Monitoring of Pesticide Residues in Special Products (지역특산품 중 잔류농약 실태조사)

  • This experiment was carried out to analyze for pesticide residues in 17 different types of the special of geographical indication. We purchased 3 cereal grains, nuts and seeds, 3 fruits, 8 vegetables, mushrooms and other plants (Korean medicines) mainly at the agricultural cooperative's joints markets. Total 209 pesticides including multi-analysed pesticides (204) and single-analysed pesticides (5 ; acephate, methamidophos, monocrotophos, omethoate, vamidothion) were analysed with a GC/MS/MS, an HPLC/UVD (PDA) and a GC/FPD. No. 83 method and single-analysed method (Screening of multi-pesticide residue in the special products of geographical indication) of Korea Food Code was selected for validation in recovery and interferences of matrice. The results were as follows: among the selected 17 the special products, the residual pesticides were detected in 8 types of the special products (40 in 302 samples, detection ratio; 13.2%). All of the samples were not detected over MRLs, but tebuconazole, procymidone and isoprothioran were detected with considerable high frequency. These results could be used as KFDA official methods for the analysis of pesticide residues in foods and reference data will be provided to the related institutions.

Survey on Current Situations of Geriatric Patients' Medication and Drug Interactions Based on Prescriptions (노인환자들의 복약실태와 처방전상의 약물상호작용 검토)

  • It is common that geriatric patients are on several medications at the same time. With this situation on hand, this study has collected prescriptions of individual geriatric patient and investigated possible drug interactions. In order to minimize the drug interactions and protect those patients from adverse reactions of medication, the gradual implementation and management of the medication history of each individual patient, the establishment of medication counseling system on medicines particularly in need of obligatory advice, the use of an inspection system on drug interactions at the time of prescription, and the implementation of a patient monitoring system on the drugs with narrow margin of safety at hospitals.

  • Purpose: This study aims to investigate the knowledge and attitudes regarding adverse drug reactions (ADR) of emergency medical services (EMS) students, so that suggestions for further education on ADR can be made. Methods: A survey on knowledge and attitude was created and modified according to Lee's and Kim's test tool. In total, 149 students' data were collected and compared with previous studies. Results: Knowledge of ADR was relatively low (54%) but attitude was relatively high (75%). Clinical experience was important in knowledge and attitude. Knowledge and attitude showed a positive correlation. Conclusion: Increased knowledge of ADR leads to a more active attitude towards it. Hence, modification of the education system to provide a more personalized education is required.

A Study on Measures of Korean Drug Policy Improvement Against Increasing Foreign Drug Offenders (외국인 마약사범 증가에 따른 국내 마약정책 개선방안)

  • Purpose: The purpose of this paper is to examine the recently rising number of foreign drug offenders in Korea and suggest improvement measures for the Korean drug policy. Method: This paper analyzed the preceding research, statistical data, etc., on related fields and referred to overseas cases to suggest improvement measures. Result: The foreign population living in Korea increased in 2000s, and the foreign drug offender cases are increasing accordingly. The survey on foreign drug offenses showed that the case of foreign drug offender is higher than the case of Korean drug offenders by 1.2 to 3.3 times. In 2021, the foreign drug offender cases increased by 6.5 times compared to 2012. Especially, the number of psychoactive drug-related drug offenses increased dramatically. The drug smuggling also remarkably increased starting from 2016 as some people earned a lot of money from smuggling the drugs Conclusion: To cope with the foreign drug offenders effectively, this paper suggested intensive monitoring of the foreign drug offenders and weighting on the drug offenders depending on their home country to intensify the entry standards on foreigners who are likely to smuggle drugs.

Monitoring and Exposure Assessment of Pesticide Residues in Domestic Agricultural Products (국내 유통 다소비 농산물의 잔류농약 모니터링 및 노출평가)

  • This study was implemented to evaluate food safety on residual pesticides in agricultural products of Korea and to use as a data base for the establishment of food policy. A total of 196 pesticide upon these products were analyzed using multi class pesticide multiresidue methods of Korean Food Code, and 232 samples of 15 agricultural products collected from 9 regions were supplied for this study. In the results, 64 kinds of pesticides were detected in 53 samples, chlorpyrifos and procymidone of them were shown a high frequency of detection in the analyzed pesticides. Among them, two samples (chlorpyrifos in perilla leaves and picoxystrobin in peach) were detected over Maximum Residue Limits (MRLs). The levels of the detected pesticide residues were within safe levels. Also, the intake assessment for pesticide residues including chlorpyrifos at multi pesticide residue monitoring were carried out. The result showed that the ratio of EDI (estimated daily intake) to ADI (acceptable daily intake) was 0.001~0.902% which means that the detected pesticide residues were in a safe range so that residual pesticides in the agricultural products in Korea are properly controlled.

  • With the diversification and internationalization of the food industry and the increased focus on health from a majority of consumers, food safety policies are being implemented based on scientific evidence. Risk analysis represents the most useful scientific approach for making food safety decisions. Total diet study (TDS) is often used as a risk assessment tool to evaluate exposure to hazardous elements. Many countries perform TDSs to screen for chemicals in foods and analyze exposure trends to hazardous elements. TDSs differ from traditional food monitoring in two major aspects: chemicals are analyzed in food in the form in which it will be consumed and it is cost-effective in analyzing composite samples after processing multiple ingredients together. In Korea, TDSs have been conducted to estimate dietary intakes of heavy metals, pesticides, mycotoxins, persistent organic pollutants, and processing contaminants. TDSs need to be carried out periodically to ensure food safety.

Web based anticancer drug management system using ubiquitous sensor network and RFID (USN과 RFID를 이용한 웹 기반 항암제 관리 시스템)

  • In order to monitor the anticancer drug in stable conditions, the Web based anticancer drug management system and alarm services were constructed and assessed in this study. Anticancer drug should be exact to the correct patient in the right environment. To overcome the restriction of existing equipment that only monitors fragmentarily, temperature and humidity were continuously monitored to maintain stable environments using sensor networks and RFID for the monitoring and management of anticancer drug. Construction drug identification and the effect of normal air outside the anticancer dispensary with obstacles were evaluated in working hour. Pre-installed control system in the dispensary could be alternated with auto sensing and alarming. We expected that the efficiency of anticancer drug management and the reliability of drug medication by handwork would be increase accordingly.

Outcomes and Use of Therapeutic Drug Monitoring in Multidrug-Resistant Tuberculosis Patients Treated in Virginia, 2009-2014

  • Background: Reports of therapeutic drug monitoring (TDM) for second-line medications to treat multidrug-resistant tuberculosis (MDR-TB) remain limited. Methods: A retrospective cohort from the Virginia state tuberculosis (TB) registry, 2009-2014, was analyzed for TDM usage in MDR-TB. Drug concentrations, measured at time of estimated peak ($C_{max}$), were compared to expected ranges. Results: Of 10 patients with MDR-TB, 8 (80%) had TDM for at least one drug (maximum 6 drugs). Second-line drugs tested were cycloserine in seven patients (mean $C_{2hr}$, $16.6{\pm}10.2{\mu}g/mL$; 4 [57%] below expected range); moxifloxacin in five (mean $C_{2hr}$, $3.2{\pm}1.5{\mu}g/mL$; 1 [20%] below); capreomycin in five (mean $C_{2hr}$, $21.5{\pm}14.0{\mu}g/mL$; 3 [60%] below); para-aminosalicylic acid in five (mean $C_{6hr}$, $65.0{\pm}29.1{\mu}g/mL$; all within or above); linezolid in three (mean $C_{2hr}$, $11.4{\pm}4.1{\mu}g/mL$, 1 [33%] below); amikacin in two (mean $C_{2hr}$, $35.3{\pm}3.7{\mu}g/mL$; 1 [50%] below); ethionamide in one ($C_{2hr}$, $1.49{\mu}g/mL$, within expected). Two patients died: a 38-year-old woman with human immunodeficiency virus/acquired immune deficiency syndrome and TB meningitis without TDM, and a 76-year-old man with fluoroquinolone-resistant (pre-extensively drug-resistant) pulmonary TB and low linezolid and capreomycin concentrations. Conclusion: Individual pharmacokinetic variability was common. A more standardized approach to TDM for MDR-TB may limit over-testing and maximize therapeutic gain.

A monitoring survey on antimicrobial resistance of bacterial isolates from companion dogs in Incheon (인천지역 반려견 유래 세균의 항생제 내성 모니터링 조사)

  • This study was performed to investigate antimicrobial resistance in bacterial isolates obtained from companion dogs in veterinary hospitals and an animal shelter in Incheon. Drug resistance was examined respectively with the isolates of Escherichia coli, Enterococcus faecalis, and Staphylococcus pseudintermedius. The prevalence of drug resistance was calculated for each bacterial species towards 163 E. coli isolates, 156 E. faecalis isolates, and 86 S. pseudintermedius isolates by using selected antimicrobials. E. coli isolates were highly resistant to ampicillin, ciprofloxacin and tetracycline (47.9%, 28.2% and 28.2%, respectively). E. faecalis isolates were highly resistant to quinupristin-dalfopristin, tetracycline, kanamycin, rifampicin (69.8%, 66.0%, 53.8% and 51.9%, respectively). Higher levels of resistance were detected for ampicillin, penicillin, tetracycline, erythromycin, trimethoprim/sulfamethoxazole, telithromycin in S. pseudintermedius isolates (83.7%~52.6%, respectively). Occurrence of methicillin-resistant S. pseudintermedius (MRSP) was confirmed by oxacillin disc diffusion method, resulted in 23.3% occurrence among the S. pseudintermedius isolates (20/86 strains). The occurrence ratio of multidrug-resistance in the isolates of E. coli, E. faecalis, and S. pseudintermedius was 34.5%, 56.9%, and 67.9%, respectively. In this study, higher levels of antimicrobial drug resistance were observed in bacterial isolates obtained from dogs in Incheon. A regular monitoring and surveillance program should be implemented to prevent the emergence and spread of the drug-resistant bacteria carried in companion dogs.

Analysis of Biurea Decomposed from Azodicarbonamide in Food Products by High Performance Liquid Chromatography Mass/Mass Spectrometry (LC/MS/MS를 이용한 식품 중 아조디카르본아미드 분해산물인 Biurea 함량 분석)

  • This study was conducted to establish a method to analyze biurea decomposed from azodicarbonamide in processed foods such as wheat flour and bread. New method was developed using high performance liquid chromatography mass/mass spectrometry to determine biurea in wheat flour and bakery products. The recovery rate was 94.3-112.5%. The limit of detection for biurea was 0.003 mg/kg, and the limit of quantification was 0.01 mg/kg. The monitoring results for biurea content using established methods showed that biurea was detected at 2.76 mg/kg in the azodicarbonamide-detected flour (detection rate, 2%). The detection rate in processed foods such as baked goods was 27% (16/59). The detection range was 0.19-18.01 mg/kg (average, 3.79 mg/kg). However, it was thought that the detection level was safe due to much lower values than the standard (45 mg/kg). As a result, the newly established biurea analytical method will contribute to the management of azodicarbonamide in processed foods such as wheat flour and bakery products.
