우리나라 노인의 신체기능 요인과 차별경험의 관련성 연구: 노인실태조사(2014, 2017) 자료 분석 (Relationship between Physical Function Factors and Discrimination Experiences of the Elderly in Korea: The 2014 and 2017 National Survey on Living Conditions and Welfare Needs of Korean Older Persons)

  • Background: Supporting the elderly population is presented as a social issue, and it affects age discrimination, which forms a negative perception and avoids the elderly. Since age discrimination lowers the quality of life of the elderly and hinders social unity, it may be important to research related factors. This study examined the physical function as a factor that influences discrimination experiences and aimed to identify the relationship between physical function factors and discrimination experiences. Methods: In this study, we analyzed 20,225 elderly from the 2014 and 2017 survey of living conditions and welfare needs of Korean older persons, conducted nationwide among older than 65 years. Physical function factors are activities of daily living, instrumental activities of daily living, sight discomfort, hearing discomfort, chewing discomfort, and leg strength as factors of interest. We performed multivariable logistic regression that reflected survey characteristics, adjusting for socio-demographic factors, health related factors, and social-support factors. Results: The number of elderly who faced discrimination experiences was 1,175 (5.8%). The results showed that the odds ratio of facing a discrimination experience in the dependent group was significantly higher compared to the independent group when being transferring out of the room (4.05; 95% confidence interval [CI], 1.50-10.88) and difficulty in hearing (1.25; 95% CI, 1.05-1.49). Even with respect to chewing ability, which was significant in models 1 and 2, they face more discrimination experiences (1.30; 95% CI, 1.11-1.53). Conclusion: These research results indicate that an important strategy for preventing age discrimination is to understand the physical function conditions of the elderly and promote the physical functions of the elderly related to transferring, hearing, and chewing; it is necessary to develop a practical intervention plan that considers these aspects.

만성요통환자의 접착용 테이핑 재활요법의 효과 (The Effect of Adhesive Taping Therapy on the Relieve of Chronic Low Back Pain)

  • The purpose of this study was to test whether adhesive taping therapy, one of rehabilitation, helps to relieve chronic low back pain. Sample were selected from 35 outpatients at oriental medicine hospital in the period from April 20 to June 20, 1998. The research design was one-group pretest - posttest design. The pretest included measuring discomfort depending on the scope and types of a range of motion, and their pain by their activity of daily living(ADL). In this research design, a treatment was to expose adhesive taping therapy to samples. The posttest included remeasuring of their discomfort and pain by ADL. The results of this study was as follows: Hypothesis I was that the discomfort score of the post-treatment group had lower than does that of the pre-treatment group. This study accepted the hypothesis (t=2.70, p=.015). Hypothesis II was that the post-treatment group had the pain score by ADL lower than does the pre-treatment group. Empirical results showed statistical significance(t=4.53, p=.000). In summary, the effect measured by the discomfort with a visual analog scale was statistically significant. The degree of pain according to postures in ADL also showed statistical significance. Consequently, these findings showed that the adhesive taping therapy was effective to alleviation of chronic low back pain.

Prevalence of Cognitive Impairment and Related Factors Among the Elderly in Rural Communities of Jeju Province

  • Purpose. The purpose of this study was to survey the prevalence rate of cognitive impairments and to identify the factors influencing cognitive impairment in the elderly in rural communities of Jeju Province. Methods. 590 elderly in 6 rural communities of Jeju Province were interviewed, using a questionnaire consisting of sociodemographic characteristics, health behavior, quality of life, and MMSE-K Results. Prevalence of cognitive impairment was 33.1 % (39.1 % of females, 16.76% of males). Prevalence of dementia was 12.4% (16.3% of females, 2.87% of males). Factors related to cognitive impairment were age, sex, education, standard of living, employment status, and subjective health state. Conclusions. In community health care for the elderly, factors relating to cognitive impairment have to be considered. When planning community health care, priority should be given to the elderly; who need care but live alone; who lack social support; who have a low standard of living; who experience discomfort in the activities of daily living; who believe they are not in a good state of health; or whose life satisfaction is low.

중년여성의 요실금과 일상생활 및 자아존중감에 관한 연구 (A Study on Discomfort of Daily Life and Self-Esteem of Urinary Incontinence in Middle-Aged Women)

  • The purpose of this study was to find out the prevalence of urinary incontinence, it's relating factors and the level of self-esteem in women. Subject of this study consisted of 153 in the age of 30-55 years old living in around Iksan. The date were collected from March, 1998 to June, 1998 with interview using structured questionare and were analysed with SPSS program. The results were as follows : 1) The prevalence of urinary incontinence is 85%. The most common amount of urinary incontinence was somewhat small expressed as 'wetting their panty' 2) The common factors related to the urinary incontinence were coughing, sneezing, laughing loudly, the activities of need for hurry and constipation. The older, the more incidence of urinary incontinence were reported. 3) Only 50% of epsodic urinary incontinence were treated and they wanted to try the herb medicine(64%), taking medicine(8%) and exercise(3%). 4) There was significant between self-esteem and urinary incontinence (T=13.83, p=.000), but between degree of urinary incontinence and self-esteem were not significant(F=.71, p=.55). In conclusion, urinary incontinence will be very serious health problem as aging so that it's Important to provide nursing intervention to prevent and be well. It need to support psychological care, too.

퇴행성관절염 노인을 위한 온요법의 적용시기에 따른 효과 (Effects of Heat Therapy according to the Application Time among the Elderly with Osteoarthritis)

  • This study was to investigate the effects(pain, discomfort, and range of motion) of heat therapy according to the application time among the elderly with osteoarthritis. Study participants were 27 elderly women, who were diagnosed as osteoarthritis, suffered from it for more than 6 months, and who were staying at a nursing home in Busan. The independent variable was heat therapy, which was applied for 20minutes, one time per week according to 3 timetable(before waking up, while in daily living, before going to bed) over 6 weeks. The dependent variables were pain and discomfort measured by 20 points visual analog scales, and range of extension and flexion measured by goniometery. The data were analyzed by descriptive statistics and repeated measures ANOVA. The results were as follows ; 1) There were significant differences for pain(F=9.77 p=.0001), discomfort(F=8.07 p=.001), range of extension(F=3.05 p=.05), and flexion(F=9.67 p=.0001) among heat therapy application times. 2) There were significant differences for pain(F=58.18 p=.0001), discomfort(F=63.68 p=.0001), range of extension(F=11.59 p=.001), and flexion(F=17.59 p=.0001) between before and after applying heat therapy. 3) There were not statistically significant differences for pain(F=.64 p=.531), discomfort(F=.18 p=.836), range of extension(F=1.33 p=.270), and fiexion(F=.26 p=.773) between before and after applying heat therapy according to the heat therapy application times. In conclusion, heat therapy was effective in reducing pain and discomfort, and in improving ROM for the elderly with osteoarthritis, but the effect of it was not different according to the time of application. We recommended further studies with larger sample size, longer and more repeatedly applied to investigate the effect of heat therapy according to the time of application.

유리 생비골 및 생비골 피부편 이식 후 공여부의 평가 (The Evaluation of Donor Site after Transfer of Free Osseous and Osteocutaneous Flap of Fibula)

  • Purpose : We have evaluated the morbidity of donor site after transfer of free fibular osseous and osteocutaneous flap to defect site of bone and soft tissue due to chronic osteomyelitis of long tubular bone, open fracture with bony defect, bone or soft tissue tumor and congenital anomaly. Materials and methods : The 54 cases of 79 cases to be carried out from May, 1982 to May, 2001 which could be followed up were reviewed. There were forty nine in male and five in female. The mean age was 35(4 to 66)years old and mean follow up period is 21.3 month(12 to 72). We have retrospectively analyzed the various postoperative complications such as compartment syndrome, donor site infection, skin defect, hypesthesia, hammer toes, ankle instability and activity of daily living by help of questionnaire, telephone, physical examination, follow up x-ray study and chart. Results : In the total 54 cases the medication period for pain control after operation were classified into three groups under 2 weeks(49 cases), from 2 weeks to 6 weeks(3 cases) and over 6 weeks(2 cases). The postoperative morbidity were occurred in total 12 cases(compartment syndrome: 0, infection : 2, skin defect: 1, hypesthesia: 5, hammer toe: 2 ankle pain: 2 discomfort in activity of daily living: 0), and also the morbidity rates of donor site were 23.5% in osseous flap and 21.6% in osteocutaneous flap were occurred. There was no statistical significonce in morbidity between osseous and osteocutaneous free fibular flap transfer(P>0.05). Discussion : In general the morbidity of free fibular flap transfer was relatively high but it did not have any effect on daily activity of living. We think that the meticulous operation technique, detailed wound care and early range of motion exercise will reduce the morbidity of donor site of flap.

  • Muscle stiffness, pain discomfort in daily activities are cardinal symtoms on arthritis. To reduce these symtoms, a nonequivalent one-group pre & posttest experiment was carried out at a rural community health post area in kwangju province during eight weeks(from 25th of January to 20th of March, 2002) The subjects were sixteen old (60-80) with osteoarthritis. And to reduce the level of pain, discomfort in daily living and to increase that of life satisfaction, 25-30minutes of muscle-joint exercise followed by 25-30minutes of local heat therapy were done three times a week. The total experimental period extended at 12 weeks and the total 36 times it executed 1. The retrogression characteristic osteoarthritis elderly with local heat and a muscle joint exercise was the telegram which it receives and after receiving, the score of the paindaily activity function appeared to the lead, there was a difference which considers statistically, this constriction was supported.(Z=4.947, p<0.0001) 2. The retrogression characteristic osteoarthritis elderly with local heat and a muscle joint exercise was the telegram which it receives and after receiving, the score of daily activity function appeared lowly, there was a difference which considers statistically, this construction was supported.(Z=2.7226, p<0.0279) 3. The retrogression characteristic osteoarthritis elderly with local heat and a muscle joint exercise was the telegram which it receives and after receiving, there was not a difference where the quality of life considers statistically, this construction was rejected.(Z=-1.2087, p>0.2171)

노인의 건강상태 및 일상생활 양식 (Health Status and Daily Life Style of the Elderly)

  • This study was planned and carried out in an attempt to investigate health status and life style. Using a guidline developed by the researcher open-ended interviews were done with a hundred and sixty Korean elderly people, from April 17, through June 2, 1988. The data were analyzed by frequencies, means, percentile scores and Chisquare-test. Major health deviations identified are ; arthritis, blood pressure discrepancies, neuralgia, and powerlessness. Discomfort in relation to elimination(75%) and to rest-activity, especially to the activities of daily living(78%) are experienced ; 11% revealed to be in need of assistance from others. One or more sensory discrepancies are complained by 75% of the subjects. Only 20% of the subjects reported being satisfied with the life-time achievements. Factors revealed that 'descendants' were the major source of pleasure. Leisure hours were spent mostly 'watching TV' 'Healthy life', 'harmonious family life' and 'frequent communication with the family' were the three most frequent wishes towards their family members. 'Insurance for ageing', 'free health care', 'work', 'cultural facilities' and 'warm attention' were the request toward the society.

Effect of Heated Red Bean Pillow Application for College Women with Dysmenorrhea

  • Purpose: Dysmenorrhea is a menstrual condition characterized by severe and frequent cramps and pain. Effective treatment methods for dysmenorrhea are not yet fully understood. This research compares the effects of pain killers and heated red bean pillows. Methods: Data were got on demographic data, menstrual cycle status, and activities of daily living (ADLs) limitations, dysmenorrhea severity and menstrual pain scores. Following a 10% drop-out rate, 44 young women satisfied the inclusion criteria. To prevent any bias, the experimental and control groups were selected from different campuses. We used two sizes of red bean pillows: $13{\times}11.5cm$, weighing 400g; and $15{\times}18cm$, weighing 220g. For analysis, we used IBM SPSS statistics 19.0. Results: Ninety-nine point seven percentage of total subjects reported moderate to severe dysmenorrhea and 63.6% reported as moderate to severe daily activities limitations. The mean pain score with visual analogue scale was $80.2{\pm}9.42$ of 100 and 86.4% used pain killers to alleviate menstrual discomfort in all the subjects. In both groups, all three variables showed significant improvement and the Moos's Menstrual Distress Questionnaire (MDQ) scores changed significantly between menstrual and post-menstrual time point at within groups and not significantly different at premenstrual and menstrual time point at between groups. However, the MDQ score was significantly higher in experimental group than control group at post-menstruation time point and the degree of satisfaction was higher in the control group. Conclusion: This research shows that red-bean pillows on the abdomen are effective in assisting the ADL and diminishing pain severity. With regard to its safety the study indicates it can be a convenient and safe option for female students with menstrual discomfort in schoolas a non-pharmacological self-help.

공동주택 거주 고령자의 건강주거 인식특성 및 요구에 관한 연구 (The Elderly's Perspectives on Apartments as a Healthy Home Environments)

  • This study focuses on a concrete understanding of the characteristics of perceptions and needs about healthy housing of the elderly, who represent the most vulnerable group of people in society, on the four evaluation dimensions of healthy housing as part of the research for evaluation indicators development for housing health performance in apartments. First, it was found that the elderly perceive an environment where their physical health is well supported as the most important factor for healthy housing. Additionally, the management support of the housing was perceived as the second most important consideration. Second, the most frequently used area for the elderly is the living room and a desired place to be added to indoor spaces was a room of their own or a 'living room', which clearly indicates the importance of the indoor environment for the elderly. Third, the biggest factor for the interior environment that affected the satisfaction level of the elderly in their housing was largely based on the convenience factor that helped lesson the discomfort in their daily life due to the physical aging process, and also on factors related to the pleasant indoor surroundings. Based on the concepts discovered in this study on healthy housing, more concrete items should be developed in the future for an extensive indicator for health performance evaluation, and actual research on the general public should follow suit.
