부채살 SPECT 데이터를 위한 정칙화된 기댓값 최대화 재구성기법 개발 (Development of Regularized Expectation Maximization Algorithms for Fan-Beam SPECT Data)

  • 목적: 부채살 단일광자단층촬영(SPECT)은 공간분해능과 민감도를 개선하는 것으로 알려져 있다. 보다 정확한 영상을 얻고 인체에 대한 SPECT의 영상화 과정을 정확하게 묘사하기 위하여 평행 데이터로 재배열하는 과정 없이 직접 부채살 데이터를 이용하여 재구성하는 알고리즘이 필요하다. 본 연구는 다양한 부채살 재구성 알고리즘을 구현하였고 각 방법의 성능을 비교하였다. 대상 및 방법: 선추적법을 적용하여 부채살 투사기와 이로부터 얻은 데이터를 직접 재구성할 수 있는 FBP, EM, OS-EM과 MAP-EM OSL 알고리즘을 구현하였다. OSL 알고리즘의 경우에는 membrane과 thin plate prior를 사용하였다. 직접 부채살 데이터를 재구성하는 방법의 성능을 평가하기 위해 양방향 최근접 이웃, 양방향 1차와 양방향 3차 보간법을 사용하여 재배열된 평행 데이터를 얻었고 이 데이터를 기존의 평행 데이터에 대한 EM 알고리즘을 사용하여 재구성하였다. Hoffman 두뇌와 Shepp/Logan 팬텀으로부터 얻은 잡음 없는 데이터와 잡음 있는 데이터는 각 방법으로 재구성하였으며 퍼센트 오차를 계산하여 각 재구성된 영상을 비교하였다. 결과: Thin-plate 사전 분포함수를 사용한 OSL 방법이 가장 낮은 오차를 가지며 잡음으로 인한 결과 영상의 불안정성을 효과적으로 제어함을 확인할 수 있었다. 부채살 데이터를 평행 데이터로 재배열시 양방향 1차 보간법이 정확성과 계산 시간 측면에서 가장 효율적인 방법임을 확인하였다. 재배열된 평행 데이터의 EM결과에 비해 직접 부채살 데이터를 재구성하여 얻은 결과영상이 더 정확하게 재구성되었다. 결론: 본 연구에서는 평행 데이터로 재배열한 경우에 비하여 보다 정확한 영상을 재구성하는 직접 부채살 재구성 알고리즘을 구현하였으며 이는 정량적으로 월등히 개선된 결과를 제공함을 확인하였다.

  • 현재 핵의학 분야에서는 디지털 컴퓨터의 급속한 발전 및 응용으로 인해 FBP 법의 대용으로 OSEM 알고리즘과 같은 고속 영상 재구성 알고리즘이 널리 이용되고 있다. 그 동안 여러 연구에서 파라미터 변경에 따른 OSEM 재구성 영상 질 변화에 대한 평가가 이루어져 왔으나, 어떠한 파라미터를 적용할 지에 관해서는 명확하게 정해진 것은 없다. 본 연구에서는 3D beam modeling을 적용한 3D OSEM 재구성 법에서 iteration 횟수와 subset 개수 변경에 따른 영상의 질 변화를 팬텀 실험과 환자 데이터을 통해 확인하고자 한다. 환자 데이터는 2010년 8월부터 9월까지 본원 핵의학과에서 Brain SPECT를 시행한 환자 5명을 대상으로 연구 분석하였다. 영상은 물과 $^{99m}Tc$ (500 MBq)을 균등하게 혼합한 Jaszczak 팬텀을 이용하여 Siemens사의 이중 헤드 감마 카메라 Symbia T2에서 획득하였다. 환자 데이터는 영상 재구성 시 환자 데이터와 팬텀 데이터 모두 iteration 횟수는 1, 4, 8, 12, 24, 48회, subset 개수는 2, 4, 8, 16, 32개로 변화를 주며 각각의 영상을 재구성하였다. 재구성된 각각의 영상에서 대조도와 영상의 잡음 정도를 가늠하기 위한 변이계수, FWHM을 산출하여 비교하였다. 팬텀 데이터와 환자 데이터에서 영상의 대조도와 공간해상력은 iteration 횟수와 subset 개수의 증가에 따라 모두 선형적으로 증가하는 경향을 나타냈으나 변이계수는 두 파라미터의 증가에 따라 향상되는 경향을 보이지 않았다. Projection 시간에 따른 비교에서도 Projection 당 10초, 20초, 30초 영상에서 모두 영상 대조도와 FWHM은 iteration 횟수와 subset 개수 증가에 따라 선형적으로 향상되는 결과를 나타냈으나 변이계수는 향상되는 경향을 보이지 않았다. 본 실험을 통해 3D beam modeling을 적용한 3D OSEM 재구성 법 영상에서도 기존의 1D와 2D OSEM 재구성 법과 같이 iteration 횟수와 부분집합 개수 증가에 따라 향상하는 영상 대조도의 선형적 관계를 확인할 수 있었다. 하지만 이는 단순한 팬텀 실험과 일부 환자 데이터 만으로 얻은 결과이고, 실제 임상에서는 보다 구조적으로 복잡한 대상과 다양한 변수들이 존재 가능하기 때문에 본 실험의 데이터만을 바탕으로 이를 일반화하기에는 무리가 있으며 차후 실험들을 통해 3D OSEM 재구성 법에 대한 평가가 추가로 이루어져야 할 것이다.

대전 도심지역의 철도진동의 영향과 대책 (A Study on Rail Vibration and Its Reduction Plan in Central Daejeon Area)

  • 도심지역 철도 주변에서 열차의 주행으로 인해 발생되는 철도 진동이 심각한 공해진동으로 부각되고 있다. 이러한 철도진동으로 부터의 피해를 저감시킬 수 있는 방안을 탐색해 보고자, 대전 도심 지역을 통과하는 철도 주변에서의 철도 진동 실태를 조사하였다. 열차가 통과하는 주행 방향과 수직한 지표면을 따라 5m 간격으로 55m까지 일정한 거리 상에서 1초 간격으로 Z축 진동값을 측정하였다. 측정 조사된 총 353개의 자료 셀마다 $L_{10}$ 값을 산출하여 진동측정치와의 거리, 열차의 속력, 차량수와의 상관 관계를 분석하였다. 분석결과 지반을 전파하는 철도진동은 철도중심에서 대략 25m 이내의 지역에서는 소음 진동규제법에서 제시한 허용기준 60dB이상으로 나타났으며, 속력 변화에 대한 진동 값의 변화가 작아 모든 속력의 열차에 대해 공해진동이 존재하였다. 한편 철도 주변 공구 밖에서는 공구가 없는 경우보다 진동값이 대체로 10%정도 감소하여 철도진동 저감에 공구 설치가 효과적인 것으로 판단된다. 실제의 지반을 대상으로 방진구를 설치하고 일정 높이에서 철구를 떨어뜨려 진동전달실험을 실시한 결과에서도 진동원으로부터 일정한 거리를 확보하면 공해진동으로부터 벗어날 수 있었으며, 공구를 설치할 경우 공구의 깊이, 위치 등의 변화에 따라 진동 저감 효과가 크게 나타났다. 대전도심지역 철도진동 실태분석 및 실험 연구결과를 종합해 볼 때 거리감쇠효과를 적용하거나 공구를 설치함으로써 철도진동으로 인한 피해를 상당히 저감시킬 수 있는 것으로 밝혀졌다. 따라서 철도에 근접한 많은 주택 및 주민의 진동 피해를 저감시키기 위하여 도심지역에서 철도와 나란하게 공구를 설치하고 이를 효율적으로 이용하는 방안이 적극 검토되어야 할 것으로 판단된다.

IF 대역 신호처리 시스템 응용을 위한 13비트 100MS/s 0.70㎟ 45nm CMOS ADC (A 13b 100MS/s 0.70㎟ 45nm CMOS ADC for IF-Domain Signal Processing Systems)

  • 본 논문에서는 IF 대역의 고속 신호처리 시스템 응용을 위해 높은 동적성능을 가지는 13비트 100MS/s ADC를 제안한다. 제안하는 ADC는 45nm CMOS 공정에서 동작 사양을 최적화하기 위해 4단 파이프라인 구조를 기반으로 하며, 광대역 고속 샘플링 입력단을 가진 SHA 회로는 샘플링 주파수를 상회하는 높은 주파수의 입력신호를 적절히 처리한다. 입력단 SHA 및 MDAC 증폭기는 요구되는 DC 이득 및 넓은 신호범위를 얻기 위해 이득-부스팅 회로 기반의 2단 증폭기 구조를 가지며, 바이어스 회로 및 증폭기에 사용되는 소자는 부정합을 최소화하기 위해 동일한 크기의 단위 소자를 반복적으로 사용하여 설계하였다. 한편, 온-칩 기준전류 및 전압회로에는 배치설계 상에서 별도의 아날로그 전원전압을 사용하여 고속 동작 시 인접 회로 블록에서 발생하는 잡음 및 간섭에 의한 성능저하를 줄였다. 또한, 미세공정상의 잠재적인 불완전성에 의한 성능저하를 완화하기 위해 다양한 아날로그 배치설계 기법을 적용하였으며, 전체 ADC 칩은 $0.70mm^2$의 면적을 차지한다. 시제품 ADC는 45nm CMOS 공정으로 제작되었으며, 측정된 DNL 및 INL은 각각 최대 0.77LSB, 1.57LSB의 값을 가지며, 동적성능은 100MS/s 동작 속도에서 각각 최대 64.2dB의 SNDR과 78.4dB의 SFDR을 보여준다. 본 시제품 ADC는 $2.0V_{PP}$의 넓은 입력신호범위를 처리하는 동시에 IF 대역에서 높은 동적성능을 확보하기 위해 사용공정상의 최소 채널 길이가 아닌 긴 채널 기반의 소자를 사용하며, 2.5V의 아날로그 전압, 2.5V 및 1.1V 두 종류의 디지털 전원전압을 사용하는 조건에서 총 425.0mW의 전력을 소모한다.

격자의 초점거리가 디지털 복부 방사선검사의 환자선량 및 노출지수 그리고 영상 품질에 미치는 영향 (The Effect of Grid Focus Distance on Patient Dose, Exposure Index, and Image Quality in Digital Abdominal Radiography)

  • 본 연구의 목적은 일반 방사선검사 시 사용되는 격자의 초점거리 차이가 노출지수와 영상의 품질에 미치는 영향을 알아보고, 임상에서 방사선검사를 위해 격자 적용 시 유용한 정보를 제공하는 데 있다. AEC 적용 후 SID 110 cm로 설정하고, 동일한 노출 조건에서 격자의 초점거리(110 cm, 140 cm, 180 cm) 별로 격자를 변경하여 각 30개의 영상을 획득하였다. 선량은 면적선량과 입사표면선량을 측정하였고, 영상 품질은 신호 대 잡음비와 대조도 대 잡음비로 평가하였다. 노출지수(EI)는 영상에서 나타난 수치를 이용했다. 획득한 자료는 정규성검정 후 일원배치분산분석(사후분석 Duncan)을 통해 평균비교분석을 실시하였으며, Pearson 상관분석을 통해 독립변수와 종속변수의 상관성을 분석하였다. 초점거리 180, 140, 110 cm의 DAP 평균값은 10.944±0.613, 10.687±0.516, 9.74±0.588 cGy×cm2이었고, ESD는 1041.75±57.92, 1019.99±49.61, 930.86±55.77 μGy, EI는 205.97±11.77, 210.59±10.37, 193.8±11.86 이었으며, SNR은 28.48±0.62, 28.41±0.64, 27.13±0.72 dB, CNR은 0.09859±0.004276, 0.09864±0.004378, 0.09026±0.004783 dB이었다. 각 평균값의 차이는 통계적으로 유의한 차이가 있었다(p<0.01). 초점거리 110 cm 보다 유의미하게 초점거리 140 cm와 180 cm에서 높게 나타났으나 초점거리 140 cm와 180 cm의 차이는 없는 것으로 나타났다. 상관관계 분석 결과 FD와 DAP(r=-0.642, p<0.01), ESD(r=-0.629, p<0.01), EI(r=-0.376, p<0.01), SNR(r=-0.615, p<0.01), CNR(r=-0.575, p<0.01)의 모든 변수에서 유의미한 음의 상관관계가 나타났다. 본 연구의 결과 격자의 초점거리는 신호 대 잡음비와 대조도 대 잡음비, 면적선량과 입사표면선량과는 중간정도 음의 상관관계가 있었고, 노출지수와는 약한 음의 상관관계가 있는 것으로 나타났다. 그러므로 방사선사는 자동노출조절장치를 이용한 방사선검사 시 동일한 조사조건을 적용해도 격자의 초점거리에 따라 노출지수 및 선량, 영상의 품질에 변화가 발생할 수 있음을 인지하고, 목표 노출지수 설정 시 유의할 필요가 있다고 사료 된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
