Effects of Dohongsamul-tang on Surgically Induced Endometriosis in Rats (도홍사물탕(桃紅四物湯)이 자궁내막증(子宮內膜症)을 유발한 흰쥐에 미치는 영향(影響))

  • Purpose : This study was to investigate the effects of Dohongsamul-tang extract on the rat with an surgically induced endometriosis. Methods : Four weeks after the operation for inducing the endometriosis to a matured female rat, the normal hyperplasia of the transplanted endometrium was identified in anatomico-histological aspects. By dividing each of the control group and Dohongsamul-tang administered experimental group into 8 rats, Dohongsamul-tang concentrates were administered orally to the experimental group with 1g/1ml/200g everyday for 40 days and then images were taken from the macroscopic tissues which were transplanted to the mesenterium, and the concentrations of progesterone, estradiol, tumor necrosis $factor-{\alpha}(TNF-{\alpha})$, and interleukin(IL)(-2, -4, -6, and -10) in the serum were measured. Results : Transplanted endometrium tissues showed the histological findings in accordance with the normal endometrium tissues and from the macroscopic findings the size of transplanted endometrium tissues showed a definite decrease. Compared to the control group, the experimental group showed significant decreases in the values of $TNF-{\alpha}$,IL-2, and IL-4 and significant increases in the values of IL-10. However, there were no significant differences in progesterone, estradiol and IL-6. Conclusion : From the above results, Dohongsamul-tang showed the strengthening of immunological function and anti-inflammatory effect. Therefore it is considered that Dohongsamul-tang will be very effective on the treatment of inflammation developed endometriosis, especially to treat the endometriosis without affecting the ovary functions.

A Case Report of Chemotherapy-Induced Hand-Foot Syndrome Treated with Modified Dohongsamul-tang (가미도홍사물탕 약욕요법 이후 호전된 항암화학요법 유발 수족증후군 1례)

  • Hand-foot syndrome (HFS) frequently occurs after receiving chemotherapy, such as sorafenib, in patients with hepatocellular carcinoma (HCC), and this impairs quality of life (QOL). The prevalence of HFS due to sorafenib is 10-28%. The effective methods to treat HFS are not established aside from temporary discontinuation of chemotherapy agents and drug dosage adjustment because the mechanism of HFS has not been elucidated. According to a previous study, modified Dohongsamul-tang was effective in the treatment of patients with HFS and improved patients' QOL. Based on these results, we administered the soaking method with modified Dohongsamul-tang to a 50-year-old male patient with HCC complaining of HFS due to sorafenib. After two weeks, the symptoms of HFS were improved despite taking sorafenib. We suggest that modified Dohongsamul-tang is a novel method for treating HFS.

Experimental Study of Dohongsamul-tang (Taohongsiwu-tang) on Fracture Healing (도홍사물탕(桃紅四物湯)이 골절 유합에 미치는 실험적 연구)

  • Objectives The purpose of this study is to evaluate the bone healing effect of Dohongsamul-tang (Taohongsiwu-tang; DH) on femur fractured mice. Methods Mice were randomly divided into 4 groups (naive, control, positive control and DH). All groups except naive group were subjected to bone fracture on both hind limb femurs. Naive group received no treatment at all. Control group was fed with normal saline, and positive control group was orally medicated with tramadol. DH-treated group was orally medicated with DH. We analysed the levels of BMP2, COX2, Col2a1, Sox9, Runx2, and Osterix genes on 3, 7 and 14 days after fracture. Alkaline phosphatase, aspartate aminotransferase, alanine aminotransferase, blood urea nitrogen, creatinine, total cholesterol, and triglyceride levels were measured for safety assessment. Results In morphological, histological analysis, callus formation process of DH-treated group was faster than the control group. BMP2, Sox9 gene expression were significantly increased at 7 days after fracture compared to the control group. COX2, Col2a1 gene expression were significantly increased at 14 days after fracture compared to the control group. Total cholesterol was significantly increased by DH at 3 days. Triglyceride was significantly decreased by DH at 3, 7 days after fracture compared to the control group. Conclusions Dohongsamul-tang promoted bone healing process after fracture by stimulating the bone regeneration factors. And DH shows no hepatotoxicity, nephrotoxicity and serum lipid abnormality. In conclusion, it seems that DH helps to promote fracture regeneration after bone fracture by regulating gene expressions related to bone repair.

Protective Effects of Dohongsamul-tang on Zinc-mediated Cytotoxicity in H9c2 Cardiomyoblast Cells (산화적 손상에 의해 유발된 심근세포 독성에 대한 도홍사물탕의 방어효과)

  • The water extract of Dohongsamul-tang(DHSMT)has been traditionally used for treatment of ischemic heart in oriental medicine. However, little is known about the mechanism by which the water extract of DHSMT rescues cells from these damages. Therefore, this study was designed to evaluate the protective effects of DHSMT on zinc-mediated cytotoxicity in H9c2 cardiomyoblast cells. This study demonstrates that treatment of H9c2 cells with zinc caused a decrease in cell viability in a dose dependent manner and a chromatin condensation. Zinc induced the cleavage of poly(ADP-ribose) polymerase (PARP). In addition, zinc induced the decrease of Bcl-2, as well as increase of Bak expression and mitochondrial dysfunction. Zinc-induced H9c2 cell death was remarkably prevented by the pretreatment of DHSMT with consistent suppression of the cleavage of poly(ADP-ribose) polymerase (PARP), mitochondrial dysfunction and the expression of Bak and Bcl-2. Taken together, the results suggest that zinc induced severe cell death in H9c2 cardiomyoblast cells via intracellular GSH(reduced glutathione) depletion and the protective effects of DHSMT against oxidative injuries may be achieved through modulation of mitochondrial dysfunction and scavenging of ROS(reactive oxygen species).

Protective Effects of Dohongsamul-tang on 2-deoxy-D-glucose Induced Autophagic Cell Death in C6 Glial Cells (도홍사물탕(挑紅四物湯)이 C6 신경교세포의 2-DG에 의한 오토파지성 세포사멸에 미치는 영향)

  • The water extract of Dohongsamul-tang(DHSMT) has been traditionally used in treatment of ischemic heart and brain diseases in Oriental Medicine. However, little is known about the mechanism by which DHSMT protects C6 glial cells from glucose deprevation induced damages. Therefore, this study was designed to evaluate the protective effects of DHSMT on 2-deoxy-D-glucose induced autophagy of C6 glial cells. Autophagic phenotype is evaluated by fluorescence microscopy and flow cytometry with specific biological staining dyes, including monodansylcadaverine and acridine orange, as well as Western blot analysis with microtubule-associated protein 1 light chain 3(LC3) and Beclin-1. Treatment with 2-deoxy-D-glucose significantly resulted in a decrease of the viability of C6 glial cells and increase of the extracellular LDH release in a dose and time-dependent manner. However, pretreatment with DHSMT protected C6 glial cells from glucose deprivation with 2-deoxy-D-glucose. The author also observed the fact that autophagy phenotype occurred by 2-deoxy-D-glucose in C6 glial cells. Pretreatment with 3-MA, a pharmacological inhibitior of autophagy, abolished the formation of acidic vesicle organelle in C6 glial cells treated with 2-deoxy-D-glucose. However, pretreatment with DHSMT inhibited the formation of autophagic phenotypes, including formation of acidic vesicle organelle, and increase of the expression of LC-3 II Beclin-1 proteins in C6 glial cells treated with 2-deoxy-D-glucose. Taken together, these data suggest that DHSMT is able to protect C6 glial cells from glucose deprivation with marked inhibition of autophagy formation.

The Effect of Dohongsamul-tang on Cytokine Production in Peripheral Blood Monocular Cells of Patients with Acute Cerebral Infarction

  • Objectives : We investigated the effect of Dohongsamul-tang (DHSMT) on the production of various cytokines in lipopolysaccaride (LPS) stimulated peripheral mononuclear cells (PBMCs) from CI patients. Methods: Cell viability was determined using MTT assay. ELISA was carried out for investigating $TNF-{\alpha}$, $IL-1{\beta}$, IL-6, IL-8, IL-4, and $TGF-{\beta}$ 1 Results : The amount of tumor necrosis factor (TNF)-${\alpha}$, interleukin (IL)-$1{\beta}$, IL-6, IL-8, IL-4, and transforming growth factor(TGF)-${\beta}$ 1 in PBMC culture supernatant significantly increased in the LPS treated cells compared to unstimulated cells. We show that DHSMT inhibited the production of TNF-${\alpha}$, IL-1${\beta}$, IL-6, IL-8, and IL-4 induced by LPS in a dose dependent manner. The maximal inhibition rate of the TNF-${\alpha}$, IL-1${\beta}$, IL-6, IL-8, and IL-4 production by pretreatment of DHSMT (1.0mg/ml) was 38.52 ${\pm}$ 2.5% (P < 0.01), 44.02 ${\pm}$ 3.5% (P < 0.05), 45.32 ${\pm}$ 2.3% (P < 0.01), 42.30 ${\pm}$ 3.1% (P < 0.05), and 49.70 ${\pm}$ 3.1%(P &lt; 0.05), respectively. On the other hand, DMSMT significantly increased the LPS-induced TGF-${\beta}$ 1 production(P<0.05). Conclusions : Taken together. these results suggest that DHSMT might have regulatory effects on cytokine production, which might explain its beneficial effect in the treatment of CI.

Effects of Dohongsamul-Tang on the Gene Expression of Photothrombotic Ischemia Mouse Model (도홍사물탕(桃紅四物湯)이 광화학적 뇌경색 마우스의 유전자 발현에 미치는 영향)

  • The water extract of Dohongsamul-Tang(DHSMT) has been traditionally used to stroke and brain injuries in Oriental Medicine. The present study was designed to investigate the effects of DHSMT on the gene expression profile of cerebral infarction by cDNA microarray in photothrombotic ischemia mouse model. Photothrombotic ischemia was induced in stereotactically held male BALB/c mice using rose bengal and cold light. MRI was performed 24 hours after inducing photothrombosis using 1.5 T MRI and 47 mm surface coil to obtain T2-weighted, and contrast-enhanced images. After MRI test, animal was sacrificed and the brain sections were stained for hematoxylin and eosin and immunohistochemistry. MRI and histological analysis revealed that lesion of thrombotic ischemia was well induced in the cortex with the evidence of biological courses of infarction. The target area of thrombotic infarction was 1 mm anterior to bregma and 3 mm lateral to midline with 2 mm in diameter, which were decreased by administration of DHSMT. To assess gene expression pattern of cerebral infarction, mRNA was isolated and reacted with microarray chip(Agilant's DNA Microarray 44K). Scatter and MA plot analysis were performed to clustering of each functional genes. M value [M=log2(R/G), A={log2(R ${\times}$ G)}/2] was between -0.5 and +0.5 with 40% difference. After pretreatment with DHSMT, the expression levels of mRNA of many genes involved in various signaling pathway such as apoptosis, cell cycle, cell proliferation, response to oxidative stress, immune response, angiogenesis, and inflammatory cytokine were markedly inhibited in photothrombotic ischemia lesion compared to the control group. These results suggest that DHSMT prevent ischemic death of brain on photothrombotic ischemia model of mice through modulation of gene expression at the transcriptional level.

  • Women with moderate weight loss have secondary or primary amenorrhea. The amenorrhea. which is usually reversible with weight gain, decreased exercise. or both, is due to hypothalamic dysfunction. But 30% of amenorrheic women with weight gain is irreversible. Recently we experienced one case of secondary amenorrhea with severe weight loss, as oriental medicine treatment secondary amenorrhea is cured, so report a medical treatment course and result.

Three Cases of Oral Mucoceles Treated with Traditional Korean Medicine (구강 점액류의 한약 치험 3례)

  • This is the first case report about oral mucoceles treated with traditional Korean medication in Korea. One case of Blandin-Nuhn mucocele and two cases of ranula were treated with Ondam-tang and Dohongsamul-tang respectively. No recurrence of ranula was found for 30 months in one case and 3 months in other recent case. According to this result, we suppose that consideration of "Blood stasis and heat" might be helpful to treat progressed or large mucoceles while common mucoceles are generally regarded as results of "Phlegm heat". We suggest that traditional Korean medication can be an alternative to preserve salivary glands and to minimize complications of surgery. Further studies with more cases and longer observation period should be performed to establish proper prescriptions for oral mucocele and the evidence of treatment rate and recurrence rate.

Case Report on Long Term Survival of Glioblastoma Patient Treated with Integrative Medicine (한양방결합치료로 장기간 생존하고 있는 교모세포종 증례 보고)

  • Objectives: This is a long-term survival case report about glioblastoma treated with western medicine and traditional Korean medicine. 28 year-old man diagnosed glioblastoma in 2003 went through 2 year's chemotherapy first. After emergency brain surgery he took acupuncture and Korean herbal medication including Dohongsamul-tang and Boan-Manyeongdan in addition to conventional radiotherapy and chemotherapy for 3 years. Since May 2008, he was exclusively treated with traditional Korean medicine except taking antiplatelets only for 9 years. Though he has sequelae of tumor necrosis such as facial palsy and hemiparesis, he has survived without recurrence more than 14 years. We suggest that integrative approach including traditional Korean medicine can be a meaningful treatment option for malignant brain tumor. Further studies with more cases should be performed to establish proper treatment protocol of integrative medicine for brain tumor.
