Noise in hospital rooms and sleep disturbance in hospitalized medical patients

  • Objectives Hospitalized patients are vulnerable to sleep disturbances because of environmental stresses including noise. While most previous studies on hospital noise and sleep have been performed for medical machines in intensive care units, there is a limited data for patients hospitalized in medical wardrooms. The purpose of present study was to measure noise level of medical wardrooms, identify patient-perceived sources of noise, and to examine the association between noise levels and sleep disturbances in hospitalized patients. Methods Noise dosimeters were used to measure noise level in 29 inpatient wardrooms at a university hospital. Sleep pattern and disturbance were assessed in 103 hospitalized patients, using the Pittsburgh Sleep Quality Index (PSQI) and Leeds Sleep Evaluation Questionnaire. Results The mean equivalent continuous noise level for 24 hours was 63.5 decibel A (dBA), which was far higher than 30 dBA recommended by the World Health Organization for hospital wardrooms. Other patients sharing a room were perceived as the most common source of noise by the patients, which was usually preventable. Of the patients in the study, 86% had bad sleep as assessed by the PSQI. The sleep disturbance was significantly correlated with increasing noise levels in a dose response manner. Conclusions Systemic organizational interventions are needed to keep wardrooms private and quiet to reduce sleep disturbance.

설문을 통한 여성 기능성 위장장애 환자의 경향과 한방치료 효과에 대한 연구 (A Questionnaire Analysis about the Female Patients with Functional Gastrointestinal Disturbance and Effect of Oriental Medical Treatment)

  • Objectives: Nowadays, the number of patients having the Functional Gastrointestinal Disturbance has been on the rise and this tendency is clear in the female group. This study is aimed to investigate their cause of disease, dietary lifestyle, digestion functions, general symptoms and any disadvantages in daily lives etc. Also, it would suggest any effective results from oriental medical treatments. Methods: The total number of 20 female patients, who have been participated in this study, have been treated at Oriental Internal Medicine, Dong-eui Hospital. We have performed the survey with 50 questionnaires. Results: Most patients were having dyspepsia and had been treated more than once before the survey. The major cause of their disease includes irregular eating habits(p<0.05) and stressful mental conditions. The patients had suffered much disadvantages in daily lives and different symptoms. Specially, pain in the below of stomach pit and early satiation were common.(p<0.05) However, the Discomfort index of patients has been decreased from 9.22 point to 2.85 point after oriental medical treatment. It means this treatment has meaningful effectiveness to the Functional Gastrointestinal Disturbance patients. Conclusion: We have investigated the features of female patients with Functional Gastrointestinal Disturbance and conclude that the Oriental Medical Treatment is effective to them.

지역사회 거주 성인의 수면문제 발생률과 위험요인 (Prevalence and Risk Factors of Sleep Disturbance in Community Dwelling Adults in Korea)

  • Purpose: The purpose of this study was to identify the prevalence and risk factors of sleep disturbance among community dwelling adults in Korea. Methods: Data were collected from nine hundred fifty nine adults dwelling in a metropolitan city in Korea. Subjects were older than 45 years, did not take medication for sleep problems on a regular basis, and did not work at night. The Pittsburgh Sleep Quality Index was used to collect data about sleep disturbance. Sociodemographic, life style, and health related factors were included as risk factors. Chi square test and logistic stepwise regression were used to identify the risk factors. Results: The prevalence of sleep disturbance was 13.1%. The significant risk factors were being female, age, number of toilet use at night, perceived health, and pain. Risk factors were somewhat different by gender. Emotional and job status were the risk factors only for men and menopausal status were for women. Conclusion: Prevalence of sleep disturbance in community dwelling adults in Korea was not greater than those dwelling in other countries. Females, older adults, the number of toilet use at night, perceived health, and pain were the most important risk factors for sleep disturbances. Gender was a factor in reported sleep disturbances. It might be useful to investigate the factors that contribute to the number of toilet use at night as this might have some underlying factors that could be corrected.

  • Objectives : The aim of this study was to report the effectiveness of Combined Korean Medicine treatment on a intracerebral hemorrhage patient with hemiplegia and gait disturbance. Methods : A 56-year-old, female patient with hemiplegia and gait disturbance was treated by Korean Medicine including acupuncture, moxibustion and herbal medicine. The effect of the treatment was evaluated by Manual Muscle Test(MMT), Modified Rankin Scale(MRS), Korean version of Modified Barthel Index(K-MBI). The gait of the patient was evaluated by 10m walk test, Timed Up&Go Test(TUG), Functional Ambulatory Category(FAC), GAITRite and Symmetry Criterion(SC). Results : After treatment, MMT, MRS, K-MBI, gait parameters and gait symmetry were improved. Conclusions : This study suggests that Combined Korean Medicine treatment might be effective for a intracerebral hemorrhage patient with hemiplegia and gait disturbance.

  • Objective: This clinical study describes the effect of Korean medicine on a post-traumatic hydrocephalus patient complaining of gait disturbance and dementia. Case presentation: A post-traumatic hydrocephalus patient complaining of gait disturbance and dementia was treated with herbal medicine, acupuncture, moxibustion, and cupping. Clinical symptoms were measured with the Korean Mini Mental Status Exam (K-MMSE), Functional Independence Measure (FIM), and Modified Barthel Index (MBI). After treatment, the patient showed improved K-MMSE, FIM, and MBI scores. Conclusion: This clinical case study provides evidence of the effect of Korean medicine on a post-traumatic hydrocephalus patient complaining of gait disturbance and dementia.

  • To protect digital communications and broadcasting that are expected to be increasingly used at frequencies above 1GHz, there is a need to determine permissible disturbance limits for frequency bands above 1GHz according to indicates that better correlates with interference level in digital systems. The quality of digital communications is expressed in terms of bit error rate(BER) or throughput. In this regard, it has been reported that degradation of BER or throughput can be estimated from the amplitude probability distribution (APD) of disturbance amplitude. Therefore, if the APD can simply be measured with a high accuracy, APD will be an optimal index for the assessment of interference in digital communications. In this paper, we show that a measurement method using statistic model of APD has compatibility about performance reduction estimation of WLAN. And through approach of this inspection, we propose numeral models about interference effects of peak value and time rate of spurious.

수면장애 연구와 실무를 위한 한국어 자가보고 수면 측정도구 (Korean Versions of Self-reported Sleep Questionnaires for Research and Practice on Sleep Disturbance)

  • Purpose: To identify Korean versions of self-reported sleep questionnaires for cross-cultural research and practice on sleep disturbance, we searched self-reported sleep questionnaires which have been developed to measure general sleep patterns. They should be qualitative, easy to take permission, do not obligate to pay, do not need any skill or professional help for scoring and have been reported high reliability and validity. Methods: General Sleep Disturbance Scale (GSDS), Leeds Sleep Evaluation Questionnaire (LSEQ), and Pittsburgh Sleep Quality Index (PSQI) were identified based on all criteria. Korean version of GSDS, LSEQ, and PSQI were developed according to the methods combined the guidelines for the process of cross-cultural adaptation of self-reported measures with decentering method. Results: Three Korean versions of self-reported sleep questionnaires were suitable for epidemiological study. They are not full length and easy to read. It requires less than 10 minutes for each subject to complete as well as for researchers to score. Conclusion: Psychometric analysis study is necessary to evaluate the reliability and validity of those Korean versions of self-reported sleep questionnaires.

남한강 상류 수계에서 어류의 다변수 모델 지수 산정 및 군집지수와의 비교평가 (Multi-metric Index Assessments of Fish Model and Comparative Analysis of Community)

  • 본 연구는 1999년 6월과 8월에 남한강 상류수계의 15개 조사지점으로부터 어류의 다변수 모델을 이용하여 생태건강성을 평가하였고, 전통적인 군집분석에 의한 지수와 서로 비교 평가하였다. 본 연구를 위해 우리나라의 생태적 특성에 맞게 보완된 10-메트릭 건강성 평가 모델을 적용하였다. 어류조사에 따르면, 총 6과 24종이 출현하였고, 생태적인 교란정도와 하천차수에 따라 일부 변이를 보였다. 생태길드분석에 따르면, 민감종과 충식종은 각각 91%, 56%로서 생태적으로 수환경이 건강한 상태로 나타났다. 그러나 S1과 S2의 2차 하천에서는 내성종이 66%, 잡식종이 76%가 채집되어 물리적 서식지의 질적 저하가 영향을 나타낸 것으로 판단되었다. 본 수계에서 생태건강도의 평균 모델값은 38로서 양호상태를 보였고, 개별적 IBI 값의 패턴은 다른 산간계류형 하천들과 유사하여 비교적 이화학적 오염에 의한 영향이 적은 것으로 사료되었다. 하천차수에 따른 IBI 값과 종다양성 지수와의 비교에 따르면, 두 변수 값은 분명한 차이가 있음을 보여주었다 Shannon-Weaver의 종다양성 지수는 실제 군집상태를 잘 반영하지 못했으며, 특히 2차 하천에서의 종다양성 지수 변이의 폭은 IBI 값으로 산정한 생태 건강도에 비해 훨씬 크게 나타났다. 본 연구결과는 다변수 메트릭에 의한 연구 접근방식은 수생태계 관리에 유용한 도구로 사용될 수 있으나, 기존의 종 다양도 지수와 같은 군집 분석법은 하천차수를 고려하지 않을 경우 비효율적인 것으로 사료되었다.

국립공원 생물다양성 평가를 위한 산림성 조류 자연성 지수 적용 (Application of Forest Bird Naturalness Index for Evaluating Biodiversity in National Parks in Korea)

  • 이 연구는 국립공원에 서식하고 있는 조류를 대상으로 국립공원 생물다양성을 평가할 수 있는 지수를 개발하여 생물다양성이 서식지 변화나 환경변화로 인해 영향을 받는 것을 알아보기 위한 것이다. 산림성 조류로 두견목, 딱다구리목, 수리목, 매목, 비둘기목, 쏙독새목, 올빼미목, 참새목 등에 해당하는 112종에 대하여 5인의 전문가 의견을 통해 교란 민감도 값을 산출한 후 자연성 지수 값을 산출하였다. 교란 민감도는 긴꼬리딱새와 팔색조가 높았으며 까치, 직박구리, 멧비둘기는 가장 낮게 나타났으며 각 공원에서 기록된 개체수 합과는 역상관으로 나타났다. 조류 자연성 지수는 속리산에서 78.70, 월악산 68.30, 주왕산 60.64, 월출산이 49.09로 나타났으며 특정 개체수를 지닌 종이 사라지는 시나리오별로 전체 종 수와 Fisher's alpha, Shannon-Wiener 다양도지수(H'), 자연성 지수가 차이를 나타내었다. 자연성 지수는 희귀종에게 민감한 반응을 나타내었고 개체수가 많은 종으로만 구성되는 경우에도 지역별 차이를 나타내었다. 추후 전체 조류 종에 대한 교란 민감도 지수를 산출하여 지점별 그리고 시간에 따라 조류의 자연성 지수가 어떻게 변화하는가를 모니터링한다면 기후변화를 포함한 다양한 환경변화로 생물다양성이 어떻게 변화하는가를 나타내는 지표로 이용될 수 있을 것으로 기대한다.

여성 류마티스 관절염 환자의 피로 예측 모형 (Prediction Model of Fatigue in Women with Rheumatoid Arthritis)

  • Rheumatoid arthritis is a chronic systemic autoimmune disease. Although the joints are the major loci of the disease activity, fatigue is a common extraarticular symptom that exists in all gradations of rheumatoid arthritis. Fatigue is defined as a subjective sense of generalized tiredness or exhaustion and has multiple dimensions. Therefore fatigue is a common and frequent problem for those with rheumatoid arthritis. In fact, 88-100% of individuals with rheumatoid arthritis experience fatigue. Especially the degree of fatigue is higher in women than men with rheumatoid arthritis. Despite the importance of fatigue among the patients with rheumatoid arthritis, the mechanism that leads to fatigue in rheumatoid arthritis is not completely understood. This study was intended to test and validate a model to predict fatigue in women with rheumatoid arthritis. Especially it was intended to identify the direct and indirect effects of the variables of pain, disability, depression, sleep disturbance, morning stiffness, and symptom duration to fatigue. Data were collected by questionnaires including Multidimensional Assesment of Fatigue(Tack, 1991), numeric scale of pain, graphic scale of joints, Ritchie Articular Index, Korean Health Assessment Questionnaire(Bae, et al., 1998), Inventory of Function Status(Tulman, et al., 1991), Center for Epidemiologic Studies-Depression, and Korean Sleep Scale(Oh, et al 1998). The sample consisted of 345 women with a mean duration of rheumatoid arthritis for 10.06 years and a mean age of 49.64 years. SPSS win and Win LISREL were used for the data analysis. Structural equation modeling revealed the overall fit of the model. Pain predicted fatigue directly and indirectly through disability, depression, and sleep disturbance. Disability, sleep disturbance predicted fatigue only directly, while depression only indirectly through disability and sleep disturbance. Also morning stiffness and symptom duration predicted fatigue through disability and depression. All predictors accounted for 65% of the variance of fatigue. Depression, pain, and disability predicted sleep disturbance. Depression had reciprocal relationship with disability and they both were predicted by pain directly and indirectly. In summary, pain, depression, disability, sleep disturbance, morning stiffness, and symptom duration contributed to the fatigue of patients with rheumatoid arthritis. The best predictor of fatigue was pain. This finding indicates that the modification of pain, depression, disability, sleep disturbance, morning stiffness could be nursing intervention for relief or prevention of fatigue.
