단순 흉부 X-선에서 양측성 폐문 림프절 비후를 보이는 대표적인 질환으로 유육종증, 림프종, 결핵, 폐암, 아밀로이드증, 브루셀라증, 콕시디오이데스진균증(Coccidioidomycosis) 등이 있다. LIP의 방사선학적 소견은 보통 비특징적이고, 초기 병변에서 양측 폐하에 망상, 망상결절성 침윤을 보이며 병이 진행됨에 따라 낭성 변화를 동반하고, 봉소상을 보이는 경우가 있으나, 본 증례의 환자와 같이 종격동과 양측 폐문에 림프절 종대를 동반하는 경우에 대한 보고는 흔치 않다. 저자들은 흉부 X-선 및 CT상 종격동과 양측 폐문부의 림프절 종대를 동반한 간질성 결절성 폐침윤 소견을 나타낸 환자에서 흉강경하 폐 생검을 통해 진단된 LIP를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

저자들은 평소 건강하던 중 기침, 호흡곤란 등으로 내원한 환자에서 흉부 X-선상 양측 폐야에 결절성 폐침윤 소견과 말초혈액상 호산구증가 등을 보여 경기관지 폐생검 및 폐포세척술 상 호산구성 폐렴을 진단하고 그 원인으로 담낭염을 동반한 간흡충증 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

An immunoglobulin G4 (IgG4)-related disease is a recently emerging entity, and a few cases of IgG4-related disease in lung and pleura have been reported. Herein, we report the case of a 74-year-old man with IgG4-related disease of lung and pleura, clinically suspicious of malignant mesothelioma. Chest computed tomography showed diffuse nodular pleural thickening, and microscopic finding disclosed diffuse thickening of visceral pleura with infiltrations of many lymphoplasma cells with increased number of IgG4-positive plasma cells and a few multinucleated giant cells. It is important for pathologists and clinicians to recognize this rare entity and its histologic finding, because it can be confused with malignant tumors on the radiologic examination although it can be treated with steroid therapy.

Varicella is a contagious infection in childhood disease typically affecting children aged 2-8 years and usually follows benign outcome. In the adult, clinical presentation is more severe and more commonly associated with complications. Varicella pneumonia, although rare, is a potentially life-threatening complication that should be suspected in any adult with varicella and respiratory symptoms. We report a case of varicella pneumonia in immunocompetent patient. The characteristic radiographic findings consisted of diffuse scattered coarse nodular infiltrations, less than 1cm sized, with ground glass opacity and consolidation in both lung fields. The patients was started on intravenous acyclovir. The chest radiograph performed 2 weeks later showed complete resolution of the pulmonary lesions.
