Association between social phobia level and dental fear level of college students (대학생의 사회공포수준과 치과공포수준의 관련성)

  • Social phobia is a mental disorder that triggers physical reactions, such as cold sweats, headaches, and panic attacks, as a result of anxiety about a particular situation. Individuals with social phobia are thought to be more sensitive to dental anxiety and may have difficulty visiting the dentist as a result. This study aimed to determine the relationship between social phobia and dental fear among college students, who are reported to have a high prevalence of social phobia. A total of 120 survey responses were analyzed. To investigate the variation in dental fear levels based on the level of social phobia, the total social phobia score was divided into two groups: 41 points or more, and 40 points or less. The disparity in dental fear scores was then analyzed using a t-test. The study found that the average score for dental fear was statistically significantly higher in the group with a total social phobia score of 41 points or more compared to the group with a total score of 40 points or less (p<0.05). Furthermore, the group that scored 41 points or higher on the social phobia scale reported experiencing symptoms such as feeling nauseous at the dentist (3.29 points), sweating upon entering the dentist (3.13 points), and feeling afraid when looking at the dentist (3.13 points). The score was high (p<0.05). Therefore, dental patients exhibiting anxiety symptoms require a comfortable treatment environment to alleviate dental fear, and safe, pain-free dental treatment techniques must be employed.

A Study of the Effect of Structured Rehabilitation Education on the Stress of the Family with Stroke Patients (구조화된 재활교육이 뇌졸중환자 가족의 스트레스에 미치는 영향)

  • Purpose: The purpose of this study is to evaluate the effect of rehabilitation education on the reduction of the stress of family members who have patients suffering from stroke and to find a new way to nurse the patients and their family. Subjects & Methods: The subjects were sixty-one family members with the patients who had been hospotalized in K hospital of oriental medicine from september the 9th, 1996 to september the fourteen, 1996. This study was performed by simulated control group pretest-posttest design; pretest was done on the control group through a questionnaire, counselling and observation while posttest was done on the experimental group 1-2 days after systemic rehabilitation education. To teach the patients and their family, the amended version of a book written by Lee Hae-jin was used as a tool for systemic rehabilitation education. As a method to estimate ADL score, modified Kang's method was applied and ADL score was measured by well-trained technician. As for the tool to estimate the degree of family stress, Choi's method adjusted to this study was applied. In the analysis of the data, social property of the patient and the characteristic of the disease were surveyed in $X^2$ examination to confirm the consistency between the experimental group and the control group. The diffrence in the degree of the stress, which is a dependent factor, was examined by t-test. The difference in ADL score between the experimental group and the control group was examined by t-test. The difference in the degree of the stress according to the general feature of the family with stroke patient, social property of the patients and the characteristic of the disease were surveyed by F examination. The difference in family stress according to the degree of ADL was surveyed by F examination. RESULTS: 1. After hypothetically-examined systemic rehabilitation education, the total of the score of family stress surveyed in 34 items of three domains was compared between the experimental group and the control group. There was no statistically significant difference between two groups; mean score of experimental group=2.28, that of control group=2.93(t=.17, df=59, p=. 66). 2. In the survey on family stress in 34 items, the items over mean score 3.0 were firstly the anxiety of possible disability and relapse of the disease and secondly to watch the patient's suffering without doing anything in the domain of the change of the disease and the difficulty in caring. And the items of the lowest stress with less than mean 2.0 score were little chance to meet the relative and friends, inconsistent treatment and attitude of the medical workers and the change of the attitude of the relative due to the patient orderly in the domain of social and personal relation and the responsibility as the family. The items which showed the difference between two groups were aggravation of neighboring patient(t=3.36, df= 59, p=.001) and the possibility of patient's death(t=2.19, df=58.38, p=.033) in the domain of the change of the disease and the difficulty in caring. 3. In the study on the stress difference according to general features of the family with the stroke patient, the score of family stress with the occupation was higher with mean 2.49 than that of the family stress without occupation with mean 2.16, but there was no significant difference. (F=5.21, df=1/59, p=.026). 4. In the study on the stress difference according to social property of the patient and the characteristic of the disease, there was significant difference in the age of the patients (F=2.98, df=3/57, p=.039). These results show that even if there is no statistically significant difference between two groups, sixteen of the experimental group are less than 3.0 in ADL score(standard 6 score)while eight of the control group are less than 3.0 and that ten of the experimental group are in the year range of 39-49 while four of the control group are in the year range of 39-49 which showed significant difference in family stress. These imply that there is a possibility that the experimental group have serious and fundamental stress resulting in high pretest stress compared with the control group. It might be due to the characteristic of simulated control group pretest-posttest design that the psychologic-supportive effect by the education was not observed. On the basis of these results, the followings are suggested. 1) A study on the nursing-mediated method to reduce the stress in the items which are not resolved by rehabilitation education, a study on nursing according to the patient's age and a study on the supportive nursing toward the family with occupation are required. 2) More than two times consecutive nursing-mediated rehabilitation education to measure the family stress is required. 3) Comprehensive and multilateral systemic education program including the instruction on western-eastern medicine, physical therapy, exercise and diet through collaboration of the experts in each field is required. 4) Family stress at home as well as in the hospital needs to be estimated and home rehabilitation and home-nursing needs to be continued.

Quality of Life, Perceived Health Status, Pain, and Difficulty of Activity of Daily Living of Degenerative Arthritis Patient in Island Residents (일 도서지역 퇴행성관절염 환자의 삶의 질, 지각된 건강상태, 통증 및 일상생활동작 장애)

  • Objectives: The purpose of this study was to identify and relationships among quality of life, perceived health status, pain and difficulty of daily activity with degenerative arthritis living in island residents. Methods: The participants were 601 degenerative arthritis patients. Data were collected using structured questionnaires from June 23th to Oct. 08th, 2008 and analyzed using the SPSS 12.0 program. and it included descriptive statistics, t-test, $x^2$-test, Pearson correlation coefficient. Results: The mean quality of life score was 115.87, perceived health status was 4.40, pain was 6.31 and difficulty of daily activity was 77.62. On the demographic variables, age, living arrangement, level of economics showed significant difference for the quality of life and sex and age showed significant difference for the perceived health status, pain, difficulty of daily activity. On the disease-related variables, duration of disease, use of analgesic drug showed significant difference for the dependent variables. There was a significant positive correlation among the quality of life, perceived health status, significant negative correlation pain and difficulty of daily activity. Conclusions: These results suggested that nursing intervention program including pain reduction, enhancing the perceived health status, ADL abilities and quality of life were necessary for the osteoarthritis patients in island residents.

Validation of the Korean Translated Dynamic Gait Index in Community-Dwelling Elderly (지역사회에 거주하는 노인을 대상으로 한 한글판 동적보행지수의 타당성)

  • The Dynamic Gait Index (DGI) was developed and widely used as a clinical tool to assess balance performance during gait. The purpose of this study was to validate the Korean translated DGI using Rasch analysis. A total of 105 community-dwelling elderly was participated in this study (age range = 65~95 years; mean = 78.0 years). The translated DGI showed sound item psychometric properties, and the 8 items were arranged in order of difficulty for the total participants. The most difficult item was 'Steps' and the easiest item was 'Level surface'. Also, each of the original 4 rating scale categories satisfied the Linacre's essential criteria suggestions for optimal rating scale category effectiveness. Although, thirty eight person (36.2%) showed the maximal high score, but the most of them was no history of fall in the preceding year. For subjects who has falling history, all of the person's ability was arranged within the item's difficulty. The 8-item Korean translated DGI can be used to measure gain in elderly person with balance disorders without compromising important clinical measurement characteristics in Korea.

Beneficial Effects of Natural Environment Therapy for Atopic Dermatitis

  • Objectives: Natural environment therapy is considered as an option for atopic dermatitis. This study was aimed to examine the beneficial effects of natural environment therapy among patients with atopic dermatitis who have moved in an atopy-free village. Methods: The questionnaire-based survey was conducted from parents of 20 children with atopic dermatitis (12 male and 8 female, median age 8 years, ranging from 6 to 13 years). The questionnaire consisted of 10 questions for the changes of two typical symptoms of pruritus and sleeping difficulty after moving into an atopy-free village. Results: 75.0% and 88.9% of patients have a family history (at least one parent and one of siblings respectively) of allergic disorders. The median staying period of patients was 27.6 months (ranging from 2 to 55 months). The score indicating pruritus was significantly improved from $6.2{\pm}2.8$ into $4.8{\pm}1.8$ (p<0.05), and sleeping difficulty was also improved from $5.7{\pm}2.9$ into $4.3{\pm}2.1$ (p<0.05) too. Conclusions: This study suggests that the natural environmental therapies could be a choice for patients with atopic dermatitis, which should be studied further for their scientific evidence and mechanisms.

Current Level of Reproductive Performances in Japanese Black Cows

  • A set of 313,169 reproduction records of Japanese Black cows calving between 1987 and 1996 were collected in nation widely Japan to investigate the current level of reproductive performances and the factors influencing the performances such as calving interval, first calving age, services per conception, calving difficulty, non-gestation period and gestation length. All the records of reproductive performances are outcome of artificial insemination. The means of first calving age, calving interval, non-gestation period and gestation length were 25.0 months, 389.9, 101.9 and 287.5 days, respectively. Services per conception were 1.41 and degree of calving difficulty was 1.07, which suggests most of the calving, did not need assistance. There were chronological tendencies that first calving age became younger while calving interval became longer. Differences in reproductive performance were found for seasons and calving numbers. Calving interval became shorter towards the fourth calving but became longer afterwards. The cows calving in May had the shortest calving interval followed by those in April and June and first calving age had a similar tendency. The cows with the standard body condition score showed more favourable reproductive performances compare with those with rather fatty or thin body condition scores.

Vocabulary Acquisition of Korean Learners for Academic Purposes -Focusing on the Effects of Instruction Introductory Methods of Context Inference and Activation of Background Knowledge (학문목적 한국어 학습자의 어휘 습득 연구 -문맥 추론과 배경지식 활성화를 통한 수업 도입을 중심으로-)

  • The purpose of this study is to deal with vocabulary in KFL. As a result of this study, learners learned vocabulary on average 43 points through contextual inference and introduction of the class to activate background knowledge. In particular, the implicit method showed the highest learning rate of 52 points, and the thematic method had a 41 point-learning rate. In contrast, the semantic method was the lowest with a 25 point-learning rate. There was no significant difference in the improvement rate of upper vocabulary learners, but in the case of the lower learner, there was significant difference in the improvement rate. The difference was not significant in the post-test relative gain rate of upper learners, but there was significant in lower learners. In the delayed test relative gain rate, the difference was significant in all groups. There was correlation between vocabulary difficulty and score, but there was no correlation with the thematic method. And there was no correlation between vocabulary difficulty, improvement rate and relative gain rate in all three classes. However, content understanding, lexical grade, improvement rate, and relative gain rate showed a significant correlation.

Estimating Average Causal Effect in Latent Class Analysis (잠재범주분석을 이용한 원인적 영향력 추론에 관한 연구)

  • Unlike randomized trial, statistical strategies for inferring the unbiased causal relationship are required in the observational studies. Recently, new methods for the causal inference in the observational studies have been proposed such as the matching with the propensity score or the inverse probability treatment weighting. They have focused on how to control the confounders and how to evaluate the effect of the treatment on the result variable. However, these conventional methods are valid only when the treatment variable is categorical and both of the treatment and the result variables are directly observable. Research on the causal inference can be challenging in part because it may not be possible to directly observe the treatment and/or the result variable. To address this difficulty, we propose a method for estimating the average causal effect when both of the treatment and the result variables are latent. The latent class analysis has been applied to calculate the propensity score for the latent treatment variable in order to estimate the causal effect on the latent result variable. In this work, we investigate the causal effect of adolescents delinquency on their substance use using data from the 'National Longitudinal Study of Adolescent Health'.

Physical Health Status and Depression of a Community-Dwelling Elderly Group (일부 지역사회 노인의 신체건강 상태와 우울)

  • The purpose of this study was to describe physical health and depression status, as well as to assessing factors that influence the physical health status. Method: The data was collected from July to August 2000. Study participants were 252 community-dwelling elderly who were recruited from 10 senior centers located in Seoul, Korea. Their physical health status was measured using the Physical Health Status Measurement Scale developed by Choi and Jung (1991), and depression was measured using BDI-II developed by Beck et al. (1996). Results: 1) The physical health status score was 4.00 $\pm$0.68 (range :1- 5). The sub-dimension that showed the highest score was personal hygiene ability at 4.62$\pm$0.95, and the lowest score was sexual function at 2.20$\pm$1.38. 2. The depression score was 17.99+9.79 (range : 0-63). Regarding the sub-dimensions, the depression scores were higher in the domain of interest with sexuality, general weakness, difficulty in concentration, and fatigue. 3. Deeper levels of depression were correlated with a declining physical health status. 4. The most influential factor on physical health was depression, and the explaining variance was 31.68%. Conclusion: It is concluded that elder subjects in senior centers had fairly good physical health and self-care ability. Also, they did not have significantly high levels of depression. Therefor, health promotion of elderly, it is recommended that elder individuals should be regarded as a respectful and useful segment of our society. Along with this basic concept, there should be a social milieu that does not snow prejudice. Moreover, health care professionals should give more attention to helping the elderly achieve a minimal level of ALD, and, particularly, to raise sexuality and help energize the lives of elder individuals.

Quality of Life after Curative Surgery in Patients with Gastric Cancer: Comparison between a Subtotal Gastrectomy and a Total Gastrectomy (위암 환자에 있어서 위의 절제범위에 따른 수술 후 삶의 질 비교)

  • Purpose: Quality of life (QOL) assessment should be applied in surgical settings to compare treatment options. We compared QOL after a subtotal versus a total gastrectomy for gastric cancer to identify which resection would produce a better QOL for the patient. Patients and Methods: We studied 362 patients with no evidence of recurrent disease after curative surgery for gastric cancer. The QOL was measured by assessing patient health perceptions according to the Spitzer index and Troidl score and by treatment-specific symptoms according to Korenaga and others with some modifications. High scores reflect a better QOL. Results: The mean score of the Spitzer index was $8.87\pm1.36$ after a subtotal gastrectomy and $8.80\pm1.23$ after a total gastrectomy. More than $80\%$ of the patients in both groups maintained good quality of life when measured by the Spitzer index. The mean Troidl score was $10.41\pm2.12$ after a subtotal gastrectomy and $9.79\pm2.16$ after a total gastrectomy (p=0.033). Swallowing difficulty was more frequent after a total gastrectomy (p=0.002). There was a statistically significant difference in the meal size (p=0.044). Other variables of the Troidl score revealed no statistically significant differences. Dizziness was significantly more frequent after a total gastrectomy than after a subtotal gastrec tomy (p=0.009). Conclusion: We conclude that in terms of postoperative quality of life, a subtotal gastrectomy has advantages over a total gastrectomy. In those cases where an adequate proximal safety margin can be achieved by a subtotal gastrectomy, that procedure is preferable to a total gastrectomy.
