Public Acceptance of a Health Information Exchange in Korea

  • Objectives: We assessed the public acceptance of a health information exchange (HIE) and examined factors that influenced the acceptance and associations among constructs of the Technology Acceptance Model (TAM). Methods: We collected data from a survey of 1,000 individuals in Korea, which was administered through a structured questionnaire. We assessed the validity and reliability of the survey instrument with exploratory factor analysis and Cronbach's alpha coefficients. We computed descriptive statistics to assess the acceptance and performed regression analyses with a structural equation model to estimate the magnitude and significance of influences among constructs of TAM. Results: Eighty-seven percent of the respondents were willing to use the technology, and the average level of agreement with the need for the technology was 4.16 on a 5-point Likert scale. The perception of ease of use of the technology significantly influenced perceptions of usefulness and attitudes about the need for HIE. Perceptions of usefulness influenced attitude and behavioral intention to use HIE, and attitude influenced intention. Age showed a wide range of influences throughout the model, and experience with offline-based information exchange and health status also showed noteworthy influences. Conclusions: The public acceptance of HIE was high, and influences posited by TAM were mostly confirmed by the study results. The study findings indicated a need for an education and communication strategy tailored by population age, health status, and prior experience with offline-based exchange to gain public buy-in for a successful introduction of the technology.

Perceptions regarding smartphone based self-care in patients with chronic heart failure : A focus group interview (만성 심부전 환자의 스마트폰을 활용한 자가관리에 대한 인식 : 포커스 그룹 인터뷰 적용)

  • This study is a qualitative study using focus group interview to explore the perception regarding smartphone use for adhering to self-care in chronic heart failure patients. Eleven patients who were diagnosed with heart failure were included. Three focus groups were held with heart failure patients at an outpatient cardiac care center. The focus group discussions were transcribed verbatim and analyzed using the method of constant comparative analysis. The 12 sub-themes were categorized into 'perception of smartphone use', 'Barriers to smartphone use', and 'Facilitators to smartphone use'. We found that the need to design tailored the messages or health information based on individual preferences. Our findings will be used to develop or design a smartphone based self-care program for patients with heart failure.

Structural Influence of SNS Social Capital on SNS Health Information Utilization Level (SNS의 사회적자본이 건강정보 활용수준에 미치는 구조적 영향력)

  • Objectives : The purpose of this study was to test fitness of the structured model of SNS activities for health information. Methods : A structured questionnaire were administered to 500 subjects. A structural equation model was applied to collected data. Results : The response rate was 73.9%. The respondents mostly used Facebook and KakaoStory. They spent 70 minutes per day and 21~30% of this usage was taken by health information. In the variances, those who has religion more actively exchanged information about diseases and medical institutions. The goodness-of-fit of the model was .81(GFI) and .90(CFI). The main path was bridging capital -> bonding capital -> credibility -> SNS activities for health information. The path from quality of sharing information to SNS activities was not significant. It could be explained by the restriction of digital literacy. Conclusions : SNS activities for health information were determined by credibility, currency and bonding social capital. Bridging social capital, indirectly, influenced SNS activities through bonding social capital. Thus building bonding social capital would be a critical success factor for SNS.

  • This study aims to develop a mobile application for the care of panic disorder, assisting in the prevention of panic attacks and the management of symptoms. Panic disorder is a psychological condition characterized by persistent fear, anxiety, and panic attacks, which requires appropriate treatment and management. The increasing need for digital therapeutics that users can access without time and space constraints is evident. The application developed through this research provides basic knowledge about panic disorder, enables self-diagnosis to monitor one's condition, and offers various therapeutic methods to effectively aid in the care of panic disorder. Additionally, it provides notifications based on real-time user location when they reach places where they have previously experienced panic attacks, and offers personalized support. This digital therapeutic aims to introduce new tools and treatment methods beyond traditional therapies. Furthermore, it is expected to contribute positively by promoting the use of digital therapeutics not only for panic disorder but also across the entire field of mental health, thereby improving users' quality of life.

Analysis of Factors that will Ensure Effective Health Care Delivery System (효율적인 의료전달체계 확보를 위한 요인 분석)

  • In order to analyze factors that support effective health care delivery system, this study aimed to research general public's perception on the functions of medical institutions(based on the levels of treatment: primary care, secondary care, and tertiary care), choices of medical institution when contracted with an unfamiliar disease, and recognition of diseases based on their severities. We collected data using self-administered survey from 400 general public living in Seoul, S. Korea from April 25 to May 18, 2011. The analysis was conducted using frequency analysis, chi-square test, and t-test; we analyzed the data to see if there are differences based on gender, age, and level of education. The result of both recognition of functional differences of medical institutions and selection of medical institutions when contracted with unfamiliar diseases showed that there were no significant differences based on the gender; however there were significant differences when considering the age and education. Looking at the result of the knowledge of the disease classification based on its severity, there were significant differences in age, gender, and education. In order to provide sustainable and effective health care delivery system, utilization of primary care as well as education and promotion regarding the functional differences of medical institutions and classification of disease based on its severity need to be encouraged.

A study on current situation of self-help group program in community-based home visiting health service (보건소 방문건강관리사업의 자조집단 운영 현황)

  • The purpose of this study was to examine and analysis self-help group program performed in community-based home visiting health service. The data was collected public servants from the Home Visiting Health Care Service of 253 public health centers all over the country and was analyzed by SPSS/Win 17.0 program. 43.1% of self-help program was performed and 56.9% was not from year 2007 to 2010. As the years passed, the number of self-help group program was getting increased. The action for activating self-help programs was voluntary participation of clients, enthusiasm of management practitioner, and encouragement about self-help group. Self-help group programs based on public health center are insufficient. Self-help programs should be activated for clients of home visiting health care service. A systematic operating model of self-help group is also needed to develop and apply in consideration of these findings.

Designed OTP Generation Method Using Health Information (건강정보를 이용한 OTP 생성 방식 설계)

  • User Authentication in Online service is essential for accurate and safe service. For this user authentication, One Time Password(OTP) is frequently used. To satisfy one-time-use characteristic of OTP, Offset information to generate OTP or final OTP value get generated through OTP generator or security card which could be lost. In this study, OTP generation method that bypasses OTP generator or security card by using health information collected from u-Health care system is proposed. Suggestion is that health information collected through wearable devices get utilized to offset information that are applied in OTP generations. OTP generated using suggested methods showed similar results than current OTP generation methods in the collision resistance test which tests how often it generate same authentication numbers, this implies that new proposed method can be applied to various on-line services.

Patients' Perception of Quality of Health Service and Utilization considering convergence factors: A Survey of Elderly Patients of Public Health Centers, Seoul, Republic of Korea (융합적 요소를 고려한 의료서비스 질 인식도와 의료이용의 관련성: 서울시 보건소를 이용하는 노인을 대상으로)

  • Health service quality is increasingly emphasized, and primary care service is no exception in this. This study identified patients' perception of quality considering convergence factors associated with increased utilization among patients over the age of 65 using community health centers. A total of 307 respondents were included in the analysis. The results showed that positive perceptions on facility and environment, patient-centered care, technical knowledge and skills of physicians, and emotional support with nurses and non-medical workers were statistically and significantly associated with higher utilization. In order to increase utilization of primary care service at community health centers, they need to manage and improve these factors.

A Review on End-of-life Care System between South Korea and the United States (한국과 미국의 생애말기케어 시스템 비교 연구)

  • This study aims to examine eligibilities, services and delivery of services for the current end-of-life care and analyze the quality control of services for end-of-life care. We analyzed the literature and laws on end-of-life systems in Korean and the United States. Current end-of-life care, hospice and palliative care in Korea is being provided mainly in hospital setting. Quality control for the services focuses on setting the criteria for structures in hospitals (i.e. staffing, facilities and equipment). Whereas American end-of-life care system has much broader eligibility for service beneficiaries and provides care mostly at home. Also quality control for services includes process (delivering service) and outcomes, such as monitoring performance indicators and consumer's satisfaction. This is linked to annual payment. The comparative analysis findings contributed to give the next direction of current Korean end-of-life care system. It is nessary to establish the better and extensive end-of-life care system in Korea in considering other countries' end-of-life care systems based on more future research.

  • 본 논문에서는 디지털헬스케어의 정의와 핵심기술, 그리고 정보보호 방안을 제안하고자 한다. 디지털 헬스케어 기기·서비스를 대상으로 하여 다양한 취약점에 대하여 사이버공격이 가능하다. 본제안의 경우 인프라공격에 대한 대응뿐만아니라 관리적인 정보보호체계수립을 통하여 보다 효과적이며, 지속적으로 대응할 수 있을 것으로 전망된다.
