급성기 해마 질환의 자기공명영상 소견: 임상화보 (MRI Findings of Acute Hippocampal Disorders: Pictorial Essay)

  • 해마는 뇌에서 가장 대사가 활발한 부위 중 하나이다. 그러므로 해마는 다양한 급성기 질환으로부터 영향을 받을 수 있다. 이 연구의 목적은 해마와 관련된 다양한 질환들을 소개 및 분류하며 특히 확산강조영상을 중심으로 자기공명영상 소견에 대해 설명하는 것이다. 급성기 해마 질환은 감염, 염증, 대사성, 허혈성, 외상성 그리고 기타 총 6가지로 분류하였다. 환자들은 임상적 소견 그리고 확산강조영상 중심의 자기공명영상을 토대로 후향적으로 검토되었다. 모든 질환들은 임상적 또는 병리학적으로 진단되었다. 급성기 해마 질환들은 임상 양상이 겹치는 경우가 많다. 그러므로 감별 진단을 위해서 급성기 해마 질환들을 분류하고 각각의 특징적인 영상 소견을 이해하는 것이 중요하다.

Studies on the Ability to Detect Lesions According to the Changes in the MR Diffusion Weighted Images

  • This study evaluated the ability of Diffusion-Weight Image (DWI), which is one of pulse sequences used in MRI based on the T2 weighted images, to detect samples placed within phantoms according to their size. Two identically sized phantoms, which could be inserted into the breast coil bilaterally, were prepared. Five samples with different sizes were placed in the phantoms, and the T2 weighted images and DWI were obtained. The Breast 2 channel coil of SIEMENS MAGNETOM Avanto 1.5 Tesla equipment was used for the experiments. 2D T2 weighted images were obtained using the following parameters: TR/TE = 6700/74 msec, Thickness/gap = 5/1 mm, Inversion Time (TI) = 130 ms, and matrix = $224{\times}448$. The parameters of DWI were that TR/TE = 8100/90 msec, Thickness/gap = 5/1 mm, matrix = $128{\times}128$, Inversion Time = 185 ms, and b-value = 0, 100, 300, 600, 1000 s/mm. The ratio of the sample volume on DWI compared to the T2 weighted images, which show excellent ability to detect lesions on MR images, was presented as the mean b-value. The measured b-value of the samples was obtained: 0.5${\times}$0.5 cm=0.33/0.34 square ${\times}$ cm (103%), 1${\times}$1 cm=1.28/1.25 square ${\times}$ cm (102.4%), 1.5${\times}$1.5 cm = 2.28/2.67 square ${\times}$ cm (85.39%), 2${\times}$2 cm=3.56/4.08 square ${\times}$ cm (87.25%), and 2.5${\times}$2.5 cm=7.53/8.77 square ${\times}$ cm (85.86%). In conclusion, the detection ability by the size of a sample was measured to be over 85% compared to T2 weighted image, but the detection ability of DWI was relatively lower than that of T2 weighted image.

  • A 59-year-old patient with a history of hepatocellular carcinoma presented with decreased consciousness and left hemiparesis. A rim-enhanced mass lesion without diffusion restriction was observed in contrast-enhanced MRI including diffusion-weighted imaging. Based on these findings, metastatic brain tumor was suspected. However, brain abscess (BA) was diagnosed after multiple bacterial colonies were observed in aspiration biopsy. Initial conventional antibiotic treatment including vancomycin had failed, so linezolid was used as second-line therapy. As a result, infection signs and clinical symptoms were resolved. We report a case with atypical imaging features and antibiotic susceptibility of a BA in an immunocompromised patient undergoing chemotherapy.

  • 항문회음부 결핵은 드문 폐외결핵의 형태로 때때로 농양을 형성할 수도 있다. 저자들은 회음부에서 촉지되는 종괴로 시행한 자기공명영상에서 T1강조 자기공명영상에서 저신호 강도, T2강조 자기공명영상에서 고신호 강도 그리고 확산강조영상에서 확산 제한을 보인 항문회음부 결핵성 농양 1예를 경험하였기에 이를 보고하고자 한다.

  • Metronidazole is an antimicrobial agent widely used for the treatment of anaerobic infection or antibiotics-associated diarrhea. It is generally thought to be safe, but can induce reversible toxic encephalopathy in the case of excessive or cumulative over-dose. Metronidazole-induced encephalopathy generally demonstrates the characteristic features of typical lesion location and bilaterality on magnetic resonance imaging (MRI). We report a case of metronidazole-induced encephalopathy with the involvement of asymmetric white matter. To our knowledge, only a few cases have been reported with respect to white matter lesion characteristics on MRI with diffusion-weighted images.

  • 전립선영상 판독과 자료체계 버전 2.1에서는 다중 매개 자기공명영상(multiparametric MRI; 이하 mpMRI)을 사용하는 버전 2의 기술적인 변수와 영상 판독 기준이 개정되었다. 이러한 변화를 통해 전립선암 평가의 발전이 예상지만, 어떤 사항들은 아직까지 해결되지 않았고 새로운 문제점들이 부각되고 있다. 본 종설에서는 전립선영상 판독과 자료체계 2.1 버전의 간단한 개요와 새롭게 부상하는 다음과 같은 문제들에 대해 비판적인 관점에서 논의하고자 한다: mpMRI의 보다 자세한 프로토콜에 대한 필요, 개정된 이행부 판독기준에 대한 검증 부족, 개정된 확산강조영상 및 조영 증강 영상 판독기준, anterior fibromuscular stroma, 중심부 평가, 주변부 신호 및 종양 공격성, 구조화된 판독문 변화에 대한 명료화의 필요, 영상 품질과 수행능력 제어에 대한 필요 및 기타 적응증을 포함하도록 시스템 확장을 위한 적응증.

Functional Magnetic Resonance Imaging in the Diagnosis of Locally Recurrent Prostate Cancer: Are All Pulse Sequences Helpful?

  • Objective: To perform a meta-analysis to quantitatively assess functional magnetic resonance imaging (MRI) in the diagnosis of locally recurrent prostate cancer. Materials and Methods: A comprehensive search of the PubMed, Embase, Cochrane Central Register of Controlled Trials, and Cochrane Database of Systematic Reviews was conducted from January 1, 1995 to December 31, 2016. Diagnostic accuracy was quantitatively pooled for all studies by using hierarchical logistic regression modeling, including bivariate modeling and hierarchical summary receiver operating characteristic (HSROC) curves (AUCs). The Z test was used to determine whether adding functional MRI to T2-weighted imaging (T2WI) results in significantly increased diagnostic sensitivity and specificity. Results: Meta-analysis of 13 studies involving 826 patients who underwent radical prostatectomy showed a pooled sensitivity and specificity of 91%, and the AUC was 0.96. Meta-analysis of 7 studies involving 329 patients who underwent radiotherapy showed a pooled sensitivity of 80% and specificity of 81%, and the AUC was 0.88. Meta-analysis of 11 studies reporting 1669 sextant biopsies from patients who underwent radiotherapy showed a pooled sensitivity of 54% and specificity of 91%, and the AUC was 0.85. Sensitivity after radiotherapy was significantly higher when diffusion-weighted MRI data were combined with T2WI than when only T2WI results were used. This was true when meta-analysis was performed on a per-patient basis (p = 0.027) or per sextant biopsy (p = 0.046). A similar result was found when $^1H$-magnetic resonance spectroscopy ($^1H$-MRS) data were combined with T2WI and sextant biopsy was the unit of analysis (p = 0.036). Conclusion: Functional MRI data may not strengthen the ability of T2WI to detect locally recurrent prostate cancer in patients who have undergone radical prostatectomy. By contrast, diffusion-weight MRI and $^1H$-MRS data may improve the sensitivity of T2WI for patients who have undergone radiotherapy.

Texture Analysis of Three-Dimensional MRI Images May Differentiate Borderline and Malignant Epithelial Ovarian Tumors

  • Objective: To explore the value of magnetic resonance imaging (MRI)-based whole tumor texture analysis in differentiating borderline epithelial ovarian tumors (BEOTs) from FIGO stage I/II malignant epithelial ovarian tumors (MEOTs). Materials and Methods: A total of 88 patients with histopathologically confirmed ovarian epithelial tumors after surgical resection, including 30 BEOT and 58 MEOT patients, were divided into a training group (n = 62) and a test group (n = 26). The clinical and conventional MRI features were retrospectively reviewed. The texture features of tumors, based on T2-weighted imaging, diffusion-weighted imaging, and contrast-enhanced T1-weighted imaging, were extracted using MaZda software and the three top weighted texture features were selected by using the Random Forest algorithm. A non-texture logistic regression model in the training group was built to include those clinical and conventional MRI variables with p value < 0.10. Subsequently, a combined model integrating non-texture information and texture features was built for the training group. The model, evaluated using patients in the training group, was then applied to patients in the test group. Finally, receiver operating characteristic (ROC) curves were used to assess the diagnostic performance of the models. Results: The combined model showed superior performance in categorizing BEOTs and MEOTs (sensitivity, 92.5%; specificity, 86.4%; accuracy, 90.3%; area under the ROC curve [AUC], 0.962) than the non-texture model (sensitivity, 78.3%; specificity, 84.6%; accuracy, 82.3%; AUC, 0.818). The AUCs were statistically different (p value = 0.038). In the test group, the AUCs, sensitivity, specificity, and accuracy were 0.840, 73.3%, 90.1%, and 80.8% when the non-texture model was used and 0.896, 75.0%, 94.0%, and 88.5% when the combined model was used. Conclusion: MRI-based texture features combined with clinical and conventional MRI features may assist in differentitating between BEOT and FIGO stage I/II MEOT patients.

Feasibility of Simultaneous Multislice Acceleration Technique in Diffusion-Weighted Magnetic Resonance Imaging of the Rectum

  • Objective: To assess the feasibility of simultaneous multislice-accelerated diffusion-weighted imaging (SMS-DWI) of the rectum in comparison with conventional DWI (C-DWI) in rectal cancer patients. Materials and Methods: This study included 65 patients with initially-diagnosed rectal cancer. All patients underwent C-DWI and SMS-DWI with acceleration factors of 2 and 3 (SMS2-DWI and SMS3-DWI, respectively) using a 3T scanner. Acquisition times of the three DWI sequences were measured. Image quality in the three DWI sequences was reviewed by two independent radiologists using a 4-point Likert scale and subsequently compared using the Friedman test. Apparent diffusion coefficient (ADC) values for rectal cancer and the normal rectal wall were compared among the three sequences using repeated measures analysis of variance. Results: Acquisition times using C-DWI, SMS2-DWI, and SMS3-DWI were 173 seconds, 107 seconds, (38.2% shorter than C-DWI), and 77 seconds (55.5% shorter than C-DWI), respectively. For all image quality parameters other than distortion (margin sharpness, artifact, lesion conspicuity, and overall image quality), C-DWI and SMS2-DWI yielded better results than did SMS3-DWI (Ps < 0.001), with no significant differences observed between C-DWI and SMS2-DWI (Ps ≥ 0.054). ADC values of rectal cancer (p = 0.943) and normal rectal wall (p = 0.360) were not significantly different among C-DWI, SMS2-DWI, and SMS3-DWI. Conclusion: SMS-DWI using an acceleration factor of 2 is feasible for rectal MRI resulting in substantial reductions in acquisition time while maintaining diagnostic image quality and similar ADC values to those of C-DWI.

Diffusion-Weighted MR Imaging of Intracerebral Hemorrhage

  • Objective: To document the signal characteristics of intracerebral hemorrhage (ICH) at evolving stages on diffusion-weighted images (DWI) by comparison with conventional MR images. Materials and Methods: In our retrospective study, 38 patients with ICH underwent a set of imaging sequences that included DWI, T1-and T2-weighted imaging, and fluid-attenuated inversion recovery (FLAIR). In 33 and 10 patients, respectively, conventional and echo-planar T2* gradient-echo images were also obtained. According to the time interval between symptom onset and initial MRI, five stages were categorized: hyperacute (n=6); acute (n=7); early subacute (n=7); late subacute (n=10); and chronic (n=8). We investigated the signal intensity and apparent diffusion coefficient (ADC) of ICH and compared the signal intensities of hematomas at DWI and on conventional MR images. Results: DWI showed that hematomas were hyperintense at the hyperacute and late subacute stages, and hypointense at the acute, early subacute and chronic stages. Invariably, focal hypointensity was observed within a hyperacute hematoma. At the hyperacute, acute and early subacute stages, hyperintense rims that corresponded with edema surrounding the hematoma were present. The mean ADC ratio was 0.73 at the hyperacute stage, 0.72 at the acute stage, 0.70 at the early subacute stage, 0.72 at the late subacute stage, and 2.56 at the chronic stage. Conclusion: DWI showed that the signal intensity of an ICH may be related to both its ADC value and the magnetic susceptibility effect. In patients with acute stroke, an understanding of the characteristic features of ICH seen at DWI can be helpful in both the characterization of intracranial hemorrhagic lesions and the differentiation of hemorrhage from ischemia.
