Blastocystis sp. is a kind of protozoa living in the intestinal tract of human and animals, which will cause intestinal diseases such as diarrhea, abdominal distension and vomiting. This paper was aimed to understand the infection of Blastocystis sp. In golden monkeys and the transmission path in North China. Thirty-seven feces samples from golden monkeys and 116 cockroach samples from Shijiazhuang Zoo were collected from July to October 2019 for PCR analysis of Blastocystis sp. Genetic diversity analysis was further conducted on the samples with positive PCR results. The results showed that the infection rate was 48.7% (18/37) in golden monkeys and 82.8% (96/116) in cockroaches, respectively. The genetic evolution analysis based on small subunit ribosomal RNA demonstrated that three subtypes (ST) of Blastocystis sp. including ST1, ST2, and ST3 existed in the intestinal tract of golden monkeys, while only ST2 was detected in the intestinal tract of cockroaches. This paper may provide supports for the quarantine and control of Blastocystis sp. for the zoo in Northern China.
Cardiogenic syncope occurs due to arrhythmia (bradycardia and tachycardia) or decreased cardiac output, and if proper treatment is not provided, it can lead to acute sudden death. A detailed medical history and physical examinations are required to determine the cause of syncope, and clinical approaches, including 12-lead ECG, are important. The 12-lead ECG does not have a chest lead in the posterior wall of the left ventricle; therefore, ECG of the isolated posterior wall myocardial infarction caused by left circumflex artery occlusion is not observed with ST elevation. Therefore, the significantly higher appearance of ST depression and R waves than S waves from V1 to V3 of the chest lead must be interpreted meaningfully. Isolated posterior wall myocardial infarction is small in the area of myocardial necrosis, and tension is increased in the necrotic area due to the contraction of the normal myocardial muscle, which can cause ventricular wall rupture. Therefore, it is necessary to additionally check Beck's triad, such as jugular venous distension and decreased heart sound, in patients with low blood pressure with an isolated posterior wall myocardial infarction on 12-lead ECG in patients with syncope.

The dog with tetraplegia was presented for magnetic resonance imaging and cervical ventral slot decompression. Intra-abdominal pressure (IAP) was measured every hour after surgery, along with respiratory rate, heart rate, and arterial pressure. Three hours after surgery, abdominal distension with agitation and respiratory distress were observed, and IAP rose to 12 mmHg, indicating mild intra-abdominal hypertension (IAH). Additional fentanyl and ketamine CRI did not alleviate IAH and acepromazine (0.01 mg/kg, IV) was administered to alleviate the agitation and respiratory distress. After acepromazine administration, the agitation subsided and IAP dropped to 4 mmHg. During the next 24 hours, the patient's vital signs and IAP remained stable, with normal urine output. This case report suggests the possibility of postoperative IAH monitoring in dogs. However, considering the nature of a single surgical case of cervical ventral slot, further study is required for indication of IAH monitoring.

Paralytic ileus is a metabolic state in which the intestines fail to transmit peristalsis due to failure of the neuromuscular mechanism in the small intestines and colon. It is a major cause of morbidity in hospitalized patients especially during late presentations and points of mismanagement. The causes include infections, electrolyte imbalance (hypokalemia, hyponatremia), surgeries and medications. When the exact cause of the disease condition is identified and corrected, paralytic ileus is usually resolved. This case report is that of a 16 year old female who was admitted and managed as a case of paralytic ileus. The patient presented with symptoms of fever, abdominal pain, abdominal distension, vomiting and inability to pass stool or flatus. There was associated body weakness, reduced urine output and weight loss. She was properly examined clinically and sent for various investigations. Investigations such plain abdominal X-Ray, serum electrolyte estimation, chest X-Ray and full blood count were carried out. The results of the investigations done were in keeping with the diagnosis of paralytic ileus, electrolyte imbalance and ongoing sepsis. She was subsequently managed through nil per oral, adequate fluid rehydration, antibiotics and correction of electrolyte imbalance. Following stable clinical state and investigation results, she was discharged and advised on follow-up.
Objectives: To report the reduction of adverse effects of chemotherapy and improvement in the quality of life in recurrent liver metastasis by Korean medicine treatments. Methods: In 2018, a 75-year-old male patient underwent surgery for primary cancer in the ampulla of Vater. In 2021, he was diagnosed with recurrent liver metastasis. Following the failure of tumor treatment with gemcitabine/cisplatin and the development of severe side effects, he decided to discontinue chemotherapy. Subsequently, with a significantly enlarged liver tumor, he resumed capecitabine/oxaliplatin treatment, alongside moxibustion, acupuncture, and herbal prescriptions from August 2021 to August 2023. The changes of chief complaints, abdominal CT, and laboratory findings were investigated. Results: After combined treatment of Korean traditional medicine and chemotherapy, the mass was decreased. Laboratory findings, and chief complaints of hand-foot syndrome, fatigue, abdominal distension, nausea and anorexia were improved. Conclustions: This case study suggests that Korean traditional medicine is effective in enhancing anticancer effects, suppressing the side effects of chemotherapy, and improving general conditions.

A 12-year-old female small-clawed Asian otter (Anoyx cinereus) with a one-week history of anorexia, chills, and abdominal distension was found dead. Grossly, yellowish-brown turbid fluids accumulated in abdominal cavity of the otter, and yellowish thread-like fibrinous materials were found on the surface of abdominal organs. Several variable sized yellowish-white crystalloids were scattered on the medullary space of kidneys. Histologically, diffuse serositis (peritonitis) characterized by the fibrinous exudates, thickened serosal capsule and the swelling of mesothelial cells were observed in the serosa of liver, spleen, stomach, and intestine. Multifocal necrosis, hemorrhage, infiltration of macrophage, and brown pigments were presented in the liver. Isolated bacteria from ascites and fibrinous materials in abdominal visceral surface were white, smooth and convex with characteristic mousy odor on blood agar plate. These bacteria were confirmed as Pasteurella (P.) multocida type A by polymerase chain reaction analysis. Based on the gross examination, histopathologic findings and bacterial experiments, this otter was diagnosed as severe peritonitis associated with P. multocida and necrotic hepatitis.
Venovenous extracorporeal membrane oxygenation (VV ECMO) is often used in cases of severe respiratory failure, especially in patients considered for lung transplantation. However, because many lung diseases can ultimately result in right heart failure, the treatment of secondary right heart failure can present a challenge when the patient is already under VV ECMO support. In such cases, an oxygenated-right ventricular assist device (OxyRVAD) can be used. OxyRVAD is designed to maintain anterograde blood flow and prevent right ventricular distension. Moreover, the pulmonary arterial cannula can be inserted percutaneously. We report a case in which percutaneous OxyRVAD was successfully implemented to manage right heart failure in a patient with respiratory failure who was on VV ECMO.
Objectives: This study aimed to report a case of alcoholic hepatitis with megaloblastic anemia that improved with Korean medicine treatment. Methods: A 27-year-old woman with alcoholic hepatitis and megaloblastic anemia was treated with herbal medicine, acupuncture, and moxibustion. The responses before and after treatment were evaluated using blood tests, ultrasound imaging, the Checklist Individual Strength (CIS), and the numerical rating scale (NRS). Results: Post-treatment, blood tests showed improved liver function test, lipid test, and anemia work-up. Ultrasound imaging showed improvement in the superoinferior diameter of right lobe, from 13.02 to 11.09, and the fatty liver improved. The CIS score decreased from 107 to 55. Fatigue diminished from NRS 10 to NRS 3, and dizziness dropped from NRS 5 to NRS 1. The abdominal distension slightly decreased, from NRS 7 to NRS 5. Conclusion: Korean medicine treatment might be effective for treating alcoholic hepatitis with megaloblastic anemia.
신생아의 피막하 간 혈종은 다양한 원인에 의해서 유발되는데 대부분은 출혈의 양이 적어 임상적으로 발견되기 보다는 부검시 발견되는 경우가 많다. 그러나 출혈의 양이 많아 혈종을 싸고 있는 피막이 파열되고 잇따른 출혈이 발생할 경우에는 창백, 혈관 허탈, 빈혈, 복부팽만, 저산소증, 산혈증 등의 증상을 보이므로 조기에 의심하여 정확한 진단을 내리고 적절한 수술적 치료를 실시하지 않으면 사망률이 높다. 저자들은 태변 흡입후 신생아 가사 소견을 보여 심폐소생술을 실시한 신생아에서 출생후 7일에 발현된 대량 피막하 간 혈종 파열 1례를 경험하였기에 보고하는 바이다.
환자는 44세 남자로 호흡곤란과 동반된 복부팽만을 주소로 입원하였다. 심초음파상 우심방내의 종괴와 삼첨판막 협착증, 우심실내강의 감소를 관찰할 수 있었다 수술을 시행하여 우심방내의 종괴를 제거하였고 삼첨판막의 개구부를 확장시킴과 동시에 심실내부의 심내막절제술을 시행하였다. 또한 우심방의 용적부하를 낮추기 위해 심방중격결손을 만들어주었다. 술 후 환자는 증세의 호전을 보였고 별다른 문제없이 퇴원하였고 조직검사상 진단은 심근내막섬유증으로 내려졌다. 술 후 18개월 뒤 환자는 진행된 호흡곤란과 청색증으로 인해 다시 입원하였고 심초음파를 시행하여 우심실내강이 이전보다 더 좁아져 있으며 폐동맥의 혈류가 거의 없음을 확인하였다. 기능적 단 심실 상태였기 때문에 양 방향성 상대정맥폐동맥 단락수술을 시행하였고 술 후 호흡곤란과 청색증은 많은 호전을 보였다. 진행된 우심실의 심근내막섬유증에서 양 방향성 상대정맥-폐동맥 단락수술의 시행은 술 후 초기에 효과적인 증상완화를 제공하지만 장기간의 추적관찰이 요구된다.
