A Comparative Study of Sasang Constitution Diagnostic Accuracy Rate between Herb Medicine Taking and Non Taking Patients (한약 복용 환자와 미복용 환자의 사상체질 진단정확률 비교 연구)

  • 1. Objectives: The purpose of this study was done to compare Sasang Constitution diagnostic rate between taking Sasang Constitution herb medicine patients and non taking group. 2. Methods: We collected data from 430 patients who were visited at the Department of Sasang Constitution, College of Oriental Medicine, Dong-eui University from November 2006 to September 2010. We classified them into two group. One was taking Sasang Constitution herb medicine patients group, the other was non taking group. For Sasang Constitution Diagnosis, they all have done SSCQ-P questionnaire. and a Sasang Constitution specialist diagnosed constitution. According to reactions after taking herb medicine, the patients of taking group were confirmed their constitutions. We checked Sasang Constitution diagnostic rate from each groups'(the whole subjects, taking herb medicine patients group, non taking herb medicine patients group) discriminant function. And to find out Sasang Constitution diagnostic rate, we applied discriminant functions to each other. 3. Results and Conclusions: 1) The whole subjects' diagnostic accuracy rate was 72.07%. 2) Taking herb medicine group's diagnostic accuracy Rate was 75.08%. Non taking herb medicine group's diagnostic accuracy rate applied taking group's discriminant function was 61.60%. 3) Non taking herb medicine group's diagnostic accuracy rate was 81.06%. Taking herb medicine group's diagnostic accuracy rate applied taking group's discriminant function was 57.14%.

Use of "Diagnostic Yield" in Imaging Research Reports: Results from Articles Published in Two General Radiology Journals

  • Objective: "Diagnostic yield," also referred to as the detection rate, is a parameter positioned between diagnostic accuracy and diagnosis-related patient outcomes in research studies that assess diagnostic tests. Unfamiliarity with the term may lead to incorrect usage and delivery of information. Herein, we evaluate the level of proper use of the term "diagnostic yield" and its related parameters in articles published in Radiology and Korean Journal of Radiology (KJR). Materials and Methods: Potentially relevant articles published since 2012 in these journals were identified using MEDLINE and PubMed Central databases. The initial search yielded 239 articles. We evaluated whether the correct definition and study setting of "diagnostic yield" or "detection rate" were used and whether the articles also reported companion parameters for false-positive results. We calculated the proportion of articles that correctly used these parameters and evaluated whether the proportion increased with time (2012-2016 vs. 2017-2022). Results: Among 39 eligible articles (19 from Radiology and 20 from KJR), 17 (43.6%; 11 from Radiology and 6 from KJR) correctly defined "diagnostic yield" or "detection rate." The remaining 22 articles used "diagnostic yield" or "detection rate" with incorrect meanings such as "diagnostic performance" or "sensitivity." The proportion of correctly used diagnostic terms was higher in the studies published in Radiology than in those published in KJR (57.9% vs. 30.0%). The proportion improved with time in Radiology (33.3% vs. 80.0%), whereas no improvement was observed in KJR over time (33.3% vs. 27.3%). The proportion of studies reporting companion parameters was similar between journals (72.7% vs. 66.7%), and no considerable improvement was observed over time. Conclusion: Overall, a minority of articles accurately used "diagnostic yield" or "detection rate." Incorrect usage of the terms was more frequent without improvement over time in KJR than in Radiology. Therefore, improvements are required in the use and reporting of these parameters.

Comparison of Diagnostic Accuracy and Prediction Rate for between two Syndrome Differentiation Diagnosis Models (중풍 변증 모델에 의한 진단 정확률과 예측률 비교)

  • In spite of abundant clinical resources of stroke patients, the objective and logical data analyses or diagnostic systems were not established in oriental medicine. In the present study we tried to develop the statistical diagnostic tool discriminating the subtypes of oriental medicine diagnostic system, syndrome differentiation (SD). Discriminant analysis was carried out using clinical data collected from 1,478 stroke patients with the same subtypes diagnosed identically by two clinical experts with more than 3 year experiences. Numerical discriminant models were constructed using important 61 symptom and syndrome indices. Diagnostic accuracy and prediction rate of 5 SD subtypes: The overall diagnostic accuracy of 5 SD subtypes using 61 indices was 74.22%. According to subtypes, the diagnostic accuracy of "phlegm-dampness" was highest (82.84%), and followed by "qi-deficiency", "fire/heat", "static blood", and "yin-deficiency". On the other hand, the overall prediction rate was 67.12% and that of qi-deficiency was highest (73.75%). Diagnostic accuracy and prediction rate of 4 SD subtypes: The overall diagnostic accuracy and prediction rate of 4 SD subtypes except "static blood" were 75.06% and 71.63%, respectively. According to subtypes, the diagnostic accuracy and prediction rate was highest in the "phlegm-dampness" (82.84%) and qi-deficiency (81.69%), respectively. The statistical discriminant model of constructed using 4 SD subtypes, and 61 indices can be used in the field of oriental medicine contributing to the objectification of SD.

  • This experiment was carried out to investigate of the early pregnancy diagnosis by finger pressure method, mucus smear method and rectal palpation method in 169 heads of pregnant dairy cow. 1. Pregnancy diagnostic rate of previous, middle and late period of pregnancy by finger pressure methods were 87.3%, 82.0% and 70% respectively. 2. Pregnancy diagnostic rate of previous, middle and late period of pregnancy by mucus smear methods were 83.1%. 94.9% and 95.0% respectively. 3. Pregnancy diagnostic rate of previous, middle and later period of pregnancy by rectal palpation methods were 83.1%, 96.1% and 100% respectively. 4. Pregnancy diagnostic rate of early pregnancy by finger pressure method, mucus smear method and rectal palpation method were 86.7%, 86.8% and 66% respectively. 5. Accordingly, finger pressure method in pregnancy diagnosis is thought to be recommendable because this early diagnostic means are simple with accurate results.

  • 1. Objectives This is the comparative study with hand-operated measurement method and Automatic measurement method, in order to convert the automatic measurement method. 2. Methods We measured the body of patients(hand-operated Width 5 Places and hand-operated circumference 8 place,Automatic Width 5 and automatic circumference 8 place by 3D body measuring instrument) and analyzed the anthropometric data divding into sex&age. 362 patient's data are used in the analysis. 3. Results and Conclusions 1) 1th circumference variable which standing was not a sasang constitutional difference. 2) Diagnostic accuracy rate of the body measurement was 50-80%. 3) Diagnostic accuracy rate of man is higher than Diagnostic accuracy rate of women 4) Diagnostic accuracy rate of Automatic & hand-operated measuring was not a big difference.

  • Background and Purpose ; Sasang Constitutional medicine is dividing individual's constitutions into four categories. Determination of an individual's constitution however, can be different according to what diagnostic standards apply. In this study, We investigated how different the applying results of the three diagnostic standards-the yong-mo(容貌), the body type(體形), and the mind(心性)-are and what the inter-diagnostic standard agreement is. Methods ; 36 patients in cerebral-infarction, admitted into the Dong-Eui Korean hospital from 1. May. 2000 to 12. October. 2000, were diagnosed by three diagnostic standards - the yong-mo(容貌), the body type(體形), and the mind(心性). Each patient was treated by Sasang constitutional medicine. The agreement of the final constitutional diagnosis and the result of each diagnostic standard was analyzed. Results 1. The number of Taeeumin was 13(40.6%), Soyangin was 10(31.3%), Soeumin was 9(28.1%). 2. In terms of analysis on the agreement of the final constitutional diagnosis and the result of each diagnostic standard, the yong-mo(容貌) showed the highest agreement of 93.8%, followed by 68.8% of the mind(心性), and the body type(體形) showed the lowest agreement of 53.1%. 3. The yong-mo(容貌) standard showed high diagnostic rate of the three constitutions(Soyangin, Taeeumin, Soeumin) generally and diagnostic rate of Soeumin was especially high. The body type(體形) standard showed low diagnostic rate of Taeeumin, and high diagnostic rate of Soeumin. The mind(心性) standard showed low diagnostic rate of Soeumin. 4. In terms of analysis on the agreement of inter-diagnostic standard result, the yong-mo(容貌) and the mind(心性) show the highest agreement, the mind(心性) and the body type(體形) showed the lowest agreement. 5. If the results come out that the three diagnostic standards have no concentration on one constitution, it was effective to choose the result of the yong-mo(容貌) standard. If it is the case the result of inter-diagnostic standard agreed more than two, it was better to follow the two agreed result than to follow the result of one specific characteristic standard.

A Study on the Concordance Rate of the Diagnostic Results Among Sasang Constitutional Medicine(SCM) Specialists (사상의학(四象醫學) 전문의(專門醫)의 체질진단(體質診斷)결과 일치도에 관한 연구)

  • 1. Objectives The purpose of this study was to investigate the concordance rate of the diagnostic results among Sasang Constitutional Medicine(SCM) specialists. 2. Methods We carried out two studies in order to investigate the concordance rate of the diagnostic results between SCM specialists. We calculated the kappa value for analyzing concordance rate(using the SPSS program). 1) Study A : We investigated 142 clients who had taken health examinations and diagnosis of Sasang constitution from March 9, 2005 to March 29, 2005 at a health examination center of Kyung-Hee medical center. Every SCM specialist diagnosed 142 clients directly. Each SCM specialist don’t know the other specialist’s diagnosis. 2) Study B : We investigated 176 clients who had taken health examinations and diagnosis of Sasang constitution from January 2005 to March 2005 at the department of Occupational and Environmental medicine of Kyung-Hee medical center. One SCM specialist diagnosed 176 clients directly and the other specialist knowing previous specialist’s diagnosis, diagnosed each person indirectly 3. Results 1) The concordance rate of the diagnostic results among SCM specialists in study A was relatively high(kappa value=0.700). 2) The concordance rate of the diagnostic results among SCM specialists in study B was high(kappa value=0.895). 4. Conclusions We can confirm the concordance rate of the diagnostic results among SCM specialists is relatively high.

Clinical Use of Cholescintigraphy in Aeute Cholecystitis: A Comparative Study with Ultrasonography (급성담낭염에서 담낭신티그라피의 임상적 이용)

  • Retrospective analysis of cholescintigraphy and ultrasonography was done in 76 patients with clinically suspected acute cholecystitis to assess the relative value of the two modalities. Excluding the Patients with obstructive jaundice, the overall results of cholescintigraphy(sensitivity 100%, specificity 95%, false positive rate 5%, false negative rate 0%, accuracy 97%) are nearly identical with or rather superior to those of the ultrasonography(sensitivity 94%, specificity 100%, false positive rate 0%, false negative rate 5%, accuracy 97%). We recommend the cholescintigraphy as the initial modality in patients with clinically suspected acute cholecystitis, and ultrasonography can be used in jaundiced patients to exclude the possibility of the false positive of cholescintigraphy.

Patterns of the Diagnosis Prevalence of Psychiatric Disorders in the Population Aged 0-18 Years Based on the Nationwide Insurance Sample Data

  • Objectives: This study aimed to examine the trend in diagnostic prevalence of psychiatric disorders in children and adolescents. Methods: Individuals aged 0-18 years were included in the study based on the National Health Insurance Claims Data. To investigate the trends in diagnosis and diagnostic prevalence of psychiatric disorders reflecting the decrease in the birth rate, data were analyzed from 2010 as a reference year to 2015. Results: The number of patients diagnosed with psychiatric disorder decreased annually, from 23,412 on 2010 to 18,821 on 2015. The most common disorder was hyperkinetic disorder in male and depressive episode in female. Although there was no significant change in overall diagnostic prevalence rate of psychiatric disorders, age groups <10 years and some disorders had significant changes in prevalence rate. This study classified the diagnostic prevalence by age into two unique patterns: group in which the diagnosis rate increases with age and group in which the diagnosis rate peaks at a certain age and then decreases. Conclusion: Diagnostic prevalence of psychiatric disorders was different according to age and sex. These patterns should be reflected in the formulation of policies related to mental health and in medical practice for pediatric patients. It is urgent to identify how these patterns change in young adults.

  • As increasing the accident of Distribution Transformer (DT), we need to manage them efficiently. In this paper, we discusses with the possibility of the diagnostic technique for distribution transformer using relative ageing rate calculation in this paper. The relative ageing rate of the DT could be calculated from the measured top oil temperature, ambient temperature and load current. In order on apply the proposed diagnostic technique we developed an on line Monitoring and Diagnostic System (MDS) which has hardware and software part. Diagnostic device is developed to measure the state information of DT and to send them with R/F(radio frequency) communication. Host computer monitors and saves the receive data. The database is constructed from the data of distributed DT and it is used for estimating loss-of-life in the MDS.
