Purpose: To evaluate the clinical and radiological results of internal fixation with tension band wiring for the fracture at the base of fifth metatarsal bone. Materials and Methods: From January 2008 to December 2009, 15 cases with displaced fracture at the base of fifth metatarsal were analyzed and average follow up period was 13.8 months. Lawrence classification was used to classify fracture type. We evaluated clinical results by American Orthopedic Foot Ankle Society (AOFAS) midfoot score and radiological results by union time. Complications was also checked. Results: According to classification, zone I fracture were 11 cases and zone II fracture were 4 cases. Bony union was achieved in all cases after 7 weeks. In the final follow-up, average AOFAS score was 94. There were no complications except hardware irritation. Conclusion: Satisfactory results were obtained after tension band wiring for the fifth Metatarsal base fracture in zone I fracture or comminuted zone II fracture for which it is not easy to be fixed with screw.

The subjects of this study consisted of 75 patients with 83 condylar fractures, who were admitted to Chonbuk National University Hospital from Jan. 1988 to Oct. 1995. The purpose of this study was to compare the imagings of conventional radiograms with those of computed tomograms and to aid in the diagnosis of condylar fractures. Also the author evaluated the usefulness of 3-dimensional reconstructive imaging in condylar fractures. The obtained results were as follows: 1. The condylar fractures were observed mainly between 2nd and 4th decades, but there was no significant difference of incidence between decades. The incidence of condyalr fractures by fracture site was subcondyalr(44.6%), condylar head(36.1%), condylar neck(19.3%) in orders. 12 of 30 condylar head fractures were sagittal splitting fractures. 2. According to the relationship of condylar head to articular fossa, the incidence of condylar fractures was higher in Type II and Type m. And the incidence of subcondylar fractures was higher in Type I classified by Mclennan. But there was no significant difference of incidence between Types. 3. The more the fracture fragments were displaced, the easier the fracture patterns were detected in conventional radiograms. The computed tomograms were also useful in the diagnosis of sagittal splitting fractures which were displaced mesially. 4. The 3-dimensional reconstructive imaging were useful in the evaluation of the fracture patterns, but they were not useful when the size of fragment and the degree of displacement were small.

Purpose: This study investigates the relationship between preoperative neutrophil-to-lymphocyte ratio (NLR), lymphocyte-to-C-reactive protein ratio (LCR), albumin, and 2-year mortality in elderly patients having hemiarthroplasty for displaced femoral neck fracture (FNF). Materials and Methods: We retrospectively reviewed 284 elderly patients who underwent hemiarthroplasty for Garden type IV FNF from September 2014 to September 2020. Using the receiver operating characteristic curve, optimal cutoff values for LCR, NLR, and albumin were established, and patients were categorized as low or high. Associations with 2-year mortality were evaluated through univariate and multivariate Cox regression analyses. Results: Of the 284 patients, 124 patients (45.9%) died within 2 years post-surgery. The optimal cutoff values were: LCR at 7.758 (specificity 58.5%, sensitivity 25.0%), NLR at 3.854 (specificity 39.2%, sensitivity 40.0%), and albumin at 3.750 (specificity 65.9%, sensitivity 21.9%). Patients with low LCR (<7.758), high NLR (≥3.854), and low albumin (<3.750) had a statistically significant reduced survival time compared to their counterparts. Conclusion: Lower preoperative LCR and albumin levels, along with higher NLR, effectively predict 2-year mortality and 30-day post-surgery complications in elderly patients with Garden type IV FNF undergoing hemiarthroplasty.
We report a patient with an anterior dislocation of the shoulder with uncommon bucket handle type fracture of the anterior glenoid fossa with intact glenoid labrum. The fracture fragment was displaced into the posterior aspect of the glenohumeral joint resulting in prevention of reduction of the shoulder. Excellent fixation was obtained with suture anchors and bioabsorbable interfragmentary screws. This allowed stable range of motion exercises, optimizing the patient's functional outcome.
Arthroscopy is recognized as an important adjunct in treatment of intra-articular fractures. The author reports on successful treatment of a displaced transverse glenoid cavity fracture, reduced and fixed with arthroscopic assist, using two cannulated screws perpendicular to the fracture surface, in a patient with frail chest. One screw passed through the Neviaser portal, and the other screw passed through the base of the coracoid process. Arthroscopy assisted reduction and 2 cannulated screw fixation through the Neviaser portal and coracoid base appears to be a good method for treatment of transverse glenoid fractures.
We presented two cases of unstable fracture of the distal clavicle in children associating superior displacement of the ipsilateral distal clavicle and widening of the coracoclavicular (CC) interval. They obtained good osteosynthesis and reduction of the CC interval by conservative treatment. The primary conservative treatment should be considered for the treatment of the unstable distal clavicular fracture with widening of the CC interval in children.
Purpose: This study was designed to evaluate the clinical efficacy of temporary K-wire fixation in F-plate fixation for displaced intra-articular calcaneal fractures. Materials and Methods: Two groups (group 1 with F-plate fixation only and group 2 with temporary K-wire fixation and F-plate fixation) of patients were included in this study. The temporary K-wire was removed six weeks after the operation. Each group consisted of 33 cases. Rotational axis angles were measured radiographically and the foot and ankle outcome score (FAOS) was used for clinical assessment. Results: In group 1, the mean rotational axis angle was reduced from $27^{\circ}$ preoperatively to $5.59^{\circ}$ postoperatively and the angle at last follow-up was $9.94^{\circ}$. There was an increase in angle of $4.35^{\circ}$ between postoperative and the last follow-up measurement. In group 2, the mean rotational axis angle was reduced from $21.2^{\circ}$ preoperatively to $4.39^{\circ}$ postoperatively and the angle at last follow-up was $5.91^{\circ}$. There was an increase in angle of $1.52^{\circ}$ between postoperative and the last follow-up measurement. Significant difference in the changes of rotational axis angle was observed between the two groups. However, no significant difference in FAOS was observed between the two groups. Conclusion: Temporary K-wire fixation can prevent reduction loss when treating displaced intra-articular calcaneal fractures with an F-plate.
Purpose: To analyze the clinical result of the arthroscopic reduction and fixation for the displaced tibial spine fractures in children and report the utility of the arthroscopic treatment with the review of the literature. Methods: Between December 2000 and July 2004, five patients (average age 9.1 years) received an arthroscopic reduction and fixation of displaced tibial spine fracture. A male-to-female ratio was 3:2 and mean follow-up was 38.2$(13{\sim}56)$ months. All five patients were classified type III by Movers and McKeever classification. The average period from injury to operation was 4.8 days $(3{\sim}8days)$, the avulsed fragment was reduced by operative arthroscopy and fixated by pull-out suture in 3 patients and by cannulated screw in 2 patients. Postoperatively long leg cast was applied for 2 weeks, and then gradual range-of-motion exercise was permitted. Full weight-bearing ambulation was permitted after 6 weeks. The clinical evaluation was performed by range of motion, Lachman and pivot shift test, KT-1000 arthrometer, Lysholm knee score and the modified Feagin score. Results: All five patients had no symptom and recovered full range of motion of the affected knees. Lachman test was positive finding of 1+ laxity in one patient, the others were negative, and all patients were negative findings for pivot shift test. The result of KT-1000 arthrometric assessment is mean maximum side-to-side differences 1.9 mm. Average Lysholm knee score was 99.4. All patients had excellent results in modified Feagin score. Conclusion: Arthroscopic reduction and fixation of displaced tibial spine fracture in children showed excellent result without complication. Both pull-out suture fixation and cannulated fixation provide an effective treatment option for fixation of the displaced anterior tibial spine fracture.
Purpose: To assess the effectiveness of the Locking compression plate (LCP) after open reduction for the treatment of the displaced clavicular shaft fracture, the clinical and radiologic outcome of the patients who were managed with the LCP for internal fixation after open reduction has been analyzed. Materials and Methods: We reviewed 26 cases with a displaced clavicular shaft fracture treated by internal fixation using Locking compression plate after open reduction between May 2003 and November 2004. The patients were followed up for at least six months period, and final postoperative outcome was evaluated using clinical results based on Kang's criteria, radiologic signs of fusion. Results: All fractures united by an average of 9.3 weeks without delayed union and showed fast recovery of motion fraction and shoulder function. In addition,24 cases without the fractures of proximal humerus recovered to normal range of shoulder notion within 2.9 weeks. Clinically, according to Kang's criteria, the outcome was good or better in 22 patients. The complications included shoulder joint dysfunction in two cases and keloid formation in one case, and no other complications were observed. Conclusion: The internal fixation using LCP for the treatment of displaced clavicular shaft fracture is a safe, reliable method of treatment, with few complications, and offers rapid recovery of shoulder joint function and bone union.
Purpose: The purpose of this study is to assess the result of conservative treatment in displaced radial head fracture. Materials and Methods: We analyzed 15 cases of displaced radial head fracture who were treated conservatively from April 1992 to May 2002. Follow-up period was more than 1 year. They were all males and the average age of patients was 20.2($11{\sim}24$) years. The average follow-up period was 13.4($12{\sim}15$) months. They were able to do active pronation and supination more than 70 degrees initially, who were applied long arm splint less than 1 week. The functional rating index modified after B.F. Morrey and no further displacement and union state of fragment in simple X-ray were used in follow-up assessment. Results: On the basis of the functional rating index modified after B.F. Morrey that included elements of pain, motion, strength, and stability, the results were 8 excellent, 7 good in patients who were treated conservatively, who had union and no further displacement of fragment in follow-up. Conclusion: We conclude that if they are able to do active pronation and supination more than 70 degrees initially, we take them to do early range of motion exercise after subsiding swelling, which can restrict limitation of range of motion exercise and return to the living early.
