Convergence Study on Ability and Accuracy in Drug Dose Calculations of Nursing Students (간호학생의 약물용량계산능력과 정확도에 관한 융합적 연구)

  • This study is to find the weaknesses in drug dose calculations of nursing students by analyzing their general characteristics, confidence(C), performances in classes(PC) and exam sheets of drug dose calculations. The data were analyzed using SPSS 19.0. There was no significant difference between male and female students in simple calculation ability(SCA), but male students' mean scores were higher in applied calculation ability(ACA) and accuracy on calculation(AC). There were no significant differences in calculation abilities according to high school departments. And students were weak in questions which are needed ACA. The explanation rate of C on PC and PC on AC were 4.2% and 3.2% respectively, so there were weak positive effects from C to PC, and PC to AC. This study suggests that there is a need to develop and implement an intervention program according to students' abilities to increase AC, C, and PC by studying math on an ongoing basis. This study needs to be cautious in generalizing, because the data set was limited to nursing students from one university.

A Comparison of Dose in Changed Technique Factor Using X-ray Imaging System (X-선 장치의 기술적 인자의 변화에 따른 선량 비교 평가)

  • With the recent development of diagnosis using radiation and increasing demand of the medical treatment, we need to minimize radiation exposure dose. So, This is the method which reduce patient dose by measuring surface dose of radiographic change factor and by comparing theoretical and actual dose, when we take an X-ray which is generally used. By changing the factor of kV, mAs, FSD, whose range is 60 to 120 kV, 20 to 100 mAs, 80 to 180 cm, we compared theoretical surface dose with actual surface dose calculated by the simple calculation program, Bit system, and NDD-M method As a result, when kV and mAs were higher, theoretical surface dose and actual surface dose were more increased. but the higher FSD was, the more decreased surface dose was. According to this, the error were measured about 0.1 to 0.2 mGy in low dose part and about 0.7 to 1.5 mGy in high dose part. Therefore, this shows that theoretical surface dose calculation method is more correct in low dose part than in high dose part. In conclusion, we will have to make constant efforts which can reduce patient and radiographer's exposure dose, studying methods which can predict patient's radiation exposure dose more exactly.

  • In this study, we summarized the results of "Pre-study for the development of Polar route space radiation forecast model", funded by National Meteorological Satellite Center, Korea Meteorological Administration. We investigated the aviation space weather-related literature and the airline companies's operation manual associated with the space weather. We also identify the strengths and weaknesses of many pre-existing space radiation calculation programs, and find the potential to be improved. Until now, we don's have our own space radiation calculation program, so we need more improved space radiation calculation program which will be developed by ourselves. Currently most space radiation calculation programs cannot reflect temporary variations in the solar activities and the space weather. Here we analyzed the strengths and weaknesses of those programs, which are widely used in typical space radiation calculations. Finally to reflect the real-time space weather effects in the forecast model, we need to develop more precise forecast model. For that purpose, we suggest the following four steps: (1) at first, we have to choose the ground-based radiation dose calculation program, (2) we have to select a proper atmospheric model in aircraft altitude, (3) we combine the selected ground cosmic radiation dose calculation program and the selected atmospheric model, and finally (4)we have to reflect the real time space weather information and space weather forecast into the newly combined model.

Evaluation of Clinical Risk according to Multi-Leaf Collimator Positioning Error in Spinal Radiosurgery (척추 방사선수술 시 다엽콜리메이터 위치 오차의 임상적 위험성 평가)

  • The purpose of this study is to evaluate the clinical risk of spinal radiosurgery by calculating the dose difference due to dose calculation algorithm and multi-leaf collimator positioning error. The images acquired by the CT simulator were recalculated by correcting the multi-leaf collimator position in the dose verification program created using MATLAB and applying stoichiometric calibration and Monte Carlo algorithm. With multi-leaf collimator positioning error, the clinical target volume (CTV) showed a dose difference of up to 13% in the dose delivered to the 95% volume, while the gross tumor volume (GTV) showed a dose difference of 9%. The average dose delivered to the total volume showed dose variation from -8.9% to 9% and -10.1% to 10.2% for GTV and CTV, respectively. The maximum dose delivered to the total volume of the spinal cord showed a dose difference from -14.2% to 19.6%, and the dose delivered to the 0.35 ㎤ volume showed a dose difference from -15.5% to 19.4%. In future research, automating the linkage between treatment planning systems and dose verification programs would be useful for spinal radiosurgery.

Three dimensional Dose reconstruction based on transit dose measurement and Monte Carlo calculation (조사문 선량 분포와 Monte Carlo 계산을 이용한 삼차원 선량 재구성에 관한 연구)

  • This is a preliminary study for developing the method of the dose reconstruction in the patients, irradiated by mega-voltage photon beams from the linear accelerator, using the transit dose distributions. In this study we present the method of three-dimensional dose reconstruction and evaluate the method by computer simulation. To acquire the dose distributions in the patients (or phantoms) we first calculate the differences between the doses at the arbitrary points in the patients and the doses at the corresponding points where the transit doses are measured. Then, we can get the dose in the patients from the measured transit dose and the calculated value of the difference. The dose differences are calculated by applying the inverse square law and using the linear attenuation coefficient. The scatter to primary dose ratios, which are calculated by the Monte Carlo program using the CT data of the patient (or phantoms), are also used in the calculations. For the evaluation of this method we used various kinds of homogeneous and inhomogeneous phantoms and calculated the transit dose distributions with the Monte Carlo program. From the distributions we reconstructed the dose distributions in the phantom. We used mono-energy Photon beam of 1.5MeV and Monte Carlo program EGS4. The comparison between the dose distributions reconstructed using the method and the distributions calculated by the Monte Carlo program was done. They agreed within errors of -4%∼+2%. This method can be used to predict the dose distributions in the patient

Calculation of Dose Distribution for SBRT Patient Using Geant4 Simulation Code (Geant4 전산모사 코드를 이용한 SBRT 환자의 선량분포 계산)

  • The Monte Carlo based dose calculation program for stereotactic body radiotherapy was developed in this study. The Geant4 toolkit widely used in the radiotherapy was used for this study. The photon energy spectrum of the medical linac studied in the previous research was applied for the patient dose calculations. The geometry of the radiation fields defined by multi-leaf collimators were taken into account in the PrimaryGeneratorAction class of the Geant4 code. The total of 8 fields were demonstrated in the patient dose calculations, where rotation matrix as a function of gantry angle was used for the determination of the source positions. The DicomHandler class converted the binary file format of the DICOM data containing the matrix number, pixel size, endian type, HU number, bit size, padding value and high bits order to the ASCII file format. The patient phantom was constructed using the converted ASCII file. The EGSnrc code was used to compare the calculation efficiency of the material data.

Comparison of plan dosimetry on multi-targeted lung radiotherapy: A phantom-based computational study using IMRT and VMAT

  • This work analyzed the dosimetric difference between the intensity modulated radiotherapy (IMRT), partial/single/double-arc volumetric modulated arc therapy (PA/SA/DA-VMAT) techniques in treatment planning for treating more than one target of lung cancer at different isocenters. IMRT and VMAT plans at different isocenters were created systematically using a Harold heterogeneous lung phantom. The conformity index (CI), homogeneity index (HI), gradient index (GI), dose-volume histogram and mean and maximum dose of the PTV were calculated and analyzed. Furthermore, the dose-volume histogram and mean and maximum doses of the OARs such as right lung, contralateral lung and non GTV were determined from the plans. The IMRT plans showed the superior target dose coverage, higher mean and maximum values than other VMAT techniques. PA-VMAT technique shows more lung sparing and DA-VMAT increases the V5/10/20 values of contralateral lung than other VMAT and IMRT techniques. The IMRT technique achieves highly conformal dose distribution to the target than other VMAT techniques. Comparing to the IMRT plans, the higher V5/10/20 and mean lung dose were observed in the contralateral lung in the DA-VMAT.

Production and Application of the Dose Calculation Program which used MS EXCEL and Bit System (MS EXCEL 및 Bit system을 이용한 피부선량 계산 프로그램의 제작 및 응용)

  • The medical diagnostic examination using ionizing radiation has improved the patients' life and brought revolution in medical examination along with the mechanical development. However, the development of medical imaging systems has also been the reason to increase the patients' exposure for ionizing radiation. ICRP recommends that each country adopts diagnostic reference levels depending on regional and national situations. The Korea Food & Drug Administration suggested the dosimetry measurement guideline for patients in 2007. Nonetheless, in reality, it is hard to know the skin dose of the patients when applying a x-ray since there is no radiation dosimeter in most of clinical situation. Therefore, this study sets a program based on the bit system to figure out easily the skin dose of a patients using MS Excel program in the PC setting. The results showed 10% better outcome.

  • The use of GEANT4 simulation toolkit has increased in the radiation medical field for the design of treatment system and the calibration or validation of treatment plans. Moreover, it is used especially on calculating dose simulation using medical data for radiation therapy. However, using internal visualization tool of GEANT4 detector constructions on expressing dose result has deficiencies because it cannot display isodose line. No one has attempted to use this code to a real patient's data. Therefore, to complement this problem, using the result of gMocren that is a three-dimensional volume-visualizing tool, we tried to display a simulated dose distribution and isodose line on medical image. In addition, we have compared cross-validation on the result of gMocren and GEANT4 simulation with commercial radiation treatment planning system. We have extracted the analyzed data of dose distribution, using real patient's medical image data with a program based on Monte Carlo simulation and visualization tool for radiation isodose mapping.
