A Comparative Study of Sasang Constitution Diagnostic Accuracy Rate between Herb Medicine Taking and Non Taking Patients (한약 복용 환자와 미복용 환자의 사상체질 진단정확률 비교 연구)

  • 1. Objectives: The purpose of this study was done to compare Sasang Constitution diagnostic rate between taking Sasang Constitution herb medicine patients and non taking group. 2. Methods: We collected data from 430 patients who were visited at the Department of Sasang Constitution, College of Oriental Medicine, Dong-eui University from November 2006 to September 2010. We classified them into two group. One was taking Sasang Constitution herb medicine patients group, the other was non taking group. For Sasang Constitution Diagnosis, they all have done SSCQ-P questionnaire. and a Sasang Constitution specialist diagnosed constitution. According to reactions after taking herb medicine, the patients of taking group were confirmed their constitutions. We checked Sasang Constitution diagnostic rate from each groups'(the whole subjects, taking herb medicine patients group, non taking herb medicine patients group) discriminant function. And to find out Sasang Constitution diagnostic rate, we applied discriminant functions to each other. 3. Results and Conclusions: 1) The whole subjects' diagnostic accuracy rate was 72.07%. 2) Taking herb medicine group's diagnostic accuracy Rate was 75.08%. Non taking herb medicine group's diagnostic accuracy rate applied taking group's discriminant function was 61.60%. 3) Non taking herb medicine group's diagnostic accuracy rate was 81.06%. Taking herb medicine group's diagnostic accuracy rate applied taking group's discriminant function was 57.14%.

Usefulness of MR Imaging for Diseases of the Small Intestine: Comparison with CT

  • Objective: To evaluate the usefulness of MR imaging for diseases of the small intestine, emphasizing a comparison with CT. Materials and Methods: Thirty-four patients who underwent both CT and MR imaging using FLASH 2D and HASTE sequences were analyzed. All patients had various small bowel diseases with variable association of peritoneal lesions. We compared the detectabilities of CT and MR imaging using different MR pulse sequences. The capability for analyzing the characteristics of small intestinal disease was also compared. Results: MR imaging was nearly equal to CT for detecting intraluminal or peritoneal masses, lesions in the bowel and mesentery, and small bowel obstruction, but was definitely inferior for detecting omental lesions. The most successful MR imaging sequence was HASTE for demonstrating bowel wall thickening, coronal FLASH 2D for mesenteric lesions, and axial FLASH 2D for omental lesions. MR imaging yielded greater information than CT in six of 12 inflammatory bowel diseases, while it was equal to CT in six of seven neoplasms and inferior in five of seven mesenteric ischemia. In determining the primary causes of 15 intestinal obstructions, MR imaging was correct in 11 (73%) and CT in nine (60%) patients. Conclusion: MR imaging can serve as an alternative diagnostic tool for patients with suspected inflammatory bowel disease, small intestinal neoplasm or obstruction.

Analysis of the priority of anatomic structures according to the diagnostic task in cone-beam computed tomographic images

  • Purpose: This study was designed to evaluate differences in the required visibility of anatomic structures according to the diagnostic tasks of implant planning and periapical diagnosis. Materials and Methods: Images of a real skull phantom were acquired under 24 combinations of different exposure conditions in a cone-beam computed tomography scanner (60, 70, 80, 90, 100, and 110 kV and 4, 6, 8, and 10 mA). Five radiologists evaluated the visibility of anatomic structures and the image quality for diagnostic tasks using a 6-point scale. results: The visibility of the periodontal ligament space showed the closest association with the ability to use an image for periapical diagnosis in both jaws. The visibility of the sinus floor and canal wall showed the closest association with the ability to use an image for implant planning. Variations in tube voltage were associated with significant differences in image quality for all diagnostic tasks. However, tube current did not show significant associations with the ability to use an image for implant planning. conclusion: The required visibility of anatomic structures varied depending on the diagnostic task. Tube voltage was a more important exposure parameter for image quality than tube current. Different settings should be used for optimization and image quality evaluation depending on the diagnostic task.

  • Functional medicine is a system which utilizes certain investigative and treatment methods that are primarily oriented toward the recognition of functional disorder. Functional disorders take place on an energetic plane. This plane is not capturable using our regular investigatory techniques, such as X -ray. ultrasound, computer tomography and laboratory analyses. The 7-zone-diagnostic system(VEGA-DFM 722 and ABR-2000, etc) is a diagnostic device Which applies pulse signals to predetermined bodily locations. Applying alternating positive and negative stress to tissues with positive and negative pulses first manifests itself as negative and positive wave forms. The skin has many functions as an organ. It is the organism's interlace with the outsider world. Hence, the manner in which the skin reacts to external stimuli reflects the current state of the organism(as meridian systems). This system make a diagnosis of functional disorder or some typical organic diseases include stress of the mind or the body, and the energetic situation, reserves. We are known a disorder or disease throw the result chart. This result chart contain disorder flow wave forms and some valuable diagnostic hints (letters ; S, OM, RA, etc). In spite of this 7-zone-diagnostic system that is applied plentifully in the clinical application, there is not a statistics which is suitable. Therefor we must carry out many various research in the future.

Two Case Reports treated by Mahwang-Haeangin-Gamcho-Seokgo-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 마황행인감초석고탕(麻黃杏仁甘草石膏湯) 투여 후 호전된 증례 2례 고찰)

  • Objectives: In this paper, two cases which showed the meaningful results on the patients' chief complaints were analyzed. The patients were treated with the Mahwang-Haengin-Gamcho-seokgo-tang herbal medication based on Shanghanlun disease pattern identification diagnostic system. Methods: The patients were diagnosed based on Shanghanlun, disease pattern identification diagnostic system. In case 1, the change of menstruation cycle was noted and pre-menstrual discomforts were measured with Menstrual Distress Questionnaire(MDQ). In case 2, Quality of life questionnaire for adult Korean asthmatics (QLQAKA) was used to estimate the quality of the patient's life. Results: All the symptoms were improved after the Mahwang-Haengin-Gamcho-seokgotang treatment. In case 1, the menstruation cycle decreased to 30 days average. MDQ score decreased 143 to 103. In case 2, the change of the QLQAKA score as 1.647 average point is considered as a meaningful improvement. Conclusion: With great difference to a 'Symptom-Medicine' diagnostic system, the disease pattern identification diagnostic system seeks the pathologic pattern through the patient's whole life. More studies and multiple cases based on the diagnostic system are needed to prove this possibility later.

Diagnostic Classification Scheme in Iranian Breast Cancer Patients using a Decision Tree

  • Background: The objective of this study was to determine a diagnostic classification scheme using a decision tree based model. Materials and Methods: The study was conducted as a retrospective case-control study in Imam Khomeini hospital in Tehran during 2001 to 2009. Data, including demographic and clinical-pathological characteristics, were uniformly collected from 624 females, 312 of them were referred with positive diagnosis of breast cancer (cases) and 312 healthy women (controls). The decision tree was implemented to develop a diagnostic classification scheme using CART 6.0 Software. The AUC (area under curve), was measured as the overall performance of diagnostic classification of the decision tree. Results: Five variables as main risk factors of breast cancer and six subgroups as high risk were identified. The results indicated that increasing age, low age at menarche, single and divorced statues, irregular menarche pattern and family history of breast cancer are the important diagnostic factors in Iranian breast cancer patients. The sensitivity and specificity of the analysis were 66% and 86.9% respectively. The high AUC (0.82) also showed an excellent classification and diagnostic performance of the model. Conclusions: Decision tree based model appears to be suitable for identifying risk factors and high or low risk subgroups. It can also assists clinicians in making a decision, since it can identify underlying prognostic relationships and understanding the model is very explicit.

Oroantral communication, its causes, complications, treatments and radiographic features: A pictorial review

  • Purpose: An oroantral communication (OAC) is an abnormal space between the maxillary sinus and oral cavity. The causes, complications, treatment, and radiographic features of OAC in 2-dimensional and 3-dimensional imaging modalities are discussed. Materials and Methods: This pictorial review presents a broad spectrum of imaging findings of OAC. Representative radiographs depicting OAC were chosen from our database. PubMed was used to conduct a comprehensive literature search of OAC. Results: Characteristic features of OAC include discontinuity of the maxillary sinus floor, thickening of the maxillary sinus mucosa, or a combination of both. Two-dimensional imaging modalities are the method of choice for identifying discontinuities in the maxillary sinus floor. However, 3-dimensional imaging modalities are also essential for determining the status of soft tissue in the maxillary sinus. Conclusion: The integration of 2-dimensional and 3-dimensional imaging modalities is crucial for the correct diagnosis and comprehensive treatment of OAC. However, the diagnosis of OAC must be confirmed clinically to prevent unnecessary mental and financial burdens to patients.

Diagnostic value of eosinopenia and neutrophil to lymphocyte ratio on early onset neonatal sepsis

  • Purpose: To determine the diagnostic value of eosinopenia and the neutrophil-to-lymphocyte ratio (NLR) in the diagnosis of early onset neonatal sepsis (EONS). Methods: This cross-sectional study was conducted in the Neonatology Ward of R.D. Kandou General Hospital Manado between July and October 2017. Samples were obtained from all neonates meeting the inclusion criteria for EONS. Data were encoded using logistic regression analysis, the point-biserial correlation coefficient, chi-square test, and receiver operating characteristic curve analysis, with a P value <0.05 considered significant. Results: Of 120 neonates who met the inclusion criteria, 73 (60.8%) were males and 47 (39.2%) were females. Ninety (75%) were included in the sepsis group and 30 (25%) in the nonsepsis group. The mean eosinophil count in EONS and non-EONS groups was $169.8{\pm}197.1cells/mm^3$ and $405.7{\pm}288.9cells/mm^3$, respectively, with statistically significant difference (P<0.001). The diagnostic value of eosinopenia in the EONS group (cutoff point: $140cells/mm^3$) showed 60.0% sensitivity and 90.0% specificity. The mean NLR in EONS and non-EONS groups was $2.82{\pm}2.29$ and $0.82{\pm}0.32$, respectively, with statistically significant difference (P<0.001). The diagnostic value of NLR in the EONS group (cutoff point, 1.24) showed 83.3% sensitivity and 93.3% specificity. Conclusion: Eosinopenia has high specificity as a diagnostic marker for EONS and an increased NLR has high sensitivity and specificity as a diagnostic marker for EONS.

Life-Threatening Lower Gastrointestinal Hemorrhage in Pediatric Crohn's Disease

  • In Crohn's disease, mild gastrointestinal bleeding often occurs; however massive gastrointestinal hemorrhage, which can have a dramatic effect on a patient's vital sign, is rare. This could result in potentially life-threatening complications, which can lead to death. Massive hemorrhagic Crohn's disease is not well known and for this reason, they are a diagnostic and therapeutic challenge. Various diagnostic and therapeutic methods are currently being developed and used. The surgical method is often used only as a last measure since this approach has the risk of serious complications that may endanger patients. However, if massive bleeding continues even after all therapeutic methods are used, the surgical method must be implemented. In this case, all therapeutic methods were found to be ineffective; therefore, surgery was used as a last option. Ultimately, the surgical method was found to be successfully used to treat life-threatening hemorrhagic Crohn's disease.

The Study to Diagnose the Road-Driver Compatibility I: Comparison of Methods for Bio-Signal Analysis (운전자 주행 적합성 진단을 위한 연구 I: 생체신호 분석방법 비교)

  • The aim of this study is to compare the methods in analyzing bio-signals representing measure driver's psychophysiological staus. This study has considered three approaches: first, the deterministic approach calculating the mean and standard deviation of bio-signal, second, probabilistic approach converting driver's bio-signal values to probability density function and identifying individual state relative to overall distribution, and third, diagnostic approach identifying the pattern change of signal over certain period of time. For evaluation of analysis methods, driver's bio-signal was collected under various road conditions, and three analysis approaches were applied respectively. In result, the deterministic approach was found to be simple to use, but generated a large variability of bio-signal. The probabilistic approach provide a relative status of individual driver among overall population, but too much affected by temporal variability of individual driver. The diagnostic approach seemed to reasonably find driver's psychophysiological change over certain period of time, but still needs to develop quantification method of the bio-signal.
