질병예방 및 건강증진 종합 대책 (Comprehensive Measures for Disease Prevention and Health Promotion)

  • While the threat from traditional communicable diseases have been decreasing non communicable chronic diseases are increasing due to the aging of population and change in life pattern of the people such as over intake of cholesterol and lack of physical exercise etc. On the other hand, since 1980s, AIDS is spreading rapidly throughout the globe and environmental pollution, accidents, addictive diseases such as drug abuse and alcoholism are becoming serious factors to hinder the health promotion of the people. In order to improve general public health and promote individual health status, existing program for communicable disease control by the government such as tuberculosis, leprosy, STD and acute communicable diseases should be effectively continued. In principle, effort should be placed on eradication of source of infection, reduction of communicability of source in infection, treatment of source of infection as well as increase of individual registance to the diseases through immunization and improvement of physical status. Since the pattern of illness is being shifted from communicable diseases to non communicable chronic diseases such as cancer, cerebral hemorrhage, heart disease and hypertention etc., special emphasis should be placed on the prevention and control of those adult diseases. Early detection of the patients, registration and treatment of patients and health education should be systematically developed for effective control of the diseases. In addition, program activities on MCH, nutrition, dentistry, mental health, environmental health, accident prevention. medical delivery system, health insurance. and all other health issues should be improved in order to achieve our goal of health promotion.

국내 돼지오제스키병의 혈청학적 감시활동(surveillance)을 위한 표본크기 (Sample size for serological surveillance of Aujeszky's disease in Korea)

  • Serological surveillance programs in animal populations are becoming increasingly important to estimate prevalence of a specific disease and subsequently to document disease-free status in a region or a country. For these purposes, the programs need to be based on both theoretical and economical aspects from the designing phase. From Aujeszky's disease (AD)-eradication program point of view, group of animals (aggregates, herds) not individual animal is the more important sampling unit of concern. In this study the authors therefore attempted to compute an appropriate sample size tailored to a current surveillance program against AD, assuming that the goal of this program is either herd-level prevalence estimation or documentation of AD-freedom. For prevalence estimation, assuming a finite population with imperfect sensitivity (Se) and specificity (Sp) of ELISA kit for AD diagnosis, the number of herds present, expected herd prevalence, and desired accuracy for a certain level of confidence, sample size was estimated at herd-level in the first stage and individual animal-level in the second stage. A two-stage sampling design was used to calculate a sample size to indicate AD-freedom. In this instance, the computation was based on the possible detection of a predetermined prevalence at a certain herd-level Se and Sp. This study indicated that the sample size varied with predetermined confidence, tolerance, Se and Sp at herd- and animal-level, and within- and among-herd prevalence. In general, smaller sample size was required to estimate AD prevalence than to document of AD-freedom. Compared to individual-based samples, two-stage sampling strategy requires a larger sample size to show disease-freedom. Statistical considerations including herd-level test characteristics when designing surveillance program also are further discussed.

  • Bovine tuberculosis (bTB), caused primarily by Mycobacterium bovis, continues to exert an economic loss, even in countries with active control measures, and is one of zoonotic diseases enable to be transmitted to human. The control and eradication of bTB are mainly based on a test and slaughter policy and/or abattoir surveillance. Various factors including limitation of diagnostic tests have been considered as major constraints to eradication. Single intradermal test (SIT) is the official diagnostic test. New diagnostic methods are needed to be developed, because of limitations of the test. In the present study SIT was compared with single intradermal comparative cervical test (SICCT) and interferon (IFN)-${\gamma}$ assay. There was very low correlation between SIT and SICCT. However, high correlation was shown between SIT and IFN-${\gamma}$ assay while no correlation was observed between SICCT and IFN-${\gamma}$ assay. Therefore, our results suggest the possibility of replacement of SIT with IFN-${\gamma}$ assay for the diagnosis of bovine tuberculosis.

제초제처리(除草劑処理)에 의(依)한 잣나무털녹병(病) 중간기주(中間奇主)(송이풀류(類)) 제거시험(除去試験) (Studies on the Eradication of Pedicularis spp., Alternate Host of White Pine Blister Rust with Several Herbicides)

  • 우리나라 잣나무털녹병(病)의 중간기주(中間寄主)인 송이풀류(類)를 제거(除去)하기 위(爲)하여 1975년(年)부터 몇 가지 제초제(除草劑)을 산지(山地)에 자생(自生)하는 송이풀류(類)에 처리(處理)하여 다음의 결과(結果)를 얻었다. 1. Roundup은 주위(周圍)의 초(草), 목본류(木本類)에 약해(薬害)를 주었으나 송이풀제거용(除去用) 제초제(除草劑)로서 가장 효과(効果)가 있었다. 2. Roundup의 송이풀 제거효과(除去効果)는 처리(處理) 1 년후(年後)에 100%, 2 년후(年後)에는 65~92%의 밀도감소효과(密度減少効果)가 있었다. 3. 2.4-D, Gramoxone, 2. 4. 5-T 및 Para-col은 처리당년도(處理當年度)에 송이풀의 경엽제거(茎葉除去)는 가능(可能)하였으나 익년도(翌年度) 봄에 재발순(再發筍)되었다. 4. 제초제(除草劑)의 살포시기(撒布時期)는 5~6月(송이풀발순완료기(發筍完了期))보다 완전(完全)히 성숙(成熟)된 8월하순(月下旬)에 살포(撒布)하는 것이 효과적(効果的)이였다.

가금티푸스의 특성 및 근절방안에 관한 연구 (The study on the characteristics of fowl typhoid and the eradication program)

  • Fowl typhoid is a septicemic disease of turkeys and chickens. The antigen is Salmonella(S) gallinarum, which cross-reacts with S pullorum because of common antigen. The organism is maintained in the ovary of carrier birds and transmitted vertically in the york. It is also transmitted horizontally through feces and broken eggs. The symptoms are weakness, drowsiness, wing drooping, hyperexcitability, paresis and diarrhea. In the many cases, the birds are found dead in the morning before any symptoms have been noticed. this experiment was carried out to investigate the characteristics of S gallanarum and scrutinized about the control systems. 1. Fowl typhoid was came about in the winter than the other seasons. The average of mortality was 46.3%. And it was came about not only adult poultry but also chicks. 2. S gallinarum was isolated from liver and spleen in infected chickens. Total isolated strains were 60. 3. The organisms were all extinct at 60$^{\circ}C$, 30mins. 4. A survival rate was higher in underground water than a secondary distilled water. It was detected in underground water until 30 days. 5. When being exposed to the solar light, it became extinct almost within 24 hours. 6. It was survived in a refrigerator until 12 months. 7. It was extinct among 5 mins in 0.1% phenol and 2% formalin. 8. When the cultured organism fluid was inoculated in SD-rat abdominal cavity, rat died within 24 hours. 9. Pullorum-typhoid related program must disciplines in order to destroy the fowl typhoid, and breeding system must be converted to cage system.

소화성 궤양에 대한 오패산의 치료효과 : 체계적 문헌고찰과 메타분석 (The Effect of Opae-san for Peptic Ulcer Disease: A Systematic Review and Meta-Analysis)

  • Objectives: The aim of this study was to perform a meta analysis of randomized controlled trials (RCTs) that applied Opae-san to peptic ulcer. Methods: The databases NDSL, RISS, KISS, KISTI, Oasis, DBpia, Cochrane, EMBASE, Pubmed, and CNKI were searched to identify RCTs that evaluated the therapeutic response to Opae-san on peptic ulcer. The selected studies were assessed using Cochrane Group's risk of bias tool. Results: 12 RCTs were selected from a total of 312 identified. Combined therapies of Opae-san plus triple therapy were superior to only triple therapy in achieving the effective rate (risk ratio=1.26, 95% CI: 1.17 to 1.35, p<0.001, I2=0%), the helicobacter pylori eradication rate (risk ratio=1.23, 95% CI: 1.12 to 1.34, p<0.001, I2=7%) and the recurrence rate (risk ratio=0.31, 95% CI: 0.12 to 0.82, p=0.02, I2=0%). But only Opae-san was not superior in achieving the effective rate compared to anti gastric secretion drugs. Conclusions: The current evidence suggests that combined therapies of Opae-san plus triple therapy yield a higher effective rate, and helicobacter pylori eradication rate and a lower recurrence rate. However, in most of these studies, it is difficult to evaluate the bias and therefore better designed studies are needed.

Influence of Helicobacter pylori Infection on Endoscopic Findings of Gastric Adenocarcinoma of the Fundic Gland Type

  • Purpose: Gastric adenocarcinoma of the fundic gland type (chief cell predominant type) (GA-FG-CCP) was first reported as a rare adenocarcinoma found in the normal fundic mucosa. Recent studies have proposed the possibility that GA-FG-CCPs were also generated in the atrophic mucosa after Helicobacter pylori (HP) eradication therapy. However, little is known on the endoscopic findings of GA-FG-CCP generated in the atrophic mucosa due to its extreme rarity. Materials and Methods: A total of 8 patients who underwent endoscopic submucosal resection and were diagnosed with GA-FG-CCP generated in the HP-uninfected mucosa (4 cases, HP-uninfected group) or HP-eradicated atrophic mucosa (4 cases, HP-eradicated group) were retrospectively analyzed, and their endoscopic findings, including magnifying endoscopy with narrow band imaging (M-NBI), and pathological features were compared. Results: While GA-FG-CCPs in the 2 groups displayed similar macroscopic appearance, M-NBI demonstrated that characteristic microvessels (tapered microvessels like withered branches) were specifically identified in the HP-eradicated group. Pathological investigation revealed that a decreasing number of fundic glands and thinned foveolar epithelium covering tumor ducts were thought to lower the thickness of the covering layer over tumor ducts in the HP-eradicated group. Moreover, dilation of vessels just under the surface of the lesions contributed to the visualization of microvessels by M-NBI. Conclusions: The change in background mucosa due to HP infection influenced the thickness of the covering layer over the tumor ducts and M-NBI finding of GA-FG-CCP.

소아 십이지장궤양에서의 H. pylori 박멸과 궤양재발에 대한 연구 (The Eradication of Helicobacter pylori in the Duodenal Ulcer in Children and the Duodenal Recurrence)

  • 목 적: H. pylori가 만성유문부 위염의 원인으로 밝혀지면서 십이지장궤양 환자의 90%이상에서 그 위점막에 H. pylori의 존재가 확인되어 H. pylori의 박멸이 십이지장궤양 치료 및 재발에 미치는 영향에 대해 많은 연구가 이루어져 왔고 여러 보고에서 H. pylori의 박멸로 십이지장궤양의 재발율이 현저히 감소한다고 하였다. 소아에서는 성인에 비해 궤양의 빈도가 낮고 내시경적 검사의 제약으로 인하여 이에 대한 관심이 상대적으로 적었으며 소아 십이지장궤양에 대한 연구는 현재까지 거의 없는 실정이다. 이에 저자들은 내시경으로 진단한 소아 십이지장궤양에서 고식적 궤양치료와 H. pylori 병행치료후의 궤양의 재발에 대해 비교 분석해 보고자 하였다. 방 법: 1987년 7월부터 1995년 8월까지 서울대학병원 어린이 병원에 내원하여 내시경검사상 십이지장궤양으로 진단받은 105명(남:여=78:27)을 대상으로, 고식적 치료(ranitidine+antacid)를 행한 군과, H. pylori 진단을 받은 경우 고식적 치료와 함께 amoxicillin(50 mg/kg) 2주, denol(8 mg/kg) 4주의 균박멸 치료를 병행한 군으로 나누어 궤양재발율에 대하여 비교 조사하여 보았다. H. pylori 양성은 내시경생검을 통한 CLO test, Giemsa 염색과 혈청 IgG 항체검사중 둘 이상이 양성이거나 생검조직의 균배양이 양성인 것으로 하였다. 결 과: 1) 91년 5월이후 진단받은 38명(남:여=30:8) 중 원발성 십이지장궤양은 30명(78.9%)이었으며 모두 H. pylori 검사가 시행되어 이중 27명이 양성으로 H. pylori의 감염율은 90.0%였다. 2) H. pylori 양성인 27명 환아에 대해 고식적 치료와 H. pylori 치료를 병행하였는데 추적내시경검사상 23명에서 궤양치유와 함께 균의 박멸이 확인되어 H. pylori 치료성공율은 85.2%였다. 3) 6주의 치료후 추적검사상 박멸에 실패한 3명은 강화요법으로 균의 소실이 확인되었고 이중 1명(33.3%)이 치료종료 2.4년후 재발하였다. 4) 십이지장궤양 환아의 치료후 추적관찰 1년이내 궤양의 재발율은 고식적 치료만을 시행한 경우 65.3%였고 H. pylori 치료를 병행한 경우는 4.3%였다. 결 론: 십이지장궤양 환아들의 대부분이 H. pylori의 감염이 있으며 H. pylori 양성인 소아 십이지장궤양에서 H. pylori 박멸 요법은 궤양 재발의 방지를 위하여 반드시 필요하다.

2016-2017년 국내 핵과류에서의 자두곰보병 발생 및 방제 (Occurrence and eradication of Plum pox virus on Ornamentals in Korea, 2016-2017)

  • 전 세계적으로 자두곰보바이러스는 핵과류에 발생하는 중요한 바이러스이다. 2015년 국내 복숭아에서 자두곰보바이러스 감염이 확인된 이후 국내 핵과류 과원에 대한 자두곰보바이러스 발생 조사가 시작되었다. 2016-2017년 국내 핵과류 1,985과원의 30,333주 나무에서 시료를 채집하여 특이 프라이머를 이용한 RT-PCR 검정 결과, 10과원의 21주 나무에서 자두곰보바이러스가 확인되었고, 박멸조치에 따라 감염 주에 대한 폐기 등 방제가 실시되었다. 2016년 자두곰보바이러스에 감염된 복숭아 나무 7주의 total RNA에서 PCR 산물 약 547 bp을 얻은 후 direct sequencing 하여 염기서열을 결정하였다. BLAST 검색 결과 Genbank에 등록된 자두곰보바이러스 D 계통 분리 주들(LC331298, LT600782)과 99% 높은 염기서열 상동성을 보였으며, 국내 복숭아 나무 7개 분리 주 간에 98-100% 염기서열 상동성을 보였다. Phylogenetic tree 분석 결과 한국, 일본, 미국, 캐나다에서 보고된 D 계통과 높은 유연관계를 가지는 것을 확인 할 수 있었다. 이 보고는 2016-2017년 국내 핵과류 과원의 자두곰보바이러스 발생 현황에 대한 첫 보고이며, 이를 바탕으로 발생 주 제거 등 국내 금지급 자두곰보병의 예방 및 방제 대책 수립을 위한 기초자료로 활용하고자 하였다.

High Prevalence of Helicobacter pylori Resistance to Clarithromycin: a Hospital-Based Cross-Sectional Study in Nakhon Ratchasima Province, Northeast of Thailand

  • Background: Helicobacter pylori is a cause of chronic gastritis, peptic ulcer disease, and gastric malignancy, infection being a serious health problem in Thailand. Recently, clarithromycin resistant H. pylori strains represent the main cause of treatment failure. Therefore this study aimed to determine the prevalence and pattern of H. pylori resistance to clarithromycin in Suranaree University of Technology Hospital, Suranree University of Technology, Nakhon Ratchasima, Northeastern Thailand, Nakhon Ratchasima province, northeast of Thailand. Materials and Methods: This hospital-based cross-sectional study was carried out between June 2014 and February 2015 with 300 infected patients interviewed and from whom gastric mucosa specimens were collected and proven positive by histology. The gastric mucosa specimens were tested for H. pylori and clarithromycin resistance by 23S ribosomal RNA point mutations analysis using real-time polymerase chain reactions. Correlation of eradication rates with patterns of mutation were analyzed by chi-square test. Results: Of 300 infected patients, the majority were aged between 47-61 years (31.6%), female (52.3%), with monthly income between 10,000-15,000 Baht (57%), and had a history of alcohol drinking (59.3%). Patient symptoms were abdominal pain (48.6%), followed by iron deficiency anemia (35.3%). Papaya salad consumption (40.3%) was a possible risk factor for H. pylori infection. The prevalence of H. pylori strains resistant to clarithromycin was 76.2%. Among clarithromycin-resistant strains tested, all were due to the A2144G point mutation in the 23S rRNA gene. Among mutations group, wild type genotype, mutant strain mixed wild type and mutant genotype were 23.8%, 35.7% and 40.5% respectively. With the clarithromycin-based triple therapy regimen, the efficacy decreased by 70% for H. pylori eradication (P<0.01). Conclusions: Recent results indicate a high rate of H. pylori resistance to clarithromycin. Mixed of wild type and mutant genotype is the most common mutant genotype in Nakhon Ratchasima province, therefore the use of clarithromycin-based triple therapy an not advisable as an empiric first-line regimen for H. pylori eradication in northeast region of Thailand.
