음향도구 착용 근로자의 소음노출 실태에 관한 연구 (Research on the Characteristics and Measures of Noise Exposure on Worker Wearing Acoustic Devices)

  • There are hundreds of thousands call center workers wearing acoustic device. However, researches and noise exposure measurements on the noise transmitted from acoustic devices have seldom been performed due to the difficulty of measurement and to the absence of the measuring method in Korea. The aim of this study is to set up management measures to protect hearing loss on the call operator by acquiring measurement data of noise transmitted from the headset. Noise exposure measurements of 17 operators were performed in 7 call centers and head and Torso simulator method in compliance with the ISO standard 11904-2 was used for the measurement of noise transmitted from the headset. Sound pressure levels(SPL) transmitted from the headset were 73.2~86 dB(A). The operator exposed to the highest SPL set up his volume control at 9 which was the highest volume level. The volume control level, adjustable from 1 to 9, could be identified 12 out of 17 operators and the range of volume levels was 4.5~9. As a result of pearson correlation analysis, the correlation between volume level and SPL transmitted from the headset showed high relation as significance at the 0.672 level(p<0.05). To protect hearing loss of call center operators, it is more practical and effective measure to limit the volume level below the noise exposure level, i.e. 85 dB(A), rather than to carry out noise monitoring considering cost-effective aspect.

청각장애인의 의사소통 경험 (A Phenomenological Study on the Communication Experiences of the Deaf)

  • 이 연구는 현상학적 연구방법을 활용하여, 청각장애인의 의사소통 경험을 이해하고자 한 것이다. 연구결과 청각장애인의 의사소통 경험은 '홀로 소리없는 세계에 갇힘', '소통하는 법을 터득함', '농사회구성원으로 정체성을 찾음'인 것으로 분석되었다. 이를 통해 청각장애인의 의사소통 경험을 "소리없는 혼자만의 세계에서 통(通)함을 배우고, 들을 수 있는 사람이 아닌 '잘 볼 수 있는 사람인 농아인'으로서 동병상련(同病相憐)의 정(情))을 나누며 살아감"으로 맥락화하였다. 홀로 소리없는 세계에서 소통을 경험하게 하는 중요한 도구는 수화였으며, 이는 청각장애인들이 세상과 소통을 배우고 이해하게 하는 가장 핵심적인 매개체 역할을 하는 것으로 나타났다. 수화를 통한 청각장애인의 의사소통 경험은 사회로 한발을 내딛을 수 있는 힘이면서, 스스로를 들을 수 없는 사람으로부터 잘 볼 수 있는 사람으로 정체성을 변화시키는 의미를 가지고 있었다. 이러한 결과를 통하여, 청각장애인의 의사소통과 관련된 사회 복지적 함의를 제시하였다.

우리나라 노인의 신체기능 요인과 차별경험의 관련성 연구: 노인실태조사(2014, 2017) 자료 분석 (Relationship between Physical Function Factors and Discrimination Experiences of the Elderly in Korea: The 2014 and 2017 National Survey on Living Conditions and Welfare Needs of Korean Older Persons)

  • Background: Supporting the elderly population is presented as a social issue, and it affects age discrimination, which forms a negative perception and avoids the elderly. Since age discrimination lowers the quality of life of the elderly and hinders social unity, it may be important to research related factors. This study examined the physical function as a factor that influences discrimination experiences and aimed to identify the relationship between physical function factors and discrimination experiences. Methods: In this study, we analyzed 20,225 elderly from the 2014 and 2017 survey of living conditions and welfare needs of Korean older persons, conducted nationwide among older than 65 years. Physical function factors are activities of daily living, instrumental activities of daily living, sight discomfort, hearing discomfort, chewing discomfort, and leg strength as factors of interest. We performed multivariable logistic regression that reflected survey characteristics, adjusting for socio-demographic factors, health related factors, and social-support factors. Results: The number of elderly who faced discrimination experiences was 1,175 (5.8%). The results showed that the odds ratio of facing a discrimination experience in the dependent group was significantly higher compared to the independent group when being transferring out of the room (4.05; 95% confidence interval [CI], 1.50-10.88) and difficulty in hearing (1.25; 95% CI, 1.05-1.49). Even with respect to chewing ability, which was significant in models 1 and 2, they face more discrimination experiences (1.30; 95% CI, 1.11-1.53). Conclusion: These research results indicate that an important strategy for preventing age discrimination is to understand the physical function conditions of the elderly and promote the physical functions of the elderly related to transferring, hearing, and chewing; it is necessary to develop a practical intervention plan that considers these aspects.

간호력(Nursing History)을 통해 본 내과 입원환자의 간호요구 (A Study of the Nursing Needs of Hospitalized medical Patients by Means of Nursing History Form)

  • Personalized Patient centered nursing care is increasingly difficult to achieve despite the fact that it remains one of our consistent goals. So, we must find away to individualized nursing care. One means, to achieve this is by use of the nursing history form, which has been developed to help the nurse make maximum use of her limited time with the patient, by obtaining systematically the information needed to plan his nursing care. The nursing history form can be used to collect data about individual nursing needs but also it lends itself to the collection of epidemiological data relevant to the needs of patient population. So this study was undertaken in an attempt to describe the general characteristics of the population studied, to find out their perceptions and expectations related to their illness and hospitalization, to find out specific basic needs and to examine the relationship between the patients nursing needs and demographic characteristics through the responses to the nursing history questions. The study population defined and selected was all the patients (70) who were admitted to Yonsei University Hospital from October 1 - 15, 1975. The direct interview method was used and the data were categorized by the investigator, according to the nature of responses to each question and were subjected to the percentile and the chi- square tests. The findings can be summarized as follows 11. General characteristics of the study population ; The population was made up mainly of urban patients who were highly educated. The age was equally distributed. 2. Patients, perceptions and expectations related to illness and hospitalization ; 88.6% of the patients knew about the reason for hospitalization but 64. 5% could state symptoms only. 37.5% recognized the cause of illness. Approximately three fourth of the patients expressed on expectation for early recovery. 60.0% of the patient responses indicated, that they expected less than 10 days of hospitalization. Of the total responses regarding hospitalization, 45.7% were negative about the post -hospitalization expectation, 45.7% expected to return to work. As a result of these findings, we can see that there is a great educational need, a psychological need and environmental need for adaptation to the hospital and a socio- economic need for the post - hospitalization period. 3. Specific basic needs : The physical needs include the problem of getting sufficient sleep (50.0%), difficulty in food intake(47.1%), problems with hospital diet (47.0%), abnormal condition of the skin(44.3%), problems with bowel elimination(35.7%), assistance with bathing(35.7%), pain(30.0%), difficulty in walking(30.0%) , difficulty in seeing(30.0%) problems in urinary elimination(20.0%) , and difficulty in hearing(10.0%), 4. Nursing needs and epidemiological characteristics ; Age distribution was related to the rendition of the mouth but no significant differences were observed statistically with the patients responses to the other nursing history questions according to the epidemiological variables of age, sex, education and residence.

産業場勤勞者에 대한 健康診斷 實態調査 -定期 健康診斷 結果를 中心으로- (A Report of the Periodic Physical Examination for the Industrial Workers in Korea)

  • This report is intended to find out the health status of the korean industrial workers based on the periodic physical examination in 1982. It has resulted in the followings: 1. As of April, 1982, total No. of industries was 81,136 of which workers was totally 3,383,700 persons classifying into 2,163,600 in male and 1,220,100 in female, and manufacturing field was occupied 44.6% of total industries and 61.0% of total workers. 2. In view of size, industries having less than 9 ordinary workers was 47.5%, those having less than 29 ordinary workers was 77.0%. 3. Applicable target industries to industrial safety & health law was 31,059 which was covered 38.3% of total establishments and No. of workers was 2,192,511, 64.8% of all industrial workers. 4. No. of establishment & workers conducting the physical examination was 26,463 which is 85.2% of all examining industries and was 2,007,091, 91.5% of all examining required workers. 5. Case having general disease by general physical examination was 3.5% of examining workers and especially it was appeared 9.7% in mining. 6. Case having general disease by special physical examination was 3.8% of examining workers and incidence rate for occupational disease was 2.4% of examining workers. 7. No. of the occupational diseased was 5,341 persons which is 2.6% of all the examining workers, 205,497 and its rate was appeared highly as 7.5% of silicosis in mining workers and 1.4% of hearing difficulty to noise in construction.

노인골절 환자의 골절 관련 특성에 대한 연구 (A Survey Study on Characteristics Associated with Fractures in Elderly People)

  • Purpose: The purpose of this study was to identify characteristics associated with fractures in elderly people in order to provide basic data for fracture preventive programs for the elderly people. Method: The participants were 84 patients over age of 65, who were admitted to the orthopedic department in a hospital in Chungnam province. Data were collected from Sep. 1, 2002 to Aug, 30, 2003 through personal interviews using a structured questionnaire. The data were analyzed using SPSSPC program. Result: Physical characteristics before the fracture included weakness or paralysis in the extremities (29.8%), need of assistance or appliances (13.1%), difficulty on balance (28.6%), visual disturbances (26.2%), hearing impairment (17.9%), speech disturbances (2.4%), urinary dysfunction (21.4%), and sleep disturbances (54.8%). The fractures occurred most frequently in winter (32.1%), between 1 pm and 6 pm (48.8%), on weekends (41.6%), in the road (58.3%) while wearing snickers (27.4%) or shoes (27.4%). The region of fractures occurred most frequently was lower extremities (57.1%), and the causes of fractures were loss of balance (31.0%) and car accident (25.0 %). A significance difference was found for type of accident, footwear at the time of the accident, place of the accident according to gender and age. Also a significance difference was found for type of accident and place of accident according to season(p<.05). Conclusion: Therefore, these results should be considered when a fracture preventive program for elderly people is designed.

Ewing's Sarcoma/Peripheral Primitive Neuroectodermal Tumor in the Cerebellopontine Angle : Diagnosis and Treatment

  • Ewing's sarcoma/primitive neuroectodermal tumor (ES/PNET) is an unusual malignancy with aggressive behavior. ES/PNET in the cerebellopontine angle (CPA) is extremely uncommon, and we report on a rare case here. A 31-year-old man presented with one month history of left facial palsy, hearing loss, swallowing difficulty, and hoarseness. Magnetic resonance images showed a large mass in the left CPA and a small one in the right cerebellar hemisphere. The patient underwent a surgery for the CPA mass lesion, and the pathology was compatible with ES/PNET. Radiation therapy and chemotherapy were administered. In contrast to the initial radiologic findings resembling vestibular schwannoma or meningioma, ES/PNET had several distinct clinical features. A patient with a CPA mass and presenting unusual clinical features should be suspected of having a rare malignancy.

언어장애인을 위한 안드로이드 기반 의사소통보조 어플리케이션 (An Android Application for Speech Communication of People with Speech Disorders)

  • Voice is the most common means for communication, but some people have difficulties in generating voice due to their congenital or acquired disorders. Individuals with speech disorders might lose their speaking ability due to hearing impairment, encephalopathy or cerebral palsy accompanied by motor skill impairments, or autism caused by mental problems. However, they have needs for communication, so some of them use various types of AAC (Augmentative & Alternative Communication) devices in order to meet their communication needs. In this paper, a mobile application for literate people having speech disorder was designed and implemented by developing accurate and fast sentence-completion functions for efficient user interaction. From a user study and the previous study on Korean text-based communication for adults having difficulty in speech communication, we identified functionality and usability requirements. Specifically, the user interface with scanning features was designed by considering the users' motor skills in using the touch-screen of a mobile device. Finally, we conducted the usability test for the application. The results of the usability test show that the application is easy to learn and efficient to use in communication with people with speech disorders.

/오/-/우/ 합성모음 연속체에 대한 중국인 한국어 학습자의 청지각적 경계 (Perceptual Boundary on a Synthesized Korean Vowel /o/-/u/ Continuum by Chinese Learners of Korean Language)

  • The present study examines the auditory boundary between Korean /o/ and /u/ on a synthesized vowel continuum by Chinese learners of Korean language. Preceding researches reported that the Chinese learners have difficulty pronouncing Korean monophthongs /o/ and /u/. In this experiment, a nine-step continuum was resynthesized using Praat from a vowel token from a recording of a male announcer who produced it in isolated form. F1 and F2 were synchronously shifted in equal steps in qtone (quarter tone), while F3 and F4 values were held constant for the entire stimuli. A forced choice identification task was performed by the advanced learners who speak Mandarin Chinese as their native language. Their experiment data were compared to a Korean native group. ROC (Receiver Operating Characteristic) analysis and logistic regression were performed to estimate the perceptual boundary. The result indicated the learner group has a different auditory criterion on the continuum from the Korean native group. This suggests that more importance should be placed on hearing and listening training in order to acquire the phoneme categories of the two vowels.

Promoting Mobility in Older People

  • Out-of-home mobility is necessary for accessing commodities, making use of neighborhood facilities, and participation in meaningful social, cultural, and physical activities. Mobility also promotes healthy aging as it relates to the basic human need of physical movement. Mobility is typically assessed either with standardized performance-based tests or with self-reports of perceived difficulty in carrying out specific mobility tasks. Mobility declines with increasing age, and the most complex and demanding tasks are affected first. Sometimes people cope with declining functional capacity by making changes in their way or frequency of doing these tasks, thus avoiding facing manifest difficulties. From the physiological point of view, walking is an integrated result of the functioning of the musculoskeletal, cardio-respiratory, sensory and neural systems. Studies have shown that interventions aiming to increase muscle strength will also improve mobility. Physical activity counseling, an educational intervention aiming to increase physical activity, may also prevent mobility decline among older people. Sensory deficits, such as poor vision and hearing may increase the risk of mobility decline. Consequently, rehabilitation of sensory functions may prevent falls and decline in mobility. To promote mobility, it is not enough to target only individuals because environmental barriers to mobility may also accelerate mobility decline among older people. Communities need to promote the accessibility of physical environments while also trying to minimize negative or stereotypic attitudes toward the physical activity of older people.
