Objective: To investigate the association of clinical, pathologic, and magnetic resonance imaging (MRI) variables with progressive disease (PD) during neoadjuvant chemotherapy (NAC) and distant metastasis-free survival (DMFS) in patients with triple-negative breast cancer (TNBC). Materials and Methods: This single-center retrospective study included 252 women with TNBC who underwent NAC between 2010 and 2019. Clinical, pathologic, and treatment data were collected. Two radiologists analyzed the pre-NAC MRI. After random allocation to the development and validation sets in a 2:1 ratio, we developed models to predict PD and DMFS using logistic regression and Cox proportional hazard regression, respectively, and validated them. Results: Among the 252 patients (age, 48.3 ± 10.7 years; 168 in the development set; 84 in the validation set), PD was occurred in 17 patients and 9 patients in the development and validation sets, respectively. In the clinical-pathologic-MRI model, the metaplastic histology (odds ratio [OR], 8.0; P = 0.032), Ki-67 index (OR, 1.02; P = 0.044), and subcutaneous edema (OR, 30.6; P = 0.004) were independently associated with PD in the development set. The clinical-pathologic-MRI model showed a higher area under the receiver-operating characteristic curve (AUC) than the clinical-pathologic model (AUC: 0.69 vs. 0.54; P = 0.017) for predicting PD in the validation set. Distant metastases occurred in 49 patients and 18 patients in the development and validation sets, respectively. Residual disease in both the breast and lymph nodes (hazard ratio [HR], 6.0; P = 0.005) and the presence of lymphovascular invasion (HR, 3.3; P < 0.001) were independently associated with DMFS. The model consisting of these pathologic variables showed a Harrell's C-index of 0.86 in the validation set. Conclusion: The clinical-pathologic-MRI model, which considered subcutaneous edema observed using MRI, performed better than the clinical-pathologic model for predicting PD. However, MRI did not independently contribute to the prediction of DMFS.

The effects of the KBCMT hot water extract & ultra-fine powder on (1) the behavior (2) expression of $IL-1{\beta}$, $TNF-{\alpha}$, MDA, CD68 and CD11b; (3) AChE in serum (4) the infarction area of the hippocampus, and brain tissue injury in Alzheimer's diseased mice induced with ${\beta}A$ were investigated. The KBCMT hot water extract suppressed the expression of $IL-1{\beta}$, IL-6 and $TNF-{\alpha}$ mRNA in BV2 microglia cell line treated with LPS. The KBCMT hot water extract suppressed the production of $IL-1{\beta}$, IL-6, $TNF-{\alpha}$, NO in BV2 microglial cell line treated with LPS. The KBCMT hot water extract & ultra-fine powder a significant inhibitory effect on the memory deficit was shown for the mice with Alzheimer's disease induced by ${\beta}A$ in the Morris water maze experiment, which measured stop-through latency and distance movemet-through latency The KBCMT ultra-fine powder suppressed the expression of TNF-a protein significantly in the microglial cell of mice with Alzheimer's disease induced by ${\beta}A$. The KBCMT hot water extract & ultra-fine powder reduced the MDA and suppressed the over-expression of CD68, CD11b in the mice with Alzheimer's disease induced by ${\beta}A$. The KBCMT hot water extract & ultra-fine powder decreased AChE significantly in the serum of the mice with Alzheimer's disease induced by ${\beta}A$. The KBCMT hot water extract & ultra-fine powder reduced infarction area of hippocampus, and controlled the injury of brain tissue in the mice with Alzheimer's disease induced by ${\beta}A$. The KBCMT hot water extract & ultra-fine powder reduced the tau protein, GFAP, and presenilin1, 2 of hippocampus in the mice with Alzheimer's disease induced by ${\beta}A$. These results suggest that the KBCMT hot water extract & ultra-fine powder may be effective for the prevention and treatment of Alzheimer's disease. Investigation into the clinical use of the KBCMT hot water extract & ultra-fine powder for Alzheimer's disease is suggested for future research.

Purpose. This study is to find the degree of variations and trends of hospital services utilization for hypertensive disease, and have done the comparative analysis of the factors affecting occurring some variations. For this, this study uses the data for patients-survey and health-survey of a regional society by Korea Institute for Heath and Social Affairs in 2008; The regional units are classified into 160 of medium size medical service areas. Methods. I understand the level of variation by using index of Extremal Quotient(EQ) and Coefficient Variation(CV), and analyze critical factors influencing some differences in hospital services utilization by using multi-regression model. Results. The main results are followed:The first, in case of rate of hospital services utilization according to standarization of sex and age by small area, I find the variations of EQ 5.3 and CV 0.3; In Ho-nam, especially, the variation of high rank of 10 of age shows higher distribution. The second, the results analyzing the factors influencing on hospital services utilization by multi regression model are that a number of bed hospitals is significant positive relationship and EQ-5D of health behavior is significant negative one. Conclusions. To increase equity of hospital services utilization for hypertensive disease, this study requests the appropriate supply management of bed hospitals by region, efficient allocation of resources, and revitalization of the health promotion program.
Purpose: The main purpose of this study was to investigate that the stages of change in smoking cessation behavior among coronary artery disease patients for six months progressed following the stages of change suggested by the transtheoretical model. Method: Subjects for this descriptive survey were 59 coronary disease patients who were smoking or who had stopped smoking for less than six months. Result: In the baseline, the distribution of the subjects’ stages of change was as follows: pre-contemplation stage 25.4%, contemplation stage 25.4%, preparation stage 22%, and action stage 27.1%. After six months, more subjects in the contemplation(33.3%) and preparation stages(30.8%) progressed to the action stage than those of the pre-contemplation stage(0%). Eighty-one percent of the subjects in the action stage at baseline progressed to the maintenance stage. The relationship between the numbers of smoking cessation attempts for six months and stages of change at baseline was significant(p=.001). However, the relationships between self-efficacy and nicotine dependence at baseline and progression in stages of change after six months were not significant. Conclusion: Progression in the stages of change for six months among subjects corresponded to the stages of change suggested by the transtheoretical model. Hence, future development and evaluation of intervention programs should be tailored individually considering each patient's stage of change.
Purpose: The purpose of this study was to identify effects of family support and quality of life on smoking cessation in patients with coronary artery disease. Method: Data were collected using a self-reported questionnaire included smoking history, family support and quality of life (QOL). The participants were 159 male patient with coronary artery disease who were current smokers or ex-smokers. A logistic model was developed to estimate the likelihood of current smoker or ex-smoker. Results: Of the participants, 28.3% were current smokers and 71.7% were ex-smokers. The mean score for family support was 27.41 for positive support and 23.11 for negative support. The mean score for QOL was 50.48. There were significant differences in QOL according to smoking status. The predictors of smoking cessation were social interaction QOL and self-control QOL, and duration of smoking. The model correctly classified 89.5% of ex-smokers and 44.4% of current smokers and the correct classification for the total was 76.8%. Conclusion: Social interaction QOL, self-control QOL and duration of smoking were significant variables in prediction of smoking cessation. QOL should be considered in developing smoking cessation interventions. It is advisable to also examine the mediating effect of family support on quality of life.
Alzheimer's disease (AD) is the most common form of dementia in the aging population and is clinically characterized by a progressive loss of cognitive abilities. Pathologically, it is defined by the appearance of senile plaques - extracellular insoluble, congophilic protein aggregates composed of amyloid $\beta$ (A$\beta$) and neurofibrillary tangles (NFTs) - inyracellular lesions consisting of paired helical filaments from hyperphosphorylated cytoskeletal tau protein as described by Alois Alzheimer a century ago. These hallmarks still serve as the major criteria for a definite diagnosis of the disease. Consequently, one of the key strategy for drug development in this disease area focuses on reducing the concentration of cerebral A$\beta$ plaque by using substances that inhibit A$\beta$ fibril formation. We focused on developing inhibitors by synthesizing several kinds of aromatic molecules. The synthetic compounds were initially screened to evaluate the effective compound by tioflavin T fluorescence assay. The selected effective compounds were tested cytotoxicity and protective effect from A$\beta$-induced neuronal toxicity by cell based MTT assay with HT22 hippocampal neurons. The BBB permeability on effectors was also tested in in vitro co-culture model(HUVEC/C6 cell line). The behavior test wea carried out in mutant APP/PS1 transgenic mouse model of Alzheimer's disease. And inhibition of A$\beta$ fibril formation by the effective compound was monitored with transmitted electron microscopic images.
The purpose of this study was to examine the path analysis of factors related to musculoskeletal diseases of shipbuilding workers by considering related factors synthetically. The survey was completed by 1,536 shipbuilding workers and except for poor responses, 1,532 were analyzed. The survey consisted of questions about lifestyle, duration of service, physical and mental stress, and musculoskeletal diseases. The collected data was analyzed with SPSS/PC+ (a descriptive statistics program) and with AMOS 4.0 (a statistical program for path modeling) to test whether the hypothesized path model fitted the collected data. The results of the correlation analysis showed that musculoskeletal disease was associated with duration of service (r=-.095), physical stress (r=.077), and mental stress (r=.602). The results of the hypothesized path model satisfied the criteria required in relation to fitting the collected data. Musculoskeletal disease was directly affected by mental stress but not by physical stress, duration of service and life style. Thus, it is suggested that reducing mental stress is a strategy for the prevention for musculoskeletal disease.
BACKGROUND: It is known that impacts of climate change on damage occurrence by insect pests and diseases are increasing. The negative effects of climate change on production will threaten our food security. It is needed that on the basis of analysis of the impacts, proper strategies in response to climate change are developed. METHODS AND RESULTS: The objective of this paper is to estimate impacts of climate change on rice damage occurrence by insect pests and diseases, using the panal model which analyzes both cross-section data and time series data. The result of an analysis on impacts of climate change on rice damage occurrence by pest insect and disease showed that the damage occurrence by Rice leaf roller and Rice water weevil increased if temperature increased, and damage occurrence by Stripe, Sheath blight, and Leaf Blast increased if precipitation(or amount of sunshine) increased(or decreased). CONCLUSION: Adaptation strategies, supplying weather forecasting information by region, developing systematical strategies for prevention of damage occurrence by pest insect and disease, analyzing the factors of damage occurrence by unexpected pest insect and disease, enforcing international cooperation for prevention of damage occurrence are needed to minimize the impacts of damage occurrence on rice production.
Recently, transfer learning techniques with a base convolutional neural network (CNN) model have widely gained acceptance in early detection and classification of crop diseases to increase agricultural productivity with reducing disease spread. The transfer learning techniques based classifiers generally achieve over 90% of classification accuracy for crop diseases using dataset of crop leaf images (e.g., PlantVillage dataset), but they have ability to classify only the pre-trained diseases. This paper provides with an evaluation scheme on selecting an effective base CNN model for crop disease transfer learning with regard to the accuracy of trained target crops as well as of untrained target crops. First, we present transfer learning models called CDC (crop disease classification) architecture including widely used base (pre-trained) CNN models. We evaluate each performance of seven base CNN models for four untrained crops. The results of performance evaluation show that the DenseNet201 is one of the best base CNN models.
