results imply that COME may be appropriate for development as a natural product to treat hair graying.

Isolation and Drug Sensitivity of Gram-Negative Rods from Bovine Udder Infections (젖소 유방염(乳房炎)으로부터 Gram 음성간균(陰性桿菌)의 분리(分離) 및 약제감수성(藥劑感受性)에 관(關)한 연구(硏究))

  • One hundred and eighteen cultures of Gram-negative rods isolated from cases of clinical bovine mastitis during lactation were examined for distribution of specific types, and activity of several antimicrobial agents to the isolates was determined by two-fold tube dilution method employing sterile whole milk as fluid medium. Of the isolates, 59.2% were Escherichia coli. Most of the remaining isolates were Klebsiella pneumoniae and Enterobacter aerogenes. Most Gram-negative rods(89.8%) were isolated from acute local and chronic mastitis. The cases of peracute systemic form with a marked symptoms of toxemia were associated with Escherichia coli and Klebsiella pneumoniae. The minimal lethal concentrations (MLC) of gentamicin and oxolinic acid in sterile whole milk were 16-128 times higher than the MLC obtained in trypticase soy broth (TSB), while the MLC of ampicillin and tetracycline in milk increased 2-4 times compared with TSB. Of the drugs tested, gentamicin was the most active antibiotics with MLC of $100{\mu}g/ml$ in sterile whole milk against all of Gram-negative rods isolated from bovine udder infections.

The Associated Factors on Suicide Ideation among Adolescents in South Korea (청소년의 자살사고(思考) 관련 요인)

  • Purpose: Suicide was considered as a big public health concern in recent years because suicide mortality has been increased rapidly. This study was to investigate the prevalence and risk factors of suicidal ideation among adolescents in South Korea. Methods: The data of 2007 Youth Health Risk Behavior Web-based Survey collected by Korean Center for Disease Control were analyzed using logistic regression for this study. Results: The prevalence of suicidal ideation was 23.7% in South Korea. The risk factors of suicide ideation were depression (odds 8.43), perceived stress(odds 4.30), drug use (odds 2.47), sexual intercourse(odds 2.36), present smoking (odds 1.76), present alcohol drink (odds 2.39), feeling unhappiness (odds 2.89), living with none of parents (odds 1.71), gender(odds 1.72), health status (odds 1.66), and socioeconomic status (odds 2.0). Conclusion : Health risk behaviors and mental health status were related to suicide ideation. Health risk behavior such as smoking and drinking prevention programs and mental health promotion programs should be implemented to decrease suicide ideation among adolescents. Suicide, mental health and health risk behaviors should be prevented through comprehensive health promotion programs.

Treatment of Epidural-Morphine-Induced Pruritus: Propofol Versus Naloxone (경막외 Morphie 투여시 생기는 소양증에 대한 Propofol과 Naloxone의 효과 비교)

  • Background: Pruritus is the most frequent undesirable symptom associated with epidural morphine. It is unpleasant and often difficult to treat. Naloxone is presently the drug of first choice for treating this symptom. Naloxone however decrease the pain threshold in some cases. Recently it was reported subhypnotic doses of propofol were efficient in relieving epidural-morphine-induced pruritus(EMIP). In a prospective. randomized, double-blinded clinical trial, we compared the efficacy of propofol and naloxone for treatment of EMIP. Methods: Forty patients with EMIP were allocated to receive either 20 mg propofol, or 1.5 ${\mu}g/kg$ naloxone intravenously. Pruritus and level of postoperative pain were assessed after 5 min, using pruritus rating scale and visual analogue scale. Results: The overall success rate in treating pruritus was similar in both groups (propofol 70% vs naloxone 65%). Twenty-five percent of the patients in the naloxone group had an increase in the level of postoperative pain versus none in the propofol group(P=0.018). Conclusions: These results suggest propofol and naloxone are equally effective in treating EMIP. However, the level of postoperative pain is significantly reduced when treated with propofol.

Clinical Study of Aged Patients with Hoarseness (노인애성환자에 대한 임상적연구)

  • The voice of aged persons is known generally to be somewhat different from that of other adults, suggesting that laryngeal change occurs with advancing age. However, because knowledge of the voice characteristics of aged persons is limited, it is difficult to judge whether their voices arc normal. Chart review and laryngoscopic examination from ninety-one patients with hoarseness over the age of 60(1st group) and one hundred sixteen patients with hoarseness below the age of 50(2nd group) were done to define aging related voice disorders. The following results were obtained. 1) Associated diseases related to laryngeal disease were hypertension(12%), pulmonary disease(4.4%), thyroid disease(1.1%) in 1st group and hypertension(9.5%), thyroid disease(1.7%) in 2nd group. 2) The underlying diseases causing hoarseness in order of frequency were benign vocal fold lesion(37.7%), inflammatory disease(36.8%), functional dysphonia(17%) in 1st group and benign vocal fold lesion(43.6%), functional dysphonia(26.3%), inflammatory disease(16.5%) in 2nd group. 3) In stroboscopic findings, atrophy and sulcus of vocal cords are more prevalent in males than in females and edema of vocal cords is more common in females. Generally the voice characteristics of aged persons depend on the mass of the vocal folds which may be decreased through atrophy or be increased by edema. However, other factors such as systemic diseases, drug side effects and compensatory mechanism to presbylaryngis must be taken into account in diagnosing and treating voice disorders in aged persons.

Analysis of the Status of Infection Controls after Application of the Healthcare Accreditation System (의료기관 인증제 도입에 따른 감염관리 실태 분석)

  • Objectives : This study was conducted to measure the effects of healthcare accreditation (HA) on the changes in infection control (IC). Methods : Questionnaires were e-mailed to 60 hospitals from 23 October to 23 December, 2011. Data were analyzed by SPSS 12.0. Results : Finally 50 hospitals (83.0%) were enrolled in the study: Seoul area (40.0%), tertiary (76.0%), and >500 beds (98.0%). Nine hospitals (18.0%) had a full time infection control nurse[ICN] with 300 beds. Among various factors, ICN (36.0%), hospital facilities (66.0%), instruments (32.0%) and supplies (88.0%) all improved. Hand hygiene increased (53.1% vs 83.2%, p<.001), but it was continued only in 34.1% of hospitals. Healthcare-associated infection (68.4%), multi drug resistant organisms (42.1%) and outbreaks (26.3%) decreased. Reasons for difficulties in satisfying the HA standards were inadequate support which included hospital facilities, instruments, budget, and a shortage of ICNs and healthcare workers (HCWs). Conclusions : HA had effects on the IC, but they were transient. Staffing in ICN and HCW staffing, hospital facilities, instruments, and supplies all need to be improved.

Systems Biology - A Pivotal Research Methodology for Understanding the Mechanisms of Traditional Medicine

  • Objectives: Systems biology is a novel subject in the field of life science that aims at a systems' level understanding of biological systems. Because of the significant progress in high-throughput technologies and molecular biology, systems biology occupies an important place in research during the post-genome era. Methods: The characteristics of systems biology and its applicability to traditional medicine research have been discussed from three points of view: data and databases, network analysis and inference, and modeling and systems prediction. Results: The existing databases are mostly associated with medicinal herbs and their activities, but new databases reflecting clinical situations and platforms to extract, visualize and analyze data easily need to be constructed. Network pharmacology is a key element of systems biology, so addressing the multi-component, multi-target aspect of pharmacology is important. Studies of network pharmacology highlight the drug target network and network target. Mathematical modeling and simulation are just in their infancy, but mathematical modeling of dynamic biological processes is a central aspect of systems biology. Computational simulations allow structured systems and their functional properties to be understood and the effects of herbal medicines in clinical situations to be predicted. Conclusion: Systems biology based on a holistic approach is a pivotal research methodology for understanding the mechanisms of traditional medicine. If systems biology is to be incorporated into traditional medicine, computational technologies and holistic insights need to be integrated.

Antioxidant and Anti-hyperglycemic Effects of a Sanghwang Mushroom(Phellinus linteusau) Water Extract (상황버섯 열수추출물의 항산화 활성과 식후 혈당 상승 억제 효과)

  • The inhibitory activities of a water extract of Sanghwang mushroom(Phellinus linteusau)(SWE) against ${\alpha}$-glucosidases were evaluated in this study. Inhibiting these enzymes involved in the absorption of disaccharides significantly decreases the postprandial increase in blood glucose level after a mixed carbohydrate diet. Oxygen radical absorbance capacity and 1,1-diphenyl-2-picryl hydrazyl scavenging activities of the SWE were evaluated to investigate the antioxidant activity of the SWE associated with complications of long-term diabetes. Furthermore, the postprandial blood glucose lowering effect of SWE was compared to a known type 2 diabetes drug($Acarbose^{(R)}$) in a Sprague-Dawley rat model. SWE significantly reduced the blood glucose increase after sucrose loading. These results suggest that SWE, which has high ${\alpha}$-glucosidase inhibitory activity and high antioxidant activities, has the potential to contribute to a useful dietary strategy for controlling postprandial hyperglycemia.

Enhanced Sensitivity to Proteasome Inhibitor Bortezomib in Nrf2 Knockdown Ovarian Cancer Cells (Nrf2 영구 넉다운 난소암 세포주의 Proteasome 저해 항암제 Bortezomib에 대한 감수성 증가)

  • NF-E2-related factor 2 (Nrf2), a master regulator of antioxidant genes in animals, has been associated with the resistance of cancer cells to several cytotoxic chemotherapeutics. Bortezomib, a reversible inhibitor of the 26S proteasome, is a novel class anti-cancer therapeutics approved for the treatment of refractory multiple myeloma. However, the molecular mechanism of drug-resistance remains elusive. In the present study, bortezomib sensitivity has been investigated in Nrf2 knockdown ovarian cancer cells. When Nrf2 expression is stably repressed using interfering RNA expression, bortezomib-induced apoptosis and cell death were significantly enhanced compared to nonspecific RNA control cells. Knockdown cells showed elevated expression in the catalytic subunit PSMB5, PSMB6, and PSMB7 compared to the control, and failed to induce heme oxygenase-1 expression following bortezomib treatment. These indicate that differential proteasome levels and altered expression of stress-response genes could be underlying mechanisms of bortezomib sensitization in Nrf2-inhibited ovarian cancer cells.
