Purpose: We evaluated the correlation of postoperative clinical outcomes and radiologic findings using computed tomography and simple X-ray in intra-articular calcaneal fractures. Materials and Methods: The current study is based on 41 feet, 38 patients with displaced intra-articular fracture who underwent surgical treatment with at least one year of follow-up. Evaluation of clinical outcome included American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score, visual analogue scale (VAS) score, and subjective satisfaction. A simple X-ray was used in evaluation of preoperative and postoperative Gissane angle, B${\ddot{O}}$hler angle, and calcaneal fracture width. Computed tomography scan was performed for evaluation of preoperative and postoperative articular step-off and articular gap in all cases. Finally, we evaluated the correlation of the postoperative clinical outcomes and radiologic findings based on the measurement. Results: The average postoperative AOFAS score and VAS score was $84.1{\pm}8.5$ and $2.2{\pm}2.2$. Subjective satisfaction was excellent in 15 cases, good in 19 cases, and fair in seven cases. The average B${\ddot{O}}$hler angle was restored from $11.1^{\circ}$ to $24.7^{\circ}$(p<0.05), Gissane angle was changed from $121.0^{\circ}$ to $119.0^{\circ}$ (p>0.05), and the average width was restored from 45.8 to 35.0 mm (p<0.05). The average articular step-off and gap were decreased from 6.3 to 2.0 mm and from 11.1 to 4.6 mm, respectively (p<0.05). No significant correlations were observed between the clinical outcome and Gissane angle, B${\ddot{O}}$hler angle, and width, and there was no significant correlation between the clinical outcome and Sanders classification. However, postoperative articular step-off showed correlation with VAS and AOFAS score and articular gap showed correlation with VAS score. Conclusion: The clinical outcome did not show correlation with B${\ddot{O}}$hler angle and Gissane angle but did show correlation with anatomical reduction of the posterior facet joint.
Purpose: The deformation characteristics induced by non-destructive stresses using piezoelectric transducer(PZT) were analyzed for 3-unit fixed partial dentures manufactured PFM, Everest(CAD/CAM) and Zirkonzahn(copy milling, MAD/MAM) by electron speckle pattern interferometery(ESPI). Methods: The ESPI analysis after loading the restoration with PZT by applying electric voltage of 900mV at the points of 10 mm above the base of the prostheses. Results: PFM and All-Ceramic Everest prostheses showed about 0.1 ${\mu}m$ while that of All- Ceramic Zirkonzahn prostheses showed 0.085 ${\mu}m$, demonstrating that Zirkonzahn displaced less. For PFM and All-Ceramic Zirkonzahn prostheses, the displacements were large at just below the loading point, while generalize displacement was shown over the loading point and weak connector areas for All-Ceramic Everest prostheses. Conclusion: We could find that the deformation characteristics induced by non-destructive stresses using PZT analyzed by ESPI were similar to the fracture strengths evaluated using universal testing machine.
Today, endoscopic procedures are common in diagnostic and other surgical procedures, with endoscopically enhanced and magnified images permitting surgical access through minimal incisions. This has expanded the treatment options for many difficult anatomic sites, and the endoscope facilitated safe anatomic reduction and fixation. The use of the endoscope may reduce the disadvantages of open fracture repair and should be considered for broad application in the treatment of displaced facial bone fractures. Optical endoscopic magnification minimizes the disadvantages associated with open surgical repair, including the risk of facial nerve injury and external facial scarring, and no postoperative complications have been attributable to the endoscopic approach. This technique was used in 14 patients treated at Ulsan University Hospital, Korea, from September 2004 to August 2006, including six mandibular subcondyle fractures, five blowout fractures and three zygomaticomaxillary complex (ZMC) fractures. Careful preoperative evaluation and proper surgical technique were essential to achieve optimal results in the selected patients.
Zygomatic fractures are the second most common facial bone fractures encountered and treated by plastic surgeons. Stable fixation of fractured fragments after adequate exposure is critical for ensuring three-dimensional anatomic reduction. Between January 2008 and December 2010, 17 patients with zygomatic fractures were admitted to our hospital; there were 15 male and 2 female patients. The average age of the patients was 41 years (range, 19 to 75 years). We exposed the inferior orbital rim and zygomatic complex through a lateral brow, intraoral, and subciliary incisions, which allowed for visualization of the bone, and then the fractured parts were corrected using the Carroll-Girard T-bar screw. Postoperative complications such as malar asymmetry, diplopia, enophthalmos, and postoperative infection were not observed. Lower eyelid retraction and temporary ectropion occurred in 1 of the 17 patients. Functional and cosmetic results were excellent in nearly all of the cases. In this report, we describe using the Carroll-Girard T-bar screw for the reduction of zygomatic fractures. Because this instrument is easy to use and can rotate to any direction and vector, it can be used to correct displaced zygomatic bone more accurately and safely than other devices, without leaving facial scars.
An Fe-25Cr steel was oxidized in Ar atmosphere at 973K with and without applying external stress of 30∼35 MPa. A 0.1$\mu\textrm{m}$ thick $Cr_2O_3$ scales formed during pre-treatment in Ar atmosphere. Initiation of cracking on the oxide scales took place at grain boundaries during the end of second creep stage, in which cracks were found nearly perpendicular to the tensile directions. On the contrary, a scale developed in $N_2$-0.1%$SO_2$ displaced a poor adherence on the metal substrate. In this sample, a fast grown of scales was observed during creep deformation, and the strength of materials was much lower than in Ar. The creep strain rate of $1.5{\times}10^{-7}/s$ and $5.8{\times}10^{-7}/s$ was determined in Ar and in $N_2$-0.1%$SO_2$ under 30MPa, respectively.
하악골 과두골절의 치로에 대해서는 여러가지 술식이 문헌상 보고되었으나 어떤 특정한 술식이 모든 증례에 적용될 수는 없으므로 각 증례에 따라 방사선 사진검사와 세밀한 임상검사 및 술자의 판단등에 따라 보존적인 치료법이나 외과적 수술법이 적절하게 선택되어야 한다. 대부분의 과두골절은 보존적인 치료만으로 만족스러운 결과를 얻을 수 있으나 보존적 처치후 장기간에 걸친 술후 검사시 하악의 기능장애, 부정교합, 하악골의 이환측으로의 전위, 위관절증 및 악관절 강직등의 병발증이 나타날 수 있으므로 골편이 심하게 전위된 경우나 무치악 환자, 구치부 고경유지가 어려운 환자 및 보존적 처치후 기능장애를 초래한 골편의 부정유합이나 비유합 환자에서는 관혈적 정복술로 보다 양호한 결과를 얻을 수 있다. 저자는 하악골 과두골절시 골편전위가 비교적 심한 6증례에서 악하부 피부절개를 통해 통상의 금속선 고정법, Kirschner 금속선을 이용한 고정법을 각 1예씩 사용하였고 4예는 골절선이 비교적 높아 통상의 수술방법으로 접근이 용이하지 않아 시상골절단과 사선골절단술을 이용하여 골편정복과 고정술을 시행하여 다소의 지견을 얻어 이에 보고하는 바이다.
Geological and electrical resistivity surveys were carried out to investigate subsurface geology and geologic structure of the Bonghwajae area in the Okchon zone. Pseudosections of the apparent electrical resistivity distribution along the three survey lines were obtained by using dipole-dipole electrode array method, and models of subsurface geology and geologic structure by using two dimensional finite difference method. The Bonghwajae fault zone exists around Bonghwajae area in the north-south direction, and is a boundary between Okchon Group and Choson Supper Group. Metabasite and hornblende gabbro intruded along the Bonghwajae fault zone remaining two fracture zones with low resistivity value of 20 ohm-m and widths of about 100m and 70-300m. They strike nearly N-S and dip westward with a high angle of $60-70^{\circ}$. Sochangri fault with a width of about 160m exists between Jisogori and Bonghwajae, by which Bonghwajae fault zone is displaced about 1km in the east-west direction. Hornblende gabbro whose electrical resistivity value is in the range of 5000-8000 ohm-m intruded the metabasite of 2000-4500 ohm-m after the Sochangri fault had formed. Great Limestone Group is widely distributed in the east of Bonghwajae fault zone, and interbeds so called Yongam formation of graphitic black slate with an extremely low electrical resistivity value of 2 ohm-m.
Purpose: The nonoperative outcome of elbow dislocations with associated radial head and coronoid fractures are often unsatisfactory because of chronic instability and stiffness from proloned immobilization, Therefore we managed these injuries with well programed surgical appproaches. Method: Ten patients with this injury were evaluated retrospectively from May 1998 to June 2004 after a minimum of 12 months. These injuries include elbow dislocation and associated fractures of both the radial head and the coronoid process. All ten patients were treated by one clinic operatively with similar scheduled surgical methods which started on the lateral side and terminated on the medial side of the elbow. Radial head and neck fractures were classified Mason types, as two and three types respectively with six and four cases and six cases were fixated. Coronoid process were fixated with screws anteroposterior directly or anchor suture in all cases, each type was classified one, two and three. where were three type one, four type two, and three type three were according to Regan and Morrey classification. Results: The outcome was three resulting in excellent, four good, two normaland and the remaining case was one poor according to the Mayo Elbow Performance score. At a terminal follow up, the range of motion of the elbow averaged flection contracture, $6^{\circ}(0{\sim}20^{\circ})$ and further flection, $129^{\circ}(115{\sim}140^{\circ})$. Two patients had complications requiring additional care. One, displaced coronoid process which was repaired with capsule and the other patient experienced, palsy of ulnar nerve and contracted elbow joint. Conclusions: Usage of early operation as the minimum injury of medial ligaments complex and the rigid fixation of fractures to prompt motion with our scheduled management for elbow dislocations with associated radial head and coracoid fractures provided excellent results.
The gravity measurement has been conducted at 61 stations with an interval of about 500 to 1,000 m along two survey lines of about 47 Km between $Chungju-Jech{\check{o}}n$ and $Salmi-D{\check{o}}cksanmy{\check{o}}n$ in order to study on the subsurface geologic structure and structural relation between $Okch{\check{o}}n$ Group and Great Limestone Group of $Chos{\check{o}}n$ Supergroup. The Bouger gravity anomalies were obtained from the reduction of the field observations, and the distribution patterns of the basement and subsurface geologic structure were interpreted by means of the Fourier-Series and Talwani method for two-dimensional body. The depth of Conrad discontinuity varies from 12.7 Km to 15.7 Km, and vertical displacements along the Osanri and Bonghwajae faults are 1.0 Km and 1.5 Km, respectively between Chungju and $Jech{\check{o}}n$. The depth of Conrad discontinuity varies from 13.8 Km to 15.4 Km, and vertical displacement along the Bonghwajae fault is 0.5 Km between Salmi and $D{\check{o}}cksanmyon$. The basement is widely exposed at several places between Chungju and $Jech{\check{o}}n$. In the unexposed area between Osanri and $W{\check{o}}lgulri$, its depth is from 1.5 Km to 2.1 Km. It is displaced downward along the Osanri and Bonghwajae faults by 0.8 Km and 0.6 Km, respectively, and is displaced upward along the Dangdusan fault by 1.6 Km. On the other hand, the depth of the basement varies abruptly by the Sindangri, Jungwon, Kounri, and Bonghwajae faults between Salmi and $D{\check{o}}cksanmy{\check{o}}n$, and it is from 2.8 Km to 3.2 Km around $Salmimy{\check{o}}n$, from 1.6 Km to 2.5 Km between the Sindangri and Bonghwajae faults, 3.0 Km near Koburangjae, and 2.5 Km at $Doj{\check{o}}nri$. The high Bouguer gravity anomalies are due to the accumulation of $Okch{\check{o}}n$ Group and $Jangs{\check{o}}nri$ Metamorphic Complex whose density is higher than the basement exposed between Sondong and Osanri, and imply the existance of Bonghwajae Metabasite or hornblende gabbro of high density distributed along the Bonghwajae fault in the vicinity of Koburangjae. The low Bouguer gravity anomalies resulted form the fracture zone associated with fault or rock of low density imply the existance of the Osanri, Bonghwajae, Dangdusan faults and $Daed{\check{o}}cksan$ thrust between Chungju and $Jech{\check{o}}n$, the uplift of the basement by the Sindangri, Jungwon, Kounri, and Bonghwajae faults, and extensive distribution of Cretaceous biotite granites between Salmi and $Docksanmy{\check{o}}n$. The thickness of $Okch{\check{o}}n$ metasediments varies from 1.5 Km to 3.2 Km, and that of Great Limestone Group of $Chos{\check{o}}n$ Supergroup from 200 m to 700 m. It is interpreted that $Okch{\check{o}}n$ Group is in contact with Great Limestone Group of $Chos{\check{o}}n$ Supergroup by the fault zones of the Bonghwajae and $Daed{\check{o}}cksan$ faults, and the Bongwhajae fault is a thrust of high angle, by which the east of the basement is displaced downward 0.5 Km between Chungju and lechon, and 1.0 Km between Salmi and $D{\check{o}}cksanmy{\check{o}}n$.
The gravity measurment has been carried out at 48 gravity stations with intervals of 1.0~1.5 km along the survey line between Samcheog, Gosari and Taebaek to study subsurface geology and geologic structure in the northeastern part of the Ockchon zone. In the study area, the depth of Conrad discontinuity is about 10 km at Samcheog, northeastern end of the survey line, and it is increased rapidly to about 12.5 km at Miro, 15 km at Gosari and 15.5 km at Dongjeom, southwestern end of the survey line, respectively. The depth of the basement of the Ockchon zone exposed at Samcheog is increased smoothly to about 2 km at 5 km from Samcheog along the survey line, and is exposed again in the area between Singiry and Gosari. Beyond Gosari its depth is increased to about 1.7 km, and displaced 2.3 km downward by Osipcheon fault near Dogyeri and 0.5 km by Baeksan thrust near Cheolam, respectively. Many V-shaped low Bouguer gravity anomalies resulted from the fracture zone associated with faults imply the existence of Osipcheon fault and several inferred faults. The low Bouguer gravity anomaly zone between Tongdong and Dongjeom is caused by Jurassic gneissose granite. A local high Bouguer gravity anomaly at 35 km along the survey line from Samcheog is interpreted by the effect of iron deposit of high density existed at subsurface. The thickness of Great Limestone Group varies from 0.5 km to 1.4 km, that of Pyeongan Supergroup from 0.4 km to 0.9 km, and that of Yangdeog Group is about 0.3 km. The thickness of Jurassic gneissose granite varies from 1.5 to 3.0 km.
