A Study on the Pathology and Prescription of the 13 Diagnostic Criteria for Panic Disorder - Based on the Uihagimmun and the Donguibogam - (공황장애 13가지 진단기준의 병리와 처방에 대한 연구 - 『의학입문』과 『동의보감』을 중심으로 -)

  • Objectives : This study aims to examine the 13 diagnostic criteria symptoms of panic disorder in Korean Medicine and to establish a literature basis for treating panic disorder in the clinical setting. Methods : Symptoms in Korean Medicine corresponding to each diagnostic criterion were comprehensively reviewed based on the Uihagimmun(醫學入門) and the Donguibogam(東醫寶鑑). Treatments were investigated as well. Results : According to Korean Medicine, panic disorder is believed to be caused by the stagnation of qi, leading to the retention of phlegm and fluids. Conclusions : Yijintang(二陳湯) can serve as a fundamental prescription for treating panic disorder, and it can be easily adapted to each patient's symptoms through modification of the ingredients.

Circulating Cell-free miRNA Expression and its Association with Clinicopathologic Features in Inflammatory and Non-Inflammatory Breast Cancer

  • Recent discovery showing the presence of microRNAs (miRNAs) in the circulation sparked interest in their use as potential biomarkers. Our previous studies showed the diagnostic potential of miR-451 as a serological marker for inflammatory breast cancer (IBC), miR-337-5p and miR-30b for non-inflammatory breast cancer (non-IBC). The aim of this study is to investigate the prognostic values of circulating miRNAs by comparing the amounts of 12 circulating miRNAs in the serum of IBC and non-IBC from Tunisian breast cancer patients, and by determinating whether correlated pairs of miRNAs could provide useful information in the diagnosis of IBC and non-IBC patients. TaqMan qPCR was performed to detect circulating expression of miRNAs in serum of 20 IBC, 20 non-IBC and 20 healthy controls. Nonparametric rank Spearman rho correlation coefficient was used to examine the prognostic value of miRNAs and to assess the correlation profile between miRNAs expression. Further, a large number of miRNAs were highly correlated (rho>0.5) in both patients groups and controls. Also, the correlations profiles were different between IBC, non-IBC and healthy controls indicating important changes in molecular pathways in cancer cells. Our results showed that miR-335 was significantly overexpressed in premenopausal non-IBC patients; miR-24 was significantly overexpressed in non-IBC postmenopausal patients. Patients with previous parity had higher serum of miR-342-5p levels than those without. Furthermore, patients with HER2+ IBC present lower serum levels of miR-15a than patients with HER2-disease. Together, these results underline the potential of miRNAs to function as diagnostic and prognostic markers for IBC and non-IBC, with links to the menopausal state, Her2 status and parity.

  • For rapid detection of iridovirus infection, a PCR-based diagnostic method was developed. The genomic DNA from mortality-associated iridovirus was cloned into pUC19 vector. The nucleotide sequences of these clones were compared with sequences of other genes from EMBL/GenBank databank. Based on the nucleotide sequences, PCR primers were prepared and used for PCR. The DNA amplification did not occur from the normal fish cells. In contrast, DNA was amplified from the iridovirus-infected fish cells and purified iridovirus. These results suggest that mortality-associated iridovirus can be detected from virus-infected cells within short time and this PCR-based diagnostic system provides a simple and accurate method for detecting the presence of iridovirus infection.

Diagnostic Value of Urine Cytology in 236 cases; a Comparison of Liquid-Based Preparation and Conventional Cytospin Method (요 세포 검사의 진단적 가치; 액상세포검사와 고식적 방법의 비교)

  • Urine cytology is an important screening tool for urinary tract neoplasms. Liquid-based preparation methods, such as $ThinPrep^{(R)}$, have been introduced for non-gynecological samples. We aimed to assess the diagnostic accuracy of liquid-based preparations in urine cytology by comparing the results of the conventional Cytospin preparation method for the same samples. A total of 236 cases subject to urine cytology were enrolled in this study from January 2005 to December 2005. All cases were subjected to cystoscopy and if a malignancy was suspected, a biopsy was performed. Urine cytology slides were made using the $ThinPrep^{(R)}$ preparation method and the conventional Cytospin and/or direct smear method from the individual samples. The results of urine cytology were compared with the final cystoscopic or histological diagnoses. We analyzed the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of both cytology preparation methods. A total of 236 slides made using the liquid based method were satisfactory for slide quality, whereas 5 slides (2.1%) prepared by conventional methods were unsatisfactory because of air-drying, a thick smear, or a bloody or inflammatory background. The $ThinPrep^{(R)}$ method showed 53.1% sensitivity, 92.6% specificity, a 92,6% positive predictive value, a 94.1% negative predictive value and 85,6% accuracy, while the conventional method showed 51% sensitivity, 98.4% specificity, a 92.6% positive predictive value, a 98.4% negative predictive value and 88,6% accuracy. Although the diagnostic values were equivalent between the use of the two methods, the quality of the cytology slides and the time consumed during the microscopic examination for a diagnosis were superior for the $ThinPrep^{(R)}$ method than for the conventional method. In conclusion, our limited studies have shown that the use of the liquid based preparation method is beneficial to improve the quality of slides and reduce the duration for a microscopic examination, but did not show better sensitivity, accuracy and predictive values.

Diagnostic Significance of Cold Agglutinin and Antimycoplasma Antibody for Mycoplasma pneumoniae Infection (항Mycoplasma 항체와 한냉응집소에 의한 Mycoplasma pneumoniae의 진단학적 의의)

  • A Study to evaluate the diagnostic significance of M. pneumoniae infection by measurements of cold agglutinin and antimycoplasma antibody titers is performed with 191 pediatric patients who have visited Yeungnam University Hospital during the period through January to July, 1987. Forty eight of 191 cases made follow up tests feasible. The results obtained are as follows : 1. It is necessary to perform routine combined measurements of cold agglutinin and antimycoplasma antibody titers for the all pediatric pneumoniae caser since a large proportion of pneumonia in children is caused by M. pneumoniae. 2. For the diagnosis of M. pneumoniae infection, measurements of cold agglutinin titers alone seems to be les significant than to check both cold agglutinin and antimycoplasma antibody titers. 3. The measurement of antimycoplasma antibody titer appeared to be more specific than cold agglutinin test in the diagnosis of M. pneumoniae infection. 4. The present study urges the necessity of follow up study of cold agglutinin and antimycoplasma antibody titers for those who initially presented with normal titers in both tests, but are clinically suspected for M. pneumoniae infection.

Fine Needle Aspiration Cytologic Findings of Inflammatory Breast Diseases (유방 염증성 질환의 세침흡인소견)

  • Fine needle aspiration of the breast is an important diagnostic tool in malignant lesions, but is also useful in differentiation of inflammatory breast diseases mimicking carcinoma clinically and radiologically. Recently, the authors have experienced eight biopsy-proven cases of chronic inflammatory diseases of the breast, which consisted of 4 cases of duct ectasia, 2 cases of fat necrosis, and a case of tuberculous mastitis and granulomatous mastitis respectively. Their cytologic features mainly based on the components and the relative frequency of inflammatory cells were evaluated for differential diagnosis of chronic inflammatory breast diseases. The results are as follows; 1. In cases of duct ectasia, varying amount of neutrophils, mononuclear leukocytes, histiocytes and multinucleated giant cells were intermixed with benign epithelial cell clusters. 2 Abundant fat tissue fragments were diagnostic for fat necrosis. Histiocytes and mononuclear cells were main components but not rich, and neutrophils and giant cells were infrequently observed. 3. Characteristic granulomas composed of epithelioid cells, mononuclear leukocytes and Langhans' type giant cells and lymphocytic infiltrates were conspicuous in tuberculous mastitis, and occasionally neutrophils, necrotic materials and epithelial cell clusters were found 4. In granulomatous mastitis, epithelioid cell granulomas were also noted but numerous neutrophils and histiocytes were intermingled within or outside the granulomas.

Fine Needle Aspiration Cytology of the Salivary Gland - An analysis of 221 cases - (타액선의 세침흡인 세포검사 - 221예 분석 -)

  • Fine needle aspiration cytology of the salivary lesions was performed on 221 patients at Soonchunhyang University Hospital for 10 years. Of 221 aspirates, 6 aspirates(2.7%) were inadequate, 116 cases(52.5%) were non-neoplastic lesions, 76(34.4%) cases were benign neoplasms and 23 cases(10.4%) were malignant neoplasms. The cytologic diagnoses could be correlated with histologic findings in 58 cases. FNAC correctly discriminated between neoplastic and nonneoplastic lesions in fifty-seven lesions and failed in a case, and overall accuracy, sensitivity, and specificity were 98.3%, 98.0%, and 100.0%. FNAC correctly discriminated malignant neoplasms from benign neoplastic/nonneoplastic lesions in fifty-three cases and fatted in five cases, and overall accuracy, sensitivity, and specificity were 91.3%, 72.7%, and 95.7%. Among three false negative cases, two mucoepidermoid carcinomas were misdiagnosed as mucocele and benign neoplasm, and an acinic cell carcinoma were misdiagnosed as Warthin's tumor. Two false positive cases were a Warthin's tumor misdiagnosed as squamous cell carcinoma and a pleomorphic adenoma misinterpretated as suggestive of malignancy. In conclusion diagnostic accuracy of FNAC of salivary lesions is high, and the possibilities of low grade mucoepidermoid carcinoma and acinic cell carcinoma should be considered on hypocellular smears with mucoid or fluidy background.

Bilateral Breast Metastases from Epstein-Barr Virus-Associated Gastric Cancer during Pregnancy: Is There a Method to Its Madness?

  • Breast metastases of extramammary malignant neoplasms are rare, with an incidence of 0.3%-2.7% among all malignant mammary tumors. Breast metastases from gastric carcinoma are very rare (<0.1%), and this event is even rarer during pregnancy. Herein, we describe a 39-year-old Caucasian woman with a history of an Epstein-Barr virus-associated gastric carcinoma (EBVaGC) that was characterized by prominent tumor infiltrating lymphocytes. Three years after undergoing radical surgery, the patient developed bilateral breast nodules during her pregnancy. A breast biopsy was performed, and histology confirmed a diagnosis of EBVaGC; tumor cells showed positivity for cytokeratin 8/18 and E-cadherin, and negativity for cytokeratin 7, cytokeratin 20, cytokeratin 5/6, caudal type homebox 2, androgen receptor, mammaglobin, gross cystic disease fluid protein-15, and estrogen and progesterone receptors. We also discuss the main diagnostic pitfalls. To our knowledge, this is the first report of an EBVaGC with lymphoid stroma that developed breast metastases during pregnancy.

Expression of Type IV Collagen and Fibronectin in Salivary Gland Tumors (타액선 종양에서 제4형 교원질과 Fibronectin 발현)

  • Objectives: Salivary gland tumors pose considerable difficulty in diagnostic and prognostic assessment based on the histopathologic features alone. We studied the expression of type IV collagen and fibronectin in salivary gland tumors with special emphasis on the differential diagnostic significance. Materials and Methods: We did immunohistochemical stain on paraffin embedded tissues of 33 benign and 24 malignant salivary gland tumors using monoclonal antibody for type IV collagen and polyclonal antibody for fibronectin. Results: 1) Well preserved linear basement membrane-like staining of type IV collagen was detected in duct-cell-derived benign salivary gland tumors. But pleomorphic adenoma exhibited a heterogeneous pattern as focal augmentation and interruption. 2) In malignant tumors, type IV collagen was distributed in an irregular, interrupted manner or completely absent. Adenoid cystic carcinomas displayed a marked staining of the basal membrane associated substances in the pseudocysts. 3) The staining pattern of fibronectin was similar to that of type IV collagen execpt more dense in the stroma. 4) Salivary gland tumors which have a prominent myoepithelial cell component revealed a particular deposition of basement membrane materials adjacent to the myoepithelial cells. Conclusion: The study of the basal membrane substances may be helpful for differential diagnosis of benign and malignant salivary gland tumors and identifying special features of salivary gland tumors such as pseudocystic pattern of adenoid cystic carcinoma. Also we think that the myoepithelial cells contribute to the formation of basement membrane materials.
