『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 대황황련사심탕(大黃黃蓮瀉心湯)을 투여한 칸디다성 질염 치험 1례 (A Case Report of VulvoVaginal Candidiasis treated by Dahuanghuanglianxiexin-tang based on Shanghanlun provisions)

  • Objective : This case report show the effect of Dahuanghuanglianxiexin-tang on VulvoVaginal Candidiasis. Methods : According to disease pattern identification diagnostic system based on shanghanlun provisions, The author diagnosed Taeyang-byung kyeolhyung. The symptoms of VulvoVaginal Candidiasis were worsen when the patient had long sitting position and digestive problems. Therefore the patient treated with Dahuanghuanglianxiexin-tang for 3 months. The severity of Candida Vaginitis was evaluated by patient's subjective statements(Vaginal pruritus, Vaginal secretion, Burning sensation). Results : After the treatment, Level of syptoms decreased from Severe to Trace and Eliminated. Conclusions : Dahuanghuanglianxiexin-tang improved the symptoms of VulvoVaginal Candidiasis on this study. Although Dahuanghuanglianxiexin-tang is not an usual treatment for VulvoVaginal Candidiasis, the author could prescribe the medicine due to shanghanlun six meridian patterns diagnostic system.

허약아클리닉에 내원한 환아들에 대한 임상적 연구 (Clinical Study on Weak Children)

  • The frequency of weak children, derived from constitutional and functional factors, has increased recently, but their symptoms have never been observed and analysed. A survey was conducted to evaluate and understand the characteristics of weak children. 311 children who had visited in the weak child clinic of Kyunghee Oriental Medical Hospital from January, 1999 to August, 1999, were the respondents of the Weak Child Questionnaire. They were divided into 4 groups by age and birth weight, and statistical significances were tested by one way analysis of variances and bivariate correlation analysis among groups. The results obtained were as follows: 1. Distribution of sex : male 191 cases, female 120 cases 2. The distribution of age showed the highest in $2{\sim}6$ years(45.3%), followed by $7{\sim}12$ years(31.8%), $0{\sim}1$ years(18.0%), over 13 years(4.8%) in order. There was a statistical difference of all symptoms' mean, except respiratory symptoms, among groups(p<0.01). 3. The distribution of birth weight showed the highest in $2.5{\sim}3.3kg(43.4%)$, followed by $3.3{\sim}4.1kg(39.5%)$, under 2.5kg(5.1%), over 4.lkg(2.1%) in order. There was a statistical difference of digestive symptoms' and total score mean among groups(p<0.01, p<0.05 respectively). 4. The correlation coefficient between general symptoms' mean and other symptoms´ is the highest(p<0.01). From the above results, it was suggested that general symptoms are representative of weak children's characteristics.

일부 치위생과 학생의 건강관련 생활습관과 자각증상 (Health-related lifestyle and self-reported symptoms in dental hygiene students)

  • Objectives : The study investigated and analyzed the relationship between health-related lifestyle and psychosomatic self-reported symptom in dental hygiene students. Methods : A self-reported questionnaire was filled out by 478 dental hygiene students in Gyeonggi-do and Gangwon-do from March 7 to June 21, 2012 by random sampling method after informed consent. Results : The health-related lifestyle showed the results as follows. Nonsmokers accounted for 89.1%. Those who never exercise accounted for 67.9% and 37.9% of the students sleep for 5-6 hours. Those who take alcohols twice per month accounted for 58.2%. The correlation between the symptoms and lifestyle included multiple subjective symptoms(I), respiratory(A), eyes and skin(B) and digestive organs(C) symptoms and smoking status(p<.01), mouth and anal(D), depression(K), nervousness(E). There existed the correlation in average sleeping time(p<.01) and impulsivess(H) and smoking status(p<.05) and lie scale(L) and regular exercise(p<.05) and aggressiveness(F) and drinking habits and irregular and life(G) and breakfast habits. The smoking habit, sleeping time, and snack intake had an influence on psychosomatic self-reported oral health-related symptoms. Conclusions : Cessation of smoking, adequate sleeping time, and reduction of snack intake can improve the oral health-related lifestyle and reduce the self-reported symptoms in the dental hygiene students.

동의보감(東醫寶鑑)을 통한 식적(食積)에 대한 고찰 (A study on Sik-jeok in Dong-ui-bo-gam)

  • 1. The concept about Jeok-chwiui(積聚) has been around since before "Hwangje-Naegyeong". Since "Hwangje-Naegyeong(黃帝內經)", Sik-jeok(食積) was made mention of specifically. In "Yu-gyeong(類經)", it is said that Sik-jeok is a combination of our body fluid and blood by bad eating and sleeping habits. 2. In the narrow sense Sik-jeok is indigestion and broadly it is inappropriately stagnant fluids in our body. 3. If studying on Sik-jeok in Dong-ui-bo-gam 1) It is located on the right side, in the epigastric region and between the skin and fascia. 2) The cause of Sik-jeok is indigestion, inappropriate temperature and weak stomach. 3) Symptoms of Sik-jeok are very diverse such as sick ascension, nausea, abdominal pain, headache, fever, etc. The right pulse is big and stressful. 4) Various symptoms related to digestive, respiratory, circulatory and reproductive system are represented by Sik-jeok. - Contemporarily women uterine or ovarian disease and back pain are mostly caused by Sik-jeok 5) Pediatric disease are mostly caused by Sik-jeok. 6) Treatment of Sik-jeok is light eating and if it is serious, you have to induce vomiting or diarrhea. Commonly used drugs are digestive medicine and invigorative medicine 7) To prevent Sik-jeok, you should forbid to eat until you are satisfied and wear warm clothes and continue to do spleen and genital do-in-beop.

소아설사(小兒泄瀉)에 대한 가감전씨백출산(加減錢氏白朮散)의 임상효능(臨床效能)에 관한 연구(硏究) (The clinical effect of Gagamjeonsibaekchul-san on child diarrhea)

  • Objectives: The objective of this study is to assess the clinical effect of Gagamjeonsibaekchul-san on child diarrhea. Methods: This clinical study was carried out with 42 cases(31male, 11female) of children aged 0 to 14 years old who visited the Department of Pediatrics, Dongguk University Bundang Oriental Hospital and were diagnosed as the diarrhea. Assessments were made with reference to sex, age, the duration of symptoms and physical history. To assess the impact of Gagamjeonsibaekchul-san, the complicated signs, the duration of treatment, the correlation between the use of Gagamjeonsibaekchul- san, san and the duration of symptom were examined before and after the treatment. Results: There were 31 males and 11 females in the incidence by sex. It showed that males were more than females. The most numerous age group was 0-2(19) followed by 3-7 (11) and 9-12 (12). According to the duration of symptom, 12 children have symptom for 0-7 days, 10 for 8-14 days, 11 for 15-20 days, 4 for 22-28 days and 5 over 28 days. With regard to the physical history of the sample, 5 children had atopic dermatitis, 3 children had enteritis, 3 children had rhinitis, 2 children had tonsillitis, 1 child had otitis media, 1 child had asthma and 1 child had pneumonia. The children also had the complicated signs: 22 children had anorexia, 17 children had abdominal pain, 13 children had vomiting and 8 children had nausea in digestive organ. In addition 11 children had dizziness, 7 children had general weakness, 1 child had enuresis and 1 child had sweat in general symptoms. For the duration of the treatment, 14 children were treated for 7 days, 13 for 8-10 days, 8 for 11-14 days, 3 for 15-21 days, 2 for 22-28 days and 2 for 29-40 days. No distinct relationship between the use of Gagamjeonsibaekchul-san and the duration of the symptom was found. Conclusion: Gagamjeonsibaekchul-san is effective in the treatment of intestine mucous membrane and improves the function of digestive system. It is also an effective short-term treatment for child diarrhea.

두드러기와 두통-현훈(頭痛-眩暈)을 주소(主訴)로 하는 담음형(痰飮型) 환자의 치험례 (Clinical Case of the Phlegm-retention Type Patient with Urticaria, Headache-Vertigo Symptoms)

  • Phlegm-retention disease, which is called dameum(痰飮), is a kind of a pathologic secretion products, accumulated excessive water in a special part of a body. Urticaria is a common dermatopathy, coming under “eun-jin(隱疹)” in Korean herbal medicine. Headache and vertigo is a very common diseases in life. There are so many causes which induce uticaria, headache and vertigo. We experienced 31-year old woman case suffering from chronic urticaria, headache, vertigo with digestive disorder, nausea. We diagnosed those symptoms as dameum(痰飮). So we administered Jeongjeongamiyijintang-gamibang(正傳加味二陳湯加味方) to the urticaria, and Banhabaekchulcheonma-tang (半夏白朮天麻湯) to the headache, vertigo. They are both famous herbal medicines for treating dameum-related symptoms, because both of them contains Yijin-tang(二陳湯) which is the main Herbal medicine for dameum(痰飮). We suggest that dameun-related urticaria, headache, vertigo can be treated by preceding medicines.

Usage Patterns and Severity Classification of Elderly Patients in a Public Hospital Emergency Department

  • This study aims to enhance the accuracy of severity classification by examining the usage patterns and characteristics of emergency department visits. It focuses on adult and elderly patients who visited a public hospital in Seoul. This descriptive study retrospectively reviewed the electronic medical records of patients who visited the emergency department of a public hospital between November and December 2023. The total number of participants was 1,033, with 46.4% (n=479) being elderly and 53.6% (n=554) being adults. The chief complaints of the participants were as follows: for the elderly, nervous system symptoms at 8.2% (n=85) and digestive symptoms at 7.5% (n=77) were the most common, while for adults, gastrointestinal symptoms at 11.0% (n=114) and trauma at 8.6% (n=89) were more prevalent. In the case of the elderly, patients classified as urgent accounted for the highest percentage at 23.9% (n=247), while for adults, non-emergency were more prevalent at 32.2% (n=333). The initial severity classification error rate for elderly patients in the urgent was 3.8%, indicating that the suitability of KTAS for elderly patients with high severity was low. To minimize severity classification errors and enhance KTAS accuracy, it's essential to address its current limitation of only classifying adults and children separately by developing a KTAS classification system that reflects the diverse characteristics of elderly patients.

부인과 수술 후 한양방 협진 치료의 효과 연구 (Effect on Oriental-Western Medicine Intergrative Care after Gynecology Surgery)

  • Purpose: The purpose of this study is to analyze the effect of oriental-western medicine integrative care after gynecology surgery by DSOM((Diagnosis System of Oriental Medicine) and improvement of major symptoms. Methods: 66 patients belonged to the experimental group and 47 patients belonged to the control group out of 113 patients taken gynecology surgery. We studied general characteristics and difference on DSOM results between the two groups. And we had patients answer questionnaires about major symptoms such as abdominal discomfort, headache, fatigue, urinary discomfort, constipation, lumbago, edema, tingling, digestive disorder, cough, pruritus, arthralgia, cold feeling by VAS (Visual Analogue Scale) in order to analyze improvement and difference between two groups. Results: Experimental group had significantly high score on deficiency of blood, blood stasis, dryness, spleen, kidney, phlegm and high frequency on deficiency of blood, dampness, kidney, phlegm pathogenesis in DSOM. It had significantly high intensity in symptoms of abdominal discomfort, headache, fatigue, urinary discomfort, constipation, lumbago at the first time, and headache at 1 month after western hospital discharge out of symptoms associated with surgery. And it had significantly high improvement in symptoms of abdominal discomfort, headache, urinary discomfort, constipation at oriental-western integrative hospital discharge, and headache, fatigue, urinary discomfort, constipation, lumbago at 1 month after western hospital discharge out of symptoms associated with surgery. Conclusion: We recognized that oriental-western medicine integrative care after gynecology surgery has significant effect.

치위생과 신입생의 신체·정신건강과 대학 생활 스트레스와의 관계 (The relationship between physical·mental health and life stress in dental hygiene freshman)

  • Objectives : The purpose of this study was to investigate the relationship between psychosocial stress and depression symptoms in dental hygiene freshman. Methods : A self-reported questionnaire of 45 questions was filled out by 207 dental hygiene students in Gyeongnam from December 9 to 11, 2013. Data were analyzed using SPSS WIN 18.0 program for descriptive statistics, t-test, ANOVA. Results : Religion tended to influence on positive effects on academic stress because those having religion had less academic stress. Economic condition also influenced on physical and mental stress in the students($8.52{\pm}5.63$)(p=0.000). Low economic status influenced on academic problems(p=0.022), family problems(p=0.014), rational relationship problems(p=0.002), and interpersonal problems(p=0.007). There was no significant difference between future problems(p=0.125) and self-efficacy(p=0.076). The physical symptoms included digestive disorders($16.73{\pm}5.37$), cardiac symptoms($14.61{\pm}5.51$), headache($9.46{\pm}3.59$), insomnia($8.32{\pm}3.67$) (p=0.000). The mental symptoms included negative emotions($15.07{\pm}3.94$)(p=0.000). Conclusions : In order to reduce the psychosocial stress symptoms in dental hygiene freshman, it is important to develop the stress debriefing program in the future.
