Statment of problem : Little in known about the importance of selection of various double crowns as retainer in determining the outcome of treatment with double crown retained removable partial denture. Purpose : To obtain information about the effects and the results of this treatment modality. Material and methods : This study describes 61 double crown retained removable partial dentures worn by 51 patients from Samsung Medical Center, Seoul a time ranging between 6 and 48 months and evaluate occlusal contacts on the denture teeth, denture movement, incidence of denture relining, denture retention, fracture of dentures and abutments, hygiene, residual ridge inflammatory changes, number of lost abutment,. interruption of denture use. Results : One tenth of all the restorations were relined. Restorations in 21 arches fractured repeatedly for various reasons . There was no apparent interrelationship between fractures and the five groups. Conclusion : Good prognoses of removable partial dentures were shown in all groups(Kennedy Classes I, II, III, combination and few remaining abutment).
PURPOSE. The objective of this literature review was to analyze the cumulative survival rates (CSRs) of rigid and non-rigid double-crown-retained removable dental prostheses. MATERIALS AND METHODS. Screening of the literature published from January 1995 to December 2019 was performed by using electronic data base (Pubmed) and manual search. The CSRs of rigid and non-rigid double crown removable dental prostheses were investigated. RESULTS. A total of 403 articles were reviewed and 56 relevant articles of them were selected. Subsequently, 25 articles were included for data extraction. These articles were classified according to rigid and non-rigid type double crowns and further subdivided into teeth, implants, and teeth-implant combination types. The CSRs of rigid type double crown ranged from 68.9% to 95.1% of 5 to 10 years in tooth abutments, 94.02% to 100% over a 3-year mean observation periods in implant abutments, and 81.8% to 97.6% in tooth-implant combination. Non-rigid type double crowns had various CSR ranges from 34% to 94% maximum during 10 years observation in teeth abutment. The CSRs of non-rigid type had over 98% in implant abutments, and ranged from 85% to 100% in tooth-implant combination. CONCLUSION. The CSRs of double crowns varies according to types. With accurate evaluation of the remaining teeth and plan of the strategic implant placement, it could be successful treatment alternatives for partially or completely edentulous patients.
Statement of problem. Various double crown systems have been used with removable partial dentures in the clinical field. Although retentive force between inner and outer crown are affected by several factors, differences between the retentive forces of different double crown system types are expected. Purpose. The purpose of this study was to evaluate the initial retentive force of outer crowns fabricated by the conventional casting technique in conus and hybrid double crowns. Material and methods. Ten double crowns were fabricated. The groups were as follows. Group 1, double crowns of hybrid inner and outer crowns using the conventional casting method; Group 2, double crowns of conus inner and outer crowns using the conventional casting method. Tensile strengths of double crowns when the inner and outer crowns were separated on a universal testing machine were measured. These values of retentive force were then statistically analyzed using the Kruskal-Wallis test. Results. Retentive force in group 2 was significantly higher than that in group 1(p<.05). Conclusion. The initial retentive forces of double crowns were affected by the types of the double crown system.
치주적으로 지지가 불리한 소수의 잔존치가 남은 환자에게 있어 하이브리드 텔레스코픽형 이중관의치는 장기적으로 안정적인 예후를 보이는 치료가 된다. 편측 악궁에 소수 잔존치가 남고 반대측 악궁에 다수의 자연치가 존재 하는 경우 임플란트를 식립하여 자연치와 혼재된 형태의 이중관 의치를 제작 할 시 의치의 지지 및 안정요소의 향상과 더불어 저작력의 균형을 맞추어 줄 수 있게 된다. 본 증례는 55세 여자 환자로 만성 치주염으로 인해 치주 지지가 감소한 소수 잔존치 상태에서 자연치만으로 하이브리드 텔레스코픽형 이중관 의치 치료를 통해 9.5년간 의치를 사용하였다. 9.5년 후 지대치 파절로 내원 하여 임플란트의 식립을 통해 자연치와 임플란트가 혼재된 하이브리드 텔레스코픽형 이중관 의치로 재수 복 하였으며, 이를 통해 재 치료에서도 장기적 예후의 안정성을 기대 할 수 있다.
이중관 가철성 국소의치는 지대치 간의 이차고정(secondary splinting), 응력의 수직 방향, 지대치 지렛점(fulcrum)의 내 외관 margin 부위 위치로 인한 지렛대 길이(the length of lever arm)의 감소로 소수 잔존치이거나 지대치의 상태가 좋지 않은 경우에도 성공적으로 사용될 수 있다. 또한 이중관(double crown)을 이용한 의치는 치아가 얼마 남지 않은 부분 무치악 환자 뿐만 아니라, 소수의 임플란트를 식립한 임플란트 오버덴처에 있어서 성공적인 치료결과를 보여주고 있다. 본 증례에서는 상악은 소수 치아가 잔존했고, 하악은 치조골 흡수 및 수축이 심하고 임플란트가 실패한 상태였다. 상악의 소수 잔존치는 치주적으로 불량하였고, 하악에는 2개의 임플란트만을 다시 식립하였다. 이러한 불리한 조건에서 이중관 가철성 국소의치를 이용한 치료를 계획하였다. 1년여 정도의 관찰 기간 동안 기능적, 심미적으로 만족스러운 결과를 얻을 수 있었으며, 이중관 가철성 국소의치의 효용성을 증명하였기에 본 증례를 보고하고자 한다.
소수 잔존치의 부분 무치악 환자들에 있어 이중관을 이용한 치료의 성공적인 결과에 대해 여러 논문에서 보고되고 있다. 특히 이중관을 이용한 가철성 국소의치는 잔존치의 치주 상태가 좋지 않고 클라스프 유지 가철성 국소의치가 실패할 가능성이 있는 경우(crown/root ratio > 1) 대체적인 치료가 될 수 있다. 본 증례에서는 상하악 양측 구치부 상실과 전반적인 치아 동요로 인하여 저작의 어려움을 주소로 내원한 환자에서 심한 치주질환으로 다수의 치아 발치 후 하악은 전치부 고정성 수복과 자연치의 구강형성을 통해 Kennedy class I 가철성 국소의치로 치료 계획을 하였고 상악은 클라스프 유지 가철성 국소의치로 수복하기에 예후가 불분명하여 프릭션 핀을 이용한 하이브리드 텔레스코픽 이중관 국소의치로 치료를 계획하였다. 이와 같이 보철 치료 완료 후 환자는 잘 적응 하였으며 발음, 저작, 유지, 심미적으로 만족하는 결과를 얻었기에 이 증례를 보고하는 바이다.
잔존 지대치의 수가 적은 경우 이중관 가철성 국소의치를 사용한 여러 보고에서 우수한 결과가 나타남을 확인할 수 있다. 특히 치주 상태가 양호하지 않고 고립된 소수 잔존치를 지대치로 이용하여 국소의치로 수복해야 하는 경우에 프릭션핀을 이용한 하이브리드 텔레스코픽 이중관은 적절한 적응증이 될 수 있다. 본 증례는 심한 치주염으로 다수 치아 발치 후 상악은 전치부 고정성 보철 수복과 Kennedy class I 가철성 국소의치로 수복하고 하악은 양측성 고립 지대치만 남아있어 클래스프 국소의치로 수복하기에 예후가 불분명하여 프릭션핀을 이용한 하이브리드 텔레스코픽 이중관 국소의치를 이용하였다. 이중관 가철성 국소의치의 특성과 효용성을 고찰해 보고자 본 증례를 보고한다.
소수 잔존치가 남아있거나 치주적으로 불리한 경우 전통적인 가철성 국소의치의 생역학적 예후가 불분명하다. 이러한 경우, 하이브리드 텔레스코픽 이중관 국소의치(hybrid telescopic double crown RPD)는 성공적인 치료의 대안이 될 수 있다. 이 치료방법의 경우 지대치 간의 이차고정 효과가 있으며 응력의 방향이 수직적이고, 지대치 발치 시에 의치의 수리가 용이하다는 점에서 전통적인 가철성 국소의치에 비해 장점을 보인다. 본 증례에서는 전치부의 과개교합을 가진 상하악의 부분 무치악 환자에 있어 상악에는 이중관 의치, 하악에는 임플란트 및 고정성 보철치료가 시행되었다. 장기간 동안 경과관찰 및 유지관리 치료를 시행한 결과, 기능 및 심미적인 면에서 만족할 만한 성과를 얻었기에 이를 보고하고자 한다.
치아가 얼마 남지 않은 부분 무치악 환자들에 있어 이중관(Double crown)을 이용한 치료의 성공적인 결과에 대해서 여러 문헌에서 보고 되고 있다. 이중관 가철성국소의치(Double crown removal partial denture)는 잔존치의 치주적인 상태가 좋지 않은 환자에게 치료의 대안이 될 수 있다. 이중관 가철성 국소의치는 특히 치주상태가 좋지 않고, 임플란트를 식립하기에는 의과적으로 문제가 있는 환자(Medically compromised patient)에게는 수술에 대한 부담 없이 치료할 수 있어 심리적이고 경제적인 이점이 존재한다. 본 증례에서는 상악은 다수의 치아가 잔존하여 고정성 보철물로 수복이 가능한 상태였고, 하악은 소수 잔존치만 남아있고 치주 상태가 매우 불량하여 잔존치를 이용한 클라스프 가철성 국소의치로는 수복하는 것이 거의 불가능하였다. 또한 환자는 1년 전에 하악 전치부 부위의 골수염으로 인한 수술병력으로 임플란트 식립이 어려운 상황이었다. 이러한 악조건에서 하악의 소수 잔존치를 이용한 이중관 의치는 2년이상의 기간동안 기능적, 심미적인 면에서 만족스러운 결과를 얻었기에 이를 보고하고자 한다.
PURPOSE. The purpose of this study was to analyze the retention force changes and wear behaviours of double-crown systems over long-term use. MATERIALS AND METHODS. Ten groups, each consisting of six samples, were evaluated. Specifically, casting gold alloy primary crown - casting gold alloy secondary crown (AA), laser sintering primary crown - laser sintering secondary crown (LL), casting Cr alloy primary crown - casting Cr alloy secondary crown, (CC) zirconia primary crown - electroformed secondary crown (ZA), and CAD/CAM titanium alloy primary crown - CAD/CAM titanium alloy secondary crown (TT) groups were evaluated at cone angles of $4^{\circ}$ and $6^{\circ}$. The samples were subjected to 5,000 insertion-separation cycles in artificial saliva, and the retention forces were measured every 500 cycles. The wear levels were analyzed via SEM at the beginning and end of the 5,000 cycles. RESULTS. In all samples, the retention forces increased when the conus angle decreased. The highest initial and final retention force values were found in the $LL-4^{\circ}$ group (32.89 N-32.65 N), and the lowest retention force values were found in the $ZA6^{\circ}$ group (5.41 N-6.27 N). The ZA groups' samples showed the least change in the retention force, and no wear was observed. In the other groups, wear was observed mostly in the primary crowns. CONCLUSION. More predictable, clinically relevant, and less excursive retention forces can be observed in the ZA groups. The retention force values of the LL groups were statically similar to those of the other groups, except the ZA groups.
