The Efficacy of Chlorhexidine in Hemodialysis Vascular Access Device Disinfection (혈액투석 혈관통로 소독에 있어 클로르헥시딘의 효과)

  • Purpose:The purpose of this study was to apply the disinfection method using chlorhexidine in practice on disinfection of vascular access for hemodialysis. Methods: This study was designed as a randomized controlled trial for examining effectiveness on infection of the vascular access device for hemodialysis when using chlorhexidine and betadine/alcohol. One-hundred-thirty study participants were separated into two groups randomly. Infection signs of the vascular access device for hemodialysis were observed and recorded before disinfection on vascular access device. Result: Before the study, there was no difference between the experimental group (chlorhexidine group) and the control group (betadine/alcohol group) in general characteristics and hematological index. Incidence of infection rate of chlorhexidine group was 0 percent and the betadine/alcohol group was 1.5 percent. There was no significant difference between the two groups. Conclusion: This study examined the effectiveness of prevention of infection with the disinfection method using chlorhexidine and betadine/alcohol. The disinfection method using chlorhexidine is considered an effective and alternative method of betadine/alcohol.

A Study on Enhancement of UV Disinfection System Performance by the Vortex Generator (와동 발생기를 이용한 자외선 살균 시스템 성능 향상에 관한 연구)

  • The effectiveness of a UV(ultra violet) disinfection system depends on the characteristics of the waste water, flow conditions, the intensity of UV radiation, the amount of time the microorganisms are exposed to the radiation, and the reactor configuration. The wast water flow conditions are important factors in the design of UV disinfection system from the point of enhancement view of UV disinfection. The turbulent energy intensity in the wake by the vortex shedding are effective for UV radiation. Therewith the effectiveness of vortex generator is considered as a enhancement of UV disinfection. The experimental results presented give important evidences and explain that it is possible to predict UV disinfection performance based on flow experiments. An experimental investigation of two types of the vortex generator is presented. The qualitative and quantitative evaluations of the wake are made by flow visualization using smoke wire method and the measurement of vortex frequencies in the wind tunnel. From the experiment, following results were obtained that the delta wing type vortex generator is more effective than circular type because of the higher vortex frequencies and the smaller drag.

Effects of Disinfection According to Durations of Surgical Hand Scrub and Type of Disinfectant (외과적 손씻기 시간과 소독제에 따른 소독효과)

  • Purpose: This study was done to identify the differences in effectiveness of disinfections for surgical hand scrubbing according to the duration of scrubbing and the type of disinfectant. Method: From June 30 to August 14, 2003, the data were collected from 30 surgical nurses and surgeons in one general hospital in P city, Korea. All participants washed their hands with two different disinfectants and four different scrubbing times, they placed both finger tips on a blood agar plate and using sterile cotton tips microbes were collected from their palms, nails and forearms. Results: The first hypothesis of this study(with same disinfectant, there will be no difference in effect of disinfection in surgical hand scrub among four time groups) was supported. The second hypothesis of this study(with same duration of surgical hand scrub, there will be no difference in effect between two disinfectants) was also supported. Conclusion: There are no differences in effectiveness of disinfection for surgical hand scrubbing according to duration of the scrubbing and the type of disinfectant. So it is concluded that the effectiveness of disinfection depends more on the exactly how the scrubbing is done rather than the duration of scrubbing or the type of disinfectant.

An aluminum-based reflective nanolens array that enhances the effectiveness of a continuous-flow ultraviolet disinfection system for livestock water

  • Climate change has worsened droughts and floods, and created conditions more likely to lead to pathogen contamination of surface water and groundwater. Thus, there is a growing need to disinfect livestock water. Ultraviolet (UV) irradiation is widely accepted as an appropriate method for disinfecting livestock water, as it does not produce hazardous chemical compounds and kills pathogens. However, UV-based disinfection inevitably consumes electricity, so it is necessary to improve UV disinfection effectiveness. Aluminum-based reflective nanolens arrays that enhanced the effectiveness of a continuous-flow UV water disinfection system were developed using electrochemical and chemical processes, including electropolishing and two-step anodization. A continuous UV disinfection system was custom designed and the parts were produced using a three-dimensional printer. Electropolished aluminum was anodized at 40 and 80 V in 0.3 M oxalic acid, at 120 and 160 V in 1.0 M phosphoric acid, and at 200 and 240 V in 1.5 M citric acid. The average nanolens diameters (D) of the aluminum-based reflective nanolens arrays prepared using 40, 80, 120, 160, 200, and 240 V anodization were 95.44, 160.98, 226.64, 309.90, 296.32, and 339.68 nm, respectively. Simple UV reflection behind irradiated water disinfected Escherichia coli O157:H7 in water more than did the non-reflective control. UV reflection and focusing behind irradiated water using an aluminum-based reflective nanolens array disinfected E. coli O157:H7 more than did simple UV reflection. Such enhancement of the UV disinfection effectiveness was significantly effective when a nanolens array with D 226.64 nm, close to the wavelength of the irradiated UV (254 nm), was used.

Feasibility Study of UV-Disinfection for Water Reuse of Effluent from Wastewater Treatment Plant (용수재이용을 위한 하수처리 유출수의 UV 소독 효율 연구)

  • The feasibility study of UV-disinfection system was performed for disinfection of effluent from wastewater treatment plant. Three low-pressure UV lamps of 17, 25, and 41 W were examined with various flow rates. Low-pressure UV lamps of 17W were examined with various turbidity, DOM (dissolved organic matter), and SS (suspended solid). The pilot plant was a flow-through type UV-disinfection system, and the range of exposure time varied from 5 to 40 seconds, turbidity from 0 to 40 NTU, DOM from 0 to 30 mg/L, and SS from 10 to 40 mg/L. The 41W lamp demonstrated complete disinfection showing no survival ratio in all the experimental conditions, and generally 17W and 25W lamps also showed high removal ratio over 97%. For the same UV dose (UV intensity times exposure time), high intensity-short exposure conditions showed better disinfection efficiency than low intensity-long exposure conditions. While the effects of turbidity and DOM were not apparent, the effects of SS was significant on the disinfection efficiency which indicates that SS control before UV-disinfection appears to be necessary to increase removal efficiency. Considering characteristics of effluent from existing wastewater treatment plants, cost-effectiveness, stable performance, and minimum maintenance, the flow-through type UV-disinfection system with high intensity and low-pressure lamps was thought to be a competitive disinfection system for wastewater reclamation.

Analyzing Image Quality and Disinfection Effectiveness of Alcohol Gel Application in Ultrasound Scans (초음파 스캔에서 알코올 겔 적용 시 화질과 소독효과 분석)

  • To analyze the image quality and disinfection effectiveness of alcohol gel application in ultrasound scans, we varied the application of alcohol gels with different alcohol content from ultrasound gels. The image quality analysis compared the Dead zone, axial and lateral resolution, sensitivity, and gray scale changes using the ATS-539 phantom, while the disinfection effectiveness analysis was performed by swabbing the three cover faces of convex probe used in clinical practice, collecting specimens and enrichment culture them for 24 hours before identification with Gram stain. image quality analysis showed that all items, regardless of the alcohol content, produced images that were not different from the ultrasonic gel application images, and pathogens such as staphylococcus aureus, streptococcus, and enterococcus were detected on all probes, and all were extinction by the application of alcohol gel. The application of alcohol gel during ultrasound examination was meaningful in terms of image quality maintenance and disinfection effect, and if the results of this study are further studied and commercialized, the application of alcohol gel as an alternative to ultrasound gel will be very useful in clinical examinations because it can consider not only image quality but also infection prevention.

A Study on CFD Methodology of the Performance Predictionfor the UV Disinfection Reactor (자외선 소독기 성능 예측을 위한 CFD 해석 기법 연구)

  • The disinfection method using UV has emerged as photodissociation in water disinfection. In order to predict performance for UV disinfection, CFD analysis was performed due to saving cost. Most CFD studies of UV reactor have used particle tracking method. However it demands additional analysis time, computing resource and phase besides working fluid. In this paper, pathogenic microorganisms' route is assumed to streamline of fluid to save computing time. the computational results are in good agreement with experimental results. The results of streamline method are compared with the particle tracking method. In conclusion, the effectiveness of streamline method for UV disinfection are confirmed.

A simple and effective method for addition silicone impression disinfection

  • PURPOSE. Although dental impression disinfection is determinant to reduce the cross-infection risk, some studies have shown that, in real practice, the disinfection procedures vary considerably. Thus, the aim of this study was to evaluate the antimicrobial effectiveness and the impact on the dimensional stability of addition silicone' impressions of water wash and the most clinically used disinfection solutions: 3% hydrogen peroxide, commercial disinfectant MD520 (Durr) and 1% and 5.25% sodium hypochlorite. MATERIALS AND METHODS. For this investigation, dental impressions were taken on 16 volunteer dental students. The antimicrobial effectiveness of each procedure was evaluated by pour plate method. The dimensional stability was evaluated using a standardized stainless-steel model, according to ANSI/ADA nº19 specification. RESULTS. The study results showed that water wash does not alter the dimensional stability of addition silicone impressions but doesn't reduce the microbial load of the material (P>.05). On the other hand, addition silicone disinfection by immersion with 3% hydrogen peroxide, MD520 (Durr), or sodium hypochlorite at 1% and 5.25% does not alter the dimensional stability significantly but reduces > 99.9% of the microbial load of the impressions (P<.001). CONCLUSION. Addition silicone impressions should always be disinfected after water wash in order to reduce effectively the cross-infection risk. All disinfectants tested showed high antimicrobial efficiency without significant changes in three-dimensional shape of impressions. Hydrogen peroxide and sodium hypochlorite are of particular importance because are easily accessible in dental setting. The less explored hydrogen peroxide could be a valuable alternative for silicone impressions disinfection.

Disinfection Characteristics of Waterborne Pathogenic Protozoa Giardia lamblia

  • Giardia lamblia is a parasitic protozoa which is transmitted in the form of a cyst through untreated water and also treated drinking water. Since its presence in water has led to frequent outbreaks of giardiasis and death in many countries, the removal and disinfection of this protozoan cyst from the water supply are of great concern for public health. This study examined the disinfection characteristics of G. lamblia cysts isolated from a Korean patient with giardiasis. When using sodium hypochlorite including 5 or 10 ppm chlorine, the killing rate was initially rapid, however, the disinfection slowed down and a 3log reduction could not be achieved even after 2h. The disinfection effectiveness was also reduced at a lower temperature, thereby implying that the risk o a giardiasis outbreak will be higher in the winter season. A CT (concentration$.$time) curve was constructed based on the results with sodium hypochlorite for use in designing and predicting disinfection performance. The organic chlorination disinfectant SDIS (sodium dichloroisocyanurate) produced a lower pH and a much higher residual effect than sodium hypochlorite. The disinfection of cysts by SDIC continued steadily throughout 2h of contact, although the initial killing rate was lower than that with sodium hypochlorite.

Evaluation of Environmental Contamination and Disinfection Effects in Patient Rooms with Carbapenem-Resistant Enterobacteriaceae Using ATP Measurements and Microbial Cultures (ATP 측정과 미생물 배양검사를 이용한 카바페넴내성장내세균 보유환자 병실 환경 오염 및 환경 소독 효과 평가)

  • Purpose: To determine the extent of environmental contamination and the effect of disinfection around patients with carbapenem-resistant Enterobacteriaceae (CRE) using adenosine triphosphate (ATP) measurements and microbial culture tests. Methods: The subjects of this study were 10 patients hospitalized in a single room due to CRE from April 13 to 21, 2021. One hundred and sixty samples were collected using cotton swabs from the patients' environment including the surface and drain of sinks and toilet seats before and after disinfection of the room after discharge. Twenty-one samples were collected from the nurses' personal digital assistants (PDAs), keyboards, and computer mice before disinfection. The relative light units (RLUs) and CRE colony-forming units (CFU) of 181 samples were measured using ATP test equipment and chrome agar plates, respectively. Results: The highest RLUs were measured at the sink drains before and after disinfection. Four CRE samples from the sink drains (2), sink surface (1), and toilet bowl (1) before disinfection were cultured. Based on the failure criteria (≥ 250 RLU/cm2 and ≥ 1 CFU/100 cm2), 90 % and 50 % of the samples from the drain exceeded the failure criteria before and after disinfection, respectively. In the culture tests, CRE was not detected after disinfection. Conclusion: According to the RLU and CFU measurements of drain samples, disinfection was not effective. Thus, improvements in the disinfection methods of drains, as well as more efficient and systematic environmental decontamination and disinfection evaluation tools, are needed to accurately evaluate the effectiveness of disinfection in various places.
